"contents": "<span style=\"font-weight: 400;\">Dear Dr Mokgethi,</span>\r\n\r\n<span style=\"font-weight: 400;\">On 20 June, your department posted two tweets with the hashtag #cancerawareness. One was a promotional tweet on “How to Manage Cancer”; the other was a reminder to the public that “early detection and management of any cancer is vital for treatment and chances of achieving [a] cure”.</span>\r\n\r\nhttps://twitter.com/GautengHealth/status/1538881166660485120\r\n\r\n<span style=\"font-weight: 400;\">Raising awareness about the risks of cancer is important. Thank you for doing so, Dr Mokgethi, but don’t you think that, before sending out bold public reminders on how cancer should be managed, Gauteng Health should first be in a position to provide timely access to quality radiation oncology services?</span>\r\n\r\n<span style=\"font-weight: 400;\">Let’s consider how long radiation oncology services have been a challenge and why this is a pressing and long-lasting issue that needs your urgent attention.</span>\r\n\r\n<span style=\"font-weight: 400;\">In December 2021, the Head of Radiation Oncology at Charlotte Maxeke Johannesburg Academic Hospital, Dr Duvern Ramiah, told </span><i><span style=\"font-weight: 400;\">Carte Blanche</span></i><span style=\"font-weight: 400;\"> that the </span><a href=\"https://www.youtube.com/watch?v=y6KaJ6q2-p4\"><span style=\"font-weight: 400;\">waiting list backlog</span></a><span style=\"font-weight: 400;\"> for radiation therapy has been growing steadily since 2018, with some breast cancer patients waiting up to 18 months for radiation. These challenges predate Covid-19 and the fire at Charlotte Maxeke hospital – both issues many of the prevailing challenges in the public health sector seem to be blamed on.</span>\r\n\r\n<span style=\"font-weight: 400;\">In the same </span><i><span style=\"font-weight: 400;\">Carte Blanche </span></i><span style=\"font-weight: 400;\">episode, Louise Turner of the Breast Health Foundation explained why breast cancer patients need timely radiation therapy:</span>\r\n\r\n<span style=\"font-weight: 400;\">“You finish chemotherapy; you have your surgery. You have a three-month window to start your radiation oncology. And the scary thing is, if you don’t get your radiation in time, there’s a very high likelihood of having a local recurrence, [of the] cancer growing back in that area. And once that happens, you start at the beginning and you go back to chemotherapy for six months, then another surgery, then you go on to the waiting list for radiation.”</span>\r\n<h4><b>Required: Solutions and results</b></h4>\r\n<span style=\"font-weight: 400;\">What are the solutions that cancer patients are waiting for, Dr Mokgethi? Where are the results? Four years is more than enough time to begin to remedy this problem. Instead, it seems waiting lists are only getting longer.</span>\r\n\r\n<span style=\"font-weight: 400;\">There have been some positive steps. Establishing the Cancer Task Team was, on paper, a very good idea, but public information on its progress has been less than forthcoming. </span>\r\n\r\n<span style=\"font-weight: 400;\">According to the Cancer Alliance’s Salomé Meyer, who sits on the task team, it first convened in April 2022 with an immediate task to investigate and reduce the Charlotte Maxeke hospital radiation oncology waiting list by looking at viable alternatives such as public-private partnerships (PPPs). </span>\r\n\r\n<span style=\"font-weight: 400;\">Meyer confirmed in response to my queries that the task team has met bi-weekly since April (over three months), but no news has reached the public domain. Can we expect an announcement soon on what the task team has achieved thus far?