"contents": "<span style=\"font-weight: 400;\">South Africa is one of 14 countries that will be</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"> <span style=\"font-weight: 400;\">financially supported by the Africa Centres for Disease Control and Prevention (Africa CDC) and World Health Organization (WHO)</span></a><span style=\"font-weight: 400;\"> from this month until February 2025 to draw up and roll out plans, including vaccination strategies, to fight mpox.</span>\r\n\r\n<span style=\"font-weight: 400;\">The budget for the “</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"><span style=\"font-weight: 400;\">mpox continental preparedness and response plan for Africa</span></a><span style=\"font-weight: 400;\">” is about</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"> <span style=\"font-weight: 400;\">$600-million</span></a><span style=\"font-weight: 400;\">, 53% of which will go towards the mpox responses of 14 of the 55 African Union member states in which cases have escalated, and human-to-human transmission of the virus has become relatively common.</span>\r\n\r\n<span style=\"font-weight: 400;\">Thirteen other countries that will be helped to design and roll out plans are: Democratic Republic of Congo (DRC), Burundi, Central African Republic, Congo, Cameroon, Côte d’Ivoire, Gabon, Liberia, Kenya, Nigeria, Republic of Guinea, Rwanda and Uganda.</span>\r\n\r\n<span style=\"font-weight: 400;\">Most of the rest of the money (45%) will be spent on funding the efforts of partners such as</span><a href=\"https://www.unicef.org/\"> <span style=\"font-weight: 400;\">Unicef,</span></a> <span style=\"font-weight: 400;\">the</span><a href=\"https://southafrica.iom.int/\"> <span style=\"font-weight: 400;\">International Organization for Migration</span></a><span style=\"font-weight: 400;\"> and the</span><a href=\"https://www.ifrc.org/\"> <span style=\"font-weight: 400;\">International Federation of Red Cross</span></a><span style=\"font-weight: 400;\">, to help countries to set up their plans, become better at testing and contact tracing, managing cases and rolling out vaccines.</span>\r\n\r\n<a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"><span style=\"font-weight: 400;\">The budget for South Africa is $45.5-million</span></a><span style=\"font-weight: 400;\"> (R813-million), of which $10.1-million (R178-million) will cover the logistics of rolling out vaccines against mpox. This money can’t be used to buy jabs. The Africa CDC and WHO say vaccines will be funded via in-kind donations and in cases where countries can afford to buy shots, the prices will depend on ongoing negotiations with drugmakers.</span>\r\n\r\n<span style=\"font-weight: 400;\">In August the WHO</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/08/WHO-Director-General-declares-mpox-outbreak-a-public-health-emergency-of-international-concern.pdf\"> <span style=\"font-weight: 400;\">declared</span></a><span style=\"font-weight: 400;\"> mpox a public health emergency of international concern, and in the same month the Africa CDC</span><a href=\"https://africacdc.org/news-item/africa-cdc-declares-mpox-a-public-health-emergency-of-continental-security-mobilizing-resources-across-the-continent/\"> <span style=\"font-weight: 400;\">announced</span></a> <span style=\"font-weight: 400;\">it a public health emergency of continental security.</span>\r\n\r\n<span style=\"font-weight: 400;\">It is the</span><a href=\"https://africacdc.org/news-item/africa-cdc-declares-mpox-a-public-health-emergency-of-continental-security-mobilizing-resources-across-the-continent/\"> <span style=\"font-weight: 400;\">first time</span></a><span style=\"font-weight: 400;\"> the Africa CDC has made such a declaration since its inception in 2017. The WHO’s declaration is the second in two years with regards to mpox – in 2022, it also</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">declared</span></a><span style=\"font-weight: 400;\"> mpox a public health emergency of international concern, after a multicountry outbreak, but announced the emergency</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">over</span></a><span