Living in one of the most fertile and biodiverse regions on Earth, millions of people still go hungry – and hungry people are voters too. </span>\r\n\r\n<span style=\"font-weight: 400;\">So if you want to be the next president of a Latin American country, then food security must be near the top of your agenda. </span>\r\n\r\n<span style=\"font-weight: 400;\">In theory, this shouldn’t be any different in South Africa. </span><a href=\"https://www.statssa.gov.za/publications/P0318/P03182022.pdf\"><span style=\"font-weight: 400;\">One in five people</span></a><span style=\"font-weight: 400;\"> in our country don’t have enough food to eat. </span><a href=\"https://www.up.ac.za/media/shared/717/Child%20PIP/Saving%20children%20reports/saving-children-2012-2013.zp207156.pdf\"><span style=\"font-weight: 400;\">Ten thousand children die from the effects of severe underfeeding each year</span></a><span style=\"font-weight: 400;\"> and a </span><a href=\"https://www.statssa.gov.za/publications/Report%2003-00-09/Report%2003-00-092016.pdf\"><span style=\"font-weight: 400;\">quarter of children under five years are stunted</span></a><span style=\"font-weight: 400;\"> because of malnutrition.</span>\r\n\r\n<span style=\"font-weight: 400;\">Food is different from other commodities which are simply a means to make money. Adequate nutrition is a basic condition for life, human development and social stability. And it must be a priority on the agenda of every political party.</span>\r\n\r\n<span style=\"font-weight: 400;\">But will South Africa’s politicians take the feeding of our nation seriously in the upcoming elections? </span>\r\n<h4><b>How food security became an election issue in Latin America </b></h4>\r\n<span style=\"font-weight: 400;\">In the 1980s and 1990s, many Latin American countries were unable to pay their debts with foreign commercial banks. So the World Bank and the International Monetary Fund bailed them out on strict conditions that included gearing agricultural production towards the export market. </span>\r\n\r\n<span style=\"font-weight: 400;\">This brought much-needed foreign currency into the region’s economies, but it came at the expense of the poor </span><span style=\"font-weight: 400;\">– because </span><span style=\"font-weight: 400;\">small-scale farming gave way to large, corporatised, single-crop farming and food companies chased profits beyond country borders. </span>\r\n\r\n<span style=\"font-weight: 400;\">Today, </span><a href=\"https://foodtank.com/news/2023/02/latin-americas-food-paradox/\"><span style=\"font-weight: 400;\">Latin America exports one-quarter of its locally produced food</span></a><span style=\"font-weight: 400;\">, compared with Asia which exports only 6%. </span>\r\n\r\n<span style=\"font-weight: 400;\">As a result of the exports, food became more expensive in Latin America and poorer people struggled to afford food at prices set by global markets in the Eighties and Nineties. Riots broke out and the</span><a href=\"https://journals.sagepub.com/doi/10.1177/0094582X9402100305\"> <span style=\"font-weight: 400;\">looting of food was widely viewed as a right</span></a> <span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> a form of restorative justice, with community leaders and academics leading activist coalitions that forced food security to the top of the political agenda. </span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-1988190\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2023/12/5E2B2042.jpg\" alt=\"hunger\" width=\"720\" height=\"443\" /> <em>A six-year-old boy carries onions and apples donated by SA Harvest in Lusikisiki, Eastern Cape, on 21 December 2023. (Photo: Hoseya Jubase)</em></p>\r\n<h4><b>More food: More degrees, better jobs</b></h4>\r\n<span style=\"font-weight: 400;\">Not that food insecurity and hunger have been done away with in Latin America. Given the cash generated by food exports, governments had little appetite to prioritise domestic consumption. </span>\r\n\r\n<span style=\"font-weight: 400;\">But one aspect of food security which was within their power and appealed to voters was to improve the nutrition of children. </span>\r\n\r\n<span style=\"font-weight: 400;\">National programmes were designed to ensure that mothers, babies and young children had enough nutritious food. Many countries </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> Chile, Peru, Mexico, Columbia, Argentina and Brazil </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> made great gains over 10 to 20 years, </span><a href=\"https://www.fao.org/3/cc3859en/cc3859en.pdf\"><span style=\"font-weight: 400;\">despite ruling parties changing</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">As a result, </span><a href=\"https://www.sciencedirect.com/science/article/pii/S0305750X1500025X\"><span style=\"font-weight: 400;\">more children did better at school</span></a><span style=\"font-weight: 400;\"> and went on to study for university degrees.