"contents": "<span style=\"font-weight: 400;\">“The best thing to do is to get as many people vaccinated as quickly as you possibly can,” said infectious diseases expert Anthony Fauci when </span><i><span style=\"font-weight: 400;\">Bhekisisa</span></i><span style=\"font-weight: 400;\"> asked him about the </span><a href=\"https://bhekisisa.org/article/2021-08-16-exclusive-every-covid-question-youve-wanted-to-ask-dr-anthony-fauci/\"><span style=\"font-weight: 400;\">goal of herd immunity</span></a><span style=\"font-weight: 400;\"> — when a large enough portion of a population acquires immunity against a disease, either from prior infection or from vaccination, to break the chain of transmission. </span>\r\n\r\n<span style=\"font-weight: 400;\">Early in Covid vaccine trials, surveys showed that </span><a href=\"https://www.nature.com/articles/s41591-020-1124-9\"><span style=\"font-weight: 400;\">more than two thirds of adults</span></a><span style=\"font-weight: 400;\"> globally said they would be willing to get on board when a vaccine became available. This was promising, but willingness to get vaccinated doesn’t necessarily translate into uptake. Also in South Africa, </span><a href=\"https://cramsurvey.org/wp-content/uploads/2021/07/2.-Burger-R.-Maughan-Brown-M.-Kohler-T.-English-R.-_-Tameris-M.-2021-Increased-openness-to-accepting-a-COVID-19-vaccine-is-a-shot-in-the-arm-for-South-Africa-Evidence-from-the-NIDS-CRAM-Wave-5-Survey.pdf\"><span style=\"font-weight: 400;\">at least one poll showed similar results</span></a><span style=\"font-weight: 400;\">, yet so far </span><a href=\"https://sacoronavirus.co.za/latest-vaccine-statistics/\"><span style=\"font-weight: 400;\">just more than 32% have followed through with getting at least one vaccine dose</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">There is some good news, though.</span>\r\n\r\n<p><img loading=\"lazy\" class=\"wp-image-1057681 size-full\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2021/10/MC-Unvaxed_1.jpg\" alt=\"vaccine messaging\" width=\"2133\" height=\"1153\" /> To persuade people to get vaccinated, messages have to be tailored to the intended audience. (Photo: genengnews.com/ Andriy Onufriyenko / Getty Images / Wikipedia)</p>\r\n\r\n<span style=\"font-weight: 400;\">According to a report in the </span><a href=\"https://www.nejm.org/doi/full/10.1056/NEJMp2106137\"><i><span style=\"font-weight: 400;\">New England Medical Journal</span></i></a><span style=\"font-weight: 400;\"> many people described as “vaccine hesitant” are in fact not outright resistant to getting the jab, but simply waiting for the right nudge. Of the almost 140,000 people surveyed, 40% had already been vaccinated and 11% were hardline refusers, who said they were very unlikely to get the jab. This means close to half of the sample in the study were sitting on the fence — “vaccine ready” and waiting. This study was not conducted in South Africa.</span>\r\n\r\n<span style=\"font-weight: 400;\">Here are six things to consider to help turn ready and steady into go: </span>\r\n\r\n<b>Know who you’re talking to</b>\r\n\r\n<span style=\"font-weight: 400;\">To persuade people to get vaccinated, messages have to be tailored to the intended audience.</span>\r\n\r\n<span style=\"font-weight: 400;\">For example, a study in the UK showed that people who are vaccine-ready and actively looking to get the shot may be receptive to messages about the </span><a href=\"https://www.thelancet.com/journals/lanpub/article/PIIS2468-2667(21)00096-7/fulltext\"><span style=\"font-weight: 400;\">public health benefits of being vaccinated</span></a><span style=\"font-weight: 400;\"> — meaning they want to get vaccinated to protect themselves and others. In contrast, people who are hesitant appear to be less interested in the greater good and respond better to learning what’s in it for them.