"contents": "<span style=\"font-weight: 400;\">In </span><a href=\"https://yerbabuenagardens.org/explore/gardens/\"><span style=\"font-weight: 400;\">Yerba Buena Gardens</span></a><span style=\"font-weight: 400;\">, two blocks of public parks and playgrounds in San Francisco’s downtown art district, Mark Harrington rallies the crowd of about 400. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Research is under attack! What do we do?” he shouts into the microphone. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Act up, fight back,” they respond. </span>\r\n\r\n<span style=\"font-weight: 400;\">“People with Aids are under attack. What do we do,” Harrington calls out. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Act up, fight back!” </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s a familiar battle cry for Harrington who was 20 when he was diagnosed with HIV in 1985. But it is also one he never expected would be needed again. </span>\r\n\r\nhttps://bhekisisa.org/wp-content/uploads/2025/03/WhatsApp-Video-2025-03-15-at-23.18.19.mp4\r\n\r\n \r\n\r\n<span style=\"font-weight: 400;\">Harrington is one of the co-founders of the </span><a href=\"https://www.treatmentactiongroup.org/about-us/history/\"><span style=\"font-weight: 400;\">Treatment Action Group</span></a><span style=\"font-weight: 400;\">, which has its origins in the US Aids activist organisation, </span><a href=\"https://dp.la/primary-source-sets/act-up-and-the-aids-crisis\"><span style=\"font-weight: 400;\">ACT UP</span></a><span style=\"font-weight: 400;\">. He co-led the “</span><a href=\"https://www.youtube.com/watch?v=s70aCOflRgY\"><span style=\"font-weight: 400;\">seize control of the Food and Drug Administration</span></a><span style=\"font-weight: 400;\">” (the FDA, the US medicines regulator) in 1988 and the </span><a href=\"https://www.icp.org/browse/archive/objects/storm-the-nih\"><span style=\"font-weight: 400;\">“storm the National Institutes of Health</span></a><span style=\"font-weight: 400;\">” (the NIH, America’s medical research agency) demonstration in 1990, which pressured both agencies to prioritise HIV. </span>\r\n\r\n<span style=\"font-weight: 400;\">But last week, he was at a crossroads again, surrounded by people holding posters with four decades’ old slogans </span><span style=\"font-weight: 400;\">– such as “silence = death” – </span><span style=\"font-weight: 400;\">when activists took on the Reagan administration. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Let me remind you,” Harrington calls out, “in the early years people with HIV had to fight for the right to be studied.” </span>\r\n\r\n<span style=\"font-weight: 400;\">He pulls the mic closer to his mouth and raises his arm: “What the people in the White House are now [again] doing is an act of premeditated murder on 40 million people living with HIV around the world.” </span>\r\n\r\n<span style=\"font-weight: 400;\">Harrington is referring to the Trump administration’s </span><a href=\"https://bhekisisa.org/multimedia/podcasts/2025-03-06-listen-trump-slashed-hiv-projects-will-the-us-kill-the-fund-that-fed-them-too/\"><span style=\"font-weight: 400;\">shutdown of US government-funded HIV projects</span></a><span style=\"font-weight: 400;\"> in the US and other countries, </span><a href=\"https://bhekisisa.org/health-news-south-africa/2025-02-27-breaking-trump-orders-usaid-funded-hiv-organisations-in-sa-to-shut-down/\"><span style=\"font-weight: 400;\">mostly states like South Africa with high HIV infections rates</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/2025_UPDATE_Save-Our-Sciences-Media-Alert-3.9.25-1.pdf\"><span style=\"font-weight: 400;\">But this protest</span></a><span style=\"font-weight: 400;\">, organised by the </span><a href=\"https://www.sfaf.org/\"><span style=\"font-weight: 400;\">San Francisco Aids Foundation</span></a><span style=\"font-weight: 400;\"> and one of many across America, is fighting the government’s </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/Open-Letter-to-Secretary-Rubio-on-the-Dismantling-of-US-Global-AIDS-Program-1.pdf\"><span style=\"font-weight: 400;\">stripping of scientific research institutions’ grants</span></a><span style=\"font-weight: 400;\">, their political independence and their freedom. </span>\r\n\r\n<iframe id=\"doc_11337\" class=\"scribd_iframe_embed\" title=\"Open Letter to Secretary Rubio on the Dismantling of US Global AIDS Program 1\" src=\"https://www.scribd.com/embeds/840986992/content?start_page=1&view_mode=scroll&access_key=key-iFjiqA7qTmHaWmy4yEoQ\" width=\"100%\" height=\"600\" frameborder=\"0\" scrolling=\"no\" data-auto-height=\"false\" data-aspect-ratio=\"0.7729220222793488\"></iframe>\r\n\r\n<span style=\"font-weight: 400;\">“When they say what we can and can’t study, they’re imposing fascist restraints on the kind of science that we need to end this pandemic,” Harrington shouts. </span>\r\n\r\n<span style=\"font-weight: 400;\">They say sex and gender are the same thing. But that’s such bullshit. It flies in the face of so much science.” </span>\r\n\r\n<span style=\"font-weight: 400;\">A few hundred metres away, more than 3,000 of the world’s top medical scientists are waiting for the renowned epidemiologist Chris Beyrer’s plenary talk to start in the </span><a href=\"https://www.moscone.com/\"><span style=\"font-weight: 400;\">Moscone Centre</span></a><span style=\"font-weight: 400;\"> at the </span><a href=\"https://www.croiconference.org/\"><span style=\"font-weight: 400;\">32nd Conference on Retroviruses and Opportunistic Infections (Croi)</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Since 1993, groundbreaking HIV science has been presented here. </span>\r\n\r\n<a href=\"https://pubmed.ncbi.nlm.nih.gov/9287228/\"><span style=\"font-weight: 400;\">Research</span></a><span style=\"font-weight: 400;\"> that proved that a cocktail of three antiretroviral (ARVs) drugs could lower the amount of virus in someone’s body to virtually undetectable levels was revealed here in 1997, two years after the pandemic peaked in 1995 with</span><a href=\"https://www.unaids.org/en/resources/fact-sheet\"> <span style=\"font-weight: 400;\">3.3 million new infections that year</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Croi also hosted the release of </span><a href=\"https://www.croiconference.org/about/\"><span style=\"font-weight: 400;\">the first data</span> </a><span style=\"font-weight: 400;\">that created hope that there could be a cure for the virus, and the </span><a href=\"https://ctv.veeva.com/study/a-comparison-of-two-ways-to-manage-anti-hiv-treatment-the-smart-study\"><span style=\"font-weight: 400;\">game-changing research</span></a><span