"contents": "<span style=\"font-weight: 400;\">On Sunday, President Cyril Ramaphosa </span><a href=\"https://www.youtube.com/watch?v=gycPveEftFs\"><span style=\"font-weight: 400;\">confirmed at the G7 Summit in Cornwall</span></a><span style=\"font-weight: 400;\"> in the UK that the US Food and Drug Administration (FDA) had found that the active substance (main ingredient) with which two million Johnson & Johnson Covid-19 vaccine doses allocated for use in South Africa was made, wasn’t safe for consumption. </span>\r\n\r\n<span style=\"font-weight: 400;\">As a result, South Africa has no J&J doses to administer right now, setting back its </span><a href=\"https://bhekisisa.org/article/2021-05-07-when-will-you-get-your-covid-jab-find-out/\"><span style=\"font-weight: 400;\">vaccine roll-out goals and timeline</span></a><span style=\"font-weight: 400;\"> amid a third Covid-19 wave that’s rapidly filling </span><a href=\"https://www.nicd.ac.za/diseases-a-z-index/covid-19/surveillance-reports/weekly-hospital-surveillance-datcov-update/\"><span style=\"font-weight: 400;\">hospital beds</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The active substance was produced by a US factory in Baltimore, but the doses were stored at the Aspen pharmaceutical company in Gqeberha. </span><a href=\"https://www.aspenpharma.com/2020/11/02/aspen-announces-agreement-with-johnson-johnson-to-manufacture-investigational-covid-19-vaccine-candidate/\"><span style=\"font-weight: 400;\">Aspen has been contracted by J&J </span></a><span style=\"font-weight: 400;\">to “fill and finish” – to import vaccine doses made in bulk, mix them with an inactive substance, then put them in vials and label the jabs. </span>\r\n\r\n<span style=\"font-weight: 400;\">The setback comes on the heels of another hold-up in April when South Africa had to pause the roll-out of J&J jabs via the </span><a href=\"https://www.samrc.ac.za/news/sisonke-together-study-open-label-pragmatic-real-world-phase-3b-clinical-trial-investigational\"><span style=\"font-weight: 400;\">Sisonke</span></a><span style=\"font-weight: 400;\"> implementation trial. The South African Health Products Regulatory Authority (SAHPRA) was </span><a href=\"https://www.sahpra.org.za/news-and-updates/guidance-on-the-use-of-the-janssen-ad26-cov2-s-covid-19-vaccine-in-pregnant-and-lactating-women/\"><span style=\"font-weight: 400;\">reviewing data relating to blood clot-related side-effects</span></a><span style=\"font-weight: 400;\"> associated with the jab. Although the occurrence of the blood clots is extremely rare, the regulator wanted to ensure the shots were safe to use and therefore temporarily stopped the study for two weeks while assessing the information. </span>\r\n\r\n<span style=\"font-weight: 400;\">But what exactly happened this time around? We look at why our two million J&J doses are unsuitable for use, when we’ll get new ones and what the implications are for our vaccine roll-out. </span>\r\n\r\n<b>What happened to our J&J vaccines?</b>\r\n\r\n<span style=\"font-weight: 400;\">South Africa has procured </span><a href=\"https://www.janssen.com/southafrica/johnson-johnson-single-dose-covid-19-vaccine-granted-registration-south-african-health-products\"><span style=\"font-weight: 400;\">31.2 million J&J vaccines.</span></a><span style=\"font-weight: 400;\"> Two million of these (a first batch of 1.1 million and a second of 900,000) had been manufactured by April/May and were ready for distribution. The jabs were kept at Aspen’s Gqeberha facility, as the company handled the final packaging. </span>\r\n\r\n<span style=\"font-weight: 400;\">The active vaccine substance Aspen used to mix with an inactive substance and then divide the vaccine into vials, was made at Emergent BioSolutions in Baltimore. In March, the FDA inspected the company’s production facility and </span><a href=\"https://www.fda.gov/media/147762/download?utm_medium=email&utm_source=govdelivery\"><span style=\"font-weight: 400;\">found the conditions to be substandard</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The regulator established that a batch of 15 million doses of J&J’s active substance had been contaminated with some of the active substance of another vaccine (AstraZeneca) which was manufactured in the same factory. The FDA therefore instructed Emergent BioSolutions to discard those doses. </span>\r\n\r\n<span style=\"font-weight: 400;\">As a precautionary measure, the FDA </span><a href=\"https://content.govdelivery.com/accounts/USFDA/bulletins/2ce64ec\"><span style=\"font-weight: 400;\">stopped all J&J production at Emergent BioSolutions</span></a><span style=\"font-weight: 400;\"> and decided to also investigate four batches of active substance which were produced during the same period as the contaminated batch. </span>\r\n\r\n<span style=\"font-weight: 400;\">But by that time some of the active substance doses had already been distributed to contractors such as Aspen to use to produce finished jabs. The FDA therefore told J&J to tell such companies to not release the vaccines until the regulator had concluded its investigation.