"contents": "<span style=\"font-weight: 400;\">The world is facing an unprecedented public health crisis. While disease control centres remain uncertain about the course of the Covid-19 pandemic, there is general agreement that</span><a href=\"https://www.dailymaverick.co.za/article/2020-03-20-covid-19-we-need-both-physical-distancing-and-social-solidarity/\"> <span style=\"font-weight: 400;\">physical distancing</span></a><span style=\"font-weight: 400;\"> and</span><a href=\"https://www.cdc.gov/coronavirus/2019-ncov/prepare/prevention.html\"> <span style=\"font-weight: 400;\">strict hygiene measures</span></a><span style=\"font-weight: 400;\"> are vital to “</span><a href=\"https://blogs.scientificamerican.com/observations/flattening-the-covid-19-curves/\"><span style=\"font-weight: 400;\">flattening the curve</span></a><span style=\"font-weight: 400;\">” by limiting the number of infections. In South Africa, 57 million citizens have been told to stay at home during a three-week government-enforced lockdown.</span>\r\n\r\n<span style=\"font-weight: 400;\">The first of several clinical trials for a vaccine to prevent infection by Sars-CoV-2, the virus that causes Covid-19,</span><a href=\"https://www.nih.gov/news-events/news-releases/nih-clinical-trial-investigational-vaccine-covid-19-begins\"> <span style=\"font-weight: 400;\">began a fortnight ago</span></a><span style=\"font-weight: 400;\">. But even if that vaccine is found to be efficacious and safe, it will take</span><a href=\"https://www.newscientist.com/article/2237742-how-soon-will-we-have-a-coronavirus-vaccine-the-race-against-covid-19/\"> <span style=\"font-weight: 400;\">at least a year</span></a><span style=\"font-weight: 400;\"> before it is available to society’s most vulnerable, such as the elderly and those</span><a href=\"https://www.dailymaverick.co.za/article/2020-03-24-what-does-covid-19-mean-for-people-with-tb/\"> <span style=\"font-weight: 400;\">living with tuberculosis</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">While many scientists are racing to develop a</span><a href=\"https://pubs.acs.org/doi/10.1021/acscentsci.0c00272\"> <span style=\"font-weight: 400;\">Sars-CoV-2-specific antiviral treatment</span></a><span style=\"font-weight: 400;\">, some are working on a different approach. They are investigating whether it might be possible to “repurpose” drugs that have been around for a long time, rather than starting from scratch. The safety of those drugs has usually already been demonstrated in humans,</span><a href=\"https://ncats.nih.gov/preclinical/repurpose\"> <span style=\"font-weight: 400;\">accelerating their transition</span></a><span style=\"font-weight: 400;\"> from the bench to the bedside.</span>\r\n\r\n<span style=\"font-weight: 400;\">Earlier this month, the World Health Organisation announced that it was launching</span><a href=\"https://news.un.org/en/story/2020/03/1059722\"> <span style=\"font-weight: 400;\">a large-scale clinical trial</span></a><span style=\"font-weight: 400;\"> for potential coronavirus therapies to “jumpstart” the search for effective treatments. </span><span style=\"font-weight: 400;\">Independent experts reviewed evidence from a host of laboratory, animal and clinical studies to identify four treatment options for inclusion in the</span><a href=\"https://sacoronavirus.co.za/2020/03/28/update-of-covid-19-28th-march-2020-2/\"> <span style=\"font-weight: 400;\">Solidarity mega-trial</span></a><span style=\"font-weight: 400;\">, of which </span><span style=\"font-weight: 400;\">South Africa is part.</span>\r\n\r\n<span style=\"font-weight: 400;\">The proposed treatments are antiviral drug remdesivir (active against coronaviruses SARS and MERS and used to treat Ebola), a combination of two anti-HIV drugs called lopinavir and ritonavir, and lopinavir-ritonavir together with a signalling protein called interferon beta. (Disappointingly, a preliminary trial of the lopinavir-ritonavir combination in adults with severe Covid-19 has</span><a href=\"https://www.nejm.org/doi/full/10.1056/NEJMoa2001282?query=featured_home\"> <span style=\"font-weight: 400;\">shown no benefit</span></a><span style=\"font-weight: 400;\">.)</span>\r\n\r\n<span style=\"font-weight: 400;\">The fourth treatment arm will comprise </span><span style=\"font-weight: 400;\">chloroquine or hydroxychloroquine,</span> <span style=\"font-weight: 400;\">two closely related drugs typically used to treat malaria and rheumatic conditions, respectively. Public </span><span style=\"font-weight: 400;\">interest in these medicines has surged as a result of recent claims by entrepreneur</span><a href=\"https://twitter.com/elonmusk/status/1239650597906898947\"> <span style=\"font-weight: 400;\">Elon Musk</span></a><span style=\"font-weight: 400;\"> and</span><a href=\"https://www.ft.com/content/46754bae-6a0d-11ea-800d-da70cff6e4d3\"> <span style=\"font-weight: 400;\">US President Donald Trump</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<b>What we know about chloroquine</b>\r\n\r\n<span style=\"font-weight: 400;\">Chloroquine is a classic antimalarial drug that revolutionised the treatment of malaria in the 20th century. It is a synthetic derivative of quinine, one of the oldest natural treatments for malaria. Since it was first administered as an antimalarial in 1944, chloroquine has saved millions of lives across the globe.