"contents": "<span style=\"font-weight: 400;\">Jessica Hinds spent her 28th birthday hunched over the toilet. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Things were really bad. She even lost a tooth,” her 38-year-old partner, Grant Hinds, recalls.</span>\r\n\r\n<span style=\"font-weight: 400;\">Jessica was pregnant and experiencing </span><a href=\"https://medlineplus.gov/ency/article/001499.htm\"><span style=\"font-weight: 400;\">hyperemesis gravidarum</span></a><span style=\"font-weight: 400;\"> (HG), a rare but particularly severe form of nausea that </span><a href=\"https://rarediseases.org/rare-diseases/hyperemesis-gravidarum/\"><span style=\"font-weight: 400;\">affects about 2% of expectant mothers</span></a><span style=\"font-weight: 400;\">. Unlike the milder form of </span><a href=\"https://www.mayoclinic.org/diseases-conditions/morning-sickness/symptoms-causes/syc-20375254\"><span style=\"font-weight: 400;\">morning sickness</span></a><span style=\"font-weight: 400;\"> that commonly occurs in the first three months of pregnancy, HG can last well beyond the first trimester, and in some cases for the entire term. </span>\r\n\r\n<span style=\"font-weight: 400;\">Some women lose </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3410506/\"><span style=\"font-weight: 400;\">more than 5% of their pre-pregnancy body weight</span></a><span style=\"font-weight: 400;\"> because of the constant vomiting. Stomach acid reaching the mouth can also </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3312266/\"><span style=\"font-weight: 400;\">wear away tooth enamel</span></a><span style=\"font-weight: 400;\">, which could compromise a pregnant woman’s dental health.</span><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">Grant says he can’t shake the image of his gaunt, exhausted partner that day, sweating and uncomfortable, and at her wit’s end.</span>\r\n\r\n<span style=\"font-weight: 400;\">Jessica was not the happy person he knew when she found out she was pregnant around four months earlier. When their daughter, JJ, was born in 2022, they agreed: they’re not going through that again.</span>\r\n<h4><b>‘It’s a family thing’</b></h4>\r\n<span style=\"font-weight: 400;\">With their newborn around one month old, Grant told his partner he was thinking of getting a </span><a href=\"https://www.mayoclinic.org/tests-procedures/vasectomy/about/pac-20384580\"><span style=\"font-weight: 400;\">vasectomy</span></a><span style=\"font-weight: 400;\">. This is a form of male contraception in which the </span><a href=\"https://bhekisisa.org/features/short-form-features/2022-09-28-we-take-the-fish-out-of-the-water-three-myths-about-vasectomies-busted/\"><span style=\"font-weight: 400;\">sperm-carrying tubes leading from the testes are cut</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">In fact, he’d been thinking about going for the snip even before the couple met, but their experience during Jessica’s pregnancy convinced him that it was the right thing to do — for both of them. </span>\r\n\r\n<span style=\"font-weight: 400;\">Grant says the vasectomy was “a no-brainer” in terms of being a way for him to support their </span><a href=\"https://www.who.int/health-topics/contraception#tab=tab_1\"><span style=\"font-weight: 400;\">family planning</span></a><span style=\"font-weight: 400;\"> decisions. </span>\r\n\r\n<span style=\"font-weight: 400;\">Using </span><a href=\"https://www.who.int/health-topics/contraception#tab=tab_1\"><span style=\"font-weight: 400;\">modern options for birth control</span></a><span style=\"font-weight: 400;\">, such as contraceptive pills or injections, an </span><a href=\"https://medlineplus.gov/ency/article/007635.htm\"><span style=\"font-weight: 400;\">intrauterine device</span></a><span style=\"font-weight: 400;\">, condoms or, as in the Hinds’ case, a vasectomy, allows a couple to decide how many children they want and how far apart they should be.