"contents": "<span style=\"font-weight: 400;\">By 2028, South Africa wants to have medicines such as </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK310658/\"><span style=\"font-weight: 400;\">methadone</span></a><span style=\"font-weight: 400;\">, which will help people get off </span><a href=\"https://www.cdc.gov/opioids/basics/heroin.html\"><span style=\"font-weight: 400;\">opioids like heroin</span></a><span style=\"font-weight: 400;\">, available at all state clinics. This is part of the country’s </span><a href=\"https://sanac.org.za/wp-content/uploads/2023/05/NSP-Document.pdf\"><span style=\"font-weight: 400;\">new plan</span></a><span style=\"font-weight: 400;\"> for services that can help to cut HIV infections over the next five years, which was released in March. </span>\r\n\r\n<span style=\"font-weight: 400;\">In 2021, South Africa recorded about </span><a href=\"https://www.unaids.org/en/regionscountries/countries/southafrica\"><span style=\"font-weight: 400;\">210,000 new HIV infections</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<a href=\"https://pubmed.ncbi.nlm.nih.gov/31643200/\"><span style=\"font-weight: 400;\">Opioids</span></a><span style=\"font-weight: 400;\"> are a group of drugs that can give you a feeling of extreme pleasure because they prompt your brain to release feel-good chemicals – but they are addictive. Other than the illegal substance heroin, the painkillers </span><a href=\"https://medlineplus.gov/druginfo/meds/a682133.html\"><span style=\"font-weight: 400;\">morphine</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://medlineplus.gov/druginfo/meds/a682065.html\"><span style=\"font-weight: 400;\">codeine</span></a><span style=\"font-weight: 400;\"> are also part of this group. </span>\r\n\r\n<span style=\"font-weight: 400;\">People who are hooked on opioids such as heroin often get their fix by injecting it. </span>\r\n\r\n<span style=\"font-weight: 400;\">Although it’s difficult to obtain exact numbers (because </span><a href=\"https://www.sahpra.org.za/wp-content/uploads/2022/07/Drug-and-Drug-Trafficking-Act-1992.pdf\"><span style=\"font-weight: 400;\">using drugs like these recreationally is illegal in South Africa</span></a><span style=\"font-weight: 400;\">), a study among 926 users published in 2019 showed that </span><a href=\"https://harmreductionjournal.biomedcentral.com/articles/10.1186/s12954-019-0298-2\"><span style=\"font-weight: 400;\">one in five people who inject drugs have HIV</span></a><span style=\"font-weight: 400;\">. Why? When drug users share or use discarded needles, there’s a big risk that they can either become infected with HIV or spread the virus. </span>\r\n\r\n<span style=\"font-weight: 400;\">Enter methadone, which is a substitute for heroin. It is </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK310658/\"><span style=\"font-weight: 400;\">a medicine that’s taken daily as a liquid or a pill</span></a><span style=\"font-weight: 400;\"> to help curb the withdrawal symptoms from the illegal drug. </span>\r\n\r\n<span style=\"font-weight: 400;\">To your brain, methadone looks like heroin, yet it doesn’t cause as much of a high. So, if you don’t get another dose soon after the high wears off, your body doesn’t experience such bad withdrawal symptoms (for example, </span><a href=\"https://medlineplus.gov/ency/article/000949.htm\"><span style=\"font-weight: 400;\">vomiting, breathing very fast, stomach cramps and feeling anxious</span></a><span style=\"font-weight: 400;\">), which is why methadone is used to help people break their heroin dependence. This is called opioid substitution therapy (OST). </span>\r\n\r\n<span style=\"font-weight: 400;\">Because people swallow their fix rather than inject it, there’s a smaller chance of opioid users sharing needles or using an old one.</span>\r\n\r\n<span style=\"font-weight: 400;\">Moreover, OST programmes don’t just hand out methadone; they also offer other </span><a href=\"https://nida.nih.gov/research-topics/harm-reduction\"><span style=\"font-weight: 400;\">harm-reduction services</span></a><span style=\"font-weight: 400;\">, such as giving clean needles and syringes to drug users who may still be injecting, so that they won’t have to share. </span>\r\n\r\n<span style=\"font-weight: 400;\">Studies show that OST can </span><a href=\"https://www.bmj.com/content/345/bmj.e5945\"><span style=\"font-weight: 400;\">halve the risk of contracting both HIV</span></a><span style=\"font-weight: 400;\"> and the </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5621373/\"><span style=\"font-weight: 400;\">hepatitis C virus </span></a><span style=\"font-weight: 