</span>\r\n\r\n<span style=\"font-weight: 400;\">A 2021 study from the US had a similar result: messages focused on personal benefits had the </span><a href=\"https://www.pnas.org/content/118/32/e2108225118\"><span style=\"font-weight: 400;\">greatest impact in convincing people to get vaccinated</span></a><span style=\"font-weight: 400;\">. Although we might wish for a society that prioritises the wellbeing of others, it might serve us well for now to be somewhat less ideological and focus instead on individual benefits — for the greater good.</span>\r\n\r\n<b>Get the word out — quickly</b>\r\n\r\n<span style=\"font-weight: 400;\">Strike while the iron is hot.</span>\r\n\r\n<span style=\"font-weight: 400;\">A </span><a href=\"https://www.nature.com/articles/s41586-021-03843-2\"><span style=\"font-weight: 400;\">study</span></a><span style=\"font-weight: 400;\"> published in the journal </span><i><span style=\"font-weight: 400;\">Nature </span></i><span style=\"font-weight: 400;\">in August found that the timing of vaccine messages can boost appointments and subsequent vaccinations. In this study, participants in the US were randomly assigned to one of two groups and were then sent a text message inviting them to schedule their vaccine appointment shortly after they became eligible to register.</span>\r\n\r\n<span style=\"font-weight: 400;\">One group received the message one day after they became eligible for getting their shot, while the other group received the message eight days after they could enrol. Both messages included a link to a website allowing the recipients to book vaccine appointments. The earlier prompt nudged one-and-a-half times more people to make appointments than the later one. </span>\r\n\r\n<span style=\"font-weight: 400;\">Simply making the booking also boosted follow-through. Nearly 90% of participants who made a booking after receiving the nudge kept their appointments — and almost everyone who received their first dose also booked their second.</span>\r\n\r\n<span style=\"font-weight: 400;\">This points to the importance of encouraging people to schedule their first shot. A similar trend was seen in an earlier </span><a href=\"https://www.pnas.org/content/118/20/e2101165118\"><span style=\"font-weight: 400;\">study designed to boost uptake of the flu vaccine</span></a><span style=\"font-weight: 400;\">. Vaccination reminders sent to people before their regular, unrelated primary healthcare visits increased vaccination rates by 5%. </span>\r\n\r\n<b>Allay people’s fears — first with empathy, then with evidence</b>\r\n\r\n<span style=\"font-weight: 400;\">The </span><a href=\"https://jamanetwork.com/journals/jama/fullarticle/2766651\"><span style=\"font-weight: 400;\">flip side of the unprecedented speed with which Covid-19 vaccines were rolled out</span></a><span style=\"font-weight: 400;\"> is that it has contributed to questions being raised about their safety.</span>\r\n\r\n<span style=\"font-weight: 400;\">Studies on people’s attitudes towards vaccines, from countries as diverse as </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0250555\"><span style=\"font-weight: 400;\">Jordan</span></a><span style=\"font-weight: 400;\"> and the </span><a href=\"https://jamanetwork.com/journals/jama/fullarticle/2780519\"><span style=\"font-weight: 400;\">US</span></a><span style=\"font-weight: 400;\">, show that fears about ingredients, safety and what many perceive as rushed approval processes deter people from getting vaccinated.</span>\r\n\r\n<a href=\"https://ebm.bmj.com/content/early/2021/08/08/bmjebm-2021-111735\"><span style=\"font-weight: 400;\">Inadequate data and a general lack of transparency</span></a><span style=\"font-weight: 400;\"> about vaccine trials, coupled with murky procurement agreements between governments and pharmaceutical companies, further fuel reluctance. </span>\r\n\r\n<span style=\"font-weight: 400;\">Although there is </span><a href=\"https://www.nature.com/articles/d41586-021-01505-x\"><span style=\"font-weight: 400;\">overwhelming evidence that the vaccines are safe</span></a><span style=\"font-weight: 400;\"> and effective, it is important to </span><a