style=\"font-weight: 400;\"> in May 2023. </span>\r\n\r\n<span style=\"font-weight: 400;\">An invitation for vaccine manufacturers to </span><a href=\"https://www.who.int/news/item/09-08-2024-who-invites-mpox-vaccine-manufacturers-to-submit-dossiers-for-emergency-evaluations\"><span style=\"font-weight: 400;\">express their interest in listing their anti-mpox shots for emergency use</span></a><span style=\"font-weight: 400;\"> was announced by the WHO on 7 August. </span>\r\n<h4><b>South Africa now has 25 confirmed mpox cases </b></h4>\r\n<span style=\"font-weight: 400;\">South Africa’s Health Department</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/Health-Department-confirms-one-more-positive-mpox-case-1.pdf\"> <span style=\"font-weight: 400;\">announced another mpox case</span></a><span style=\"font-weight: 400;\"> on Monday – a 38-year old man who was diagnosed at a private medical facility in Cape Town – on 6 September.</span>\r\n\r\n<span style=\"font-weight: 400;\">This brings South Africa’s total number of 2024 cases to 25. Twelve cases were diagnosed in Gauteng, 11 in KwaZulu-Natal and two in the Western Cape. Three have died.</span>\r\n\r\n<span style=\"font-weight: 400;\">The country’s first 2024 case was</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/06/13.05.2024_Minister-Phaahla-calls-for-vigilance-against-Monkey-Pox-disease.pdf\"> <span style=\"font-weight: 400;\">reported</span></a><span style=\"font-weight: 400;\"> on 8 May.</span>\r\n<h4><b>Where and when were South Africa’s mpox cases diagnosed? </b></h4>\r\n<span style=\"font-weight: 400;\">Use the interactive map, created by Bhekisisa’s health reporter, Zano Kunene, to see the details.</span>\r\n<div class=\"flourish-embed flourish-map\" data-src=\"visualisation/18601166\"><script src=\"https://public.flourish.studio/resources/embed.js\"></script></div>\r\n<a href=\"https://www.nicd.ac.za/mpox-outbreak-alert-africas-crisis-and-south-africas-response/\"><span style=\"font-weight: 400;\">None of the country’s cases</span></a><span style=\"font-weight: 400;\"> is the relatively new strain, clade 1b, which seems to be deadlier than other forms of the virus. All of South Africa’s cases were infected with clade 2b, the same type that caused outbreaks in Western Europe and the UK in 2022 and 2023, according to Health Department spokesperson Foster Mohale.</span>\r\n\r\n<span style=\"font-weight: 400;\">Almost all South Africa’s cases were </span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"><span style=\"font-weight: 400;\">among gay and bisexual men</span></a><span style=\"font-weight: 400;\">, most in their twenties or thirties, and many of them were infected with HIV, but </span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"><span style=\"font-weight: 400;\">untreated</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa’s latest case</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/08/Health-Department-calls-for-calm-as-WHO-declares-Mpox-a-public-health-emergency-of-international-concern.pdf\"> <span style=\"font-weight: 400;\">does not have an international travel history</span></a> <span style=\"font-weight: 400;\">and went to the doctor on 4 September after he noticed typical mpox-like blisters on his face, chest and genitals. He wasn’t admitted to hospital, but is in a stable condition and isolating at home.</span>\r\n<h4><b>The differences between clades 1 and 2</b></h4>\r\n<span style=\"font-weight: 400;\">Mpox is caused by the monkeypox virus and there are two known clades:</span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"> <span style=\"font-weight: 400;\">clade 1 and clade 2</span></a><span style=\"font-weight: 400;\">. Mpox starts off with a fever, muscle aches and a sore throat, followed by blister-like bumps on the skin on any part of the body. Most people recover spontaneously, but those with weakened immune systems,</span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"> <span style=\"font-weight: 400;\">such as those with untreated HIV, often fall very ill with the disease, and some