400;\"> (which leads to an infection spread through blood and which damages the liver) and help HIV-positive users to </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5036913/\"><span style=\"font-weight: 400;\">stay on antiretroviral (ARV) treatment</span></a><span style=\"font-weight: 400;\"> because people can often </span><a href=\"https://ascpjournal.biomedcentral.com/articles/10.1186/s13722-017-0084-y\"><span style=\"font-weight: 400;\">get their HIV medicine at the same place</span></a><span style=\"font-weight: 400;\"> they get methadone. </span>\r\n\r\n<span style=\"font-weight: 400;\">Andrew Scheibe, a public health specialist at the health organisation, TB HIV Care, told </span><i><span style=\"font-weight: 400;\">Bhekisisa</span></i><span style=\"font-weight: 400;\"> that they estimate there are about 400,000 heroin users in South Africa. </span>\r\n\r\n<span style=\"font-weight: 400;\">The substitute drug is currently</span><a href=\"https://www.sapc.za.org/Media/Default/Documents/STG%20hospital%20level%20adult%202019_v2.0.pdf\"><span style=\"font-weight: 400;\"> available only at district hospitals</span></a><span style=\"font-weight: 400;\"> for people with withdrawal symptoms (because they’ve stopped using altogether) – and then only for up to 10 days.</span>\r\n\r\n<span style=\"font-weight: 400;\">This means that getting methadone to help wean people off heroin gradually is not a realistic option if they rely on public healthcare. </span>\r\n\r\n<span style=\"font-weight: 400;\">With the new plan, though, the aim is to give all opioid drug users who want methadone, the medicine for free at state clinics. </span>\r\n\r\n<span style=\"font-weight: 400;\">But for a countrywide roll-out to work, the government has to do five things over the next five years, says Scheibe, who also helped to write </span><a href=\"https://www.unodc.org/documents/hiv-aids/2022/22-10821_eBook_OAT_implementing_tool.pdf\"><span style=\"font-weight: 400;\">guidelines for the United Nations Office on Drugs and Crime</span></a><span style=\"font-weight: 400;\"> on putting OST programmes in place.</span>\r\n\r\n<span style=\"font-weight: 400;\">“A programme like this wouldn’t start everywhere at once; it would have to be a stepwise process.” </span>\r\n<h4><b>Five things over the next five years</b></h4>\r\n<h4><b>Get a plan in place</b><span style=\"font-weight: 400;\"> </span></h4>\r\n<span style=\"font-weight: 400;\">A good start is to have policies that identify people who use drugs as a focus for the public health sector. This has been done by including substance users in the </span><a href=\"https://sanac.org.za/wp-content/uploads/2023/05/NSP-Document.pdf\"><span style=\"font-weight: 400;\">new HIV plan</span></a><span style=\"font-weight: 400;\"> and the </span><a href=\"https://www.health.gov.za/wp-content/uploads/2022/06/Published-Health-Sector-Drug-Master-Plan-2019-2025.pdf\"><span style=\"font-weight: 400;\">National Drug Master Plan</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">With this in place, medication can be added to the government’s </span><a href=\"https://www.sapc.za.org/Media/Default/Documents/STG%20hospital%20level%20adult%202019_v2.0.pdf\"><span style=\"font-weight: 400;\">list of essential treatments</span></a><span style=\"font-weight: 400;\"> (here’s the current list for </span><a href=\"https://knowledgehub.health.gov.za/elibrary/hospital-level-tertiary-and-quaternary-essential-medicines-list-2019-2021\"><span style=\"font-weight: 400;\">hospitals</span></a><span style=\"font-weight: 400;\"> and primary </span><a href=\"https://knowledgehub.health.gov.za/elibrary/primary-healthcare-standard-treatment-guidelines-and-essential-medicines-list-7th-edition\"><span style=\"font-weight: 400;\">healthcare facilities</span></a><span style=\"font-weight: 400;\">), which names all the medicines that are available in public health facilities, and treatment guidelines can be written up. </span>\r\n\r\n<span style=\"font-weight: 400;\">Earlier this year, the government started making plans for how a methadone programme could be rolled out for the country and getting input from experts to guide them, says Kgalabi Ngako, deputy director for the mental health and substance abuse directorate in the Health Department.