href=\"https://bhekisisa.org/article/2021-09-06-when-empathy-meets-evidence-addressing-covid-vaccine-hesitancy-in-sa/\"><span style=\"font-weight: 400;\">acknowledge that people’s fears are valid</span></a><span style=\"font-weight: 400;\">. Showing empathy when talking to people who are reluctant or uncertain may help to make them more receptive to balanced, evidence-based messages. </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s equally important to be honest about issues such as the common occurrence of minor side-effects — and to use evidence to contextualise the extremely rare risk of severe side-effects.</span>\r\n\r\n<span style=\"font-weight: 400;\">For instance, the scaremongering media coverage about blood clots developing after getting the Johnson & Johnson (J&J) or AstraZeneca vaccine created the impression that clots occur far more often than they do.</span>\r\n\r\n<span style=\"font-weight: 400;\">Investigations by international medicine regulators showed that the risk of developing a blood clot after getting the jab was very low — less than one in a million for </span><a href=\"https://www.fda.gov/news-events/press-announcements/fda-and-cdc-lift-recommended-pause-johnson-johnson-janssen-covid-19-vaccine-use-following-thorough\"><span style=\"font-weight: 400;\">J&J</span></a><span style=\"font-weight: 400;\"> and between</span><a href=\"https://www.health.gov.au/initiatives-and-programs/covid-19-vaccines/is-it-true/is-it-true-does-the-vaxzevria-astrazeneca-covid-19-vaccine-cause-blood-clots\"><span style=\"font-weight: 400;\"> four and six in a million for AstraZeneca.</span></a><span style=\"font-weight: 400;\"> Typically, a </span><a href=\"https://bhekisisa.org/article/2021-05-07-how-rare-is-very-rare-your-questions-about-blood-clots-answered/\"><span style=\"font-weight: 400;\">very rare adverse event</span></a><span style=\"font-weight: 400;\"> after taking a medicine is defined as affecting fewer than one in 10,000 people, but in this case the chances of developing blood clots were deemed really slim, and the vaccines were cleared for use after the temporary pause.</span>\r\n\r\n<b>Don’t be afraid to drop a name or two</b>\r\n\r\n<span style=\"font-weight: 400;\">If Elvis could do it, so can you.</span>\r\n\r\n<p><img loading=\"lazy\" class=\"wp-image-1057682 size-full\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2021/10/MC-Unvaxed_2.jpg\" alt=\"elvis polio shot\" width=\"1004\" height=\"609\" /> In 1956, the then rising rock ’n’ roll star Elvis Presley appeared on public television, getting his polio shot. The Elvis effect seemed to have helped to spur </span><a href=\"https://www.tandfonline.com/doi/full/10.1080/14780038.2016.1145393\"><span style=\"font-weight: 400;\">US teenagers to launch campaigns to recruit their friends</span></a><span style=\"font-weight: 400;\">. Vaccinations surged, and by the next year active cases of polio had </span><a href=\"https://ourworldindata.org/polio#the-vaccine-against-polio\"><span style=\"font-weight: 400;\">dropped from 15,140 to 5,485</span></a><span style=\"font-weight: 400;\">. Five years later, by 1962, </span><a href=\"https://ourworldindata.org/polio#the-vaccine-against-polio\"><span style=\"font-weight: 400;\">fewer than 1,000 cases</span></a><span style=\"font-weight: 400;\"> were reported. </span>\r\n\r\n<span style=\"font-weight: 400;\">Studies about social influence on </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/31259597/\"><span style=\"font-weight: 400;\">boosting the uptake of flu vaccines</span></a><span style=\"font-weight: 400;\"> suggest that talking about other people getting their shots might help to improve the uptake for Covid-19 vaccines, too. It need not only be social influencers– even examples from </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7834519/\"><span style=\"font-weight: 400;\">within a peer group</span></a><span style=\"font-weight: 400;\"> can help to nudge people to get on board.