"contents": "<span style=\"font-weight: 400;\">This year, 2022, is perhaps most notable for what didn’t happen. While SARS-CoV-2 continued to mutate rapidly, we did not see another devastating wave of </span><a href=\"https://www.spotlightnsp.co.za/2022/07/15/covid-19-the-role-of-next-generation-vaccines-in-immunity/\"><span style=\"font-weight: 400;\">Covid-19</span></a><span style=\"font-weight: 400;\"> deaths, as some feared. To what extent we remain vulnerable to future mutations is hard to gauge. Most people in South Africa have either been infected with SARS-CoV-2, vaccinated against it, or both. The </span><a href=\"https://www.spotlightnsp.co.za/2022/09/14/covid-19-binning-of-jabs-a-shocking-indictment-of-vaccination-campaign-says-health-expert/\"><span style=\"font-weight: 400;\">vaccination rate</span></a><span style=\"font-weight: 400;\"> slowed to a trickle and by the end of the year only a little more than half of South Africa’s adult population had been fully </span><a href=\"https://www.spotlightnsp.co.za/2022/08/16/covid-19-benefits-of-vaccines-still-far-outweigh-risks-experts-say/\"><span style=\"font-weight: 400;\">vaccinated</span></a><span style=\"font-weight: 400;\">. Updated vaccines that provide greater protection against new variants have not yet been made available in the country.</span>\r\n\r\n<span style=\"font-weight: 400;\">With the threat from Covid-19 waning, much of the focus in 2022 was on recovering from Covid-19-related disruptions. In some areas, such as tuberculosis (TB) testing, the recovery has been </span><a href=\"https://www.spotlightnsp.co.za/2022/09/15/as-testing-volumes-improve-government-outlines-tb-recovery-plan/\"><span style=\"font-weight: 400;\">impressive</span></a><span style=\"font-weight: 400;\">. But as is apparent from recent measles outbreaks </span><a href=\"https://www.spotlightnsp.co.za/2022/07/28/poor-vaccine-coverage-risks-measles-outbreak-experts-say/\"><span style=\"font-weight: 400;\">predicted earlier in the year</span></a><span style=\"font-weight: 400;\">, significant gaps remain.</span>\r\n\r\n<span style=\"font-weight: 400;\">The big picture remains bleak when it comes to South Africa’s public healthcare system.</span>\r\n\r\n<span style=\"font-weight: 400;\">As noted in several reports this year from community healthcare monitoring group </span><a href=\"https://ritshidze.org.za/\"><span style=\"font-weight: 400;\">Ritshidze</span></a><span style=\"font-weight: 400;\"> and confirmed in several government reports, healthcare facilities remain beset by staff shortages. There is little sign that the government has a grip on the situation as a promising </span><a href=\"https://www.spotlightnsp.co.za/2020/09/01/government-strategy-shows-billions-needed-to-avert-healthcare-worker-crisis/\"><span style=\"font-weight: 400;\">strategy document</span></a><span style=\"font-weight: 400;\"> on the issue appears to be gathering dust, while the nurse population is </span><a href=\"https://www.spotlightnsp.co.za/2021/10/07/ageing-nurses-a-crisis-on-the-horizon/\"><span style=\"font-weight: 400;\">ageing</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://www.spotlightnsp.co.za/2022/08/08/in-depth-what-are-fuelling-concerns-over-nurse-training-in-south-africa/\"><span style=\"font-weight: 400;\">nurse training</span></a><span style=\"font-weight: 400;\"> in the country has hit several </span><a href=\"https://www.spotlightnsp.co.za/2022/08/08/in-depth-what-are-fuelling-concerns-over-nurse-training-in-south-africa/\"><span style=\"font-weight: 400;\">snags</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-1504824\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/12/MC-NHI-Review_2.jpg\" alt=\"\" width=\"720\" height=\"411\" /> The National Health Insurance Bill will be tabled for approval in the National Assembly in 2023. (Photo: Rosetta Msimango / Spotlight)</p>\r\n\r\n<span style=\"font-weight: 400;\">Question marks also remain over the government’s commitment to </span><a href=\"https://www.spotlightnsp.co.za/2022/06/21/editorial-government-claims-to-value-healthcare-workers-its-actions-suggest-it-does-not/\"><span style=\"font-weight: 400;\">support healthcare workers</span></a><span style=\"font-weight: 400;\">, especially after steps were taken against Dr Tim de Maayer at the Rahima Moosa Mother and Child Hospital when he spoke out about conditions there.