</span>\r\n\r\n<span style=\"font-weight: 400;\">It was a remarkably effective, safe and inexpensive treatment against malaria until the emergence and spread of resistance led to a</span><a href=\"https://academic.oup.com/jid/article/203/1/11/878765\"> <span style=\"font-weight: 400;\">sharp decline in its use</span></a><span style=\"font-weight: 400;\">. Hydroxychloroquine, a derivative of chloroquine, is also effective against malaria and is usually preferred because of its</span><a href=\"https://link.springer.com/article/10.1007/s10787-015-0239-y\"> <span style=\"font-weight: 400;\">more tolerable safety profile</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">It is well established that chloroquine and hydroxychloroquine inhibit an important metabolic pathway in the malaria parasite called haemozoin biocrystallisation, but the precise way in which this leads to parasite death remains unclear. These drugs are also likely to disturb other aspects of metabolism within the malaria parasite. Part of my doctoral research investigated how chloroquine</span><a href=\"https://pubs.acs.org/doi/10.1021/acschembio.8b00583\"> <span style=\"font-weight: 400;\">distributes within the parasite and interacts with protein targets</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Despite their longstanding use as antimalarials, chloroquine and hydroxychloroquine are also used in the clinical treatment of other diseases such as</span><a href=\"https://academic.oup.com/rheumatology/article/36/7/799/1782792\"> <span style=\"font-weight: 400;\">rheumatoid arthritis and lupus</span></a><span style=\"font-weight: 400;\">, an autoimmune disease. Chloroquine has even been proposed</span><span style=\"font-weight: 400;\"> as an adjunct in some</span><a href=\"https://cancerres.aacrjournals.org/content/73/1/3\"> <span style=\"font-weight: 400;\">cancer therapy regimes</span></a><span style=\"font-weight: 400;\">. However,</span><a href=\"https://apps.who.int/iris/bitstream/handle/10665/65773/WHO_MAL_79.906.pdf?sequence=1&isAllowed=y\"> <span style=\"font-weight: 400;\">serious side effects</span></a><span style=\"font-weight: 400;\"> can result from acute and chronic use of these drugs and so appropriate doses need to be selected carefully.</span>\r\n\r\n<span style=\"font-weight: 400;\">In addition to these broad-spectrum effects on the immune system, chloroquine also exhibits potent antiviral activity in a petri dish (“in vitro”)</span><a href=\"https://virologyj.biomedcentral.com/articles/10.1186/1743-422X-2-69\"> <span style=\"font-weight: 400;\">against Sars-CoV</span></a><span style=\"font-weight: 400;\">, the strain of coronavirus that was identified in 2003 to cause severe acute respiratory syndrome (SARS).</span>\r\n\r\n<span style=\"font-weight: 400;\">How chloroquine diminishes viral replication is unclear; it appears to modulate vesicular pH and may also interfere with virus-receptor binding. A</span><a href=\"https://www.youtube.com/watch?v=BIymfznD7YA\"> <span style=\"font-weight: 400;\">recent hypothesis</span></a><span style=\"font-weight: 400;\"> suggests that chloroquine may act as a</span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0109180\"> <span style=\"font-weight: 400;\">zinc “ionophore”</span></a><span style=\"font-weight: 400;\"> (meaning that it has a high affinity for zinc ions), leading to a high cellular concentration of this metal which could</span><a href=\"https://journals.plos.org/plospathogens/article?id=10.1371/journal.ppat.1001176\"> <span style=\"font-weight: 400;\">effectively impair replication</span></a><span style=\"font-weight: 400;\"> of the virus.</span>\r\n\r\n<span style=\"font-weight: 400;\">Interestingly, these inhibitory effects are observed when cells are treated with chloroquine either before or after exposure to the virus, suggesting that chloroquine may have both prophylactic and therapeutic potential.</span>\r\n\r\n<b>Chloroquine and Covid-19</b>\r\n\r\n<span style=\"font-weight: 400;\">Because of the similarity between Sars-CoV and the C0vid-19 virus (both pathogens</span><a href=\"https://link.springer.com/article/10.1007%2Fs11427-020-1637-5\"> <span style=\"font-weight: 400;\">share similar “spike” proteins on their outer coat</span></a><span style=\"font-weight: 400;\">), researchers wondered whether chloroquine could be effective against Sars-CoV-2. Indeed, chloroquine was shown to</span><a href=\"https://www.nature.com/articles/s41422-020-0282-0\"> <span style=\"font-weight: 400;\">effectively inhibit the virus in vitro</span></a><span style=\"font-weight: 400;\">. In a similar cell line model, hydroxychloroquine was found to be</span><a href=\"https://academic.oup.com/cid/advance-article/doi/10.1093/cid/ciaa237/5801998\"> <span style=\"font-weight: 400;\">even more potent</span></a><span style=\"font-weight: 400;\"> against Sars-CoV-2 than chloroquine.