style=\"font-weight: 400;\"> that showed putting someone on antiretroviral therapy (ART) as soon as possible after they had been diagnosed worked better than starting treatment at a later stage. </span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-2637038\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2025/03/SILENCE-STILL-EQUALS-DEATH.jpg\" alt=\"\" width=\"960\" height=\"1280\" /> <em>Posters at a San Francisco rally against the Trump administration’s defunding of science. Missing from the conference: people working for US government agencies, such as the NIH, Centres for Disease Control and Prevention (CDC) and the US Agency for International Development (USAID), which funded much of the research presented. </span>\r\n\r\n<span style=\"font-weight: 400;\">Over the past seven weeks the Trump administration has cut travel budgets and laid off hundreds of staff, including the heads of all the agencies. </span>\r\n\r\n<span style=\"font-weight: 400;\">Although some attended online, and the conference organisers imposed no restrictions on what researchers could and couldn’t say, some US government-funded presentations were retracted; others were redacted, had their titles changed, or the names of government authors removed, to adhere to the new doublespeak of the Trump era, say delegates. </span>\r\n\r\nhttps://bhekisisa.org/wp-content/uploads/2025/03/WhatsApp-Video-2025-03-15-at-23.05.17.mp4\r\n\r\n<span style=\"font-weight: 400;\">Researchers who did make it to the venue often had to watch their language out of fear of losing more funding. </span>\r\n\r\n<span style=\"font-weight: 400;\">That meant </span><a href=\"https://www.nytimes.com/interactive/2025/03/07/us/trump-federal-agencies-websites-words-dei.html\"><span style=\"font-weight: 400;\">no words like bias, minorities, racism, equitable or sexual preferences</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">And being careful about how they framed their studies on groups of people Trump doesn’t like </span><span style=\"font-weight: 400;\">– or acknowledge</span><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Like transgender people. </span>\r\n\r\n<span style=\"font-weight: 400;\">On the night of his inauguration, on 20 January, the president </span><a href=\"https://www.whitehouse.gov/presidential-actions/2025/01/defending-women-from-gender-ideology-extremism-and-restoring-biological-truth-to-the-federal-government/\"><span style=\"font-weight: 400;\">signed an executive order</span></a><span style=\"font-weight: 400;\"> banning people’s right to identify as transgender or nonbinary individuals. There are only two genders, the executive order declared: male and female. </span>\r\n\r\n<span style=\"font-weight: 400;\">In the world of HIV, that’s a train smash. </span>\r\n\r\n<span style=\"font-weight: 400;\">Science shows that </span><a href=\"https://bhekisisa.org/multimedia/2023-08-03-watch-why-do-transgender-people-gay-and-bisexual-men-have-a-higher-chance-of-getting-hiv/\"><span style=\"font-weight: 400;\">some of these groups, particularly transgender women, have a much higher chance of contracting the virus</span></a><span style=\"font-weight: 400;\"> than the general population and therefore need </span><a href=\"https://bhekisisa.org/multimedia/2023-06-05-how-two-women-are-changing-the-way-trans-women-are-treated/\"><span style=\"font-weight: 400;\">tailor-made health services</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\nhttps://www.youtube.com/watch?v=GomX-uI_gys\r\n\r\n<span style=\"font-weight: 400;\">“What you found in some cases was someone presenting a study about men who have sex with men, that would have included, for example, people who are transgender,” explains Mitchell Warren from the New York-based advocacy organisation, Avac. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Sixty one days [before] you might have seen this study focused on gay men, other men who have sex with men, transgender women and men and nonbinary individuals. But now you’d see it saying this is for sexually active people or men who have sex with men. </span>\r\n\r\n<span style=\"font-weight: 400;\">“People are very guarded right now.” </span>\r\n\r\n<span style=\"font-weight: 400;\">But Beyrer, </span><a href=\"https://corporate.dukehealth.org/people/chris-beyrer-md-mph\"><span style=\"font-weight: 400;\">the director of the </span><span style=\"font-weight: 400;\">Duke Global Health Institute</span></a><span style=\"font-weight: 400;\">,</span><span style=\"font-weight: 400;\"> isn’t one of them. </span>\r\n\r\n<span style=\"font-weight: 400;\">On stage, he’s ready for his plenary.</span>\r\n\r\n<span style=\"font-weight: 400;\">“I want to start with a few words that define the HIV response and our field,” he begins. “Diversity. Equity. Inclusion.” </span>\r\n\r\n<span style=\"font-weight: 400;\">Trump weaponised these three words when he signed an executive order </span><a href=\"https://www.whitehouse.gov/presidential-actions/2025/01/ending-radical-and-wasteful-government-dei-programs-and-preferencing/\"><span style=\"font-weight: 400;\">to end all diversity, equity and inclusion programmes</span></a><span style=\"font-weight: 400;\"> in January. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Diversity is in our DNA. It’s who we are. We can’t not address diversity, because we’re dealing with a diverse HIV pandemic,” Beyrer proclaims. “And the fact that </span><a href=\"https://www.unaids.org/en/resources/fact-sheet\"><span style=\"font-weight: 400;\">three-quarters of all people</span></a><span style=\"font-weight: 400;\"> living with HIV on this planet access antiretroviral therapy, is the greatest achievement of equity in global health. </span>\r\n\r\n<span style=\"font-weight: 400;\">We’re not going to give it up.”