</span>\r\n\r\n<i><span style=\"font-weight: 400;\">Read in </span></i><span style=\"font-weight: 400;\">Daily Maverick</span><i><span style=\"font-weight: 400;\">: “</span></i><a href=\"https://www.dailymaverick.co.za/article/2022-06-12-rahima-moosa-hospital-problem-has-metastasised-throughout-the-healthcare-system-in-south-africa/\"><i><span style=\"font-weight: 400;\">Rahima Moosa hospital problem ‘has metastasised throughout the healthcare system in South Africa’</span></i></a><i><span style=\"font-weight: 400;\">”</span></i>\r\n\r\n<span style=\"font-weight: 400;\">Apart from healthcare worker shortages, most provincial health departments also remain beset by mismanagement and corruption. The Gauteng health department dominated headlines this year for the botched rebuild of </span><a href=\"https://www.spotlightnsp.co.za/2022/04/29/a-year-after-the-fire-little-progress-at-charlotte-maxeke-hospital/\"><span style=\"font-weight: 400;\">Charlotte Maxeke Johannesburg Academic Hospital</span></a><span style=\"font-weight: 400;\"> following last year’s fire, the attack on De Maayer, Covid-19-related corruption, and a still-unfolding web of corruption first exposed by Babita Deokoran who was assassinated last year – </span><i><span style=\"font-weight: 400;\">News24’s</span></i><span style=\"font-weight: 400;\"> coverage of this affair was one of the year’s journalistic highlights.</span>\r\n\r\n<span style=\"font-weight: 400;\">It came as little surprise when Dr Nomathemba Mokgethi was axed as Gauteng’s health MEC in October. She was replaced by </span><a href=\"https://www.spotlightnsp.co.za/2022/10/24/in-the-hot-seat-new-gauteng-health-mec-responds-to-10-questions-from-spotlight/\"><span style=\"font-weight: 400;\">Nomantu Nkomo-Ralehoko</span></a><span style=\"font-weight: 400;\">. The new MEC responded to </span><a href=\"https://www.spotlightnsp.co.za/2022/10/24/in-the-hot-seat-new-gauteng-health-mec-responds-to-10-questions-from-spotlight/\"><span style=\"font-weight: 400;\">10 questions</span></a><span style=\"font-weight: 400;\"> from </span><i><span style=\"font-weight: 400;\">Spotlight</span></i><span style=\"font-weight: 400;\">, but whether she actually has the stomach for the extensive reforms needed in the province remains unclear.</span>\r\n\r\n<span style=\"font-weight: 400;\">Meanwhile, more than three years after it was </span><a href=\"https://www.spotlightnsp.co.za/2022/02/08/in-depth-on-nhi-common-themes-emerging-from-parliamentary-hearings-part-1/\"><span style=\"font-weight: 400;\">first introduced in Parliament</span></a><span style=\"font-weight: 400;\">, work on the National Health Insurance Bill is being wrapped up in the National Assembly. Following the </span><a href=\"https://www.spotlightnsp.co.za/2022/06/02/analysis-nhi-bill-clause-by-clause-headbutting-begins-in-parliament/\"><span style=\"font-weight: 400;\">clause-by-clause deliberations</span></a><span style=\"font-weight: 400;\"> and the department’s responses last week, the state legal adviser now has to respond to the recommended amendments in the bill. When Parliament resumes next year, the bill will be tabled before a full sitting of the </span><a href=\"https://www.spotlightnsp.co.za/2022/02/08/in-depth-where-we-are-in-the-nhi-legislative-process-and-what-happens-next/\"><span style=\"font-weight: 400;\">National Assembly and if adopted will be referred to the National Council of Provinces</span></a><span style=\"font-weight: 400;\"> for further consultation. For now, questions about how NHI will be funded, the changing role of provinces, and the powers of the minister remain largely unanswered – with the majority party in Parliament seemingly intent on ramming the bill through in roughly its original form.</span>\r\n\r\n<i><span style=\"font-weight: 400;\">Read in </span></i><span style=\"font-weight: 400;\">Daily Maverick</span><i><span style=\"font-weight: 400;\">: “</span></i><a href=\"https://www.dailymaverick.co.za/opinionista/2022-12-06-nhi-not-perfect-legislation-but-critical-for-universal-health-coverage/\"><i><span style=\"font-weight: 400;\">The NHI is not a perfect piece of legislation, but is critical for universal health coverage</span></i></a><i><span style=\"font-weight: 400;\">”</span></i>\r\n\r\n<span style=\"font-weight: 400;\">Though it went somewhat under the radar compared with the NHI, a new Public Service Professionalisation Framework adopted by Cabinet this year offers significant potential for improving the public service, including healthcare services. The framework contains a set of concrete reforms, including some legislative amendments, that together amount to much more than just Batho Pele 2.0. Unfortunately, it seems unlikely that the framework will get the political backing it will need to make a real impact.