"contents": "<span style=\"font-weight: 400;\">Mays Chemist is a bright, bustling pharmacy on Main Road in Melville, Johannesburg. Since July 2023, a small revolution has been taking place there – one that could make a real difference to HIV prevention in South Africa. </span>\r\n\r\n<span style=\"font-weight: 400;\">With the support of pharmacists and an in-house nurse, 210 people have started getting </span><a href=\"https://www.hiv.gov/hiv-basics/hiv-prevention/using-hiv-medication-to-reduce-risk/pre-exposure-prophylaxis\"><span style=\"font-weight: 400;\">PrEP (pre-exposure prophylaxis)</span></a><span style=\"font-weight: 400;\"> here, which is medication that can protect someone from contracting HIV. PrEP has been </span><a href=\"https://sahivsoc.org/Files/PREP%2520and%2520TT%2520Policy%2520-%2520Final%2520Draft%2520-%25205%2520May%25202016%2520(HIV%2520news).pdf\"><span style=\"font-weight: 400;\">available in South Africa since 2016</span></a><span style=\"font-weight: 400;\">, and </span><a href=\"https://bhekisisa.org/health-news-south-africa/2023-10-25-over-a-million-sas-have-used-the-hiv-prevention-pill/\"><span style=\"font-weight: 400;\">more than a million people</span></a><span style=\"font-weight: 400;\"> have already started taking it to help protect them against HIV infection. </span>\r\n\r\n<span style=\"font-weight: 400;\">So why are the new clients getting their medication from Mays so important? </span>\r\n\r\n<span style=\"font-weight: 400;\">Because they’re taking part in a project that’s looking at whether letting people obtain HIV prevention medicines from healthcare facilities that are part of their everyday routines, such as pharmacies that are near to where they work, study or shop, can work. (At the moment, someone has to go to a government clinic or hospital to get PrEP.) </span>\r\n\r\n<span style=\"font-weight: 400;\">So far, the project has proven popular – more than 1,200 people across 10 pharmacies have started PreP. </span>\r\n\r\n<span style=\"font-weight: 400;\">The medicine is a </span><a href=\"https://www.sahpra.org.za/wp-content/uploads/2022/05/SAHPGL-CEM-NS-02_v3-Guideline_Scheduling-of-Substances-and-Medicines.pdf\"><span style=\"font-weight: 400;\">schedule 4 drug</span></a><span style=\"font-weight: 400;\"> under the </span><a href=\"https://www.gov.za/sites/default/files/gcis_document/201505/act-101-1965.pdf\"><span style=\"font-weight: 400;\">Medicines and Related Substances Act</span></a><span style=\"font-weight: 400;\">, so a private pharmacy can only give it to someone who has a prescription from a doctor or a specially licensed nurse who works at a state clinic or hospital. </span>\r\n\r\n<span style=\"font-weight: 400;\">We think that also allowing pharmacists and nurses working at private facilities to prescribe PrEP and other anti-HIV drugs, which are also Schedule 4 medicines, is more than an urgent medical need, it is a human right. </span>\r\n<h4><b>Take your pick, then tell your pharmacist</b></h4>\r\n<span style=\"font-weight: 400;\">About </span><a href=\"https://www.unaids.org/en/regionscountries/countries/southafrica\"><span style=\"font-weight: 400;\">90,000 women and 50,000 men</span></a><span style=\"font-weight: 400;\"> were infected with HIV in South Africa in 2023 – on top of the </span><a href=\"https://hsrc.ac.za/press-releases/hsc/new-hiv-survey-highlights-progress-and-ongoing-disparities-in-south-africas-hiv-epidemic\"><span style=\"font-weight: 400;\">7.8 million</span></a><span style=\"font-weight: 400;\"> who already have HIV. We have to prevent new infections with every tool we have, and to do this we have to make it as easy as possible for people to get anti-HIV drugs. </span>\r\n\r\n<span style=\"font-weight: 400;\">But more than that, we have to give people a choice about how they want to take their medication. </span>\r\n\r\n<span style=\"font-weight: 400;\">Until now, PrEP has been available only as a daily pill. It contains the two antiretrovirals emtricitabine and tenofovir (drugs that stop the virus from multiplying in your body). It works so well that it lowers your chance of getting HIV through sex to almost zero – but for that level of protection you have to take it diligently every day.