</span>\r\n<h4><b>Find the money </b></h4>\r\n<span style=\"font-weight: 400;\">Methadone treatment is costly. To help someone stop using opioids completely, they should ideally be on OST for </span><a href=\"https://archives.nida.nih.gov/sites/default/files/podat-3rdEd-508.pdf\"><span style=\"font-weight: 400;\">at least 12 months</span></a><span style=\"font-weight: 400;\">, starting with a dose of </span><a href=\"http://apps.who.int/iris/bitstream/handle/10665/43948/9789241547543_eng.pdf;jsessionid=BF0C526DC386D4808734D2A146EA41F9?sequence=1\"><span style=\"font-weight: 400;\">between 10mg and 30mg per day</span></a><span style=\"font-weight: 400;\">. This is then gradually upped to a level where they don’t have withdrawal symptoms (the maintenance level). At a starting dose of, say, 20mg per day, a single pop could cost about R12.25 (if we take the </span><a href=\"https://medicineprices.org.za/#search:methadone\"><span style=\"font-weight: 400;\">lowest price at which methadone sells</span></a><span style=\"font-weight: 400;\">). A month’s supply at this price would therefore work out to almost R400 per user. </span>\r\n\r\n<span style=\"font-weight: 400;\">For a maintenance level, a dose of at least </span><a href=\"http://apps.who.int/iris/bitstream/handle/10665/43948/9789241547543_eng.pdf;jsessionid=BF0C526DC386D4808734D2A146EA41F9?sequence=1\"><span style=\"font-weight: 400;\">60mg per day is advised</span></a><span style=\"font-weight: 400;\">. A daily fix would then cost just over R36 at the lowest price, and for a month it would work out to about R1,080. </span>\r\n<blockquote>The risk of diversion exists for every Schedule 6 medication; it’s not different just because it’s methadone.</blockquote>\r\n<span style=\"font-weight: 400;\">For comparison, it costs the Health Department about </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/32004205/\"><span style=\"font-weight: 400;\">R60 for a month’s supply</span></a><span style=\"font-weight: 400;\"> of the </span><a href=\"https://www.truvada.com/\"><span style=\"font-weight: 400;\">oral HIV prevention pill</span></a><span style=\"font-weight: 400;\"> for one patient (this increases to about R90 when administration costs are included). In the private sector, it costs about </span><a href=\"https://medicineprices.org.za/#search:truvada\"><span style=\"font-weight: 400;\">R700 for a month’s supply</span></a><span style=\"font-weight: 400;\">, which works out to R58 per day.</span>\r\n<h4><b>Get enough hands on deck</b></h4>\r\n<span style=\"font-weight: 400;\">Methadone is a </span><a href=\"https://www.sahpra.org.za/wp-content/uploads/2021/11/Medicines-Act-General-Regulations-2017.pdf\"><span style=\"font-weight: 400;\">Schedule 6 medication</span></a><span style=\"font-weight: 400;\">, which means that only a doctor can prescribe it. But for a full OST programme, </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK310658/\"><span style=\"font-weight: 400;\">a whole team of health workers is needed</span></a><span style=\"font-weight: 400;\">, including nurses, social workers, pharmacists, counsellors and </span><a href=\"https://bhekisisa.org/features/2023-02-23-lets-pray-youll-be-okay-my-escape-from-a-backyard-drug-rehab/\"><span style=\"font-weight: 400;\">peer educators</span></a><span style=\"font-weight: 400;\">. And, says Scheibe, health workers would need training and support on how to prescribe methadone correctly over a treatment course and help people stay with the programme. </span>\r\n<h4><b>Set up systems to manage supply </b></h4>\r\n<span style=\"font-weight: 400;\">In an OST programme it’s possible that people who get an opioid substitute such as methadone for free </span><a href=\"https://bhekisisa.org/wp-content/uploads/2023/03/14659891.2012.734539.pdf\"><span style=\"font-weight: 400;\">might sell it to other people</span></a><span style=\"font-weight: 400;\">. Hospitals and clinics would therefore have to keep good records of who gets treatment and their progress. </span>\r\n\r\n<span style=\"font-weight: 400;\">“We already have systems in place [for monitoring how much of a Schedule 6 medicine, like methadone, has been given out], and we just have to strengthen them to make sure that they account for the stock,” Scheibe says. “The risk of diversion exists for every Schedule 6 medication; it’s not different just because it’s methadone.” </span>\r\n<h4><b>Have a network of other services too</b></h4>\r\n<span style=\"font-weight: 400;\">For OST programmes to work at community health centres, people would need to be able to dispose of needles and syringes safely. Such services are already available for patients who use injectable medicines, such as insulin, says Scheibe. </span>\r\n<blockquote>There’s no doubt about the effectiveness of methadone [to treat substance use]. It’s just a question of whether or not our primary healthcare system can feasibly and safely deliver it</blockquote>\r\n<span style=\"font-weight: 400;\">Offering HIV and