die</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Clade 1 has been rampant in the DRC, where mpox has been recorded for more than a decade, but in August cases were also detected in</span><a href=\"https://www.folkhalsomyndigheten.se/the-public-health-agency-of-sweden/communicable-disease-control/disease-information-about-mpox/one-case-of-mpox-clade-i-reported-in-sweden/\"> <span style=\"font-weight: 400;\">Sweden</span></a><span style=\"font-weight: 400;\"> and</span><a href=\"https://www.cdc.gov/media/releases/2024/s0822-mpox-outbreak.html\"> <span style=\"font-weight: 400;\">Thailand</span></a><span style=\"font-weight: 400;\">.</span><a href=\"https://www.ecdc.europa.eu/en/news-events/epidemiological-update-mpox-due-monkeypox-virus-clade-i\"> <span style=\"font-weight: 400;\">Moreover, from 2023 onwards</span></a><span style=\"font-weight: 400;\">, clade 1 cases were found in Kenya, Rwanda, Burundi, the Central African Republic and Uganda; all countries close to the DRC. The cases in Sweden and Thailand had both travelled to African countries where clade 1 cases had been detected.</span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-2357016\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2024/09/Mpox-Map-9-September-2024.jpg\" alt=\"mpox\" width=\"1080\" height=\"1080\" /> <em>Source: Department of Health, 9 September 2024</em></p>\r\n\r\n<span style=\"font-weight: 400;\">The WHO says</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">clade 1 cases in the DRC have increased steadily</span></a><span style=\"font-weight: 400;\"> over the past decade, especially in 2023.</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">At more than 15,600 cases and 537 deaths</span></a><span style=\"font-weight: 400;\">, the numbers reported so far in 2024 are more than last year’s total. </span>\r\n\r\n<span style=\"font-weight: 400;\">Clades 1 and 2 cause the same type of disease, but infection with clade 1 makes people feel sicker and</span><a href=\"https://www.pnas.org/doi/full/10.1073/pnas.2220415120\"> <span style=\"font-weight: 400;\">can lead to more deaths</span></a><span style=\"font-weight: 400;\">. Although symptoms of a clade 2 infection are milder,</span><a href=\"https://www.pnas.org/doi/full/10.1073/pnas.2220415120\"> <span style=\"font-weight: 400;\">virus of this type spreads easier</span></a><span style=\"font-weight: 400;\">. Clade 2b cases have mainly spread among gay and bisexual men with HIV, but who are not on treatment, or don’t use their treatment correctly. Clade 1 cases, however,</span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2491159/\"> <span style=\"font-weight: 400;\">can also spread</span></a><span style=\"font-weight: 400;\"> via children and heterosexual adults who live closely together in a household and may share a bed or linen. </span>\r\n<h4><b>Worldwide vaccine shortage</b></h4>\r\n<span style=\"font-weight: 400;\">Last week, the DRC</span><a href=\"https://africacdc.org/news-item/joint-press-release-the-democratic-republic-of-congo-receives-first-mpox-vaccines/\"> <span style=\"font-weight: 400;\">received two shipments</span></a><span style=\"font-weight: 400;\"> from the European Union donation programme of about 100,000 vaccine doses used against mpox each – but it’s not nearly enough. Two such jabs are currently being used globally, both </span><a href=\"https://www.who.int/news/item/09-08-2024-who-invites-mpox-vaccine-manufacturers-to-submit-dossiers-for-emergency-evaluations\"><span style=\"font-weight: 400;\">recommended</span></a><span style=\"font-weight: 400;\"> by the WHO’s advisory group on immunisation.</span>\r\n\r\n<span style=\"font-weight: 400;\">Vaccines specifically against mpox </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10883320/\"><span style=\"font-weight: 400;\">are being developed</span></a><span style=\"font-weight: 400;\">, but there have not yet been clinical trials that include thousands of people, which is a necessary step in getting any medicine into commercial production.