"contents": "<span style=\"font-weight: 400;\">Prioritising local vaccine manufacturing, distribution, and roll-out remains crucial to addressing some of the challenges faced by the African continent’s health systems. Ensuring that vulnerable communities have access to life-saving health innovations will not only ensure that millions of people live longer but also yield economic benefits for developing economies.</span>\r\n\r\n<span style=\"font-weight: 400;\">The African health landscape faces several complex challenges that call on all of us to act with the utmost urgency. Some of the</span><a href=\"https://pmc.ncbi.nlm.nih.gov/articles/PMC10626816/\"> <span style=\"font-weight: 400;\">challenges</span></a><span style=\"font-weight: 400;\"> stem from colonialism and persist to this day, such as malnutrition, social inequality, uncoordinated regulatory bodies, poor health infrastructure development and the decline in funding for healthcare by governments.</span>\r\n\r\n<span style=\"font-weight: 400;\">While all of these issues and challenges have been identified and progress made to address them as envisioned in the</span><a href=\"https://www.afro.who.int/sites/default/files/2018-08/State%20of%20health%20in%20the%20African%20Region.pdf\"> <span style=\"font-weight: 400;\">Framework of Actions</span></a><span style=\"font-weight: 400;\"> (FAS) which seeks to strengthen healthcare systems for Universal Health Coverage (UHC) and the Sustainable Development Goals (SDGs) for Africa, the Covid-19 pandemic exposed the vulnerabilities that persist in many health systems on the continent.</span>\r\n\r\n<span style=\"font-weight: 400;\">Adding to the challenges is the climate emergency which has increased emerging infectious diseases that have spread outside their endemic locations. In the past decade and more, Africa has been ravaged by ebola, measles, yellow fever, zika virus, chikungunya virus, Rift Valley fever and coronavirus,</span><a href=\"https://www.tandfonline.com/doi/full/10.1080/23744235.2021.2022195#d1e301\"> <span style=\"font-weight: 400;\">outbreaks</span></a><span style=\"font-weight: 400;\"> that added to the existing burden of diseases caused by malaria, tuberculosis (TB) and the human immunodeficiency virus (HIV).</span>\r\n\r\n<span style=\"font-weight: 400;\">According to the</span><a href=\"https://www.afro.who.int/sites/default/files/2019-03/Productivity%20cost%20of%20illness%202019-03-21.pdf\"> <span style=\"font-weight: 400;\">World Health Organization</span></a><span style=\"font-weight: 400;\"> (WHO), high-burden diseases such as malaria were responsible for an estimated</span><a href=\"https://www.who.int/news-room/fact-sheets/detail/malaria#:~:text=Disease%20burden&text=The%20estimated%20number%20of%20malaria,cases%20and%2095%25%20of%20deaths.\"> <span style=\"font-weight: 400;\">608,000 deaths</span></a><span style=\"font-weight: 400;\"> globally in 2022, with 95% of cases in Africa. TB was responsible for an estimated</span><a href=\"https://www.afro.who.int/news/rate-tb-diagnosis-treatment-africa-increasing#:~:text=Globally%20TB%20continues%20to%20claim,000%20lives%20lost%20in%202022.\"><span style=\"font-weight: 400;\"> 424,000 deaths in 2022</span></a><span style=\"font-weight: 400;\"> and an estimated 25.6 million people on the continent are living with HIV, with up to</span><a href=\"https://www.afro.who.int/health-topics/hivaids\"> <span style=\"font-weight: 400;\">380,000 deaths</span></a><span style=\"font-weight: 400;\"> caused by Aids-related illnesses. These three diseases and other infections are responsible for the loss of economic productivity amounting to more than $800-billion annually.</span>\r\n\r\n<a href=\"https://www.afro.who.int/health-topics/noncommunicable-diseases\"><span style=\"font-weight: 400;\">Non-communicable diseases</span></a><span style=\"font-weight: 400;\"> (NCDs) like cardiovascular disease, diabetes, hypertension and obesity have also reached catastrophic levels with devastating outcomes resulting in more than 41 million deaths annually, with 77% of premature deaths caused by NCDs occurring in low-middle-income countries.