</span>\r\n\r\n<hr />\r\n\r\n<strong>Visit <a href=\"https://www.dailymaverick.co.za?utm_source=direct&utm_medium=in_article_link&utm_campaign=homepage\"><em>Daily Maverick's</em> home page</a> for more news, analysis and investigations</strong>\r\n\r\n<hr />\r\n\r\n<span style=\"font-weight: 400;\">Away from the corridors of Parliament, it was an exciting year for TB. Pilots of </span><a href=\"https://www.spotlightnsp.co.za/2022/10/21/digital-x-rays-boosting-tb-diagnosis-assessment-finds/\"><span style=\"font-weight: 400;\">mobile X-ray screening</span></a><span style=\"font-weight: 400;\"> and the </span><a href=\"https://www.spotlightnsp.co.za/2022/06/08/positive-signs-for-new-tb-testing-strategy/\"><span style=\"font-weight: 400;\">testing of asymptomatic people</span></a><span style=\"font-weight: 400;\"> thought to be at high risk of TB suggested these new TB detection strategies may warrant a wider roll-out. Early findings from a </span><a href=\"https://www.spotlightnsp.co.za/2022/11/15/tb-treatment-can-be-cut-to-two-months-for-some-finds-landmark-study/\"><span style=\"font-weight: 400;\">landmark study</span></a><span style=\"font-weight: 400;\"> confirmed that for many people drug-susceptible TB can be cured in as little as two months, compared with the standard six. Several studies, most notably one called TB-PRACTECAL, confirmed that drug-resistant TB can be cured in six months with a treatment regimen that does not require any injections – plans are already afoot to </span><a href=\"https://www.spotlightnsp.co.za/2022/06/22/shorter-simpler-dr-tb-regimen-expected-later-this-year/\"><span style=\"font-weight: 400;\">introduce this regimen in South Africa</span></a><span style=\"font-weight: 400;\">. There are also plans to start treating most children with TB for </span><a href=\"https://www.spotlightnsp.co.za/2022/09/16/tb-treatment-to-be-shortened-to-4-months-for-most-kids/\"><span style=\"font-weight: 400;\">four rather than six months</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<img loading=\"lazy\" class=\"alignnone size-full wp-image-1504825\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/12/2022-12-15-12_45_17-Greenshot.jpg\" alt=\"\" width=\"720\" height=\"821\" />\r\n\r\n<span style=\"font-weight: 400;\">The biggest HIV news of the year came two weeks ago when the </span><a href=\"https://www.spotlightnsp.co.za/2022/12/13/analysis-how-well-did-sahpra-do-in-2022/\"><span style=\"font-weight: 400;\">South African Health Products Regulatory Authority</span></a> <a href=\"https://www.spotlightnsp.co.za/2022/12/01/first-hiv-prevention-injection-approved-in-sa/\"><span style=\"font-weight: 400;\">approved</span></a><span style=\"font-weight: 400;\"> the first HIV prevention injection in South Africa. </span><a href=\"https://www.spotlightnsp.co.za/2022/11/22/pilot-projects-set-to-inform-rollout-of-hiv-prevention-shot/\"><span style=\"font-weight: 400;\">Pilot projects</span></a><span style=\"font-weight: 400;\"> assessing how to best provide the every-other-month injection are set to start early in 2023, but the price of the injection seems likely to stand in the way of a mass roll-out. Less headline-making is the fact that HIV prevention pills have steadily become more widely available at clinics in South Africa over the past two years. This is one of several positive developments in the country’s HIV response, although </span><a href=\"https://www.spotlightnsp.co.za/2022/12/02/editorial-world-aids-day-2022-choice-convenience-and-respect-should-be-the-cornerstone-of-sas-hiv-response/\"><span style=\"font-weight: 400;\">much remains to be done</span></a><span style=\"font-weight: 400;\">. A </span><a href=\"https://sanac.org.za/wp-content/uploads/2022/11/NSPn-for-HIV-TB-STIs-2023-2028-Draft-1.pdf\"><span style=\"font-weight: 400;\">draft</span></a><span style=\"font-weight: 400;\"> National Strategic Plan (NSP) for HIV, TB and STIs 2023-2028 was published for public comment late in November. The draft plan makes much of mental health support. The Department of Health’s new </span><a href=\"https://www.spotlightnsp.co.za/2021/10/11/in-depth-sas-mental-health-policy-has-lapsed-what-happens-next/\"><span style=\"font-weight: 400;\">mental health framework</span></a><span style=\"font-weight: 400;\">, however, remains unpublished – the prior one having expired in 2020.</span>\r\n\r\n<span style=\"font-weight: 400;\">In terms of numbers – the </span><a href=\"https://www.samrc.ac.za/sites/default/files/files/2022-12-14/weekly10Dec2022.pdf\"><span style=\"font-weight: 400;\">MRC estimates</span></a><span style=\"font-weight: 400;\"> more than 52,000 excess natural deaths in South Africa in 2022 (about a quarter of the 2021 number). The WHO recently estimated that there were about </span><a href=\"https://www.spotlightnsp.co.za/2022/10/28/56-000-tb-deaths-in-sa-in-2021-estimates-who/\"><span style=\"font-weight: 400;\">56,000</span></a><span style=\"font-weight: 400;\"> TB deaths in South Africa in 2021 (2022 estimates will only be published next year). According to the latest estimates from the Thembisa model, there were about </span><a href=\"https://www.spotlightnsp.co.za/2022/07/26/spotlight-on-hiv-six-graphs-that-tell-the-story/\"><span style=\"font-weight: 400;\">52,000</span></a><span style=\"font-weight: 400;\"> HIV-related deaths in 2021 (we also saw the Thembisa team producing their </span><a href=\"https://www.sciencedirect.com/science/article/pii/S1201971222004386\"><span style=\"font-weight: 400;\">first TB estimates</span></a><span style=\"font-weight: 400;\"> this year).