</span>\r\n\r\n<span style=\"font-weight: 400;\">Although the US medicines regulator, the Food and Drug Administration, issued an </span><a href=\"https://www.cdc.gov/poxvirus/mpox/interim-considerations/overview.html\"><span style=\"font-weight: 400;\">emergency use licence for a new type of smallpox vaccine to be used against mpox infection</span></a><span style=\"font-weight: 400;\"> in August 2022, in most other places earlier </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10598838/\"><span style=\"font-weight: 400;\">anti-smallpox jabs are used against mpox</span></a><span style=\"font-weight: 400;\">, because the two diseases are caused by similar-looking viruses. </span>\r\n\r\n<span style=\"font-weight: 400;\">In South Africa, people were vaccinated against smallpox until the 1980s, when the pandemic was eradicated; those who were vaccinated have about </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-06-14-mpox-jabs-and-treatment-could-arrive-in-days/\"><span style=\"font-weight: 400;\">85% protection</span></a><span style=\"font-weight: 400;\"> against getting infected with mpox.</span>\r\n\r\n<span style=\"font-weight: 400;\">The Health Department has </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/06/12_06-Min-of-health.pdf\"><span style=\"font-weight: 400;\">announced</span></a><span style=\"font-weight: 400;\"> that it will start to vaccinate four groups of people with a higher chance than the general population to contract the mpox virus – men who have sex with men, sex workers and health and lab workers – as soon as jabs arrive in the country. </span>\r\n\r\n<span style=\"font-weight: 400;\">But the department has not yet managed to secure donations.</span>\r\n\r\n<span style=\"font-weight: 400;\">Mohale says the department has tried to source jabs from Western countries, but “it’s difficult to tell when we can expect the vaccines. The department is still considering several options, while at the same time looking at the epidemiological surveillance data.” </span>\r\n\r\n<span style=\"font-weight: 400;\">With a huge shortage of mpox jabs, the current spread of clade 1 cases is unfolding against the background of vaccine inequity “and yet another looming failure”, experts wrote on</span><a href=\"https://www.bmj.com/content/386/bmj.q1803\"> <span style=\"font-weight: 400;\">15 August in the British Medical Journal</span></a><span style=\"font-weight: 400;\"> (BMJ). </span>\r\n\r\n<span style=\"font-weight: 400;\">“We have seen this before, and it does not end well for anyone,” they said. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Vaccine inequity is already evident”, the authors wrote, “with</span><a href=\"https://africacdc.org/news-item/africa-cdc-declares-mpox-a-public-health-emergency-of-continental-security-mobilizing-resources-across-the-continent/\"> <span style=\"font-weight: 400;\">Africa CDC reporting a need</span></a><span style=\"font-weight: 400;\"> for approximately 10 million vaccine doses to control the outbreak,</span><a href=\"https://www.nature.com/articles/d41586-024-02607-y\"> <span style=\"font-weight: 400;\">of which only about 280,000 are available</span></a><span style=\"font-weight: 400;\"> – less than 3% of the estimated need, even as wealthy countries hoard, stockpile, and refuse to share vaccines.”</span>\r\n\r\n<span style=\"font-weight: 400;\">The WHO and Africa CDC’s continental plan aims to get 10 million people in Africa vaccinated within the next six months.</span>\r\n\r\n<span style=\"font-weight: 400;\">The BMJ authors, however,</span><a href=\"https://www.bmj.com/content/386/bmj.q1803\"> <span style=\"font-weight: 400;\">concluded</span></a><span style=\"font-weight: 400;\">: “Having observed Covid-19 and previous epidemics, we have every reason to worry that high-income nations will make the same mistakes again – from vaccine hoarding to unfair travel bans and anti-Black racism.” </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">Bhekisisa health reporter, Zano Kunene, put together a timeline of how mpox has unfolded in South Africa and added the cases to an interactive map. "description": "<span style=\"font-weight: 400;\">South Africa is one of 14 countries that will be</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"> <span style=\"font-weight: 400;\">financially supported by the Africa Centres for Disease Control and Prevention (Africa CDC) and World Health Organization (WHO)</span></a><span style=\"font-weight: 400;\"> from this month until February 2025 to draw up and roll out plans, including vaccination strategies, to fight mpox.