As simple as this sounds, it is difficult to achieve, an issue that health experts discussed during a virtual conference on </span><a href=\"http://iserp.columbia.edu/article/meeting-challenge-covid-19-africa\"><span style=\"font-weight: 400;\">Covid-19 in Africa</span></a><span style=\"font-weight: 400;\"> on Thursday. </span>\r\n\r\n<span style=\"font-weight: 400;\">Delivering a keynote address, Professor </span><a href=\"https://www.publichealth.columbia.edu/people/our-faculty/wme1\"><span style=\"font-weight: 400;\">Wafaa El-Sa</span></a><span style=\"font-weight: 400;\">dr said that population mitigation measures such as lockdowns and </span><a href=\"https://www.dailymaverick.co.za/article/2020-09-04-copy-paste-physical-distancing-measures-have-had-dire-effects-on-developing-countries/\"><span style=\"font-weight: 400;\">physical distancing</span></a><span style=\"font-weight: 400;\"> were unsustainable and had had devastating social and economic effects from job losses to gender-based violence, and disruptions to education.</span>\r\n\r\n<span style=\"font-weight: 400;\">As the pandemic continues to grow, testing is crucial for identifying both symptomatic and asymptomatic individuals, while successful contact tracing allows for the prompt isolation of those infected and ensuring those at risk are quarantined – breaking the chain of transmission.</span>\r\n\r\n<span style=\"font-weight: 400;\">“</span><span style=\"font-weight: 400;\">T</span><span style=\"font-weight: 400;\">here are still a lot of gaps that exist,” said </span><a href=\"https://www.publichealth.columbia.edu/people/our-faculty/wme1\"><span style=\"font-weight: 400;\">El-Sa</span></a><span style=\"font-weight: 400;\">dr, who is the founder and director of </span><a href=\"https://icap.columbia.edu/\"><span style=\"font-weight: 400;\">ICAP</span></a><span style=\"font-weight: 400;\"> (formerly the International Center for AIDS Care and Treatment Programs) at Columbia University in the US</span><span style=\"font-weight: 400;\">.</span><span style=\"font-weight: 400;\"> But as the race for a vaccine continues, effective testing, tracing and treatment measures are critical. </span>\r\n\r\n<span style=\"font-weight: 400;\">Her address was followed by a panel discussion moderated by Martin Veller, dean of the Health Sciences faculty at the University of the Witwatersrand, and included fellow panellists Professor Glenda Gray, CEO of the South African Medical Research Council; Dr Meredith McMorrow from the US Centers for Disease Control (CDC); Mazyanga Mazaba Liwewe from Africa CDC; and Jennifer Dohrn from Columbia University. </span>\r\n\r\n<span style=\"font-weight: 400;\">El-Sadr explained that there were two main types of tests: diagnostic tests that identify current infection, and serologic tests that indicate signs of previous infection. Both have shortcomings, according to McMorrow, who’s based in South Africa, such as the costs of testing kits, and maintaining equipment.</span>\r\n\r\n<span style=\"font-weight: 400;\">Quoted in El-Sadr’s talk was a </span><a href=\"https://academic.oup.com/cid/advance-article/doi/10.1093/cid/ciaa695/5849218\"><span style=\"font-weight: 400;\">study</span></a><span style=\"font-weight: 400;\"> from the Africa CDC that showed some of the key challenges to testing, tracing and treatment on the continent, such as: </span>\r\n<ul>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Limited availability of testing facilities and laboratories </span></li>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Long turnaround times for results</span></li>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Misinformation</span></li>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Shortage of reagents</span></li>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Limited contact tracing due to PPE shortages</span></li>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Hard-to-reach populations</span></li>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The high cost of testing (particularly in private healthcare) </span></li>\r\n \t<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Poorly trained community healthcare workers </span></li>\r\n</ul>\r\n<span style=\"font-weight: 400;\">Gray said that using innovative, digital tracing mechanisms was important. South Africa, for example, recently launched a Bluetooth-enabled mobile app called Covid Alert. </span>\r\n\r\n<span style=\"font-weight: 400;\">“It uses Bluetooth technology to notify users if they have been in contact with [the] coronavirus, including strangers in retail stores, in public transport or in any public space,” a statement from the Presidency read. However, global concerns have been raised over </span><a href=\"https://www.dailymaverick.co.za/article/2020-07-28-the-trial-and-error-of-covid-19-digital-contact-tracing-in-south-africa/\"><span style=\"font-weight: 400;\">data privacy</span></a><span style=\"font-weight: 400;\"> infringements in relation to contact tracing apps. South Africa, for example, had relatively restrictive testing criteria initially requiring a person to have symptoms such as a dry cough or fever. </span>\r\n\r\n<span style=\"font-weight: 400;\">Concerns over this testing strategy in densely populated settings such as informal settlements were raised. Asymptomatic carriers were a significant threat to close contacts, especially in multigenerational homes.