</span>\r\n\r\n<b>Don’t focus on hardline refusers, try to convince those sitting on the fence </b>\r\n\r\n<span style=\"font-weight: 400;\">A small percentage of people say they will never get the jab — and studies suggest that chances are slim that they will change their minds. In the US, they number about </span><a href=\"https://www.kff.org/coronavirus-covid-19/dashboard/kff-covid-19-vaccine-monitor-dashboard/#intentions\"><span style=\"font-weight: 400;\">one in eight people</span></a><span style=\"font-weight: 400;\">, in South Africa, it’s about </span><a href=\"https://cramsurvey.org/wp-content/uploads/2021/07/2.-Burger-R.-Maughan-Brown-M.-Kohler-T.-English-R.-_-Tameris-M.-2021-Increased-openness-to-accepting-a-COVID-19-vaccine-is-a-shot-in-the-arm-for-South-Africa-Evidence-from-the-NIDS-CRAM-Wave-5-Survey.pdf\"><span style=\"font-weight: 400;\">one in 15</span></a><span style=\"font-weight: 400;\"> (although the proportion may </span><a href=\"https://cramsurvey.org/wp-content/uploads/2021/07/2.-Burger-R.-Maughan-Brown-M.-Kohler-T.-English-R.-_-Tameris-M.-2021-Increased-openness-to-accepting-a-COVID-19-vaccine-is-a-shot-in-the-arm-for-South-Africa-Evidence-from-the-NIDS-CRAM-Wave-5-Survey.pdf\"><span style=\"font-weight: 400;\">differ in pockets of the population</span></a><span style=\"font-weight: 400;\">).</span>\r\n\r\n<span style=\"font-weight: 400;\">The uptake among vaccine-ready people can change over time, but hardline anti-vaxxers almost never change their minds. So trying to persuade committed anti-vaxxers is likely to be a waste of time and money; it’s more effective to focus on people who are vaccine-ready and receptive to new information. </span>\r\n\r\n<b>Understand people’s realities</b>\r\n\r\n<span style=\"font-weight: 400;\">Historically, vaccination campaigns focused on busting myths and providing evidence-based information about vaccine safety and benefits. Yet, opposition to Covid-19 vaccines (</span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0248892\"><span style=\"font-weight: 400;\">and non-pharmaceutical interventions</span></a><span style=\"font-weight: 400;\"> such as wearing masks and physical distancing) seems more strongly rooted in people’s </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0248892\"><span style=\"font-weight: 400;\">lack of institutional trust</span></a><span style=\"font-weight: 400;\">, and even a </span><a href=\"https://www.nature.com/articles/s41591-021-01454-y\"><span style=\"font-weight: 400;\">mistrust of the government in general</span></a><span style=\"font-weight: 400;\"> when faced with a large-scale epidemic. </span>\r\n\r\n<span style=\"font-weight: 400;\">A separate — but valid — issue is a distrust of vaccines that stems from historical injustice and inequality. For example, American intelligence services </span><a href=\"https://academic.oup.com/jeea/advance-article-abstract/doi/10.1093/jeea/jvab018/6271320\"><span style=\"font-weight: 400;\">used a vaccination campaign in Pakistan as a cover</span></a><span style=\"font-weight: 400;\"> to capture Osama bin Laden — a nugget used effectively by the Taliban to discredit subsequent vaccination drives in that country. </span>\r\n\r\n<span style=\"font-weight: 400;\">Historic racism has also contributed to lower trust in many communities of colour — and has been so pervasive that the effect is seen even </span><a href=\"https://www.thelancet.com/journals/lanepe/article/PIIS2666-7762(21)00157-5/fulltext\"><span style=\"font-weight: 400;\">among healthcare workers</span></a><span style=\"font-weight: 400;\">. Low levels of trust are compounded by many of these communities </span><a href=\"https://journals.plos.org/plosbiology/article?id=10.1371/journal.pbio.3001167\"><span style=\"font-weight: 400;\">still experiencing obstacles</span></a><span style=\"font-weight: 400;\"> in accessing medical care for Covid-19. </span>\r\n\r\n<span style=\"font-weight: 400;\">In the end, turning nudges into numbers may be </span><a href=\"https://bhekisisa.org/article/2021-09-06-when-empathy-meets-evidence-addressing-covid-vaccine-hesitancy-in-sa/\"><span