</span>\r\n\r\n<span style=\"font-weight: 400;\">Researchers analysing a sample of 4.5 million births from 77 countries found that, specifically in poorer countries, the chance of an infant dying </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6667399/\"><span style=\"font-weight: 400;\">increases if a woman has two babies less than three years apart</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Moreover, when siblings are further apart, they may have better nutrition and be healthier, because mothers may have more money to spend on food and time to </span><a href=\"https://www.sciencedirect.com/science/article/abs/pii/S2213398420301093\"><span style=\"font-weight: 400;\">invest in their upbringing</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">But </span><a href=\"https://population.un.org/dataportal/data/indicators/12/locations/947/start/1990/end/2030/table/pivotbylocation\"><span style=\"font-weight: 400;\">data</span></a><span style=\"font-weight: 400;\"> shows that in sub-Saharan Africa, about four out of 10 women’s needs for family planning are not met at the moment, with almost 30% of pregnancies in the region </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0220970\"><span style=\"font-weight: 400;\">being unplanned</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">In a </span><a href=\"https://reproductive-health-journal.biomedcentral.com/articles/10.1186/s12978-019-0749-y\"><span style=\"font-weight: 400;\">2019 study</span></a><span style=\"font-weight: 400;\"> from KwaZulu-Natal about male attitudes towards family planning, health workers said that including men in the conversation could help to empower women. </span>\r\n\r\n<span style=\"font-weight: 400;\">“It [family planning] ends up being a feminine thing, yet it’s not supposed to be; it’s a family thing,” said one nurse.</span>\r\n<h4><b>Partner support</b></h4>\r\n<a href=\"https://journals.sagepub.com/doi/10.1177/0272684X16685254\"><span style=\"font-weight: 400;\">Studies</span></a><span style=\"font-weight: 400;\"> found that when men don’t support their partners in family planning, contraceptive use drops. The resistance could be fuelled, in part, by misconceptions about how birth control works. </span>\r\n\r\n<span style=\"font-weight: 400;\">“You find that your queen [female partner] is loose like jelly, and that is caused by the injection,” said one man in a focus group discussion in the </span><a href=\"https://reproductive-health-journal.biomedcentral.com/articles/10.1186/s12978-019-0749-y\"><span style=\"font-weight: 400;\">KwaZulu-Natal study</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The fear of violent behaviour when men discover that their partners are on birth control can also cause women to use contraception either in secret or not at all. </span>\r\n\r\n<span style=\"font-weight: 400;\">“I am forced not to use family planning because my partner is going to hit me,” one woman told the researchers. </span>\r\n\r\n<span style=\"font-weight: 400;\">But the opposite also holds. In many cases, </span><a href=\"https://panafrican-med-journal.com/content/article/19/349/full/\"><span style=\"font-weight: 400;\">men’s approval of their partners using birth control</span></a><span style=\"font-weight: 400;\"> can increase uptake of contraception, encourage women to keep on using it, or even help them find the best method for them. </span>\r\n\r\n<span style=\"font-weight: 400;\">One woman from KwaZulu-Natal recalls her partner talking to her about the option of using a contraceptive implant. </span>\r\n\r\n<span style=\"font-weight: 400;\">“We have to get it even if we are using condoms,” her boyfriend said.</span>\r\n<h4><span style=\"font-weight: 400;\"> </span><b>Why less will come to mean more</b></h4>\r\n<span style=\"font-weight: 400;\">Lower fertility rates spur economic growth, say population studies researchers, because it </span><a href=\"https://futures.issafrica.org/thematic/03-demographic-dividend/\"><span style=\"font-weight: 400;\">increases the proportion of working-age people in a population</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The more people who are able to work — and pay taxes — in a country, the more money governments have to spend on supporting children and the elderly, such as by building schools and hospitals and supplying electricity, water and sanitation. </span>\r\n\r\n<span style=\"font-weight: 400;\">Jakkie Cilliers, who heads the Institute for Security Studies’ </span><a href=\"https://futures.issafrica.org/\"><span style=\"font-weight: 400;\">African Futures</span></a><span style=\"font-weight: 400;\"> programme, says that when a country enters its </span><a href=\"https://www.imf.org/external/pubs/ft/fandd/2006/09/basics.htm\"><span style=\"font-weight: 400;\">demographic