</span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa also faced challenges of self-isolation and self-quarantine among the poor. For example, the Western Cape dealt with </span><a href=\"https://www.dailymaverick.co.za/article/2020-06-25-western-cape-concerned-about-high-rejection-rates-for-isolation-facilities/\"><span style=\"font-weight: 400;\">high-rejection rates for facilities </span></a><span style=\"font-weight: 400;\">that had been set up to accommodate those who could not isolate or quarantine at home. Some people had concerns about restrictions to freedom of movement while others did not want to be separated from their families. Further concerns were stigmatisation from community members who would witness a person being taken to a facility by a government vehicle.</span>\r\n\r\n<span style=\"font-weight: 400;\">But there have been successes. Innovative strategies such as ‘</span><a href=\"https://www.dailymaverick.co.za/article/2020-05-09-south-africas-covid-19-testing-strategy-needs-urgent-fixing-heres-how-to-do-it/\"><span style=\"font-weight: 400;\">pooled testing</span></a><span style=\"font-weight: 400;\">’ were effective in Ghana to shorten results’ turnaround time from six days to two and was a cost-effective, low-resource strategy to curb the country’s outbreak. Pooled testing allows saliva from several people to be analysed by one test.</span>\r\n\r\n<span style=\"font-weight: 400;\">W</span><a href=\"https://www.dailymaverick.co.za/article/2020-07-14-sa-needs-a-surveillance-programme-as-covid-19-rna-signals-show-up-in-wastewater-samples/\"><span style=\"font-weight: 400;\">astewater surveillance </span></a><span style=\"font-weight: 400;\"> has also been punted as a cost-effective early warning system to help identify hotspots as the pandemic continues to progress. In South Africa, t</span><span style=\"font-weight: 400;\">he Water Research Commission (WRC) in partnership with the South African Local Government Association (Salga), is leading a water surveillance initiative called the Water Quality (Wastewater and Non-sewer) National Surveillance Programme.</span>\r\n\r\n<span style=\"font-weight: 400;\">Global resources, however, are being pumped into vaccine development. As Africa CDC director John Nkengasong said during Wednesday’s dialogue, “The discussion on a vaccine represents one of the greatest geopolitical conversations that are currently going on.”</span>\r\n\r\n<span style=\"font-weight: 400;\">According to Nkengasong, to vaccinate 20% of the continent’s population would cost between $5- and $7-billion. Innovative strategies such as ‘</span><a href=\"https://www.dailymaverick.co.za/article/2020-05-09-south-africas-covid-19-testing-strategy-needs-urgent-fixing-heres-how-to-do-it/\"><span style=\"font-weight: 400;\">pooled testing</span></a><span style=\"font-weight: 400;\">’ were effective in Ghana to shorten results’ turnaround time from six days to two and was a cost-effective, low-resource strategy to curb the country’s outbreak. Pooled testing allows saliva from several people to be analysed by one test.</span>\r\n\r\n<span style=\"font-weight: 400;\">W</span><a href=\"https://www.dailymaverick.co.za/article/2020-07-14-sa-needs-a-surveillance-programme-as-covid-19-rna-signals-show-up-in-wastewater-samples/\"><span style=\"font-weight: 400;\">astewater surveillance </span></a><span style=\"font-weight: 400;\"> has also been punted as a cost-effective early warning system to help identify hotspots as the pandemic continues to progress. In South Africa, t</span><span style=\"font-weight: 400;\">he Water Research Commission (WRC) in partnership with the South African Local Government Association (Salga), is leading a water surveillance initiative called the Water Quality (Wastewater and Non-sewer) National Surveillance Programme.</span>\r\n\r\n<span style=\"font-weight: 400;\">Global resources, however, are being pumped into vaccine development. As Africa CDC director John Nkengasong said during Wednesday’s dialogue, “The discussion on a vaccine represents one of the greatest geopolitical conversations that are currently going on.”</span>\r\n\r\n<span style=\"font-weight: 400;\">According to Nkengasong, to vaccinate 20% of the continent’s population would cost between $5- and $7-billion.