style=\"font-weight: 400;\">more about </span><i><span style=\"font-weight: 400;\">how</span></i><span style=\"font-weight: 400;\"> we talk to the unvaccinated than </span><i><span style=\"font-weight: 400;\">what</span></i><span style=\"font-weight: 400;\"> we say</span></a><span style=\"font-weight: 400;\">. </span><b>DM/MC</b>\r\n\r\n<i><span style=\"font-weight: 400;\">This story was produced by the</span></i><a href=\"https://bhekisisa.org/\"> <i><span style=\"font-weight: 400;\">Bhekisisa Centre for Health Journalism</span></i></a><i><span style=\"font-weight: 400;\">. This was promising, but willingness to get vaccinated doesn’t necessarily translate into uptake. Also in South Africa, </span><a href=\"https://cramsurvey.org/wp-content/uploads/2021/07/2.-Burger-R.-Maughan-Brown-M.-Kohler-T.-English-R.-_-Tameris-M.-2021-Increased-openness-to-accepting-a-COVID-19-vaccine-is-a-shot-in-the-arm-for-South-Africa-Evidence-from-the-NIDS-CRAM-Wave-5-Survey.pdf\"><span style=\"font-weight: 400;\">at least one poll showed similar results</span></a><span style=\"font-weight: 400;\">, yet so far </span><a href=\"https://sacoronavirus.co.za/latest-vaccine-statistics/\"><span style=\"font-weight: 400;\">just more than 32% have followed through with getting at least one vaccine dose</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">There is some good news, though.</span>\r\n\r\n[caption id=\"attachment_1057681\" align=\"aligncenter\" width=\"2133\"]<img class=\"wp-image-1057681 size-full\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2021/10/MC-Unvaxed_1.jpg\" alt=\"vaccine messaging\" width=\"2133\" height=\"1153\" /> To persuade people to get vaccinated, messages have to be tailored to the intended audience. (Photo: genengnews.com/ Andriy Onufriyenko / Getty Images / Wikipedia)[/caption]\r\n\r\n<span style=\"font-weight: 400;\">According to a report in the </span><a href=\"https://www.nejm.org/doi/full/10.1056/NEJMp2106137\"><i><span style=\"font-weight: 400;\">New England Medical Journal</span></i></a><span style=\"font-weight: 400;\"> many people described as “vaccine hesitant” are in fact not outright resistant to getting the jab, but simply waiting for the right nudge. Of the almost 140,000 people surveyed, 40% had already been vaccinated and 11% were hardline refusers, who said they were very unlikely to get the jab. This means close to half of the sample in the study were sitting on the fence — “vaccine ready” and waiting. This study was not conducted in South Africa.</span>\r\n\r\n<span style=\"font-weight: 400;\">Here are six things to consider to help turn ready and steady into go: </span>\r\n\r\n<b>Know who you’re talking to</b>\r\n\r\n<span style=\"font-weight: 400;\">To persuade people to get vaccinated, messages have to be tailored to the intended audience.</span>\r\n\r\n<span style=\"font-weight: 400;\">For example, a study in the UK showed that people who are vaccine-ready and actively looking to get the shot may be receptive to messages about the </span><a href=\"https://www.thelancet.com/journals/lanpub/article/PIIS2468-2667(21)00096-7/fulltext\"><span style=\"font-weight: 400;\">public health benefits of being vaccinated</span></a><span style=\"font-weight: 400;\"> — meaning they want to get vaccinated to protect themselves and others. In contrast, people who are hesitant appear to be less interested in the greater good and respond better to learning what’s in it for them.</span>\r\n\r\n<span style=\"font-weight: 400;\">A 2021 study from the US had a similar result: messages focused on personal benefits had the </span><a href=\"https://www.pnas.org/content/118/32/e2108225118\"><span style=\"font-weight: 400;\">greatest impact in convincing people to get vaccinated</span></a><span style=\"font-weight: 400;\">. Although we might wish for a society that prioritises the wellbeing of others, it might serve us well for now to be somewhat less ideological and focus instead on individual benefits — for the greater good.