</span>\r\n\r\n<span style=\"font-weight: 400;\">The results of that investigation, as far as they pertain to South Africa’s doses, were shared with SAHPRA and Aspen over the weekend. The </span><a href=\"https://www.fda.gov/news-events/press-announcements/fda-takes-steps-increase-availability-covid-19-vaccine\"><span style=\"font-weight: 400;\">FDA cleared two batches, with special conditions</span></a><span style=\"font-weight: 400;\">, and found “several other batches are not suitable for use”. Some batches were still under review. </span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa’s two million J&J jabs were made with active substance from batches the FDA found unsuitable for use. Over the weekend, SAHPRA reviewed the data provided by the FDA and decided to “</span><a href=\"https://www.sahpra.org.za/press-releases/sahpra-update-covid-19-vaccine-janssen-fda-developments/\"><span style=\"font-weight: 400;\">not release the vaccine produced using the drug substance batches that were not suitable</span></a><span style=\"font-weight: 400;\">”.</span>\r\n\r\n<span style=\"font-weight: 400;\">Aspen CEO Stephen Saad </span><a href=\"https://www.youtube.com/watch?v=MQYBQ9gdMzo\"><span style=\"font-weight: 400;\">told eNCA on Monday night the FDA found that none of the doses used in South Africa were contaminated</span></a><span style=\"font-weight: 400;\">, but that the vaccines made with them shouldn’t be used, as a precautionary measure.</span>\r\n\r\n<b>Why couldn’t we just release the jabs, why did we have to wait for the FDA’s data? </b>\r\n\r\n<span style=\"font-weight: 400;\">First, Emergent BioSolutions falls under the FDA’s jurisdiction, since the active substance they produce is made in the US. It is the FDA’s responsibility – just like it’s SAHPRA’s responsibility for medicine produced in South Africa – to ensure good manufacturing practices are followed by drug companies in the US. </span>\r\n\r\n<span style=\"font-weight: 400;\">Although South Africa’s two million J&J doses were mixed and packaged here, the active substance used was produced in the US. So it’s the FDA’s responsibility to ensure the product used by Aspen is of a good standard. Moreover, the doses don’t belong to Aspen, but to J&J, since Aspen is only a manufacturer contracted by J&J to complete the production process of the jabs. </span>\r\n\r\n<span style=\"font-weight: 400;\">SAHPRA </span><a href=\"https://www.sahpra.org.za/press-releases/sahpra-update-on-the-covid-19-vaccine-janssen-gmp-concerns/\"><span style=\"font-weight: 400;\">registered J&J’s vaccine in South Africa on 31 March</span></a><span style=\"font-weight: 400;\">, but with conditions, including that it be “manufactured under conditions of good manufacturing practice as determined by SAHPRA and aligned with global best practice”.</span>\r\n\r\n<span style=\"font-weight: 400;\">Almost all vaccines used in South Africa are made in other countries, so SAHPRA always needs documentation from the regulatory authority in the country of manufacture that confirms the quality of the jabs. The regulator explains that “through an arrangement called reliance” it has “formal relationships with certain regulatory authorities that allow for the confidential exchange of information”. </span>\r\n\r\n<span style=\"font-weight: 400;\">This allows SAHPRA to access reports produced by other regulatory agencies on inspections, testing and various other aspects of manufacturing and testing compliance. SAHPRA has such a reliance arrangement with the FDA. So, our regulator relies “on lot release testing undertaken by these partnering agencies, which involves reviewing manufacturing documentation and performing certain quality tests to ensure compliance with the approved and registered requirements”.