</span>\r\n\r\n<span style=\"font-weight: 400;\">The budget for the “</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"><span style=\"font-weight: 400;\">mpox continental preparedness and response plan for Africa</span></a><span style=\"font-weight: 400;\">” is about</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"> <span style=\"font-weight: 400;\">$600-million</span></a><span style=\"font-weight: 400;\">, 53% of which will go towards the mpox responses of 14 of the 55 African Union member states in which cases have escalated, and human-to-human transmission of the virus has become relatively common.</span>\r\n\r\n<span style=\"font-weight: 400;\">Thirteen other countries that will be helped to design and roll out plans are: Democratic Republic of Congo (DRC), Burundi, Central African Republic, Congo, Cameroon, Côte d’Ivoire, Gabon, Liberia, Kenya, Nigeria, Republic of Guinea, Rwanda and Uganda.</span>\r\n\r\n<span style=\"font-weight: 400;\">Most of the rest of the money (45%) will be spent on funding the efforts of partners such as</span><a href=\"https://www.unicef.org/\"> <span style=\"font-weight: 400;\">Unicef,</span></a> <span style=\"font-weight: 400;\">the</span><a href=\"https://southafrica.iom.int/\"> <span style=\"font-weight: 400;\">International Organization for Migration</span></a><span style=\"font-weight: 400;\"> and the</span><a href=\"https://www.ifrc.org/\"> <span style=\"font-weight: 400;\">International Federation of Red Cross</span></a><span style=\"font-weight: 400;\">, to help countries to set up their plans, become better at testing and contact tracing, managing cases and rolling out vaccines.</span>\r\n\r\n<a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/MPOX-CONTINENTAL-PREPAREDNESS-AND-RESPONSE-PLAN-FOR-AFRICA_Final-Edit.pdf\"><span style=\"font-weight: 400;\">The budget for South Africa is $45.5-million</span></a><span style=\"font-weight: 400;\"> (R813-million), of which $10.1-million (R178-million) will cover the logistics of rolling out vaccines against mpox. This money can’t be used to buy jabs. The Africa CDC and WHO say vaccines will be funded via in-kind donations and in cases where countries can afford to buy shots, the prices will depend on ongoing negotiations with drugmakers.</span>\r\n\r\n<span style=\"font-weight: 400;\">In August the WHO</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/08/WHO-Director-General-declares-mpox-outbreak-a-public-health-emergency-of-international-concern.pdf\"> <span style=\"font-weight: 400;\">declared</span></a><span style=\"font-weight: 400;\"> mpox a public health emergency of international concern, and in the same month the Africa CDC</span><a href=\"https://africacdc.org/news-item/africa-cdc-declares-mpox-a-public-health-emergency-of-continental-security-mobilizing-resources-across-the-continent/\"> <span style=\"font-weight: 400;\">announced</span></a> <span style=\"font-weight: 400;\">it a public health emergency of continental security.</span>\r\n\r\n<span style=\"font-weight: 400;\">It is the</span><a href=\"https://africacdc.org/news-item/africa-cdc-declares-mpox-a-public-health-emergency-of-continental-security-mobilizing-resources-across-the-continent/\"> <span style=\"font-weight: 400;\">first time</span></a><span style=\"font-weight: 400;\"> the Africa CDC has made such a declaration since its inception in 2017. The WHO’s declaration is the second in two years with regards to mpox – in 2022, it also</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">declared</span></a><span style=\"font-weight: 400;\"> mpox a public health emergency of international concern, after a multicountry outbreak, but announced the emergency</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">over</span></a><span