</span>\r\n\r\n<span style=\"font-weight: 400;\">The human papillomavirus (HPV) has the potential to cause cervical cancer in women and the HPV vaccine remains crucial in preventing 90% of deaths related to cervical cancer.</span>\r\n<h4><b>Vaccine manufacturing</b></h4>\r\n<span style=\"font-weight: 400;\">Furthermore, prioritising vaccine manufacturing in Africa would curb the maternal mortality ratio (MMR) by at least 20.3% to achieve the SDG target by 2030. Africa accounts for 69% of global maternal deaths, while up to 50% of deaths of children under five are from pneumonia, malaria, measles, diarrhoea, HIV and tuberculosis.</span>\r\n\r\n<span style=\"font-weight: 400;\">Mpox has become the latest</span><a href=\"https://africacdc.org/disease-outbreak/outbreak-brief-16-monkeypox-in-africa-union-member-states/\"> <span style=\"font-weight: 400;\">public health emergency</span></a><span style=\"font-weight: 400;\"> with more than 74,000 reported cases and 193 deaths globally. Africa has since 2022 reported over 6,800 cases and 173 deaths.</span>\r\n\r\n<span style=\"font-weight: 400;\">The resurgence of these diseases is a public health emergency that requires the urgent strengthening of local vaccine manufacturing capabilities that will foster sustainable equitable access to life-saving products that will restore the continent’s health.</span>\r\n\r\n<span style=\"font-weight: 400;\">Strides have been made to turn Africa into a self-reliant region that is capable of having a thriving vaccine manufacturing industry. This has been demonstrated through the 13 operational companies in South Africa, Egypt, Morocco, Senegal, Ethiopia and Ghana that either have fill-and-finish capabilities, or are manufacturing drug substances. Nigeria, Ethiopia, Algeria, Rwanda and Uganda are also making efforts to establish manufacturing capabilities.</span>\r\n\r\n<span style=\"font-weight: 400;\">These strides are encouraging, but more needs to be done to boost the research and development capabilities crucial to countries succeeding in their efforts to manufacture vaccines locally. Enabling frameworks like the Priority Vaccine Manufacturing (PAVM) Framework outline the continent’s strategic direction to address the 22 legacy diseases, expanding diseases and outbreak diseases. The framework seeks to scale up local vaccine manufacturing to address the reliance on global suppliers.</span>\r\n\r\n<span style=\"font-weight: 400;\">Some of the notable milestones that align with the PAVM include the RTS, S/AS01 malaria vaccine for children in areas with moderate-to-high malaria transmission, and the WHO-recommended R21/Matrix-M malaria vaccine.</span>\r\n\r\n<span style=\"font-weight: 400;\">While there is currently no vaccine for HIV, the development of the longstanding, twice-a-year injectable Pre-Exposure Prophylaxis (PreP) Lenacapavir changes the landscape in the prevention of HIV transmission among key populations in Africa.</span>\r\n\r\n<span style=\"font-weight: 400;\">The third priority disease for Africa, TB, currently has 11 vaccine candidates undergoing clinical trials that are critical to reducing the global burden of TB.</span>\r\n\r\n<span style=\"font-weight: 400;\">The African Medicines Agency (AMA), African Continental Free Trade Area (AfCFTA) and the Pharmaceutical Manufacturing Plan for Africa (PMPA) are some of the mechanisms that will play a crucial role in improving access to locally manufactured vaccines.</span>\r\n\r\n<span style=\"font-weight: 400;\">They will do this by addressing the challenges of poor access to markets and introducing financial mechanisms to support vaccine manufacturing, streamlining regulatory processes to fast-track the approval of vaccines, training and equipping the local workforce with practical experience and global collaboration and developing partnerships with multinational companies to create an enabling environment that will support local manufacturing. </span><b>DM</b>",