</span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-2637034\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2025/03/Diversity-is-in-our-DNA.jpg\" alt=\"\" width=\"960\" height=\"558\" /> <em>‘Diversity is in our DNA. (Photo: Jirair Ratevosian)</em></p>\r\n\r\n<span style=\"font-weight: 400;\">Beyrer has felt the impact of funding cuts first-hand. His partner worked for an organisation funded by </span><a href=\"https://www.state.gov/pepfar/\"><span style=\"font-weight: 400;\">Pepfar</span></a><span style=\"font-weight: 400;\">, the US government’s Aids fund, and has been furloughed. And he’s had to roll out austerity measures at his Institute because the NIH, which</span><a href=\"https://www.nature.com/articles/d41586-025-00754-4\"><span style=\"font-weight: 400;\"> funds more than three-quarters of biomedical research</span></a><span style=\"font-weight: 400;\"> around the world, slashed the budgets grantees use to pay for overheads, known as indirect costs, from an average of 40% of a grant value to 15%, although it </span><a href=\"https://www.nature.com/articles/d41586-025-00436-1\"><span style=\"font-weight: 400;\">is facing legal challenges</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\"><strong>Watch:</strong> <a href=\"https://bhekisisa.org/multimedia/2023-06-05-how-two-women-are-changing-the-way-trans-women-are-treated/\">How two women re changing the way trans women are treated</a> </span>\r\n\r\n<span style=\"font-weight: 400;\">Beyrer isn’t backing down. </span>\r\n\r\n<span style=\"font-weight: 400;\">“The reality is that you can’t pick and choose which kinds of populations you care about and which you don’t in a pandemic,” he told Bhekisisa after his plenary. </span>\r\n\r\n<span style=\"font-weight: 400;\">“You have to deal with everybody who’s affected. And you have to deal with those populations who have disproportionate burdens. </span>\r\n\r\n<span style=\"font-weight: 400;\">You don’t have a choice because that’s where the virus is. And whatever you think about transgender women, the fact is that they’re among the groups that have the highest risk and the highest prevalence for HIV.</span>\r\n\r\n<span style=\"font-weight: 400;\">“So they have to be part of service provision.”</span>\r\n<h4><b>Will the next generation of scientists be erased? </b></h4>\r\n<span style=\"font-weight: 400;\">At Yerba Buena Gardens, </span><span style=\"font-weight: 400;\">Franco Chevalier, </span><span style=\"font-weight: 400;\">a young doctor and researcher originally from the Dominican Republic, which shares the island of </span><span style=\"font-weight: 400;\">Hispaniola with Haiti in the</span> <span style=\"font-weight: 400;\">Caribbean Sea, has taken the stage. </span>\r\n\r\n<span style=\"font-weight: 400;\">In the audience someone waves a poster saying “Fund HIV research, not cyber trucks”, referring to Trump’s chief fund cutter, Elon Musk’s </span><a href=\"https://www.kbb.com/tesla/cybertruck/\"><span style=\"font-weight: 400;\">electric Tesla trucks</span></a><span style=\"font-weight: 400;\"> with a sticker price upwards of $80,000 (R1.46-million). </span>\r\n\r\n<span style=\"font-weight: 400;\">“As an immigrant, I had to navigate a system that wasn’t always designed for people who looked like me and who had to endure the things I had to as a gay Afro-Latino,” Chevalier reveals. </span>\r\n\r\n<span style=\"font-weight: 400;\">“But I found mentors, scientists who believed in me.”</span>\r\n\r\n<span style=\"font-weight: 400;\">Today </span><a href=\"https://cfar.ucsf.edu/people/franco-chevalier\"><span style=\"font-weight: 400;\">Chevalier</span></a><span style=\"font-weight: 400;\"> is an internal medicine doctor and infectious diseases fellow at the </span><a href=\"https://www.ucsf.edu/\"><span style=\"font-weight: 400;\">University of California San Francisco</span></a><span style=\"font-weight: 400;\">, as well as a master of public health candidate at the </span><a href=\"https://www.berkeley.edu/\"><span style=\"font-weight: 400;\">University of California, Berkeley</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">But he’s also the</span><a href=\"https://www.linkedin.com/in/fcmd/\"> <span style=\"font-weight: 400;\">deputy medical director</span></a><span style=\"font-weight: 400;\"> at the San Francisco City Clinic, where he helps to prevent new HIV infections among the Latino gay community, one of the groups with the </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/cdc_156513_DS1.pdf\"><span style=\"font-weight: 400;\">highest chance of contracting the virus</span> </a><span style=\"font-weight: 400;\">in the US. </span>\r\n\r\n<span style=\"font-weight: 400;\">Chevalier looks up from his phone from which he’s reading his speech, and pauses. </span>\r\n\r\n<span style=\"font-weight: 400;\">“None of this would have been possible without the investment of the US government in research,” he warns. “Without proper funding we not only risk losing the entire next generation of scientists, which the cuts will disproportionately impact, but we run the danger of losing the resources that have kept millions alive and have prevented countless people from becoming infected in the first place.”</span>\r\n\r\n<span style=\"font-weight: 400;\">Each year, young scientists like Chevalier start the Croi conference with a workshop for new investigators and advocates; many of the more experienced scientists at the meeting have shaped at least part of their careers here. </span>\r\n\r\n<span style=\"font-weight: 400;\">This year, Warren says, the most common question from attendees was: “I’m a 30-year-old postdoctoral student and thinking about a career in HIV. Do I have a future?” </span>\r\n\r\n<span style=\"font-weight: 400;\">Almost all of the academic positions for HIV research in the US, and also many in other countries, are funded by the NIH.</span>\r\n\r\n<span style=\"font-weight: 400;\">Now almost all of them are in danger. </span>\r\n\r\n<span style=\"font-weight: 400;\">“If a young researcher is wondering if they have a future in HIV research, and don’t join this field because of that, in 10, 20, 30 years they’re not going to have created the next big discoveries,” Warren cautions. </span>\r\n\r\n<span style=\"font-weight: 400;\">“You have to create a constant cycle of new ideas, new investigators, new advocates, and that’s also being disrupted here.” </span>\r\n<h4><b>Why PrEP studies had to shut down</b><span style=\"font-weight: 400;\"> </span></h4>\r\n<span style=\"font-weight: 400;\">The consequences of research funding cuts have already been felt, explains Beyrer </span><span style=\"font-weight: 400;\">– many studies are on pause. </span>\r\n\r\n<span style=\"font-weight: 400;\">One such trial is the </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/CATALYST-Overview_FINAL_18.5.2023.pdf\"><span style=\"font-weight: 400;\">Catalyst study</span></a><span style=\"font-weight: 400;\"> – with </span><a href=\"https://wrhi.ac.za/projects/south-africa-catalyst-study-launch/\"><span style=\"font-weight: 400;\">six sites in South Africa</span></a><span style=\"font-weight: 400;\"> – testing the best ways to roll out a daily anti-HIV pill, a two-monthly HIV prevention injection and a monthly vaginal ring.</span>\r\n\r\n<span style=\"font-weight: 400;\">What is more, </span><a href=\"https://www.science.org/content/article/fear-spreads-nih-will-terminate-grants-involving-south-africa\"><span style=\"font-weight: 400;\">Science</span> <span style=\"font-weight: 400;\">magazine reports</span></a><span style=\"font-weight: 400;\"> that health researchers who work in South Africa are on red alert after hearing last week that the NIH could terminate all grants that fund work in the country.