</span>\r\n\r\n<span style=\"font-weight: 400;\">In December, though, an injectable form of the drug cabotegravir (</span><a href=\"https://iris.who.int/bitstream/handle/10665/360869/9789240054097-eng.pdf\"><span style=\"font-weight: 400;\">CAB-LA</span></a><span style=\"font-weight: 400;\">) was </span><a href=\"https://www.sahpra.org.za/press-releases/sahpra-registers-new-long-acting-hiv-pre-exposure-prophylaxis/\"><span style=\"font-weight: 400;\">approved as PrEP in South Africa</span></a><span style=\"font-weight: 400;\">. The first two injections are given four weeks apart and then one every two months, which makes it convenient to use because you don’t have to remember to take a pill every day. </span><a href=\"https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(22)00538-4/fulltext\"><span style=\"font-weight: 400;\">Studies have shown</span></a><span style=\"font-weight: 400;\"> that taking the injection virtually wipes out someone’s chance of getting HIV through sex. </span>\r\n\r\n<span style=\"font-weight: 400;\">CAB-LA will be available from some state health facilities </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-07-22-breaking-sa-has-taken-up-us-donations-of-cab-la-and-will-roll-out-the-anti-hiv-jab-before-the-end-of-the-year/\"><span style=\"font-weight: 400;\">before the end of the year</span></a><span style=\"font-weight: 400;\">, with the Health Department also </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-07-29-sa-wants-to-buy-the-2-monthly-anti-hiv-jab-18-days-after-a-us-donation-deal/\"><span style=\"font-weight: 400;\">looking to buy more</span></a><span style=\"font-weight: 400;\">, if the price is right,</span> <span style=\"font-weight: 400;\">Bhekisisa reported as part of its coverage of the </span><a href=\"https://www.iasociety.org/conferences/aids2024\"><span style=\"font-weight: 400;\">International Aids Conference</span></a><span style=\"font-weight: 400;\"> in July.</span>\r\n\r\n<span style=\"font-weight: 400;\">Our project also offers CAB-LA as a PrEP option in a substudy called Axis, which is run at some facilities in Gauteng, under the umbrella of our bigger PrEP project. We want to see whether people prefer to take the daily tablet or the two-monthly injection, and also want to know what people think of the injection and how practical a PrEP option it would be in real life for people in South Africa. </span>\r\n\r\n<span style=\"font-weight: 400;\">Since we started the Axis study in March, 143 out of the 200 people enrolled chose CAB-LA over the daily PrEP pill, and 108 came back for their first follow-up shot a month later. </span>\r\n\r\n<span style=\"font-weight: 400;\">In in-depth interviews, people told us they prefer the long-acting injectable form of PrEP over the daily pill because it’s convenient, easy to take, more attractive than having to swallow a big pill, and they know that they’re protected against infection. </span>\r\n\r\n<span style=\"font-weight: 400;\">But the prevention pill is still useful to people who are scared of needles and those who are used to taking tablets every day. </span>\r\n<h4><b>Why the focus on private pharmacies?</b><span style=\"font-weight: 400;\"> </span></h4>\r\n<span style=\"font-weight: 400;\">Participants told us they like private pharmacies because they’re conveniently located at malls and near busy taxi ranks, are open on weekends and the service is fast and efficient. Moreover, they trusted the professionalism of the staff and the confidentiality of their relationship, and they faced less scrutiny from other customers. </span>\r\n\r\n<span style=\"font-weight: 400;\">In our project, people who want to start PrEP speak to a pharmacist, who refers them to an in-house nurse for advice on things like contraception and lowering their HIV risk, and to do tests for HIV and other sexually transmitted infections, </span><a href=\"https://www.mayoclinic.org/diseases-conditions/hepatitis-b/symptoms-causes/syc-20366802\"><span style=\"font-weight: 400;\">hepatitis B</span></a><span style=\"font-weight: 400;\"> and kidney function, as set out in the </span><a href=\"https://knowledgehub.health.gov.za/system/files/elibdownloads/2022-08/PrEP%2520Guidelines%2520Update%252012%2520%2520Nov%2520%25202021%2520Final.pdf\"><span style=\"font-weight: 400;\">Health Department’s guidelines</span></a><span style=\"font-weight: 400;\">. Once all that is done, people have a phone conversation with a doctor, who then writes the prescription.