</span>\r\n\r\n<span style=\"font-weight: 400;\">Today, one in </span><a href=\"https://www.dhet.gov.za/Planning%20Monitoring%20and%20Evaluation%20Coordination/Report%20on%20Skills%20Supply%20and%20Demand%20in%20South%20Africa%20-%202022%20%281%29.pdf\"><span style=\"font-weight: 400;\">six adults in Brazil and Mexico</span></a><span style=\"font-weight: 400;\"> has a bachelor’s degree, compared with </span><a href=\"https://www.dhet.gov.za/Planning%20Monitoring%20and%20Evaluation%20Coordination/Report%20on%20Skills%20Supply%20and%20Demand%20in%20South%20Africa%20-%202022%20%281%29.pdf\"><span style=\"font-weight: 400;\">one in 12 in South Africa</span></a><span style=\"font-weight: 400;\">. Consequently, those countries’ labour markets became more skilled as human capital pipelines began to flow. </span>\r\n<h4><b>Back home: SA grows enough crops, but exports result in local hunger </b></h4>\r\n<span style=\"font-weight: 400;\">South Africa, on the other hand, is stuck.</span>\r\n\r\n<span style=\"font-weight: 400;\">Right now, j</span><a href=\"https://www.ijr.org.za/home/wp-content/uploads/2023/12/SA-Reconciliation-Barometer-2023_Final.pdf\"><span style=\"font-weight: 400;\">obs, housing and food are pressing issues for young voters</span></a><span style=\"font-weight: 400;\"> in South Africa and these provoke grassroots movements and political protests. Curiously, a review by one of our projects reveals that political parties have latched onto jobs and housing, but none has yet placed food security at the centre of its 2024 election manifesto. </span>\r\n\r\n<span style=\"font-weight: 400;\">Yet hungry people have the vote. </span>\r\n\r\n<span style=\"font-weight: 400;\">About </span><a href=\"https://www.statssa.gov.za/publications/P0318/P03182022.pdf\"><span style=\"font-weight: 400;\">one in five people in South Africa</span> <span style=\"font-weight: 400;\">(</span><span style=\"font-weight: 400;\">22%) had too little access to food in 2022 and 12.9% reported going hungry</span></a><span style=\"font-weight: 400;\">. That is a large constituency </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> 13,505,000 (22%) of the country’s estimated 61,384,000 people. </span>\r\n\r\n<span style=\"font-weight: 400;\">When it comes to who has access to how much food, South Africa and Latin American countries are in the same boat. </span>\r\n\r\n<span style=\"font-weight: 400;\">Our country produces enough food to feed everyone if it really wanted to, but </span><a href=\"https://apps.fas.usda.gov/newgainapi/api/Report/DownloadReportByFileName?fileName=Retail%20Foods_Pretoria_South%20Africa%20-%20Republic%20of_SF2023-0019.pdf\"><span style=\"font-weight: 400;\">29% of our food output is exported</span></a><span style=\"font-weight: 400;\"> because consumers in other countries can pay more for those products than most South Africans can. </span>\r\n<h4><b>What political parties can do to fight to end hunger </b></h4>\r\n<span style=\"font-weight: 400;\">There isn’t much leeway to change the extent of international trade since the government would like to increase exports, not decrease them. But we can learn from Latin America: There is still a lot of room to improve food security – starting with children. </span>\r\n\r\n<span style=\"font-weight: 400;\">Imagine if every political party committed to </span><a href=\"https://www.gov.za/documents/constitution/constitution-republic-south-africa-1996-04-feb-1997\"><span style=\"font-weight: 400;\">section 28(1)(c) of the Constitution</span> </a><span style=\"font-weight: 400;\">which gives every child the right to basic nutrition, shelter, healthcare and social services. If they made adequate child nutrition a cornerstone of their election promises – and delivered on them – over the next five years it would be possible to:</span>\r\n<ul>\r\n \t<li><a href=\"https://www.up.ac.za/media/shared/717/Child%20PIP/Saving%20children%20reports/saving-children-2012-2013.zp207156.pdf\">Stop the 10,000 child deaths a year</a> caused by severe acute malnutrition;</li>\r\n \t<li>Reduce the <a href=\"https://ilifalabantwana.co.za/wp-content/uploads/2019/09/SA-ECR_2019_12_09_2019_online_pages.pdf\">rate of low birth weight</a> from <a href=\"https://www.who.int/publications/i/item/WHO-NMH-NHD-14.2\">14% to below 10%</a> (this is aligned with World Health Organization <a href=\"https://www.who.int/publications/i/item/WHO-NMH-NHD-14.2\">targets</a>);</li>\r\n \t<li>Halve the <a href=\"https://data.unicef.org/wp-content/uploads/2023/05/JME-2023-Levels-and-trends-in-child-malnutrition.pdf\">rate of stunting among children under two from 30% to 15%</a>, and</li>\r\n \t<li>Reduce the levels of <a href=\"https://data.unicef.org/wp-content/uploads/2023/05/JME-2023-Levels-and-trends-in-child-malnutrition.pdf\">child obesity from 13.5% to below 10%</a>.