</span>\r\n<h4><b>Departmental staff shortages</b></h4>\r\n<span style=\"font-weight: 400;\">The task team has also been mandated to look into staffing and equipment shortfalls at both Charlotte Maxeke and Steve Biko hospitals. In the </span><i><span style=\"font-weight: 400;\">Carte Blanche</span></i><span style=\"font-weight: 400;\"> feature and in an early April meeting that I attended, Dr Ramiah alluded to departmental staff shortages. </span>\r\n\r\n<span style=\"font-weight: 400;\">In March this year, you announced that Gauteng Health had experienced </span><a href=\"https://www.news24.com/citypress/news/charlotte-maxeke-hospital-hits-another-wall-as-radiology-department-falls-apart-20220601\"><span style=\"font-weight: 400;\">a number of resignations</span></a><span style=\"font-weight: 400;\"> since May last year. Considering Dr Ramiah’s comments, it is conceivable that radiation oncology was among the departments affected. </span>\r\n\r\n<span style=\"font-weight: 400;\">Despite this, I have not seen any advertisements for radiation oncology staff positions at Charlotte Maxeke hospital. May I ask if you have made any appointments to address the staffing shortages? Will you share the current numbers with the public?</span>\r\n<h4><b>Unused equipment</b></h4>\r\n<span style=\"font-weight: 400;\">I’m sure your current task has not been made easier after the </span><a href=\"https://www.citizen.co.za/lifestyle/health/3048392/cancer-r500-million-linac-accelerator-machines/\"><span style=\"font-weight: 400;\">R500-million investment</span></a><span style=\"font-weight: 400;\"> made in linear accelerator (Linac) radiation machines in 2019. It has been reported that the department procured these machines without building the </span><a href=\"https://www.medicalbrief.co.za/new-radiotherapy-machines-in-storage-while-gauteng-patients-are-turned-away/\"><span style=\"font-weight: 400;\">bunkers</span></a><span style=\"font-weight: 400;\"> needed to house them, which means they are in storage and cannot be used. Have any of the new machines been allocated to Charlotte Maxeke hospital?</span>\r\n\r\n<span style=\"font-weight: 400;\">In April, Dr Ramiah admitted that only four Linac machines were operational at Charlotte Maxeke hospital. This means that the hospital remains </span><a href=\"https://www.spotlightnsp.co.za/2021/06/28/opinion-stark-realities-facing-cancer-patients-at-charlotte-maxeke-hospital/\"><span style=\"font-weight: 400;\">short of at least six radiation machines</span></a><span style=\"font-weight: 400;\">, including the Linacs, to cater for patient requirements adequately. </span>\r\n\r\n<span style=\"font-weight: 400;\">Are there currently radiation machines in need of repair? If so, these machines have existing bunkers (too small for the new Linacs), so would repairing them not be a short-term option to reduce the numbers on the waiting lists? Do you have the staff to operate them? Perhaps, if you had procured machines that would have fitted into the existing bunkers, years would not have been wasted and more patients could have been treated?</span>\r\n<h4><b>Required: Truth and transparency</b></h4>\r\n<span style=\"font-weight: 400;\">Being transparent and honest about the department’s progress in addressing the radiation oncology challenges is crucial, Dr Mokgethi. Cancer patients deserve better than to be kept in the dark about measures that are a matter of life and death.</span>\r\n\r\n<span style=\"font-weight: 400;\">This need for honesty and transparency also extends to healthcare workers in your employ, as I’ve already mentioned in an earlier </span><a href=\"https://copyfounder.com/an-open-letter-to-dr-nolutshungu/\"><span style=\"font-weight: 400;\">open letter to Dr Nomonde Nolutshungu</span></a><span style=\"font-weight: 400;\">. Doctors should also be frank with cancer patients.</span>\r\n\r\n<span style=\"font-weight: 400;\">Are doctors in your employ less forthcoming with the state of affairs due to fear of being disciplined by their superiors? We are all aware of what happened when </span><a href=\"https://www.dailymaverick.co.za/article/2022-06-09-dr-tim-de-maayer-who-blew-the-whistle-on-shocking-state-of-childrens-healthcare-at-rahima-moosa-hospital-is-suspended/\"><span style=\"font-weight: 400;\">Dr Tim de Maayer</span></a><span style=\"font-weight: 400;\"> spoke up about the state of affairs at Rahima Moosa Mother and Child Hospital. Transparency is important to dispel any misperceptions, Dr Mokgethi.</span>\r\n\r\n<span style=\"font-weight: 400;\">Cancer patients, their loved ones, cancer organisations and other members of the voting public need urgent solutions or, at the very least, frequent updates from the Gauteng Department of Health on the steps being implemented to address these challenges. </span>\r\n\r\n<span style=\"font-weight: 400;\">My hope is that these questions will also be raised in the Gauteng Legislature so that we can get official answers. Many people are dying and there is a perception that you and your department are not acting with the necessary urgency.