</span>\r\n\r\n<span style=\"font-weight: 400;\">HIV prevention medication, also called pre-exposure prophylaxis or PrEP, especially the </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-10-11-almost-40-of-the-worlds-anti-hiv-pill-users-live-in-sa/\"><span style=\"font-weight: 400;\">daily pill</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-07-29-sa-wants-to-buy-the-2-monthly-anti-hiv-jab-18-days-after-a-us-donation-deal/\"><span style=\"font-weight: 400;\">two-monthly jab</span></a><span style=\"font-weight: 400;\">, can lower people’s chances of getting HIV through sex to close to zero, but only if taken as prescribed.</span>\r\n\r\n<span style=\"font-weight: 400;\">And that’s the most difficult part of the equation: just because there’s a pill, jab and ring, doesn’t automatically mean people will use it. </span>\r\n\r\n<span style=\"font-weight: 400;\">So, to get HIV-negative people who have a high chance of contracting the virus to take PrEP, researchers need to know which type different groups of people prefer and from where they feel most comfortable collecting it. </span>\r\n\r\nhttps://bhekisisa.org/wp-content/uploads/2025/03/WhatsApp-Video-2025-03-16-at-00.01.08.mp4\r\n\r\n<span style=\"font-weight: 400;\">The Catalyst study, funded by Pepfar through USAID, did exactly this. </span>\r\n\r\n<span style=\"font-weight: 400;\">It was conducted across </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/CATALYST-Study-Protocol.pdf\"><span style=\"font-weight: 400;\">South Africa, Kenya, Lesotho, Uganda and Zimbabwe</span></a><span style=\"font-weight: 400;\">, but when the Trump administration put a hold on foreign aid, all of those sites were stripped of their funding. </span>\r\n\r\n<span style=\"font-weight: 400;\">That meant two Johannesburg clinics – one for sex workers and another for transgender people – which were </span><a href=\"https://wrhi.ac.za/projects/south-africa-catalyst-study-launch/\"><span style=\"font-weight: 400;\">run</span></a><span style=\"font-weight: 400;\"> by the Wits Reproductive Health Institute, which led the South African arm of the study, have been shut down. </span>\r\n\r\n<span style=\"font-weight: 400;\">The other four sites, hosted by government clinics in the Free State but funded by the US government, also closed. Although HIV services for which the government pays in the clinics are still running, </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-07-29-sa-wants-to-buy-the-2-monthly-anti-hiv-jab-18-days-after-a-us-donation-deal/\"><span style=\"font-weight: 400;\">the facilities can now only offer clients the daily pill</span></a><span style=\"font-weight: 400;\">, because the vaginal rings and two-monthly shots aren’t on the Health Department’s shopping list of essential medicines, but were, instead, sponsored by the trial. </span>\r\n\r\n<span style=\"font-weight: 400;\">“One of the reasons South Africa has done so well with HIV research is because of the extraordinary depth of scientific talent,” Beyrer says. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Now we’re worried about our international partnerships for collaborative research with countries like South Africa, and also China.”</span>\r\n\r\n<strong>Listen:</strong> <a href=\"https://bhekisisa.org/multimedia/podcasts/2025-03-06-listen-trump-slashed-hiv-projects-will-the-us-kill-the-fund-that-fed-them-too/\">Trump slashed HIV projects. Will the US kill the fund that fed them too?</a>\r\n\r\n<span style=\"font-weight: 400;\">For South Africa, Trump </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/02/Trump-EO-halting-Aid-to-SA.pdf\"><span style=\"font-weight: 400;\">signed an additional executive order</span></a><span style=\"font-weight: 400;\"> banning all foreign aid to the country, which includes money used for research projects. HIV programmes in the country mostly operate with Pepfar money and get their funds either through USAID, which is hosted by the state department, or the Centers for Disease Control and Prevention (CDC), which falls under the Department of Health and Human Services. </span>\r\n\r\n<a href=\"https://www.science.org/content/article/fear-spreads-nih-will-terminate-grants-involving-south-africa\"><span style=\"font-weight: 400;\">Science</span> <span style=\"font-weight: 400;\">magazine reports</span> </a><span style=\"font-weight: 400;\">that a</span><span style=\"font-weight: 400;\"> 12 March email on behalf of the NIH’s acting director, Matthew Memoli, asked researchers at the NIH to compile lists of South Africa-related grants. “The email instructed NIH’s institutes and centres to ‘please provide any information related to NIH investment in South Africa to [NIH’s executive secretary] by 5pm’.”</span>\r\n\r\n<span style=\"font-weight: 400;\">One of the lists, compiled by the National Institute of Allergy and Infectious Diseases, names 237 grants. Most involve HIV or tuberculosis research.</span>\r\n\r\n<i><span style=\"font-weight: 400;\">“Similar wording accompanied earlier NIH requests for information that led to the termination of dozens of grants involving transgender health, vaccine hesitancy, and other topics the administration does not support,” Science reports. </span></i>\r\n\r\n<span style=\"font-weight: 400;\">Although projects – research, treatment and prevention programmes – funded through the CDC in South Africa </span><a href=\"https://bhekisisa.org/health-news-south-africa/2025-02-13-breaking-defying-trump-some-pepfar-projects-can-now-restart-in-full-without-a-waiver/\"><span style=\"font-weight: 400;\">are still running for now</span></a><span style=\"font-weight: 400;\">, following a court order, many organisations say they have only received budgets until the end of September. </span>\r\n\r\n<span style=\"font-weight: 400;\">All projects that received their funds through USAID </span><a href=\"https://bhekisisa.org/health-news-south-africa/2025-02-27-breaking-trump-orders-usaid-funded-hiv-organisations-in-sa-to-shut-down/\"><span style=\"font-weight: 400;\">have been shut down</span></a><span style=\"font-weight: 400;\">, or are in the process of closing down. </span>\r\n\r\n<span style=\"font-weight: 400;\">HIV prevention research, Beyrer says, is unlikely to be funded by the US government in future. </span>\r\n\r\n<span style=\"font-weight: 400;\">But he’s not giving up. Beyrer has faith in court proceedings taking their course. </span>\r\n\r\n<span style=\"font-weight: 400;\">“I lost my first partner to Aids in 1991 when I was just finishing my medical training. Of a whole circle of guys in New York who were our friends, there are only two of us who survived,” he says. </span>\r\n\r\n<span style=\"font-weight: 400;\">“I’ve held on to those men and those losses my whole life. What do we do?” he shouts into the microphone. