"contents": "<span style=\"font-weight: 400;\">Nearly 5% of the world’s carbon emissions come from the healthcare sector, says a </span><a href=\"https://lancetcountdown.org/2024-report/\"><span style=\"font-weight: 400;\">report</span></a><span style=\"font-weight: 400;\"> launched at </span><a href=\"https://cop29.az/en/home\"><span style=\"font-weight: 400;\">COP29</span></a><span style=\"font-weight: 400;\">, the UN’s annual climate change conference on Tuesday – and money for dealing with the disastrous effects of changing weather patterns on people’s health is “urgently needed”. </span>\r\n\r\n<span style=\"font-weight: 400;\">But South Africa is not yet a member of a </span><a href=\"https://www.who.int/\"><span style=\"font-weight: 400;\">World Health Organization (WHO)</span></a><span style=\"font-weight: 400;\"> group called the </span><a href=\"https://www.who.int/initiatives/alliance-for-transformative-action-on-climate-and-health\"><span style=\"font-weight: 400;\">Alliance for Transformative Action on Climate and Health</span></a><span style=\"font-weight: 400;\">, for which plans were put in place in 2021 at </span><a href=\"https://www.un.org/en/climatechange/cop26\"><span style=\"font-weight: 400;\">COP26 in Glasgow</span></a><span style=\"font-weight: 400;\">, as a way to help countries make their health services greener. </span>\r\n\r\n<span style=\"font-weight: 400;\">Keeping hospitals running adds to the rising level of greenhouse gases in the air, because of the electricity used to power buildings and equipment, fuel needed to transport patients and get supplies delivered and dealing with waste, says Azeeza Rangunwala, coordinator for Africa at </span><a href=\"https://greenhospitals.org/\"><span style=\"font-weight: 400;\">Global Green and Healthy Hospitals</span></a><span style=\"font-weight: 400;\">, a network of people who help healthcare facilities across the world to be more environmentally friendly. </span>\r\n\r\n<span style=\"font-weight: 400;\">Burning fuels like coal and oil to generate electricity and making products releases a lot of carbon dioxide into the air. This forms a layer in the atmosphere that traps heat rising up from Earth’s surface after being baked hot by the sun all day. Because the heat can’t escape, the air heats up – much like in a greenhouse – and over time, </span><a href=\"https://bhekisisa.org/article/2023-12-06-heating-up-heres-how-climate-change-works/\"><span style=\"font-weight: 400;\">the air gets warmer and warmer</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">In fact, last year </span><a href=\"https://library.wmo.int/viewer/68835/download?file=1347_Global-statement-2023_en.pdf&type=pdf&navigator=1\"><span style=\"font-weight: 400;\">the air temperature was 1.45°C higher</span></a><span style=\"font-weight: 400;\"> than about 150 years ago when the world started burning coal and oil at a large scale to run factories and transport systems such as cars and planes. It’s dangerously close to </span><span style=\"font-weight: 400;\">the 1.5°C rise in temperature that 196 countries, including South Africa, who signed </span><a href=\"https://unfccc.int/process-and-meetings/the-paris-agreement\"><span style=\"font-weight: 400;\">a legally binding agreement in Paris in 2015</span></a><span style=\"font-weight: 400;\">, pledged not to exceed in order to avoid the catastrophic consequences of more floods, droughts and illness.</span>\r\n\r\n<span style=\"font-weight: 400;\">But in South Africa, </span><a href=\"https://www.sahpra.org.za/wp-content/uploads/2020/01/Reprocessing-of-Single-Use-Medical-Devices_Communication-to-Stakeholders_Nov2019.pdf\"><span style=\"font-weight: 400;\">rules about how many types of modern healthcare products are used</span></a><span style=\"font-weight: 400;\"> are essentially blocking simple ways in which hospitals – of which the government runs about 395 – can cut down on how much greenhouse gases caring for patients put into the air. </span>\r\n\r\n<span style=\"font-weight: 400;\">Here’s why this is a problem.</span>\r\n\r\n<b>Waste from hospitals </b>\r\n\r\n<span style=\"font-weight: 400;\">The amount of greenhouse gas the South African economy </span><a href=\"https://www.iea.org/countries/south-africa/emissions\"><span style=\"font-weight: 400;\">produces is close to 400 million</span></a><span style=\"font-weight: 400;\"> metric tonnes of carbon dioxide per year (a metric tonne is 1,000kg) and makes up about 1% of the world’s carbon emissions. Both of these things run on diesel. Such items </span><a href=\"https://pmc.ncbi.nlm.nih.gov/articles/PMC9858835/\"><span style=\"font-weight: 400;\">include waste like</span></a><span style=\"font-weight: 400;\"> needles, medicine vials and bandages that have come into contact with blood.