style=\"font-weight: 400;\"> in May 2023. </span>\r\n\r\n<span style=\"font-weight: 400;\">An invitation for vaccine manufacturers to </span><a href=\"https://www.who.int/news/item/09-08-2024-who-invites-mpox-vaccine-manufacturers-to-submit-dossiers-for-emergency-evaluations\"><span style=\"font-weight: 400;\">express their interest in listing their anti-mpox shots for emergency use</span></a><span style=\"font-weight: 400;\"> was announced by the WHO on 7 August. </span>\r\n<h4><b>South Africa now has 25 confirmed mpox cases </b></h4>\r\n<span style=\"font-weight: 400;\">South Africa’s Health Department</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/09/Health-Department-confirms-one-more-positive-mpox-case-1.pdf\"> <span style=\"font-weight: 400;\">announced another mpox case</span></a><span style=\"font-weight: 400;\"> on Monday – a 38-year old man who was diagnosed at a private medical facility in Cape Town – on 6 September.</span>\r\n\r\n<span style=\"font-weight: 400;\">This brings South Africa’s total number of 2024 cases to 25. Twelve cases were diagnosed in Gauteng, 11 in KwaZulu-Natal and two in the Western Cape. Three have died.</span>\r\n\r\n<span style=\"font-weight: 400;\">The country’s first 2024 case was</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/06/13.05.2024_Minister-Phaahla-calls-for-vigilance-against-Monkey-Pox-disease.pdf\"> <span style=\"font-weight: 400;\">reported</span></a><span style=\"font-weight: 400;\"> on 8 May.</span>\r\n<h4><b>Where and when were South Africa’s mpox cases diagnosed? </b></h4>\r\n<span style=\"font-weight: 400;\">Use the interactive map, created by Bhekisisa’s health reporter, Zano Kunene, to see the details.</span>\r\n<div class=\"flourish-embed flourish-map\" data-src=\"visualisation/18601166\"><script src=\"https://public.flourish.studio/resources/embed.js\"></script></div>\r\n<a href=\"https://www.nicd.ac.za/mpox-outbreak-alert-africas-crisis-and-south-africas-response/\"><span style=\"font-weight: 400;\">None of the country’s cases</span></a><span style=\"font-weight: 400;\"> is the relatively new strain, clade 1b, which seems to be deadlier than other forms of the virus. All of South Africa’s cases were infected with clade 2b, the same type that caused outbreaks in Western Europe and the UK in 2022 and 2023, according to Health Department spokesperson Foster Mohale.</span>\r\n\r\n<span style=\"font-weight: 400;\">Almost all South Africa’s cases were </span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"><span style=\"font-weight: 400;\">among gay and bisexual men</span></a><span style=\"font-weight: 400;\">, most in their twenties or thirties, and many of them were infected with HIV, but </span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"><span style=\"font-weight: 400;\">untreated</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa’s latest case</span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/08/Health-Department-calls-for-calm-as-WHO-declares-Mpox-a-public-health-emergency-of-international-concern.pdf\"> <span style=\"font-weight: 400;\">does not have an international travel history</span></a> <span style=\"font-weight: 400;\">and went to the doctor on 4 September after he noticed typical mpox-like blisters on his face, chest and genitals. He wasn’t admitted to hospital, but is in a stable condition and isolating at home.</span>\r\n<h4><b>The differences between clades 1 and 2</b></h4>\r\n<span style=\"font-weight: 400;\">Mpox is caused by the monkeypox virus and there are two known clades:</span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"> <span style=\"font-weight: 400;\">clade 1 and clade 2</span></a><span style=\"font-weight: 400;\">. Mpox starts off with a fever, muscle aches and a sore throat, followed by blister-like bumps on the skin on any part of the body. Most people recover spontaneously, but those with weakened immune systems,</span><a href=\"https://www.who.int/emergencies/disease-outbreak-news/item/2024-DON525\"> <span style=\"font-weight: 400;\">such as those with untreated HIV, often fall very ill with the disease, and some