</span>\r\n\r\n<span style=\"font-weight: 400;\">Last month, preliminary clinical investigations in China found that chloroquine may be effective and safe in treating Covid-19 associated pneumonia. The</span><a href=\"https://www.biosciencetrends.com/article/1883\"> <span style=\"font-weight: 400;\">report stated</span></a><span style=\"font-weight: 400;\"> that:</span>\r\n\r\n<span style=\"font-weight: 400;\">“Results from more than 100 patients have demonstrated that chloroquine phosphate is superior to the control treatment in inhibiting the exacerbation of pneumonia, improving lung imaging findings, promoting a virus negative conversion, and shortening the disease course. [...] Severe adverse reactions to chloroquine phosphate were not noted in these patients.”</span>\r\n\r\n<span style=\"font-weight: 400;\">To investigate further the potential of chloroquine as a treatment for Covid-19, there are currently over a dozen ongoing</span><a href=\"https://linkinghub.elsevier.com/retrieve/pii/S0883944120303907\"> <span style=\"font-weight: 400;\">clinical trials in China</span></a><span style=\"font-weight: 400;\">. </span><span style=\"font-weight: 400;\">However, experts have voiced concern that the randomised controlled trials in China are small and</span><a href=\"https://www.who.int/blueprint/priority-diseases/key-action/RD-Blueprint-expert-group-on-CQ-call-Mar-13-2020.pdf\"> <span style=\"font-weight: 400;\">may not be powered enough</span></a><span style=\"font-weight: 400;\"> to reach the statistical conclusions that are needed.</span>\r\n\r\n<span style=\"font-weight: 400;\">For hydroxychloroquine, a non-randomised trial</span><a href=\"https://www.sciencedirect.com/science/article/pii/S0924857920300996\"> <span style=\"font-weight: 400;\">published earlier this month</span></a><span style=\"font-weight: 400;\"> by researchers in France concluded that the drug significantly reduced viral load in nasal swabs of Covid-19 patients when it was administered in combination with</span><span style=\"font-weight: 400;\"> antibacterial drug azithromycin</span><span style=\"font-weight: 400;\">. Despite being a small, open-label trial, this preliminary result is encouraging.</span>\r\n\r\n<span style=\"font-weight: 400;\">Further clinical trials</span><a href=\"https://clinicaltrials.gov/ct2/results?cond=Coronavirus&term=Chloroquine&cntry=&state=&city=&dist=\"> <span style=\"font-weight: 400;\">are scheduled to start soon</span></a><span style=\"font-weight: 400;\"> in the UK, Canada, Egypt, Thailand and Norway, among other countries. Importantly, the doses and side effects of these interventions will need to be closely monitored to avoid acute toxicity. To assist these global efforts, pharmaceutical giants</span><a href=\"https://www.magazine.bayer.com/en/coronavirus-an-active-substance-from-bayer-could-help.aspx\"> <span style=\"font-weight: 400;\">Bayer</span></a><span style=\"font-weight: 400;\"> and</span><a href=\"https://www.novartis.com/news/media-releases/novartis-commits-donate-130-million-doses-hydroxychloroquine-support-global-covid-19-pandemic-response\"> <span style=\"font-weight: 400;\">Novartis</span></a><span style=\"font-weight: 400;\"> have announced that they will donate vast quantities of chloroquine and hydroxychloroquine, respectively.</span>\r\n\r\n<span style=\"font-weight: 400;\">Finding ways to manage and mitigate Covid-19 infections is an urgent need, especially for countries such as South Africa in which a large proportion of the population is vulnerable to infection. While the promise of repositioning “old” drugs such as chloroquine and hydroxychloroquine is tantalising, it is critical that the hazards of using unapproved treatments are shared with the public to prevent</span><a href=\"https://www.bbc.com/news/52012242\"> <span style=\"font-weight: 400;\">needless fatalities</span></a><span style=\"font-weight: 400;\"> and</span><a href=\"https://www.wsj.com/articles/trump-claim-that-malaria-drugs-treat-coronavirus-sparks-warnings-shortages-11584981897?tesla=y&mod=article_inline\"> <span style=\"font-weight: 400;\">supply shortages</span></a><span style=\"font-weight: 400;\"> that may further burden already strained healthcare systems.</span>\r\n\r\n<span style=\"font-weight: 400;\">It is important that robust data, gathered from properly managed clinical trials, are obtained quickly in order to make informed clinical decisions for patients with C0vid-19. </span><span style=\"font-weight: 400;\">Fortunately, while we wait, there are a growing number of online resources for research scientists to make their contribution to “</span><a href=\"http://crowdfightcovid19.org/\"><span style=\"font-weight: 400;\">crowdfighting</span></a><span style=\"font-weight: 400;\">” this alarming pandemic</span><span style=\"font-weight: 400;\">. </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">John Woodland is a postdoctoral medicinal chemist with the Drug Discovery and Development Centre (H3D) at the University of Cape Town.</span></i>",