</span>\r\n\r\n<span style=\"font-weight: 400;\">If pharmacists and in-house nurses could issue the prescriptions themselves, the process could be cheaper and faster. While this is a change from the way things are done now, it’s not a radical one. </span>\r\n\r\n<span style=\"font-weight: 400;\">In South Africa, pharmacists are already part of the process to get people started on treatment for a health condition, give advice on using medicines, check how well someone sticks with their therapy and follow up with clients to make sure they know what to do in case of a missed dose or how to deal with possible side-effects. They also dispense emergency contraceptives without prescriptions. </span>\r\n\r\n<span style=\"font-weight: 400;\">The </span><a href=\"https://pharmcouncil.co.za/\"><span style=\"font-weight: 400;\">South African Pharmacy Council</span></a><span style=\"font-weight: 400;\">, the regulatory body to which pharmacists belong and which audits practitioners’ service level, backs the </span><a href=\"https://sahivsoc.org/Subheader/Index/pimart-online-course\"><span style=\"font-weight: 400;\">Pharmacy Initiated Management of Antiretroviral Therapy (Pimart) course</span></a><span style=\"font-weight: 400;\">. This programme, which has been part of pharmacy practice since August 2021, trains pharmacists and in-house nurses to prescribe ARVs, HIV prevention medicines (whether taken before or after sex) and tuberculosis treatment. </span>\r\n\r\n<span style=\"font-weight: 400;\">But Pimart is </span><a href=\"https://sapc.pharmaciae.org.za/pimart/\"><span style=\"font-weight: 400;\">on hold for now</span></a><span style=\"font-weight: 400;\"> after the Independent Practitioner Association Foundation, a network of general practitioners and dentists, took the pharmacy council to court to stop the programme from running. Even though the North Gauteng High Court in Pretoria </span><a href=\"https://bhekisisa.org/health-news-south-africa/2023-08-23-why-not-enough-people-with-hiv-are-on-treatment-and-how-a-new-court-ruling-could-help/\"><span style=\"font-weight: 400;\">ruled in favour of the Council</span></a><span style=\"font-weight: 400;\"> in August 2023, the network of doctors and dentists was </span><a href=\"https://sapc.pharmaciae.org.za/pimart-update-court-ruling-in-favour-of-pimart-appealed/\"><span style=\"font-weight: 400;\">allowed to appeal</span></a><span style=\"font-weight: 400;\"> and a new court date for that is yet to be set. </span>\r\n\r\n<span style=\"font-weight: 400;\">Dara Vucevic, a pharmacist at Mays, completed the Pimart training, but because the programme has been stalled she hasn’t received a certificate and so isn’t allowed to add the tasks to her day-to-day work. </span>\r\n\r\n<span style=\"font-weight: 400;\">But, she says, with her four-year pharmacy degree, 28 years’ experience and this extra course under her belt, she understands what clients need and that she could put that knowledge and skills to good use when prescribing HIV prevention medicine. And because she’s able to build a relationship with clients, she can guide them through starting a new treatment. They can pop in and ask questions, which is often not possible in busy clinics, and she knows immediately if they don’t arrive to pick up a refill.</span>\r\n\r\n<span style=\"font-weight: 400;\">Excluding the close to </span><a href=\"https://www.frontiersin.org/journals/reproductive-health/articles/10.3389/frph.2023.1173576/full\"><span style=\"font-weight: 400;\">3,600 private pharmacies</span></a><span style=\"font-weight: 400;\"> in South Africa from delivering HIV prevention services will be a wasted chance to halt new infections in the country. </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">Angela Tembo is a social scientist with extensive experience in implementation research at Ezintsha.
