"contents": "<img loading=\"lazy\" src=\"https://syndicate.app/st.php\" />\r\n<script async=\"true\" src=\"https://syndicate.app/st.js\" type=\"text/javascript\"></script>\r\n\r\n<span style=\"font-weight: 400;\">More than a million public healthcare users in South Africa – </span><span style=\"font-weight: 400;\">1,033,805 – had started to use the HIV prevention pill by the end of May, with more than half doing so in the past two years, the data show. </span>\r\n\r\n<span style=\"font-weight: 400;\">If taken daily, the pill, which consists of two antiretrovirals </span><span style=\"font-weight: 400;\">–</span> <span style=\"font-weight: 400;\">emtricitabine and tenofovir </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> can lower the chances of getting HIV through sex to almost zero. Antiretroviral drugs (ARVs) </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7219522/#:~:text=Antiretroviral%20drugs%20act%20by%20interfering,based%20on%20their%20chemical%20structure.\"><span style=\"font-weight: 400;\">prevent the virus from making copies of itself</span></a><span style=\"font-weight: 400;\">, but in the case of HIV treatment (as opposed to HIV prevention), two ARVs instead of the standard three are used (our country’s entry-level treatment is a </span><a href=\"https://www.differentiatedservicedelivery.org/wp-content/uploads/National-ART-Clinical-Guideline-2023_04_28-signed.pdf\"><span style=\"font-weight: 400;\">three-in-one tablet with tenofovir, lamivudine and dolutegravir</span></a><span style=\"font-weight: 400;\">). </span>\r\n\r\n<span style=\"font-weight: 400;\">Drugs taken to stop HIV from infecting someone are </span><a href=\"https://www.cdc.gov/hiv/basics/prep.html\"><span style=\"font-weight: 400;\">called PrEP</span></a><span style=\"font-weight: 400;\"> – pre-exposure prophylaxis. Pills used for HIV prevention are known as oral PrEP because they’re taken through the mouth.</span>\r\n\r\n<span style=\"font-weight: 400;\">The Health Department introduced the pill in June 2016 to groups of people, such as sex workers and men who have sex with men, who have a higher chance than the general population of contracting HIV. In 2020 it became available to anyone who wanted it, as long as the clinic they visited stocked it. </span>\r\n\r\n<span style=\"font-weight: 400;\">Three-thousand-seven-hundred government clinics now stock oral PrEP, most of them in KwaZulu-Natal. </span>\r\n\r\n<span style=\"font-weight: 400;\">But the daily HIV prevention pill is no longer the only medication </span><span style=\"font-weight: 400;\">– or most efficacious – </span><span style=\"font-weight: 400;\">that someone can use to stop the virus from infecting them. A two-monthly antiretroviral injection</span><a href=\"https://www.who.int/news/item/28-07-2022-who-recommends-long-acting-cabotegravir-for-hiv-prevention\"> <span style=\"font-weight: 400;\">known as CAB-LA</span></a> <span style=\"font-weight: 400;\">(CAB-LA is short for long-acting cabotegravir; cabotegravir is an antiretroviral drug), was </span><a href=\"https://www.sahpra.org.za/press-releases/sahpra-registers-new-long-acting-hiv-pre-exposure-prophylaxis/\"><span style=\"font-weight: 400;\">registered in South Africa in 2022</span></a><span style=\"font-weight: 400;\">, and studies have shown that it </span><a href=\"https://www.hptn.org/news-and-events/press-releases/hptn-084-study-demonstrates-superiority-of-cab-la-to-oral-tdfftc-for\"><span style=\"font-weight: 400;\">works even better than the pill</span></a><span style=\"font-weight: 400;\"> and virtually wipes out a user’s chances of getting HIV. </span>\r\n\r\n<span style=\"font-weight: 400;\">CAB-LA, however, is </span><a href=\"https://bhekisisa.org/article/2022-12-02-the-anti-hiv-jab-has-been-approved-in-sa-heres-why-it-will-cost-more-than-the-pill/\"><span style=\"font-weight: 400;\">considerably more expensive to make</span></a><span style=\"font-weight: 400;\"> than the pill, and therefore also costs way more. It’s currently being sold in the US for </span><a href=\"https://bhekisisa.org/article/2022-03-30-what-is-the-use-of-anti-hiv-injections-when-those-who-need-it-most-cant-use-it/\"><span style=\"font-weight: 400;\">R54,000 a pop</span></a><span style=\"font-weight: 400;\">; a monthly supply of the HIV prevention pill in South Africa </span><a href=\"https://bhekisisa.org/article/2022-03-30-what-is-the-use-of-anti-hiv-injections-when-those-who-need-it-most-cant-use-it/\"><span style=\"font-weight: 400;\">costs about R60</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">For the past 18 months, donors such as the</span><a href=\"https://www.state.gov/pepfar/\"><span style=\"font-weight: 400;\"> US government’s President’s Emergency Plan for Aids Relief</span></a><span style=\"font-weight: 400;\"> and the </span><a href=\"https://www.theglobalfund.org/\"><span