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Act up, fight back,” they respond. </span>\r\n\r\n<span style=\"font-weight: 400;\">“People with Aids are under attack. What do we do,” Harrington calls out. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Act up, fight back!” </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s a familiar battle cry for Harrington who was 20 when he was diagnosed with HIV in 1985. But it is also one he never expected would be needed again. </span>\r\n\r\nhttps://bhekisisa.org/wp-content/uploads/2025/03/WhatsApp-Video-2025-03-15-at-23.18.19.mp4\r\n\r\n \r\n\r\n<span style=\"font-weight: 400;\">Harrington is one of the co-founders of the </span><a href=\"https://www.treatmentactiongroup.org/about-us/history/\"><span style=\"font-weight: 400;\">Treatment Action Group</span></a><span style=\"font-weight: 400;\">, which has its origins in the US Aids activist organisation, </span><a href=\"https://dp.la/primary-source-sets/act-up-and-the-aids-crisis\"><span style=\"font-weight: 400;\">ACT UP</span></a><span style=\"font-weight: 400;\">. He co-led the “</span><a href=\"https://www.youtube.com/watch?v=s70aCOflRgY\"><span style=\"font-weight: 400;\">seize control of the Food and Drug Administration</span></a><span style=\"font-weight: 400;\">” (the FDA, the US medicines regulator) in 1988 and the </span><a href=\"https://www.icp.org/browse/archive/objects/storm-the-nih\"><span style=\"font-weight: 400;\">“storm the National Institutes of Health</span></a><span style=\"font-weight: 400;\">” (the NIH, America’s medical research agency) demonstration in 1990, which pressured both agencies to prioritise HIV. </span>\r\n\r\n<span style=\"font-weight: 400;\">But last week, he was at a crossroads again, surrounded by people holding posters with four decades’ old slogans </span><span style=\"font-weight: 400;\">– such as “silence = death” – </span><span style=\"font-weight: 400;\">when activists took on the Reagan administration. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Let me remind you,” Harrington calls out, “in the early years people with HIV had to fight for the right to be studied.” </span>\r\n\r\n<span style=\"font-weight: 400;\">He pulls the mic closer to his mouth and raises his arm: “What the people in the White House are now [again] doing is an act of premeditated murder on 40 million people living with HIV around the world.” </span>\r\n\r\n<span style=\"font-weight: 400;\">Harrington is referring to the Trump administration’s </span><a href=\"https://bhekisisa.org/multimedia/podcasts/2025-03-06-listen-trump-slashed-hiv-projects-will-the-us-kill-the-fund-that-fed-them-too/\"><span style=\"font-weight: 400;\">shutdown of US government-funded HIV projects</span></a><span style=\"font-weight: 400;\"> in the US and other countries, </span><a href=\"https://bhekisisa.org/health-news-south-africa/2025-02-27-breaking-trump-orders-usaid-funded-hiv-organisations-in-sa-to-shut-down/\"><span style=\"font-weight: 400;\">mostly states like South Africa with high HIV infections rates</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/2025_UPDATE_Save-Our-Sciences-Media-Alert-3.9.25-1.pdf\"><span style=\"font-weight: 400;\">But this protest</span></a><span style=\"font-weight: 400;\">, organised by the </span><a href=\"https://www.sfaf.org/\"><span style=\"font-weight: 400;\">San Francisco Aids Foundation</span></a><span style=\"font-weight: 400;\"> and one of many across America, is fighting the government’s </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/Open-Letter-to-Secretary-Rubio-on-the-Dismantling-of-US-Global-AIDS-Program-1.pdf\"><span style=\"font-weight: 400;\">stripping of scientific research institutions’ grants</span></a><span style=\"font-weight: 400;\">, their political independence and their freedom. </span>\r\n\r\n<iframe id=\"doc_11337\" class=\"scribd_iframe_embed\" title=\"Open Letter to Secretary Rubio on the Dismantling of US Global AIDS Program 1\" src=\"https://www.scribd.com/embeds/840986992/content?start_page=1&view_mode=scroll&access_key=key-iFjiqA7qTmHaWmy4yEoQ\" width=\"100%\" height=\"600\" frameborder=\"0\" scrolling=\"no\" data-auto-height=\"false\" data-aspect-ratio=\"0.7729220222793488\"></iframe>\r\n\r\n<span style=\"font-weight: 400;\">“When they say what we can and can’t study, they’re imposing fascist restraints on the kind of science that we need to end this pandemic,” Harrington shouts. </span>\r\n\r\n<span style=\"font-weight: 400;\">They say sex and gender are the same thing. But that’s such bullshit. It flies in the face of so much science.” </span>\r\n\r\n<span style=\"font-weight: 400;\">A few hundred metres away, more than 3,000 of the world’s top medical scientists are waiting for the renowned epidemiologist Chris Beyrer’s plenary talk to start in the </span><a href=\"https://www.moscone.com/\"><span style=\"font-weight: 400;\">Moscone Centre</span></a><span style=\"font-weight: 400;\"> at the </span><a href=\"https://www.croiconference.org/\"><span style=\"font-weight: 400;\">32nd Conference on Retroviruses and Opportunistic Infections (Croi)</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Since 1993, groundbreaking HIV science has been presented here. </span>\r\n\r\n<a href=\"https://pubmed.ncbi.nlm.nih.gov/9287228/\"><span style=\"font-weight: 400;\">Research</span></a><span style=\"font-weight: 400;\"> that proved that a cocktail of three antiretroviral (ARVs) drugs could lower the amount of virus in someone’s body to virtually undetectable levels was revealed here in 1997, two years after the pandemic peaked in 1995 with</span><a href=\"https://www.unaids.org/en/resources/fact-sheet\"> <span style=\"font-weight: 400;\">3.3 million new infections that year</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Croi also hosted the release of </span><a href=\"https://www.croiconference.org/about/\"><span style=\"font-weight: 400;\">the first data</span> </a><span style=\"font-weight: 400;\">that created hope that there could be a cure for the virus, and the </span><a href=\"https://ctv.veeva.com/study/a-comparison-of-two-ways-to-manage-anti-hiv-treatment-the-smart-study\"><span style=\"font-weight: 400;\">game-changing research</span></a><span style=\"font-weight: 400;\"> that showed putting someone on antiretroviral therapy (ART) as soon as possible after they had been diagnosed worked better than starting treatment at a later