dividend</span></a><span style=\"font-weight: 400;\">, its economy starts to grow faster. </span>\r\n\r\n<span style=\"font-weight: 400;\">This “sweet spot” occurs when there are 1.7 people of working age (15 t0 65) for every dependent child or elder. At that point, the amount of money a country can generate (its </span><a href=\"https://www.imf.org/en/Publications/fandd/issues/Series/Back-to-Basics/gross-domestic-product-GDP\"><span style=\"font-weight: 400;\">gross domestic product</span></a><span style=\"font-weight: 400;\">) starts to make a real difference to people’s social and economic development. </span>\r\n\r\n<b>Visit </b><a href=\"https://www.dailymaverick.co.za/?utm_source=direct&utm_medium=in_article_link&utm_campaign=homepage\"><b><i>Daily Maverick’s</i></b><b> home page</b></a><b> for more news, analysis and investigations</b>\r\n\r\n<span style=\"font-weight: 400;\">It also applies to how families can use their money. </span>\r\n\r\n<span style=\"font-weight: 400;\">“A woman who has two children can invest in the welfare of those kids better than a woman who has five or six kids,” Cilliers notes. That means parents can feed their children better, give them better education and afford better healthcare. </span>\r\n\r\n<span style=\"font-weight: 400;\">“But that’s not the situation in most of Africa,” says Cilliers. </span>\r\n\r\n<span style=\"font-weight: 400;\">Researchers look at graphs called </span><a href=\"https://ourworldindata.org/age-structure\"><span style=\"font-weight: 400;\">population pyramids</span></a><span style=\"font-weight: 400;\"> – showing the proportion of people in different age groups – to make sense of countries’ development outlooks.</span>\r\n\r\n<span style=\"font-weight: 400;\">For example, in </span><a href=\"https://futures.issafrica.org/thematic/03-demographic-dividend/\"><span style=\"font-weight: 400;\">Niger</span></a><span style=\"font-weight: 400;\"> “there are so many more kids than working-age people that the country can’t build schools fast enough or provide better healthcare”, Cilliers explains. </span>\r\n\r\n<span style=\"font-weight: 400;\">But for a country like </span><a href=\"https://futures.issafrica.org/thematic/03-demographic-dividend/\"><span style=\"font-weight: 400;\">Mauritius</span></a><span style=\"font-weight: 400;\">, whose population pyramid “looks more like the Taj Mahal” — bulging around the middle, where the working-age population sit in the graph — there’s more money available in the state’s purse for education, healthcare and services. </span>\r\n\r\n<span style=\"font-weight: 400;\">According to a </span><a href=\"https://esaro.unfpa.org/en/publications/seeing-unseen\"><span style=\"font-weight: 400;\">report</span></a><span style=\"font-weight: 400;\"> from the United Nations Population Fund, the rates of unplanned pregnancies tend to be higher in countries where men hold more power in society — likely because women in these countries don’t have a say in how many children they want to have or even the power to say no to sex.</span>\r\n<h4><b>The solution?</b></h4>\r\n<span style=\"font-weight: 400;\">For starters, keep girls in school. Research shows that girls who had finished at least primary education were </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0220970\"><span style=\"font-weight: 400;\">less likely to have unplanned pregnancies</span></a><span style=\"font-weight: 400;\"> than those with no education, because </span><a href=\"https://journals.plos.org/plosone/article/file?id=10.1371/journal.pone.0220970&type=printable\"><span style=\"font-weight: 400;\">girls are likely to learn about modern contraceptives</span></a><span style=\"font-weight: 400;\"> at school.</span><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">Then, create more paid jobs for women, improve health services so that fewer mothers and infants die, and target family planning education for both men and women. Women with more money tend to have a better chance at avoiding unintended pregnancies, because they’re </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0220970\"><span style=\"font-weight: 400;\">better able to afford family planning services</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Cilliers argues that the best way for Africa to get to that demographic sweet spot is to slow the continent’s population growth.