</span>\r\n\r\n<span style=\"font-weight: 400;\">We need answers, Dr Mokgethi. "description": "<span style=\"font-weight: 400\">Dr Tim De Maayer is a dedicated paediatric gastroenterologist who works at Rahima Moosa Mother and Child Hospital and serves as a lecturer at the University of the Witwatersrand. </span>\r\n\r\n<span style=\"font-weight: 400\">At Rahima Moosa Hospital, he established a specialised Paediatric Gastroenterology service, which provides care to children with complex gastrointestinal, liver, or nutritional diseases. </span>\r\n\r\n<span style=\"font-weight: 400\">In May 2022, he wrote an </span><a href=\"https://www.dailymaverick.co.za/article/2022-05-22-a-wake-up-call-for-health-department-heads-children-are-dying-because-of-horrendous-state-of-our-public-hospitals/\"><span style=\"font-weight: 400\">open letter </span></a><span style=\"font-weight: 400\">to administrators at the Gauteng Department of Health, which was </span><a href=\"https://www.dailymaverick.co.za/article/2022-05-22-a-wake-up-call-for-health-department-heads-children-are-dying-because-of-horrendous-state-of-our-public-hospitals/\"><span style=\"font-weight: 400\">published</span></a><span style=\"font-weight: 400\"> by Daily Maverick. His open letter was widely praised as he exposed the shocking state of children’s healthcare at the Rahima Moosa Mother and Child Hospital.</span>\r\n\r\n<span style=\"font-weight: 400\">Dr Tim De Maayer did what brave health workers have always done. Having done everything in his power to care for the desperately ill children at Rahima Moosa Mother and Child Hospital, he raised issues with management and, having seen no change, he went public. </span>\r\n\r\n<span style=\"font-weight: 400\">In June 2022, </span><a href=\"https://www.dailymaverick.co.za/article/2022-06-09-dr-tim-de-maayer-who-blew-the-whistle-on-shocking-state-of-childrens-healthcare-at-rahima-moosa-hospital-is-suspended/\"><span style=\"font-weight: 400\">he was placed on precautionary suspension</span></a><span style=\"font-weight: 400\"> with immediate effect after criticising the state of healthcare provided to patients at the hospital. The resulting public outcry saw him </span><a href=\"https://www.dailymaverick.co.za/article/2022-06-10-whistleblower-paediatricians-suspension-lifted-after-massive-public-outcry/\"><span style=\"font-weight: 400\">being reinstated.</span></a>\r\n\r\n<span style=\"font-weight: 400\">The consequences of the punitive action against those speaking out in healthcare led to many South African healthcare workers joining the</span><a href=\"https://www.dailymaverick.co.za/article/2022-06-21-i-am-movement-of-sa-health-workers-throws-down-the-gauntlet-to-health-minister-and-gauteng-premier/\"><span style=\"font-weight: 400\"> “I am” movement</span></a><span style=\"font-weight: 400\"> in 2022, speaking up against continued victimisation. </span>\r\n\r\n<span style=\"font-weight: 400\">The Office of the Health Ombud released </span><a href=\"https://healthombud.org.za/publications/reports/investigation-report-into-allegations-against-rahima-moosa-mother-and-child/\"><span style=\"font-weight: 400\">its report</span></a><span style=\"font-weight: 400\"> to minister of health Dr Joe Phaahla on Tuesday 14 March 2023, </span><a href=\"https://www.dailymaverick.co.za/article/2023-03-14-rahima-moosa-mother-child-hospital-is-a-filthy-and-neglected-mess-says-health-ombud/\"><span style=\"font-weight: 400\">noting</span></a><span style=\"font-weight: 400\"> that the health and dignity of patients and the wellbeing of healthcare workers were severely compromised.</span>\r\n\r\n<span style=\"font-weight: 400\">The investigative team spent close to a year investigating the allegations and concluded that there was incontrovertible proof that confirmed the complaints. The team obtained video footage that corroborated their findings, and whistle-blower Dr Tim De Maayer contributed significantly to the report.</span>",