</span>\r\n\r\n<b>Get the word out — quickly</b>\r\n\r\n<span style=\"font-weight: 400;\">Strike while the iron is hot.</span>\r\n\r\n<span style=\"font-weight: 400;\">A </span><a href=\"https://www.nature.com/articles/s41586-021-03843-2\"><span style=\"font-weight: 400;\">study</span></a><span style=\"font-weight: 400;\"> published in the journal </span><i><span style=\"font-weight: 400;\">Nature </span></i><span style=\"font-weight: 400;\">in August found that the timing of vaccine messages can boost appointments and subsequent vaccinations. In this study, participants in the US were randomly assigned to one of two groups and were then sent a text message inviting them to schedule their vaccine appointment shortly after they became eligible to register.</span>\r\n\r\n<span style=\"font-weight: 400;\">One group received the message one day after they became eligible for getting their shot, while the other group received the message eight days after they could enrol. Both messages included a link to a website allowing the recipients to book vaccine appointments. The earlier prompt nudged one-and-a-half times more people to make appointments than the later one. </span>\r\n\r\n<span style=\"font-weight: 400;\">Simply making the booking also boosted follow-through. Nearly 90% of participants who made a booking after receiving the nudge kept their appointments — and almost everyone who received their first dose also booked their second.</span>\r\n\r\n<span style=\"font-weight: 400;\">This points to the importance of encouraging people to schedule their first shot. A similar trend was seen in an earlier </span><a href=\"https://www.pnas.org/content/118/20/e2101165118\"><span style=\"font-weight: 400;\">study designed to boost uptake of the flu vaccine</span></a><span style=\"font-weight: 400;\">. Vaccination reminders sent to people before their regular, unrelated primary healthcare visits increased vaccination rates by 5%. </span>\r\n\r\n<b>Allay people’s fears — first with empathy, then with evidence</b>\r\n\r\n<span style=\"font-weight: 400;\">The </span><a href=\"https://jamanetwork.com/journals/jama/fullarticle/2766651\"><span style=\"font-weight: 400;\">flip side of the unprecedented speed with which Covid-19 vaccines were rolled out</span></a><span style=\"font-weight: 400;\"> is that it has contributed to questions being raised about their safety.</span>\r\n\r\n<span style=\"font-weight: 400;\">Studies on people’s attitudes towards vaccines, from countries as diverse as </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0250555\"><span style=\"font-weight: 400;\">Jordan</span></a><span style=\"font-weight: 400;\"> and the </span><a href=\"https://jamanetwork.com/journals/jama/fullarticle/2780519\"><span style=\"font-weight: 400;\">US</span></a><span style=\"font-weight: 400;\">, show that fears about ingredients, safety and what many perceive as rushed approval processes deter people from getting vaccinated.</span>\r\n\r\n<a href=\"https://ebm.bmj.com/content/early/2021/08/08/bmjebm-2021-111735\"><span style=\"font-weight: 400;\">Inadequate data and a general lack of transparency</span></a><span style=\"font-weight: 400;\"> about vaccine trials, coupled with murky procurement agreements between governments and pharmaceutical companies, further fuel reluctance. </span>\r\n\r\n<span style=\"font-weight: 400;\">Although there is </span><a href=\"https://www.nature.com/articles/d41586-021-01505-x\"><span style=\"font-weight: 400;\">overwhelming evidence that the vaccines are safe</span></a><span style=\"font-weight: 400;\"> and effective, it is important to </span><a href=\"https://bhekisisa.org/article/2021-09-06-when-empathy-meets-evidence-addressing-covid-vaccine-hesitancy-in-sa/\"><span style=\"font-weight: 400;\">acknowledge that people’s fears are valid</span></a><span style=\"font-weight: 400;\">. Showing empathy when talking to people who are reluctant or uncertain may help to make them more receptive to balanced, evidence-based messages. </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s equally important to be honest about issues such as the common occurrence of minor side-effects — and to use evidence to contextualise the extremely rare risk of severe side-effects.