</li>\r\n</ul>\r\n<span style=\"font-weight: 400;\">In my opinion, the five best – </span><span style=\"font-weight: 400;\">and </span><span style=\"font-weight: 400;\">most urgent – </span><span style=\"font-weight: 400;\">ways to do this, are: </span>\r\n<h4><b>1 Close the gap between</b> <b>the </b><a href=\"https://www.gov.za/services/child-care-social-benefits/child-support-grant\"><b>Child Support Grant of R510</b></a><b> and the </b><a href=\"https://www.statssa.gov.za/publications/P03101/P031012023.pdf\"><b>food poverty line of R760</b></a><b>, starting with</b> <b>children younger than three.</b></h4>\r\n<span style=\"font-weight: 400;\">A food poverty line of R760 means someone in South Africa needs at least R760 per month to buy enough food for a healthy diet (according to the Food and Agricultural Organization of the United Nations,</span><a href=\"https://www.fao.org/policy-support/tools-and-publications/resources-details/en/c/446032/\"> <span style=\"font-weight: 400;\">this translates to an intake of 2,100 kilocalories</span></a><span style=\"font-weight: 400;\"> per adult per day). </span>\r\n\r\n<span style=\"font-weight: 400;\">Ideally, the Child Support Grant should be increased from R510 to R760 for all eligible children, but our current fiscus can’t afford this. However, National Treasury should set aside an extra R6-billion for under-three-year-olds, </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/21944378/\"><span style=\"font-weight: 400;\">which would protect children</span> </a><span style=\"font-weight: 400;\">during the most important stage of their development. </span>\r\n<h4><b>2</b> <b>Ensure pregnant women have enough nutritious food</b><span style=\"font-weight: 400;\"> </span></h4>\r\n<span style=\"font-weight: 400;\">Existing policies such as iron and folate supplementation (</span><a href=\"https://www.who.int/tools/elena/interventions/daily-iron-pregnancy#:~:text=Pregnant%20women%20require%20additional%20iron,as%20well%20as%20fetal%20development.\"><span style=\"font-weight: 400;\">too little iron and folic acid during pregnancy can make a pregnant woman sick and her unborn baby develop</span><span style=\"font-weight: 400;\"> inadequately</span></a><span style=\"font-weight: 400;\">) </span><span style=\"font-weight: 400;\">must stay in place, but we need to do more than this to keep moms-to-be healthy. The Child Support Grant should be extended backwards, so that expecting women receive grants from 12 weeks of pregnancy instead of from birth. Mothers who are underweight should receive phone food vouchers to buy milk powder and eggs at spaza shops or supermarkets. These foods are high in proteins and other vitamins and minerals </span><a href=\"https://www.ajol.info/index.php/sajcn/article/view/97787\"><span style=\"font-weight: 400;\">essential for helping their unborn babies to grow at the right pace.</span></a>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-1900462\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2023/10/MC-TuesdayEditorial_4.jpg\" alt=\"hunger\" width=\"720\" height=\"427\" /> <em>Hunger is linked to many societal challenges. (Photo: Gallo Images / Brenton Geach)</em></p>\r\n<h4><b>3</b> <b>Spot and treat children under three who fail to thrive </b></h4>\r\n<span style=\"font-weight: 400;\">We should train and equip community health workers to identify and refer children who fail to thrive. Failing to thrive means that their </span><a href=\"https://www.hopkinsmedicine.org/health/conditions-and-diseases/failure-to-thrive\"><span style=\"font-weight: 400;\">development falls more and more behind </span></a><span style=\"font-weight: 400;\">the normal growth of children.