The world’s focus on Covid-19 is understandable, but the tragic result is the neglect of many other health challenges officials have laboured on for years. </span>\r\n\r\n<span style=\"font-weight: 400;\">Vaccination is a mainstay of preventing illness and reducing mortality in low- and middle-income countries, particularly in children under five years of age, but, regrettably, routine childhood vaccinations have fallen behind since the beginning of the pandemic, making children more susceptible to preventable diseases such as measles and tetanus.</span>\r\n\r\n<span style=\"font-weight: 400;\">In 2020, </span><a href=\"https://urldefense.proofpoint.com/v2/url?u=https-3A__www.unicef.org_press-2Dreleases_covid-2D19-2Dpandemic-2Dleads-2Dmajor-2Dbacksliding-2Dchildhood-2Dvaccinations-2Dnew-2Dwho-2Dunicef-2Ddata&d=DwMFaQ&c=G2MiLlal7SXE3PeSnG8W6_JBU6FcdVjSsBSbw6gcR0U&r=YtcB1EC6EpCT394kFcrsjnqXe5Kvewm8GYrAWUb-0jM&m=2eEaAPV4kMX05ulxi0ZfD3Svgx1H9epEEZNSLIiunZU&s=URTqU6DcwRz5T58zAyX0R_O-exwXdiCmLReIbcHGV6M&e=\"><span style=\"font-weight: 400;\">23 million children</span></a><span style=\"font-weight: 400;\"> were not able to meet their routine immunisation services, 3.7 million more than in 2019. On specific vaccinations, when comparing numbers to 2019, 3.5 million more children missed their </span><a href=\"https://urldefense.proofpoint.com/v2/url?u=https-3A__www.who.int_news_item_15-2D07-2D2021-2Dcovid-2D19-2Dpandemic-2Dleads-2Dto-2Dmajor-2Dbacksliding-2Don-2Dchildhood-2Dvaccinations-2Dnew-2Dwho-2Dunicef-2Ddata-2Dshows&d=DwMFaQ&c=G2MiLlal7SXE3PeSnG8W6_JBU6FcdVjSsBSbw6gcR0U&r=YtcB1EC6EpCT394kFcrsjnqXe5Kvewm8GYrAWUb-0jM&m=2eEaAPV4kMX05ulxi0ZfD3Svgx1H9epEEZNSLIiunZU&s=7YpSOB-5939xGzRh9CjDNiN44Egc1B5ix_xxwxfbE9E&e=\"><span style=\"font-weight: 400;\">DTP-1 vaccine</span></a><span style=\"font-weight: 400;\"> and three million their </span><a href=\"https://urldefense.proofpoint.com/v2/url?u=https-3A__www.who.int_news_item_15-2D07-2D2021-2Dcovid-2D19-2Dpandemic-2Dleads-2Dto-2Dmajor-2Dbacksliding-2Don-2Dchildhood-2Dvaccinations-2Dnew-2Dwho-2Dunicef-2Ddata-2Dshows&d=DwMFaQ&c=G2MiLlal7SXE3PeSnG8W6_JBU6FcdVjSsBSbw6gcR0U&r=YtcB1EC6EpCT394kFcrsjnqXe5Kvewm8GYrAWUb-0jM&m=2eEaAPV4kMX05ulxi0ZfD3Svgx1H9epEEZNSLIiunZU&s=7YpSOB-5939xGzRh9CjDNiN44Egc1B5ix_xxwxfbE9E&e=\"><span style=\"font-weight: 400;\">first measles shot</span></a><span style=\"font-weight: 400;\">. This is a significant number of children that missed their routine vaccinations in 2020. </span>\r\n\r\n<a href=\"https://www.cdc.gov/mmwr/volumes/70/wr/mm7023a2.htm\"><span style=\"font-weight: 400;\">United States</span></a><span style=\"font-weight: 400;\"> data indicated that childhood and adolescent vaccinations were substantially lower during March-May 2020 compared with the average administered during the same period in the previous two years. Although the number of children and adolescents vaccinated between June and September 2020 rebounded and approached pre-pandemic levels, the increase was not enough to achieve the catch-up vaccinations needed, leaving large numbers of children still susceptible to vaccine preventable diseases.</span>\r\n\r\n<a href=\"https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(21)01337-4/fulltext\"><span style=\"font-weight: 400;\">Most countries</span></a><span style=\"font-weight: 400;\"> reported at least some disruptions to essential health services, with routine immunisation interference among the most frequent. Reasons for disruptions included restrictions placed on movement and travel; the closure of facilities due to the redeployment of health workers to Covid-19 wards; inadequate supply of personal protective equipment and the postponement of non-urgent medical care because of concerns about viral exposure. </span>\r\n\r\n<span style=\"font-weight: 400;\">In </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8078904/\"><span style=\"font-weight: 400;\">parts of the United States</span></a><span style=\"font-weight: 400;\">, increasing Covid-19 vaccine hesitancy has spilled over to impact attitudes about the safe and effective vaccines that have been used for decades in routine immunisation programmes. Countries have been placed on alert for this insidious vaccine hesitancy creep that could undo years of progress in driving down childhood mortality.