die</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Clade 1 has been rampant in the DRC, where mpox has been recorded for more than a decade, but in August cases were also detected in</span><a href=\"https://www.folkhalsomyndigheten.se/the-public-health-agency-of-sweden/communicable-disease-control/disease-information-about-mpox/one-case-of-mpox-clade-i-reported-in-sweden/\"> <span style=\"font-weight: 400;\">Sweden</span></a><span style=\"font-weight: 400;\"> and</span><a href=\"https://www.cdc.gov/media/releases/2024/s0822-mpox-outbreak.html\"> <span style=\"font-weight: 400;\">Thailand</span></a><span style=\"font-weight: 400;\">.</span><a href=\"https://www.ecdc.europa.eu/en/news-events/epidemiological-update-mpox-due-monkeypox-virus-clade-i\"> <span style=\"font-weight: 400;\">Moreover, from 2023 onwards</span></a><span style=\"font-weight: 400;\">, clade 1 cases were found in Kenya, Rwanda, Burundi, the Central African Republic and Uganda; all countries close to the DRC. The cases in Sweden and Thailand had both travelled to African countries where clade 1 cases had been detected.</span>\r\n\r\n[caption id=\"attachment_2357016\" align=\"alignnone\" width=\"1080\"]<img class=\"size-full wp-image-2357016\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2024/09/Mpox-Map-9-September-2024.jpg\" alt=\"mpox\" width=\"1080\" height=\"1080\" /> <em>Source: Department of Health, 9 September 2024</em>[/caption]\r\n\r\n<span style=\"font-weight: 400;\">The WHO says</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">clade 1 cases in the DRC have increased steadily</span></a><span style=\"font-weight: 400;\"> over the past decade, especially in 2023.</span><a href=\"https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern\"> <span style=\"font-weight: 400;\">At more than 15,600 cases and 537 deaths</span></a><span style=\"font-weight: 400;\">, the numbers reported so far in 2024 are more than last year’s total. </span>\r\n\r\n<span style=\"font-weight: 400;\">Clades 1 and 2 cause the same type of disease, but infection with clade 1 makes people feel sicker and</span><a href=\"https://www.pnas.org/doi/full/10.1073/pnas.2220415120\"> <span style=\"font-weight: 400;\">can lead to more deaths</span></a><span style=\"font-weight: 400;\">. Although symptoms of a clade 2 infection are milder,</span><a href=\"https://www.pnas.org/doi/full/10.1073/pnas.2220415120\"> <span style=\"font-weight: 400;\">virus of this type spreads easier</span></a><span style=\"font-weight: 400;\">. Clade 2b cases have mainly spread among gay and bisexual men with HIV, but who are not on treatment, or don’t use their treatment correctly. Clade 1 cases, however,</span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2491159/\"> <span style=\"font-weight: 400;\">can also spread</span></a><span style=\"font-weight: 400;\"> via children and heterosexual adults who live closely together in a household and may share a bed or linen. </span>\r\n<h4><b>Worldwide vaccine shortage</b></h4>\r\n<span style=\"font-weight: 400;\">Last week, the DRC</span><a href=\"https://africacdc.org/news-item/joint-press-release-the-democratic-republic-of-congo-receives-first-mpox-vaccines/\"> <span style=\"font-weight: 400;\">received two shipments</span></a><span style=\"font-weight: 400;\"> from the European Union donation programme of about 100,000 vaccine doses used against mpox each – but it’s not nearly enough. Two such jabs are currently being used globally, both </span><a href=\"https://www.who.int/news/item/09-08-2024-who-invites-mpox-vaccine-manufacturers-to-submit-dossiers-for-emergency-evaluations\"><span style=\"font-weight: 400;\">recommended</span></a><span style=\"font-weight: 400;\"> by the WHO’s advisory group on immunisation.</span>\r\n\r\n<span style=\"font-weight: 400;\">Vaccines specifically against mpox </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10883320/\"><span style=\"font-weight: 400;\">are being developed</span></a><span style=\"font-weight: 400;\">, but there have not yet been clinical trials that include thousands of people, which is a necessary step in getting any medicine into commercial production.