</span>\r\n\r\n<span style=\"font-weight: 400;\">Each community health worker should have a scale to weigh children and track their progress, and they should all be allowed to dispense vitamin A and deworming medication. In most provinces, these field workers are not allowed to do so because it is felt that handing out medicines is outside their scope of work. Yet anyone can buy them </span><a href=\"https://www.sahpra.org.za/wp-content/uploads/2021/06/Consolidated-schedules_25-May-2021.docx.pdf\"><span style=\"font-weight: 400;\">over the counter</span></a><span style=\"font-weight: 400;\"> and dispensing them requires minimal training. Vitamin A </span><a href=\"https://www.who.int/data/nutrition/nlis/info/children-6-59-months-receiving-vitamin-a-supplements#\"><span style=\"font-weight: 400;\">protects children from falling seriously ill by boosting their immune systems</span></a><span style=\"font-weight: 400;\">, and deworming medication </span><a href=\"https://www.who.int/news/item/29-09-2017-who-recommends-large-scale-deworming-to-improve-children-s-health-and-nutrition\"><span style=\"font-weight: 400;\">helps them to get the right number of nutrients</span></a><span style=\"font-weight: 400;\"> (chemical compounds such as proteins and fats contained in food); worms lower the ability to absorb nutrients, which in turn </span><a href=\"https://www.who.int/news/item/29-09-2017-who-recommends-large-scale-deworming-to-improve-children-s-health-and-nutrition\"><span style=\"font-weight: 400;\">leads to children growing at a slower pace than they need to</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Once malnourished children are identified, they should be treated, just as we do any other health condition. The right treatment for malnutrition is providing the right food. Caregivers should get vouchers to buy powdered milk and eggs (</span><a href=\"https://www.ajol.info/index.php/sajcn/article/view/97787\"><span style=\"font-weight: 400;\">which are important for child growth</span></a><span style=\"font-weight: 400;\">) for malnourished children they take care of.</span>\r\n<h4><b>4 Link organisations who help people to grow food with government training programmes </b></h4>\r\n<span style=\"font-weight: 400;\">Supporting smallholder farmers financially is not enough to ensure they succeed </span><span style=\"font-weight: 400;\">– that’s a </span><a href=\"https://repository.uwc.ac.za/bitstream/handle/10566/8721/COVID19%20impacts-%20%20PLAAS%20RR58%20WEB4.pdf?sequence=1&isAllowed=y\"><span style=\"font-weight: 400;\">lesson learnt from broad-based black economic empowerment and Covid-19 relief</span></a><span style=\"font-weight: 400;\">. Small-scale farmers also </span><a href=\"https://doi.org/10.1371/journal.pone.0182399\"><span style=\"font-weight: 400;\">need training and mentoring, as well as interaction with their peers,</span> </a><span style=\"font-weight: 400;\">to become players in local food supply chains. For instance, </span><a href=\"https://thanda.org/organic-farming/\"><span style=\"font-weight: 400;\">Thanda</span></a> <span style=\"font-weight: 400;\">in southern KwaZulu-Natal supports more than 400 local farmers with seedlings, training and access to school feeding schemes and local spaza shops, to which they sell their products. Linking the resources of government extension services (</span><a href=\"https://www.sciencedirect.com/topics/agricultural-and-biological-sciences/extension-programs\"><span style=\"font-weight: 400;\">government-operated programmes that provide educational learning experiences to farmers</span></a><b>) </b><span style=\"font-weight: 400;\">and nonprofit organisations would clearly boost local markets.</span>\r\n<h4><b>5 Make 10 nutritious foods cheaper </b></h4>\r\n<span style=\"font-weight: 400;\">About 2.5 million children in South Africa who </span><a href=\"https://www.unicef.org/southafrica/media/7971/file/%20ZAF-update-study-exclusion-error-children-eligible-receive-child-support-grant-June-2023.pdf\"><span style=\"font-weight: 400;\">qualify for Child Support Grants can’t access the payments</span></a><span style=\"font-weight: 400;\"> because they don’t have the required documents (such as identity documents) or their parents are foreigners. That’s why we need plans which will also help such groups. These include the food industry’s agreement to forgo profits on selected items, matched by a publicly funded retail subsidy, so that all children can benefit from the lower prices. </span><a href=\"https://www.uobgroup.com/web-resources/uobgroup/pdf/research/MN_230922D.pdf\"><span style=\"font-weight: 400;\">Malaysia has done this to great effect</span></a><span style=\"font-weight: 400;\">, making food more accessible while capping food inflation. </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">David Harrison is a medical doctor and the chief executive at the </span></i><a href=\"https://dgmt.co.za/about/\"><i><span style=\"font-weight: 400;\">DG Murray Trust</span></i></a><span style=\"font-weight: 400;\">, </span><i><span style=\"font-weight: 400;\">a public innovation fund based in Cape Town.</span></i>\r\n\r\n<i><span style=\"font-weight: 400;\">This story was produced by the</span></i><a href=\"http://bhekisisa.org./\"><i><span style=\"font-weight: 400;\"> Bhekisisa Centre for Health Journalism</span></i></a><i><span style=\"font-weight: 400;\">.