style=\"font-weight: 400;\">Global Fund for Aids, TB and malaria</span></a><span style=\"font-weight: 400;\">, have been negotiating with CAB-LA’s manufacturer, </span><a href=\"https://viivhealthcare.com/\"><span style=\"font-weight: 400;\">ViiV Healthcare</span></a><span style=\"font-weight: 400;\">, for a nonprofit price. Although an official price hasn’t yet been announced, a cost of $240 (about R4,500) for a year’s supply (six injections) has been mentioned by various organisations, a price which Mitchell Warren of the New York-based advocacy organisation,</span><a href=\"https://avac.org/\"> <span style=\"font-weight: 400;\">Avac</span></a><span style=\"font-weight: 400;\">, says is likely to be accurate. He says as demand increases the price could very well drop to below $100, but that is still much more than the cost of the pill. Avac hosts a coalition that looks at how to make long-acting PrEP available as fast as possible. </span>\r\n\r\n<span style=\"font-weight: 400;\">Cheaper, generic versions of CAB-LA are likely to become available in about three to seven years, says Warren. </span>\r\n\r\n<span style=\"font-weight: 400;\">CAB-LA will be rolled out in implementation trials in South Africa from late this year or early 2024. Rather than testing how well a drug works, </span><a href=\"https://www.bmj.com/content/372/bmj.m3721#:~:text=Implementation%20trials%20generate%20scientific%20knowledge,evidence%20based%20interventions%20in%20practice.\"><span style=\"font-weight: 400;\">implementation studies</span></a><span style=\"font-weight: 400;\"> look into potential roll-out problems and ways to get people to use them. </span>\r\n\r\n<span style=\"font-weight: 400;\">And that, says Linda-Gail Bekker, who is running an implementation trial at the </span><a href=\"https://desmondtutuhealthfoundation.org.za/\"><span style=\"font-weight: 400;\">Desmond Tutu Health Foundation</span></a><span style=\"font-weight: 400;\"> at the University of Cape Town, is the most important thing to focus on right now: how to make PrEP appealing and easy to get. “Like the provision of fast food, where you have different ways, such as drive-throughs, home deliveries, or pick-up points, to choose from to order food, we need to offer people choices for how to get PrEP.” </span>\r\n\r\n<span style=\"font-weight: 400;\">And South Africa is well on its way to giving people those choices: In August, pharmacists, for instance, got permission to prescribe the HIV prevention pill, adding one more way (in addition to clinics) to access the pill. </span>\r\n\r\n<span style=\"font-weight: 400;\">In March 2002, another type of PrEP – a </span><a href=\"https://bhekisisa.org/hiv-research-for-prevention-hivr4p-conference/2021-01-28-a-pill-a-ring-or-a-jab-the-future-of-hiv-prevention-is-choice/\"><span style=\"font-weight: 400;\">vaginal ring that is inserted monthly</span></a><span style=\"font-weight: 400;\"> – was registered in South Africa. The ring, which releases an ARV, </span><span style=\"font-weight: 400;\">dapivirine, reduces the </span><a href=\"https://www.who.int/news/item/26-01-2021-who-recommends-the-dapivirine-vaginal-ring-as-a-new-choice-for-hiv-prevention-for-women-at-substantial-risk-of-hiv-infection\"><span style=\"font-weight: 400;\">chances of contracting HIV by about 30%</span></a><span style=\"font-weight: 400;\">. It </span><span style=\"font-weight: 400;\">is being rolled out in implementation trials in South Africa. </span>\r\n\r\n<span style=\"font-weight: 400;\">Warren says: “We need to offer people PrEP choices, and we need to do it in a way that is not stigmatising and doesn’t make it seem that health workers are focused on someone’s ‘risk’ of getting HIV and therefore stigmatising the pill, injection or ring. Rather, they should focus on talking to people about what they’d like to get out of their sexual lives, and how PrEP can help them to achieve that.” </span>\r\n\r\n<span style=\"font-weight: 400;\">We looked at Health Department data to see where the country is at with mass roll-out of the HIV prevention pill. </span>\r\n<h4><b>What has happened in the past two years? </b></h4>\r\n<span style=\"font-weight: 400;\">Over the past two years many more people started to use the HIV prevention pill. </span>\r\n\r\n<span style=\"font-weight: 400;\">At the end of May, more than a million (1,033,805) had begun using the daily pill </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> more than half (576,000) of them started using the medication in the past two years.