</span>\r\n\r\n<span style=\"font-weight: 400;\">Food and supply deliveries, together with transport for staff to a health facility, </span><a href=\"https://global.noharm.org/sites/default/files/documents-files/5961/HealthCaresClimateFootprint_092319.pdf\"><span style=\"font-weight: 400;\">add still more greenhouse gas emissions</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">And food that’s left uneaten or thrown away, and which </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/11/j.jand_.2020.01.001.pdf\"><span style=\"font-weight: 400;\">can make up 20% to 30%</span></a><span style=\"font-weight: 400;\"> of a hospital’s waste, </span><a href=\"https://www.fao.org/4/i3347e/i3347e.pdf\"><span style=\"font-weight: 400;\">adds extra pressure</span></a><span style=\"font-weight: 400;\">. Not only were the emissions from making and delivering the food unnecessary, but the waste has to be collected and driven away by trucks and then usually gets dumped on landfill sites. Here it </span><a href=\"https://researchspace.csir.co.za/server/api/core/bitstreams/26b93ae6-84aa-429c-b71e-da3a6d0bb18c/content\"><span style=\"font-weight: 400;\">breaks down and releases methane,</span></a><span style=\"font-weight: 400;\"> another powerful greenhouse gas, into the atmosphere. </span>\r\n\r\n<b>What is South Africa doing about it? </b>\r\n\r\n<a href=\"https://www.fda.gov/medical-devices/products-and-medical-procedures/reprocessing-single-use-medical-devices-information-health-care-facilities#:~:text=The%20FDA%20ensures%20the%20safety,original%20equipment%20manufacturer%20(OEM).\"><span style=\"font-weight: 400;\">Research</span></a><span style=\"font-weight: 400;\"> from the </span><a href=\"https://www.fda.gov/\"><span style=\"font-weight: 400;\">Food and Drug Administration</span></a><span style=\"font-weight: 400;\"> shows that devices such as forceps used during biopsies, drill bits and bite blocks for dental work and some fittings attached to instruments used in surgeries done by camera can be safely sterilised and reused. </span>\r\n\r\n<span style=\"font-weight: 400;\">But the </span><a href=\"https://www.sahpra.org.za/\"><span style=\"font-weight: 400;\">South African Health Products Regulatory Authority (Sahpra)</span></a><span style=\"font-weight: 400;\"> does not allow this because the manufacturers’ instructions say that the devices can be used only once.</span>\r\n\r\n<span style=\"font-weight: 400;\">John Lazarus, head of urology at the University of Cape Town, has, together with other healthcare workers, </span><a href=\"https://www.scielo.org.za/scielo.php?script=sci_arttext&pid=S0256-95742022000400002\"><span style=\"font-weight: 400;\">called</span></a><span style=\"font-weight: 400;\"> for reusing these devices. But without support from the regulator, he says “our hands are tied”.</span>\r\n\r\n<span style=\"font-weight: 400;\">“Hospitals and individual clinicians would not want to work outside the rules,” he says, and despite the group having met Sahpra on the issue, the regulating body “has been slow to make a decision”.</span>\r\n\r\n<span style=\"font-weight: 400;\">Another way to lower the health sector’s carbon emissions is to build so-called </span><a href=\"https://www.sciencedirect.com/science/article/pii/S2405844024008004\"><span style=\"font-weight: 400;\">green hospitals</span></a><span style=\"font-weight: 400;\">. These are buildings that run on, for example, solar power instead of electricity generated from coal, or have been designed to benefit from sunlight instead of switching on lights, or using materials that keep buildings naturally cool in summer and warm in winter. </span>\r\n\r\n<span style=\"font-weight: 400;\">New public facilities like the </span><a href=\"https://www.westerncape.gov.za/tpw/eco-friendly-khayelitsha-hospital\"><span style=\"font-weight: 400;\">Khayelitsha </span></a><span style=\"font-weight: 400;\">and </span><a href=\"https://d7.westerncape.gov.za/news/r500-million-mitchells-plain-hospital-nearing-completion\"><span style=\"font-weight: 400;\">Mitchells Plain</span></a><span style=\"font-weight: 400;\"> hospitals in the Western Cape are examples where this works.</span>\r\n\r\n<span style=\"font-weight: 400;\">Spokesperson for the province’s health department, Dwayne Evans, says that through their energy-saving programme at pilot sites such as the Red Cross Children’s Hospital and Paarl Hospital, the department has saved about 4,000 tonnes of carbon dioxide emissions since 2022 — equal to about 13,750 homes not having to rely on electricity from coal for a year.</span>\r\n\r\n<span style=\"font-weight: 400;\">But getting the health sector on board to change how they work and so help slow climate change may prove difficult. </span>\r\n\r\n<span style=\"font-weight: 400;\">Says Lazarus: “In general the motivation to transform health for sustainability is not well established in South Africa.” </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">This story was produced by the</span></i><a href=\"http://bhekisisa.org./\"> <i><span style=\"font-weight: 400;\">Bhekisisa Centre for Health Journalism</span></i></a><i><span style=\"font-weight: 400;\">.