stage. </span>\r\n\r\n[caption id=\"attachment_2637038\" align=\"alignnone\" width=\"960\"]<img class=\"size-full wp-image-2637038\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2025/03/SILENCE-STILL-EQUALS-DEATH.jpg\" alt=\"\" width=\"960\" height=\"1280\" /> <em>Posters at a San Francisco rally against the Trump administration’s defunding of science. Missing from the conference: people working for US government agencies, such as the NIH, Centres for Disease Control and Prevention (CDC) and the US Agency for International Development (USAID), which funded much of the research presented. </span>\r\n\r\n<span style=\"font-weight: 400;\">Over the past seven weeks the Trump administration has cut travel budgets and laid off hundreds of staff, including the heads of all the agencies. </span>\r\n\r\n<span style=\"font-weight: 400;\">Although some attended online, and the conference organisers imposed no restrictions on what researchers could and couldn’t say, some US government-funded presentations were retracted; others were redacted, had their titles changed, or the names of government authors removed, to adhere to the new doublespeak of the Trump era, say delegates. </span>\r\n\r\nhttps://bhekisisa.org/wp-content/uploads/2025/03/WhatsApp-Video-2025-03-15-at-23.05.17.mp4\r\n\r\n<span style=\"font-weight: 400;\">Researchers who did make it to the venue often had to watch their language out of fear of losing more funding. </span>\r\n\r\n<span style=\"font-weight: 400;\">That meant </span><a href=\"https://www.nytimes.com/interactive/2025/03/07/us/trump-federal-agencies-websites-words-dei.html\"><span style=\"font-weight: 400;\">no words like bias, minorities, racism, equitable or sexual preferences</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">And being careful about how they framed their studies on groups of people Trump doesn’t like </span><span style=\"font-weight: 400;\">– or acknowledge</span><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Like transgender people. </span>\r\n\r\n<span style=\"font-weight: 400;\">On the night of his inauguration, on 20 January, the president </span><a href=\"https://www.whitehouse.gov/presidential-actions/2025/01/defending-women-from-gender-ideology-extremism-and-restoring-biological-truth-to-the-federal-government/\"><span style=\"font-weight: 400;\">signed an executive order</span></a><span style=\"font-weight: 400;\"> banning people’s right to identify as transgender or nonbinary individuals. There are only two genders, the executive order declared: male and female. </span>\r\n\r\n<span style=\"font-weight: 400;\">In the world of HIV, that’s a train smash. </span>\r\n\r\n<span style=\"font-weight: 400;\">Science shows that </span><a href=\"https://bhekisisa.org/multimedia/2023-08-03-watch-why-do-transgender-people-gay-and-bisexual-men-have-a-higher-chance-of-getting-hiv/\"><span style=\"font-weight: 400;\">some of these groups, particularly transgender women, have a much higher chance of contracting the virus</span></a><span style=\"font-weight: 400;\"> than the general population and therefore need </span><a href=\"https://bhekisisa.org/multimedia/2023-06-05-how-two-women-are-changing-the-way-trans-women-are-treated/\"><span style=\"font-weight: 400;\">tailor-made health services</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\nhttps://www.youtube.com/watch?v=GomX-uI_gys\r\n\r\n<span style=\"font-weight: 400;\">“What you found in some cases was someone presenting a study about men who have sex with men, that would have included, for example, people who are transgender,” explains Mitchell Warren from the New York-based advocacy organisation, Avac. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Sixty one days [before] you might have seen this study focused on gay men, other men who have sex with men, transgender women and men and nonbinary individuals. But now you’d see it saying this is for sexually active people or men who have sex with men. </span>\r\n\r\n<span style=\"font-weight: 400;\">“People are very guarded right now.” </span>\r\n\r\n<span style=\"font-weight: 400;\">But Beyrer, </span><a href=\"https://corporate.dukehealth.org/people/chris-beyrer-md-mph\"><span style=\"font-weight: 400;\">the director of the </span><span style=\"font-weight: 400;\">Duke Global Health Institute</span></a><span style=\"font-weight: 400;\">,</span><span style=\"font-weight: 400;\"> isn’t one of them. </span>\r\n\r\n<span style=\"font-weight: 400;\">On stage, he’s ready for his plenary.</span>\r\n\r\n<span style=\"font-weight: 400;\">“I want to start with a few words that define the HIV response and our field,” he begins. “Diversity. Equity. Inclusion.” </span>\r\n\r\n<span style=\"font-weight: 400;\">Trump weaponised these three words when he signed an executive order </span><a href=\"https://www.whitehouse.gov/presidential-actions/2025/01/ending-radical-and-wasteful-government-dei-programs-and-preferencing/\"><span style=\"font-weight: 400;\">to end all diversity, equity and inclusion programmes</span></a><span style=\"font-weight: 400;\"> in January. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Diversity is in our DNA. It’s who we are. We can’t not address diversity, because we’re dealing with a diverse HIV pandemic,” Beyrer proclaims. “And the fact that </span><a href=\"https://www.unaids.org/en/resources/fact-sheet\"><span style=\"font-weight: 400;\">three-quarters of all people</span></a><span style=\"font-weight: 400;\"> living with HIV on this planet access antiretroviral therapy, is the greatest achievement of equity in global health. </span>\r\n\r\n<span style=\"font-weight: 400;\">We’re not going to give it up.”</span>\r\n\r\n[caption id=\"attachment_2637034\" align=\"alignnone\" width=\"960\"]<img class=\"size-full wp-image-2637034\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2025/03/Diversity-is-in-our-DNA.jpg\" alt=\"\" width=\"960\" height=\"558\" /> <em>‘Diversity is in our DNA. His partner worked for an organisation funded by </span><a href=\"https://www.state.gov/pepfar/\"><span style=\"font-weight: 400;\">Pepfar</span></a><span style=\"font-weight: 400;\">, the US government’s Aids fund, and has been furloughed. And he’s had to roll out austerity measures at his Institute because the NIH, which</span><a href=\"https://www.nature.com/articles/d41586-025-00754-4\"><span style=\"font-weight: 400;\"> funds more than three-quarters of biomedical research</span></a><span style=\"font-weight: 400;\"> around the world, slashed the budgets grantees use to pay for overheads, known as indirect costs, from an average of 40% of a grant value to 15%, although it </span><a href=\"https://www.nature.com/articles/d41586-025-00436-1\"><span