</span>\r\n\r\n<span style=\"font-weight: 400;\">For instance, the scaremongering media coverage about blood clots developing after getting the Johnson & Johnson (J&J) or AstraZeneca vaccine created the impression that clots occur far more often than they do.</span>\r\n\r\n<span style=\"font-weight: 400;\">Investigations by international medicine regulators showed that the risk of developing a blood clot after getting the jab was very low — less than one in a million for </span><a href=\"https://www.fda.gov/news-events/press-announcements/fda-and-cdc-lift-recommended-pause-johnson-johnson-janssen-covid-19-vaccine-use-following-thorough\"><span style=\"font-weight: 400;\">J&J</span></a><span style=\"font-weight: 400;\"> and between</span><a href=\"https://www.health.gov.au/initiatives-and-programs/covid-19-vaccines/is-it-true/is-it-true-does-the-vaxzevria-astrazeneca-covid-19-vaccine-cause-blood-clots\"><span style=\"font-weight: 400;\"> four and six in a million for AstraZeneca.</span></a><span style=\"font-weight: 400;\"> Typically, a </span><a href=\"https://bhekisisa.org/article/2021-05-07-how-rare-is-very-rare-your-questions-about-blood-clots-answered/\"><span style=\"font-weight: 400;\">very rare adverse event</span></a><span style=\"font-weight: 400;\"> after taking a medicine is defined as affecting fewer than one in 10,000 people, but in this case the chances of developing blood clots were deemed really slim, and the vaccines were cleared for use after the temporary pause.</span>\r\n\r\n<b>Don’t be afraid to drop a name or two</b>\r\n\r\n<span style=\"font-weight: 400;\">If Elvis could do it, so can you.</span>\r\n\r\n[caption id=\"attachment_1057682\" align=\"aligncenter\" width=\"1004\"]<img class=\"wp-image-1057682 size-full\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2021/10/MC-Unvaxed_2.jpg\" alt=\"elvis polio shot\" width=\"1004\" height=\"609\" /> In 1956, the then rising rock ’n’ roll star Elvis Presley appeared on public television, getting his polio shot. Vaccinations surged, and by the next year active cases of polio had </span><a href=\"https://ourworldindata.org/polio#the-vaccine-against-polio\"><span style=\"font-weight: 400;\">dropped from 15,140 to 5,485</span></a><span style=\"font-weight: 400;\">. Five years later, by 1962, </span><a href=\"https://ourworldindata.org/polio#the-vaccine-against-polio\"><span style=\"font-weight: 400;\">fewer than 1,000 cases</span></a><span style=\"font-weight: 400;\"> were reported. </span>\r\n\r\n<span style=\"font-weight: 400;\">Studies about social influence on </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/31259597/\"><span style=\"font-weight: 400;\">boosting the uptake of flu vaccines</span></a><span style=\"font-weight: 400;\"> suggest that talking about other people getting their shots might help to improve the uptake for Covid-19 vaccines, too. It need not only be social influencers– even examples from </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7834519/\"><span style=\"font-weight: 400;\">within a peer group</span></a><span style=\"font-weight: 400;\"> can help to nudge people to get on board.</span>\r\n\r\n<b>Don’t focus on hardline refusers, try to convince those sitting on the fence </b>\r\n\r\n<span style=\"font-weight: 400;\">A small percentage of people say they will never get the jab — and studies suggest that chances are slim that they will change their minds. In the US, they number about </span><a href=\"https://www.kff.org/coronavirus-covid-19/dashboard/kff-covid-19-vaccine-monitor-dashboard/#intentions\"><span style=\"font-weight: 400;\">one in eight people</span></a><span style=\"font-weight: 400;\">, in South Africa, it’s about </span><a href=\"https://cramsurvey.org/wp-content/uploads/2021/07/2.-Burger-R.-Maughan-Brown-M.-Kohler-T.-English-R.-_-Tameris-M.