hepatitis testing, handing out ARVs and making counselling part of the treatment package can also help to cut HIV infections among people who inject drugs, </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4560909/\"><span style=\"font-weight: 400;\">a study from Canada has shown</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The World Health Organization also recommends having help on hand for people who have overdosed or are dealing with withdrawal symptoms, which </span><a href=\"https://www.sapc.za.org/Media/Default/Documents/STG%20hospital%20level%20adult%202019_v2.0.pdf\"><span style=\"font-weight: 400;\">are already available at district hospitals</span></a><span style=\"font-weight: 400;\">. </span>\r\n<h4><b>Why swop rather than stop</b></h4>\r\n<a href=\"https://www.unodc.org/documents/hiv-aids/2022/22-10821_eBook_OAT_implementing_tool.pdf\"><span style=\"font-weight: 400;\">Evidence shows</span></a><span style=\"font-weight: 400;\"> that weaning people off opioids gradually by using substitutes (called maintenance) works better to help them beat the addiction than using these medicines simply to treat withdrawal symptoms after they’ve suddenly stopped using (which is called detoxification). </span>\r\n\r\n<span style=\"font-weight: 400;\">For example, in </span><a href=\"https://www.sciencedirect.com/science/article/pii/S030646031630394X?via=ihub\"><span style=\"font-weight: 400;\">a 2017 study</span></a><span style=\"font-weight: 400;\">, 199 opioid users agreed to be admitted to an abstinence programme at a Cape Town health centre that does not offer substitution therapy. The users were first referred for detoxification before entering the programme if they couldn’t manage their withdrawal symptoms. Only 23 participants completed the two-month treatment.</span>\r\n\r\n<span style=\"font-weight: 400;\">In contrast, a </span><a href=\"http://www.samj.org.za/index.php/samj/article/view/11913/8075\"><span style=\"font-weight: 400;\">pilot study in the Western Cape in 2014</span></a><span style=\"font-weight: 400;\"> showed that two out of three people who received the opioid substitute </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK459126/#:~:text=Buprenorphine%20is%20FDA%2Dapproved%20for,opioid%20(such%20as%20heroin).\"><span style=\"font-weight: 400;\">buprenorphine</span><span style=\"font-weight: 400;\"> (which is similar to methadone) completed a 12-week treatment programme</span></a><span style=\"font-weight: 400;\"> to curb their use of heroin, whereas just under half of the group stuck with the programme when they received only the normal treatment – counselling, group therapy and urine tests – to prevent relapse. </span>\r\n<h4><b>Done right, a national roll-out can work</b></h4>\r\n<span style=\"font-weight: 400;\">Mauritius is a good example of a country where </span><a href=\"https://www.hri.global/files/2018/12/10/Sub-SaharanAfrica-harm-reduction.pdf\"><span style=\"font-weight: 400;\">OST programmes funded mainly by the government</span></a><span style=\"font-weight: 400;\"> work well. </span>\r\n\r\n<span style=\"font-weight: 400;\">In 2007, this island nation had </span><a href=\"https://www.unodc.org/documents/wdr/WDR_2010/World_Drug_Report_2010_lo-res.pdf\"><span style=\"font-weight: 400;\">about 24,000 opioid drug users</span></a><span style=\"font-weight: 400;\"> – </span><a href=\"https://www.unodc.org/documents/wdr/WDR_2010/World_Drug_Report_2010_lo-res.pdf\"><span style=\"font-weight: 400;\">one of the highest prevalences </span></a><span style=\"font-weight: 400;\">in Africa. Data show that two years before, </span><a href=\"https://www.unaids.org/sites/default/files/country/documents/MUS_narrative_report_2015.pdf\"><span style=\"font-weight: 400;\">nine out of 10</span></a><span style=\"font-weight: 400;\"> Mauritians who were HIV positive injected drugs. </span>\r\n\r\n<span style=\"font-weight: 400;\">After rolling out the substitution programme to </span><a href=\"https://health.govmu.org/Documents/Main%20Page/New/NDO_MOH_FINAL_JOSE_VERSION_05July_2018_Brown.pdf\"><span style=\"font-weight: 400;\">40 government sites (including healthcare facilities and prisons)</span><span style=\"font-weight: 400;\"> i</span></a><span style=\"font-weight: 400;\">n 2014 following a small pilot project, new HIV cases among this group dropped to </span><a href=\"https://health.govmu.org/Documents/Departments-Hospitals/Departments/Documents/HIV%202020.pdf\"><span style=\"font-weight: 400;\">two in 10 by 2020</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">“There’s no doubt about the effectiveness of methadone [to treat substance use].