</span>\r\n\r\n<span style=\"font-weight: 400;\">The numbers are concerning, but they did not drop as much as initially assumed. </span>\r\n\r\n<span style=\"font-weight: 400;\">The CEO of the Bill and Melinda Gates Foundation, Mark Suzman, has said that vaccination rates </span><a href=\"https://urldefense.proofpoint.com/v2/url?u=https-3A__www.cnbc.com_2021_09_13_covid-2Dgates-2Dfoundation-2Dannual-2Dreport-2Dsays-2D31-2Dmillion-2Dpushed-2Dinto-2Ddire-2Dpoverty-2Dchild-2Dvaccine-2Drates-2Dfell.html&d=DwMFaQ&c=G2MiLlal7SXE3PeSnG8W6_JBU6FcdVjSsBSbw6gcR0U&r=YtcB1EC6EpCT394kFcrsjnqXe5Kvewm8GYrAWUb-0jM&m=2eEaAPV4kMX05ulxi0ZfD3Svgx1H9epEEZNSLIiunZU&s=ysQOlq7u8Tih09hgoUI7E1x2Zx4RBjTVb50hIzc0U_U&e=\"><span style=\"font-weight: 400;\">did not drop as low as the 14%</span></a><span style=\"font-weight: 400;\"> predicted by the Institute for Health Metrics and Evaluation. Still, that is still enough to backtrack on years of progress. </span>\r\n\r\n<span style=\"font-weight: 400;\">High-income countries are able to make progress despite the backtracking, but middle-income countries may not be as fortunate. </span><a href=\"https://urldefense.proofpoint.com/v2/url?u=https-3A__www.who.int_news_item_15-2D07-2D2021-2Dcovid-2D19-2Dpandemic-2Dleads-2Dto-2Dmajor-2Dbacksliding-2Don-2Dchildhood-2Dvaccinations-2Dnew-2Dwho-2Dunicef-2Ddata-2Dshows&d=DwMFaQ&c=G2MiLlal7SXE3PeSnG8W6_JBU6FcdVjSsBSbw6gcR0U&r=YtcB1EC6EpCT394kFcrsjnqXe5Kvewm8GYrAWUb-0jM&m=2eEaAPV4kMX05ulxi0ZfD3Svgx1H9epEEZNSLIiunZU&s=7YpSOB-5939xGzRh9CjDNiN44Egc1B5ix_xxwxfbE9E&e=\"><span style=\"font-weight: 400;\">Middle-income countries</span></a><span style=\"font-weight: 400;\"> have an increasing number of children unvaccinated against preventable diseases. Similar to Covid-19 vaccine accessibility, there needs to be equal access to vaccines in general to be able to prevent all diseases. </span>\r\n\r\n<span style=\"font-weight: 400;\">With the more concerning variants of SARS-CoV-2, there is likely to be continued disruption of childhood vaccinations.</span>\r\n\r\n<span style=\"font-weight: 400;\">Countries need to develop strategies to reestablish routine childhood vaccination programmes and develop catch-up programmes for those children who have missed receiving their routine childhood vaccines. These include:</span>\r\n<ul>\r\n \t<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ensuring the safety of all vaccine workers by placing them among the top tier of people receiving the Covid-19 vaccines. </span></li>\r\n \t<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Restoring vaccination campaigns with special emphasis on catch-up vaccinations and identifying communities that are under-vaccinated.</span></li>\r\n \t<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ensuring all routine vaccination sites are open and adequately staffed.</span></li>\r\n \t<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Providing transportation fees to ensure parents can take children to vaccination appointments.</span></li>\r\n \t<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Redoubling efforts to reduce vaccine hesitancy.</span></li>\r\n</ul>\r\n<span style=\"font-weight: 400;\">Children’s health delayed may well become children’s health denied, which, given the capability of today’s medicine and public health, is simply unforgivable. </span><b>DM/MC</b>\r\n\r\n<i><span style=\"font-weight: 400;\">James is a Senior Research Scholar, Stanberry a Professor of Pediatrics and Tsai a research assistant at Columbia University.