style=\"font-weight: 400;\">is facing legal challenges</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\"><strong>Watch:</strong> <a href=\"https://bhekisisa.org/multimedia/2023-06-05-how-two-women-are-changing-the-way-trans-women-are-treated/\">How two women re changing the way trans women are treated</a> </span>\r\n\r\n<span style=\"font-weight: 400;\">Beyrer isn’t backing down. </span>\r\n\r\n<span style=\"font-weight: 400;\">“The reality is that you can’t pick and choose which kinds of populations you care about and which you don’t in a pandemic,” he told Bhekisisa after his plenary. </span>\r\n\r\n<span style=\"font-weight: 400;\">“You have to deal with everybody who’s affected. And you have to deal with those populations who have disproportionate burdens. </span>\r\n\r\n<span style=\"font-weight: 400;\">You don’t have a choice because that’s where the virus is. And whatever you think about transgender women, the fact is that they’re among the groups that have the highest risk and the highest prevalence for HIV.</span>\r\n\r\n<span style=\"font-weight: 400;\">“So they have to be part of service provision.”</span>\r\n<h4><b>Will the next generation of scientists be erased? </b></h4>\r\n<span style=\"font-weight: 400;\">At Yerba Buena Gardens, </span><span style=\"font-weight: 400;\">Franco Chevalier, </span><span style=\"font-weight: 400;\">a young doctor and researcher originally from the Dominican Republic, which shares the island of </span><span style=\"font-weight: 400;\">Hispaniola with Haiti in the</span> <span style=\"font-weight: 400;\">Caribbean Sea, has taken the stage. </span>\r\n\r\n<span style=\"font-weight: 400;\">In the audience someone waves a poster saying “Fund HIV research, not cyber trucks”, referring to Trump’s chief fund cutter, Elon Musk’s </span><a href=\"https://www.kbb.com/tesla/cybertruck/\"><span style=\"font-weight: 400;\">electric Tesla trucks</span></a><span style=\"font-weight: 400;\"> with a sticker price upwards of $80,000 (R1.46-million). </span>\r\n\r\n<span style=\"font-weight: 400;\">“As an immigrant, I had to navigate a system that wasn’t always designed for people who looked like me and who had to endure the things I had to as a gay Afro-Latino,” Chevalier reveals. </span>\r\n\r\n<span style=\"font-weight: 400;\">“But I found mentors, scientists who believed in me.”</span>\r\n\r\n<span style=\"font-weight: 400;\">Today </span><a href=\"https://cfar.ucsf.edu/people/franco-chevalier\"><span style=\"font-weight: 400;\">Chevalier</span></a><span style=\"font-weight: 400;\"> is an internal medicine doctor and infectious diseases fellow at the </span><a href=\"https://www.ucsf.edu/\"><span style=\"font-weight: 400;\">University of California San Francisco</span></a><span style=\"font-weight: 400;\">, as well as a master of public health candidate at the </span><a href=\"https://www.berkeley.edu/\"><span style=\"font-weight: 400;\">University of California, Berkeley</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">But he’s also the</span><a href=\"https://www.linkedin.com/in/fcmd/\"> <span style=\"font-weight: 400;\">deputy medical director</span></a><span style=\"font-weight: 400;\"> at the San Francisco City Clinic, where he helps to prevent new HIV infections among the Latino gay community, one of the groups with the </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/cdc_156513_DS1.pdf\"><span style=\"font-weight: 400;\">highest chance of contracting the virus</span> </a><span style=\"font-weight: 400;\">in the US. </span>\r\n\r\n<span style=\"font-weight: 400;\">Chevalier looks up from his phone from which he’s reading his speech, and pauses. </span>\r\n\r\n<span style=\"font-weight: 400;\">“None of this would have been possible without the investment of the US government in research,” he warns. “Without proper funding we not only risk losing the entire next generation of scientists, which the cuts will disproportionately impact, but we run the danger of losing the resources that have kept millions alive and have prevented countless people from becoming infected in the first place.”</span>\r\n\r\n<span style=\"font-weight: 400;\">Each year, young scientists like Chevalier start the Croi conference with a workshop for new investigators and advocates; many of the more experienced scientists at the meeting have shaped at least part of their careers here. </span>\r\n\r\n<span style=\"font-weight: 400;\">This year, Warren says, the most common question from attendees was: “I’m a 30-year-old postdoctoral student and thinking about a career in HIV. Do I have a future?” </span>\r\n\r\n<span style=\"font-weight: 400;\">Almost all of the academic positions for HIV research in the US, and also many in other countries, are funded by the NIH.</span>\r\n\r\n<span style=\"font-weight: 400;\">Now almost all of them are in danger. </span>\r\n\r\n<span style=\"font-weight: 400;\">“If a young researcher is wondering if they have a future in HIV research, and don’t join this field because of that, in 10, 20, 30 years they’re not going to have created the next big discoveries,” Warren cautions. </span>\r\n\r\n<span style=\"font-weight: 400;\">“You have to create a constant cycle of new ideas, new investigators, new advocates, and that’s also being disrupted here.” </span>\r\n<h4><b>Why PrEP studies had to shut down</b><span style=\"font-weight: 400;\"> </span></h4>\r\n<span style=\"font-weight: 400;\">The consequences of research funding cuts have already been felt, explains Beyrer </span><span style=\"font-weight: 400;\">– many studies are on pause. </span>\r\n\r\n<span style=\"font-weight: 400;\">One such trial is the </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/CATALYST-Overview_FINAL_18.5.2023.pdf\"><span style=\"font-weight: 400;\">Catalyst study</span></a><span style=\"font-weight: 400;\"> – with </span><a href=\"https://wrhi.ac.za/projects/south-africa-catalyst-study-launch/\"><span style=\"font-weight: 400;\">six sites in South Africa</span></a><span style=\"font-weight: 400;\"> – testing the best ways to roll out a daily anti-HIV pill, a two-monthly HIV prevention injection and a monthly vaginal ring.</span>\r\n\r\n<span style=\"font-weight: 400;\">What is more, </span><a href=\"https://www.science.org/content/article/fear-spreads-nih-will-terminate-grants-involving-south-africa\"><span style=\"font-weight: 400;\">Science</span> <span style=\"font-weight: 400;\">magazine reports</span></a><span style=\"font-weight: 400;\"> that health researchers who work in South Africa are on red alert after hearing last week that the NIH could terminate all grants that fund work in the country.