-2021-Increased-openness-to-accepting-a-COVID-19-vaccine-is-a-shot-in-the-arm-for-South-Africa-Evidence-from-the-NIDS-CRAM-Wave-5-Survey.pdf\"><span style=\"font-weight: 400;\">one in 15</span></a><span style=\"font-weight: 400;\"> (although the proportion may </span><a href=\"https://cramsurvey.org/wp-content/uploads/2021/07/2.-Burger-R.-Maughan-Brown-M.-Kohler-T.-English-R.-_-Tameris-M.-2021-Increased-openness-to-accepting-a-COVID-19-vaccine-is-a-shot-in-the-arm-for-South-Africa-Evidence-from-the-NIDS-CRAM-Wave-5-Survey.pdf\"><span style=\"font-weight: 400;\">differ in pockets of the population</span></a><span style=\"font-weight: 400;\">).</span>\r\n\r\n<span style=\"font-weight: 400;\">The uptake among vaccine-ready people can change over time, but hardline anti-vaxxers almost never change their minds. So trying to persuade committed anti-vaxxers is likely to be a waste of time and money; it’s more effective to focus on people who are vaccine-ready and receptive to new information. </span>\r\n\r\n<b>Understand people’s realities</b>\r\n\r\n<span style=\"font-weight: 400;\">Historically, vaccination campaigns focused on busting myths and providing evidence-based information about vaccine safety and benefits. Yet, opposition to Covid-19 vaccines (</span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0248892\"><span style=\"font-weight: 400;\">and non-pharmaceutical interventions</span></a><span style=\"font-weight: 400;\"> such as wearing masks and physical distancing) seems more strongly rooted in people’s </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0248892\"><span style=\"font-weight: 400;\">lack of institutional trust</span></a><span style=\"font-weight: 400;\">, and even a </span><a href=\"https://www.nature.com/articles/s41591-021-01454-y\"><span style=\"font-weight: 400;\">mistrust of the government in general</span></a><span style=\"font-weight: 400;\"> when faced with a large-scale epidemic. </span>\r\n\r\n<span style=\"font-weight: 400;\">A separate — but valid — issue is a distrust of vaccines that stems from historical injustice and inequality. For example, American intelligence services </span><a href=\"https://academic.oup.com/jeea/advance-article-abstract/doi/10.1093/jeea/jvab018/6271320\"><span style=\"font-weight: 400;\">used a vaccination campaign in Pakistan as a cover</span></a><span style=\"font-weight: 400;\"> to capture Osama bin Laden — a nugget used effectively by the Taliban to discredit subsequent vaccination drives in that country. </span>\r\n\r\n<span style=\"font-weight: 400;\">Historic racism has also contributed to lower trust in many communities of colour — and has been so pervasive that the effect is seen even </span><a href=\"https://www.thelancet.com/journals/lanepe/article/PIIS2666-7762(21)00157-5/fulltext\"><span style=\"font-weight: 400;\">among healthcare workers</span></a><span style=\"font-weight: 400;\">. Low levels of trust are compounded by many of these communities </span><a href=\"https://journals.plos.org/plosbiology/article?id=10.1371/journal.pbio.3001167\"><span style=\"font-weight: 400;\">still experiencing obstacles</span></a><span style=\"font-weight: 400;\"> in accessing medical care for Covid-19. </span>\r\n\r\n<span style=\"font-weight: 400;\">In the end, turning nudges into numbers may be </span><a href=\"https://bhekisisa.org/article/2021-09-06-when-empathy-meets-evidence-addressing-covid-vaccine-hesitancy-in-sa/\"><span style=\"font-weight: 400;\">more about </span><i><span style=\"font-weight: 400;\">how</span></i><span style=\"font-weight: 400;\"> we talk to the unvaccinated than </span><i><span style=\"font-weight: 400;\">what</span></i><span style=\"font-weight: 400;\"> we say</span></a><span style=\"font-weight: 400;\">. </span><b>DM/MC</b>\r\n\r\n<i><span style=\"font-weight: 400;\">This story was produced by the</span></i><a href=\"https://bhekisisa.org/\"> <i><span style=\"font-weight: 400;\">Bhekisisa Centre for Health Journalism</span></i></a><i><span style=\"font-weight: 400;\">.
