"contents": "<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">Mothers whose children’s HIV status is disclosed in their Road to Health Booklets see it as a violation of their privacy and their right to dignity, respect and freedom.</span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">HIV-positive </span></span></span><a href=\"https://health-e.org.za/multimedia/being-a-young-hiv-positive-mother-aphiwes-story/\"><span style=\"color: #0000ff;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><u>mother</u></span></span></span></a><span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"> of a two-year-old, Thandi Zondo* </span></span></span><span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">says she finds the practice unconstitutional.</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“</span><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">What is being done is disturbing because if you are not prepared to disclose your status, you shouldn’t be exposed like this.</span><span style=\"color: #000000;\">”</span></span></span>\r\n\r\n<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">Zondo says it</span><span style=\"color: #000000;\">’s the reason most mothers, </span><span style=\"color: #000000;\">such as herself,</span><span style=\"color: #000000;\"> are not comfortable sharin</span><span style=\"color: #000000;\">g </span><span style=\"color: #000000;\">their chil</span><span style=\"color: #000000;\">dren’s health</span><span style=\"color: #000000;\"> cards even though</span><span style=\"color: #000000;\"> they are</span><span style=\"color: #000000;\"> needed in creches and by Home Affairs.</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“</span><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">I do believe that a child’s card has to be confidential; it has to be between the mother and a health worker. But sometimes that isn’t happening. You can’t always be there to take your child for immunisation, especially if you’re a working mother.”</span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">Health workers give this free booklet to every mother after they’ve given birth in both public and private health facilities. It contains an infant’s health information, including immunisations and the health of the baby at every clinic visit. In many public schools, parents are requested to produce their children’s health booklet when registering them for the first time. </span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><b>Two couples, same experience</b></span></span></span>\r\n\r\n<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">Jabu Mwelase* says: “I thought it was only my son’s card, a mistake done </span><span style=\"color: #000000;\">by the </span><span style=\"color: #000000;\">hospital, but when I did the research I found out this is done in all hospitals and clinics. They tick the HIV positive box purposely.”</span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">When she asked a nurse about it, she was told it was for the sake of the child’s health.</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“</span><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">I find it difficult to send my nanny to a clinic with </span><span style=\"color: #000000;\">this booklet</span><span style=\"color: #000000;\"> because my status is revealed so it isn’t private anymore</span><span style=\"color: #000000;\">,” she adds. </span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">When her son is due for his immunisation, Mwelase takes a day’s leave from work so she can take him to the clinic herself.</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“</span><span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">I wish the Department of Health could find another way of writing kids</span></span></span><span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">’</span></span></span><span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"> clinic cards because we’re not all ready to reveal our </span></span></span><a href=\"https://health-e.org.za/2019/07/17/get-a-haircut-while-testing-for-hiv-at-the-tutu-truck/\">HIV</a><span style=\"color: #0000ff;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><u> status</u></span></span></span><span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"> yet</span></span></span><span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">.”</span></span></span>\r\n\r\n<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">Sindi Mhlongo* says even though she is HIV negative, she </span><span style=\"color: #000000;\">has</span><span style=\"color: #000000;\"> seen many mothers’ cards indicating their status. </span><span style=\"color: #000000;\">Mhlongo says women at the</span><span style=\"color: #000000;\"> clinic would borrow one another's’ cards intentionally to see if the HIV positive box was ticked or not.</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">I think the National Department of Health is being ignorant about this and it has been happening for years, but nothing is being done, there is no confidentiality anymore,” says Mhlongo.</span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><b>Between parents, child and healthcare professionals</b></span></span></span>\r\n\r\n<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">The N</span><span style=\"color: #000000;\">ational </span><span style=\"color: #000000;\">Department o</span><span style=\"color: #000000;\">f Health</span><span style=\"color: #000000;\"> Child, Youth and School Health acting chief director Dr Lesley Bamford says they have received a small number of complaints in the past about this issue.</span><span style=\"color: #000000;\"> However </span><span style=\"color: #000000;\">HIV status remains an important cause of death in young children, she says.</span></span></span>\r\n\r\n<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">According to Bamford, i</span><span style=\"color: #000000;\">n order to provide appropriate healthcare for children in South Africa, the healthcare worker </span><span style=\"color: #000000;\">needs</span><span style=\"color: #000000;\"> to know which children are HIV positive so that these children can get the services that they need.</span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">About the mother’s status, she says:</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">The information recorded on the card is included so that the child can receive treatment that is required to ensure that the child does not [get] HIV. We regard this as part of the process of normalising HIV infection. So one is balancing the mother’s right to privacy and the child’s right to receive the care that they need.</span></span><span style=\"font-family: Georgia, sans-serif;\">”</span></span>\r\n\r\n<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">Bamford says all</span><span style=\"color: #000000;\"> records written on the car</span><span style=\"color: #000000;\">d </span><span style=\"color: #000000;\">are confidential between the child’s parents and a healthcare worker</span><span style=\"color: #000000;\">, but this hasn’t stopped some institutions from asking for the card as part of application processes.</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“</span><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">We have worked closely with the Department of Home Affairs to ensure that th</span><span style=\"color: #000000;\">e </span><span style=\"color: #000000;\">Road To Health Booklet is not required for any official purposes, which is the official policy, although there may be cases where officials don’t follow the policy correctly</span><span style=\"color: #000000;\">.”</span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\">Proof of immunisation is also required at school entry, but Bamford says they should only get a copy of the immunisation page, not the entire booklet.</span></span></span>\r\n\r\n<span style=\"color: #000000;\"><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><b>Intention versus reality </b></span></span></span>\r\n\r\n<span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">Public health nurse and HIV specialist Siboniso Nene sa</span><span style=\"color: #000000;\">ys the </span><span style=\"color: #000000;\">aim to record statuses on the clinic card is for the continuation of care</span><span style=\"color: #000000;\">, which</span><span style=\"color: #000000;\"> is good, but may have adverse implications.</span></span></span>\r\n\r\n<span style=\"color: #000000;\">“</span><span style=\"font-family: Georgia, sans-serif;\"><span style=\"font-size: large;\"><span style=\"color: #000000;\">Currently as a country, we have failed to destigmatise HIV.