</span>\r\n\r\n<span style=\"font-weight: 400;\">HIV prevention medication, also called pre-exposure prophylaxis or PrEP, especially the </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-10-11-almost-40-of-the-worlds-anti-hiv-pill-users-live-in-sa/\"><span style=\"font-weight: 400;\">daily pill</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-07-29-sa-wants-to-buy-the-2-monthly-anti-hiv-jab-18-days-after-a-us-donation-deal/\"><span style=\"font-weight: 400;\">two-monthly jab</span></a><span style=\"font-weight: 400;\">, can lower people’s chances of getting HIV through sex to close to zero, but only if taken as prescribed.</span>\r\n\r\n<span style=\"font-weight: 400;\">And that’s the most difficult part of the equation: just because there’s a pill, jab and ring, doesn’t automatically mean people will use it. </span>\r\n\r\n<span style=\"font-weight: 400;\">So, to get HIV-negative people who have a high chance of contracting the virus to take PrEP, researchers need to know which type different groups of people prefer and from where they feel most comfortable collecting it. </span>\r\n\r\nhttps://bhekisisa.org/wp-content/uploads/2025/03/WhatsApp-Video-2025-03-16-at-00.01.08.mp4\r\n\r\n<span style=\"font-weight: 400;\">The Catalyst study, funded by Pepfar through USAID, did exactly this. </span>\r\n\r\n<span style=\"font-weight: 400;\">It was conducted across </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/03/CATALYST-Study-Protocol.pdf\"><span style=\"font-weight: 400;\">South Africa, Kenya, Lesotho, Uganda and Zimbabwe</span></a><span style=\"font-weight: 400;\">, but when the Trump administration put a hold on foreign aid, all of those sites were stripped of their funding. </span>\r\n\r\n<span style=\"font-weight: 400;\">That meant two Johannesburg clinics – one for sex workers and another for transgender people – which were </span><a href=\"https://wrhi.ac.za/projects/south-africa-catalyst-study-launch/\"><span style=\"font-weight: 400;\">run</span></a><span style=\"font-weight: 400;\"> by the Wits Reproductive Health Institute, which led the South African arm of the study, have been shut down. </span>\r\n\r\n<span style=\"font-weight: 400;\">The other four sites, hosted by government clinics in the Free State but funded by the US government, also closed. Although HIV services for which the government pays in the clinics are still running, </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-07-29-sa-wants-to-buy-the-2-monthly-anti-hiv-jab-18-days-after-a-us-donation-deal/\"><span style=\"font-weight: 400;\">the facilities can now only offer clients the daily pill</span></a><span style=\"font-weight: 400;\">, because the vaginal rings and two-monthly shots aren’t on the Health Department’s shopping list of essential medicines, but were, instead, sponsored by the trial. </span>\r\n\r\n<span style=\"font-weight: 400;\">“One of the reasons South Africa has done so well with HIV research is because of the extraordinary depth of scientific talent,” Beyrer says. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Now we’re worried about our international partnerships for collaborative research with countries like South Africa, and also China.”</span>\r\n\r\n<strong>Listen:</strong> <a href=\"https://bhekisisa.org/multimedia/podcasts/2025-03-06-listen-trump-slashed-hiv-projects-will-the-us-kill-the-fund-that-fed-them-too/\">Trump slashed HIV projects. Will the US kill the fund that fed them too?</a>\r\n\r\n<span style=\"font-weight: 400;\">For South Africa, Trump </span><a href=\"https://bhekisisa.org/wp-content/uploads/2025/02/Trump-EO-halting-Aid-to-SA.pdf\"><span style=\"font-weight: 400;\">signed an additional executive order</span></a><span style=\"font-weight: 400;\"> banning all foreign aid to the country, which includes money used for research projects. HIV programmes in the country mostly operate with Pepfar money and get their funds either through USAID, which is hosted by the state department, or the Centers for Disease Control and Prevention (CDC), which falls under the Department of Health and Human Services. </span>\r\n\r\n<a href=\"https://www.science.org/content/article/fear-spreads-nih-will-terminate-grants-involving-south-africa\"><span style=\"font-weight: 400;\">Science</span> <span style=\"font-weight: 400;\">magazine reports</span> </a><span style=\"font-weight: 400;\">that a</span><span style=\"font-weight: 400;\"> 12 March email on behalf of the NIH’s acting director, Matthew Memoli, asked researchers at the NIH to compile lists of South Africa-related grants. “The email instructed NIH’s institutes and centres to ‘please provide any information related to NIH investment in South Africa to [NIH’s executive secretary] by 5pm’.”</span>\r\n\r\n<span style=\"font-weight: 400;\">One of the lists, compiled by the National Institute of Allergy and Infectious Diseases, names 237 grants. Most involve HIV or tuberculosis research.</span>\r\n\r\n<i><span style=\"font-weight: 400;\">“Similar wording accompanied earlier NIH requests for information that led to the termination of dozens of grants involving transgender health, vaccine hesitancy, and other topics the administration does not support,” Science reports. </span></i>\r\n\r\n<span style=\"font-weight: 400;\">Although projects – research, treatment and prevention programmes – funded through the CDC in South Africa </span><a href=\"https://bhekisisa.org/health-news-south-africa/2025-02-13-breaking-defying-trump-some-pepfar-projects-can-now-restart-in-full-without-a-waiver/\"><span style=\"font-weight: 400;\">are still running for now</span></a><span style=\"font-weight: 400;\">, following a court order, many organisations say they have only received budgets until the end of September. </span>\r\n\r\n<span style=\"font-weight: 400;\">All projects that received their funds through USAID </span><a href=\"https://bhekisisa.org/health-news-south-africa/2025-02-27-breaking-trump-orders-usaid-funded-hiv-organisations-in-sa-to-shut-down/\"><span style=\"font-weight: 400;\">have been shut down</span></a><span style=\"font-weight: 400;\">, or are in the process of closing down. </span>\r\n\r\n<span style=\"font-weight: 400;\">HIV prevention research, Beyrer says, is unlikely to be funded by the US government in future. </span>\r\n\r\n<span style=\"font-weight: 400;\">But he’s not giving up. Beyrer has faith in court proceedings taking their course. </span>\r\n\r\n<span style=\"font-weight: 400;\">“I lost my first partner to Aids in 1991 when I was just finishing my medical training. Of a whole circle of guys in New York who were our friends, there are only two of us who survived,” he says. </span>\r\n\r\n<span style=\"font-weight: 400;\">“I’ve held on to those men and those losses my whole life.