</span>\r\n\r\n<b>Are the J&J jabs used by the Sisonke trial contaminated? </b>\r\n\r\n<span style=\"font-weight: 400;\">Professor Linda-Gail Bekker, one of the co-lead investigators on the trial, says all the jabs used in the study were manufactured at J&J’s Leiden factory in The Netherlands – </span><a href=\"https://twitter.com/LindaGailBekker/status/1403413363569446916\"><span style=\"font-weight: 400;\">so none was manufactured by Emergent BioSolutions</span></a><span style=\"font-weight: 400;\"> in Baltimore where contamination was an issue. </span>\r\n\r\n<span style=\"font-weight: 400;\">On Monday night, J&J confirmed that “doses previously distributed to South Africa as part of the Sisonke study have met all product specifications and requirements for quality, safety and efficacy”.</span>\r\n\r\n<span style=\"font-weight: 400;\">The 479,768 healthcare workers who received J&J shots in the Sisonke study have therefore </span><a href=\"https://twitter.com/LindaGailBekker/status/1404383651316568065\"><span style=\"font-weight: 400;\">all had safe and uncontaminated jabs</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<b>Was only South Africa’s consignment made with contaminated ingredients? </b>\r\n\r\n<span style=\"font-weight: 400;\">No, many more millions of doses have been contaminated. </span><a href=\"https://www.nytimes.com/2021/06/11/us/politics/johnson-covid-vaccine-emergent.html\"><span style=\"font-weight: 400;\">The New York Times reports</span></a><span style=\"font-weight: 400;\"> that the FDA told J&J that 60 million doses couldn’t be used “because of possible contamination”, although J&J told </span><i><span style=\"font-weight: 400;\">Bhekisisa</span></i><span style=\"font-weight: 400;\"> it couldn’t confirm this number. </span>\r\n\r\n<span style=\"font-weight: 400;\">However, Aspen also “fills and finishes” doses for</span><a href=\"https://www.aspenpharma.com/2021/03/29/stephen-saad-aspen-chief-executive-address-to-president-ramaphosa-and-his-delegation-at-aspens-flagship-manufacturing-site-in-gqberha-eastern-cape/\"><span style=\"font-weight: 400;\"> 220,000 million jabs that J&J is providing to the Africa Union’s African Vaccine Acquisition Trust</span></a><span style=\"font-weight: 400;\"> for distribution to countries on the continent. </span>\r\n\r\n<span style=\"font-weight: 400;\">Aspen senior executive Stavros </span><span style=\"font-weight: 400;\">Nicolaou confirmed to </span><i><span style=\"font-weight: 400;\">Bhekisisa</span></i><span style=\"font-weight: 400;\"> that there are “several millions of other doses” at Aspen which have also been affected by the FDA’s decision. </span>\r\n\r\n<span style=\"font-weight: 400;\">Several European countries </span><a href=\"https://www.ema.europa.eu/en/news/covid-19-vaccine-janssen-authorities-eu-take-steps-safeguard-vaccine-quality\"><span style=\"font-weight: 400;\">will also not be using vaccines </span></a><span style=\"font-weight: 400;\">with an active substance produced by Emergent BioSolutions during the same time as the batch that the FDA found to be contaminated in March.</span>\r\n\r\n<b>When will we get new J&J jabs? </b>\r\n\r\n<span style=\"font-weight: 400;\">J&J will have to replace the shots South Africa can’t use. </span><a href=\"https://www.youtube.com/watch?v=MQYBQ9gdMzo\"><span style=\"font-weight: 400;\">On Monday night on </span><i><span style=\"font-weight: 400;\">eNCA</span></i><span style=\"font-weight: 400;\">, Saad said</span></a><span style=\"font-weight: 400;\"> the two million doses will be replaced by J&J within the next two weeks.</span>\r\n\r\n<a href=\"https://www.sharenet.co.za/feeds/IR/sens_view.php?key=01814189cd2606403d6055060b28f318&link=20210614142500@28\"><span style=\"font-weight: 400;\">In a statement on Monday</span></a><span style=\"font-weight: 400;\">, Aspen said </span><span style=\"font-weight: 400;\">300,000 J&J doses will arrive in South Africa “within days”. Saad told </span><i><span style=\"font-weight: 400;\">eNCA</span></i><span style=\"font-weight: 400;\"> they are likely to arrive within the next two days. The Health Department says these shots are specifically for teachers and other staff at primary and high schools. </span>\r\n\r\n<span style=\"font-weight: 400;\">J&J’s medical director in South Africa, Abeda Williams, told </span><i><span style=\"font-weight: 400;\">Bhekisisa</span></i><span style=\"font-weight: 400;\"> that these doses have been made with active substance that comes from the two batches that the FDA has cleared with special conditions. The requirement is that J&J and Emergent BioSolutions agree that “</span><a href=\"https://www.fda.gov/news-events/press-announcements/fda-takes-steps-increase-availability-covid-19-vaccine\"><span