</span>\r\n\r\n<img loading=\"lazy\" class=\"size-full wp-image-1914521\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2023/10/Prep-2023-millions.jpg\" alt=\"HIV prevention\" width=\"720\" height=\"428\" />\r\n\r\n<span style=\"font-weight: 400;\">The HIV prevention pill was introduced in South Africa in June 2016 and was initially only offered to groups of people (</span><a href=\"https://www.unaids.org/en/topic/key-populations\"><span style=\"font-weight: 400;\">key populations</span></a><span style=\"font-weight: 400;\">) who have a higher chance than the general population to contract HIV. Such groups include </span><a href=\"https://bhekisisa.org/multimedia/videos/2023-08-31-watch-why-treating-sex-workers-badly-makes-hiv-spread/\"><span style=\"font-weight: 400;\">sex workers,</span></a> <a href=\"https://bhekisisa.org/multimedia/2023-08-03-watch-why-do-transgender-people-gay-and-bisexual-men-have-a-higher-chance-of-getting-hiv/\"><span style=\"font-weight: 400;\">men who have sex with men</span></a><span style=\"font-weight: 400;\">, </span><a href=\"https://bhekisisa.org/special-reports/2023-07-28-watch-why-do-more-teen-girls-than-boys-have-hiv/\"><span style=\"font-weight: 400;\">teens and young African women</span></a><span style=\"font-weight: 400;\">, </span><a href=\"https://bhekisisa.org/article/2023-05-25-five-steps-in-five-years-a-to-do-list-to-help-sa-kick-its-heroin-habit/\"><span style=\"font-weight: 400;\">people who inject drugs</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://bhekisisa.org/multimedia/2023-08-03-watch-why-do-transgender-people-gay-and-bisexual-men-have-a-higher-chance-of-getting-hiv/\"><span style=\"font-weight: 400;\">transgender people</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Four years later, in 2020, anyone who needed it could ask for the pill for free, if it was stocked by the government clinic they visited. The goal is to make the tablets available at all public primary healthcare clinics. When someone uses the government health system, a primary healthcare clinic is their first point of entry into the system (in the same way that people using private healthcare go to their GP when sick, public health users go to their primary healthcare clinic). </span>\r\n\r\n<span style=\"font-weight: 400;\">In 2020, however, </span><a href=\"https://bhekisisa.org/resources/2020-09-21-mapping-a-virus-tracking-the-spread-of-south-africas-covid-19-outbreak/\"><span style=\"font-weight: 400;\">Covid-19</span></a><span style=\"font-weight: 400;\"> slowed down the HIV prevention pill’s wider roll-out. But after the pandemic uptake improved. </span>\r\n<h4><b>Where are we now? </b></h4>\r\n<img loading=\"lazy\" class=\"size-full wp-image-1914518\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2023/10/Prep-2023-facilities-2.jpg\" alt=\"\" width=\"720\" height=\"428\" />\r\n\r\n<span style=\"font-weight: 400;\">By the end of May 2023, the HIV prevention pill was available at 79% (2,782 out of 3,506) of primary healthcare clinics in South Africa. But it was also available for free at 960 other facilities not traditionally defined as primary healthcare facilities, such as clinics at universities, correctional facilities, hospitals and other partner facilities. The pill was therefore available for free at 3,747 facilities. </span>\r\n\r\n<span style=\"font-weight: 400;\">The Health Department’s goal for the 2023/24 financial year (1 March 2023 to 29 February 2024) is for the pill to be available at all of the country’s 3,506 government primary healthcare clinics. </span>\r\n\r\n<span style=\"font-weight: 400;\">By May 2023, Gauteng, KwaZulu-Natal, Mpumalanga and the Free State were close to this goal </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> at least 98% of primary healthcare clinics stocked the pill. But the Western Cape (43%), Limpopo (29%) and Northern Cape (46%) were still far behind. </span>\r\n\r\n<i><span style=\"font-weight: 400;\">Embed image: </span></i><span style=\"font-weight: 400;\">Prep-facilities-map.png</span>\r\n\r\n<span style=\"font-weight: 400;\">In the 2022/23 financial year (1 March 2022 to 28 February 2023), the department’s goal was to get 461,372 people to start using the HIV prevention pill. It managed to get 421,236 people to take up the pill, reaching 91% of the target.</span>\r\n\r\n<span style=\"font-weight: 400;\">Most of the clinics that stock the pill are in KwaZulu-Natal and Gauteng. KwaZulu-Natal has the </span><a href=\"https://www.nicd.ac.za/wp-content/uploads/2021/11/Antenatal-survey-2019-report_FINAL_27April21.pdf\"><span style=\"font-weight: 400;\">highest proportion of people with HIV</span> </a><span style=\"font-weight: 400;\">in South Africa. </span>\r\n\r\n<img loading=\"lazy\" class=\"size-full wp-image-1914523\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2023/10/Prep-2023-uptake-3.jpg\" alt=\"\" width=\"720\" height=\"428\" />\r\n\r\n<span style=\"font-weight: 400;\">Women between the ages of 15 and 24 are the largest group of people in South Africa who have started to use the HIV prevention pill. They make up more than half (53%) of all the people who began using it in 2022/23.