style=\"font-weight: 400;\">the FDA may share relevant information about the manufacture of the batches, under an appropriate confidentiality agreement, with the regulatory authorities of the countries in which the vaccine may be used”</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Williams has confirmed that this data was supplied to SAHPRA, and that the regulator reviewed the information and approved the jabs for use in South Africa. SAHPRA confirmed this in a statement on Sunday, saying the active substance used in the jabs “</span><a href=\"https://www.sahpra.org.za/press-releases/sahpra-update-covid-19-vaccine-janssen-fda-developments/\"><span style=\"font-weight: 400;\">meets the requirements and will subsequently be released and shipped to South Africa</span></a><span style=\"font-weight: 400;\">”. </span>\r\n\r\n<span style=\"font-weight: 400;\">The 300,000 doses will arrive in the country as finished vaccines, so Aspen won’t need to package them. The jabs will, however, first need to be quality checked by the National Control Lab at the University of the Free State, according to Dr Nicholas Crisp, the deputy director-general in the Health Department who is managing the distribution of Covid vaccines. The quality checking is a routine procedure that all vaccines arriving in the country undergo – it’s not a special procedure followed because the shots were produced by Emergent BioSolutions.</span>\r\n\r\n<span style=\"font-weight: 400;\">Saad says Aspen will release one million J&J doses in the week of 21 June, probably on Monday. Aspen started to “fill and finish” these jabs about a month ago, with the active substance supplied by J&J’s factory in Leiden. These shots have already been quality tested, so they wouldn’t have to go to the National Control Lab first and will therefore be flown to OR Tambo International Airport in Johannesburg as soon as they’re released and then be distributed to provinces. Aspen said </span><a href=\"http://www.itradedata.co.za/senss.asp?id=387707\"><span style=\"font-weight: 400;\">it will release more doses in July</span></a><span style=\"font-weight: 400;\"> (for South Africa and other African countries). </span>\r\n\r\n<span style=\"font-weight: 400;\">Aspen said J&J will also be “delivering substantial quantities of compliant, finished vaccines to South Africa to replace the lost stock” within the next few weeks (these will be produced with active substance made in J&J’s Leiden factory). </span>\r\n\r\n<b>Will South Africa have to pay to replace the unusable jabs? </b>\r\n\r\n<span style=\"font-weight: 400;\">Neither the South African government nor Aspen would have to cover any costs to replace the jabs, since it’s not their fault that the shots have to be destroyed. </span><a href=\"https://www.youtube.com/watch?v=MQYBQ9gdMzo\"><span style=\"font-weight: 400;\">Saad says the cost will be covered</span></a><span style=\"font-weight: 400;\"> by either J&J, Emergent BioSolutions or the companies’ respective insurers. </span>\r\n\r\n<b>Will we reach our original vaccination goals? </b>\r\n\r\n<span style=\"font-weight: 400;\">South Africa will almost certainly not reach its goal of having 90% of people of 60 and older vaccinated by the end of June. Crisp told </span><i><span style=\"font-weight: 400;\">eNCA</span></i><span style=\"font-weight: 400;\"> that, if we continue to vaccinate at our current pace, we will only have 52% of the population group vaccinated by the end of the month. </span>\r\n\r\n<span style=\"font-weight: 400;\">The main reason for this is that we don’t have enough jabs to speed up our vaccination campaign. Crisp says that because we’ve only had Pfizer shots to use since the start of our mass roll-out on 17 May, we haven’t been able to scale up to more than 80,000 vaccinations per day, as we’ll risk running out of jabs. We could administer double the number of doses if we had access to the J&J shots we were promised, he said. </span>\r\n\r\n<span style=\"font-weight: 400;\">But with two million J&J shots that Saad says will be delivered within the next two weeks, we will be in a position to catch up – eventually. The most important thing, according to former co-chair of South Africa’s Covid-19 Ministerial Advisory Committee, Professor Salim Abdool Karim, is that we get Phase 2 of the roll-out (people older than 40 and those living in congregate settings) done by December, before a potential fourth wave hits: “Those are the people who are most likely to be hospitalised or die.