</span>\r\n\r\n<span style=\"font-weight: 400;\">Although the US medicines regulator, the Food and Drug Administration, issued an </span><a href=\"https://www.cdc.gov/poxvirus/mpox/interim-considerations/overview.html\"><span style=\"font-weight: 400;\">emergency use licence for a new type of smallpox vaccine to be used against mpox infection</span></a><span style=\"font-weight: 400;\"> in August 2022, in most other places earlier </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10598838/\"><span style=\"font-weight: 400;\">anti-smallpox jabs are used against mpox</span></a><span style=\"font-weight: 400;\">, because the two diseases are caused by similar-looking viruses. </span>\r\n\r\n<span style=\"font-weight: 400;\">In South Africa, people were vaccinated against smallpox until the 1980s, when the pandemic was eradicated; those who were vaccinated have about </span><a href=\"https://bhekisisa.org/health-news-south-africa/2024-06-14-mpox-jabs-and-treatment-could-arrive-in-days/\"><span style=\"font-weight: 400;\">85% protection</span></a><span style=\"font-weight: 400;\"> against getting infected with mpox.</span>\r\n\r\n<span style=\"font-weight: 400;\">The Health Department has </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/06/12_06-Min-of-health.pdf\"><span style=\"font-weight: 400;\">announced</span></a><span style=\"font-weight: 400;\"> that it will start to vaccinate four groups of people with a higher chance than the general population to contract the mpox virus – men who have sex with men, sex workers and health and lab workers – as soon as jabs arrive in the country. </span>\r\n\r\n<span style=\"font-weight: 400;\">But the department has not yet managed to secure donations.</span>\r\n\r\n<span style=\"font-weight: 400;\">Mohale says the department has tried to source jabs from Western countries, but “it’s difficult to tell when we can expect the vaccines. The department is still considering several options, while at the same time looking at the epidemiological surveillance data.” </span>\r\n\r\n<span style=\"font-weight: 400;\">With a huge shortage of mpox jabs, the current spread of clade 1 cases is unfolding against the background of vaccine inequity “and yet another looming failure”, experts wrote on</span><a href=\"https://www.bmj.com/content/386/bmj.q1803\"> <span style=\"font-weight: 400;\">15 August in the British Medical Journal</span></a><span style=\"font-weight: 400;\"> (BMJ). </span>\r\n\r\n<span style=\"font-weight: 400;\">“We have seen this before, and it does not end well for anyone,” they said. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Vaccine inequity is already evident”, the authors wrote, “with</span><a href=\"https://africacdc.org/news-item/africa-cdc-declares-mpox-a-public-health-emergency-of-continental-security-mobilizing-resources-across-the-continent/\"> <span style=\"font-weight: 400;\">Africa CDC reporting a need</span></a><span style=\"font-weight: 400;\"> for approximately 10 million vaccine doses to control the outbreak,</span><a href=\"https://www.nature.com/articles/d41586-024-02607-y\"> <span style=\"font-weight: 400;\">of which only about 280,000 are available</span></a><span style=\"font-weight: 400;\"> – less than 3% of the estimated need, even as wealthy countries hoard, stockpile, and refuse to share vaccines.”</span>\r\n\r\n<span style=\"font-weight: 400;\">The WHO and Africa CDC’s continental plan aims to get 10 million people in Africa vaccinated within the next six months.</span>\r\n\r\n<span style=\"font-weight: 400;\">The BMJ authors, however,</span><a href=\"https://www.bmj.com/content/386/bmj.q1803\"> <span style=\"font-weight: 400;\">concluded</span></a><span style=\"font-weight: 400;\">: “Having observed Covid-19 and previous epidemics, we have every reason to worry that high-income nations will make the same mistakes again – from vaccine hoarding to unfair travel bans and anti-Black racism.” </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">Bhekisisa health reporter, Zano Kunene, put together a timeline of how mpox has unfolded in South Africa and added the cases to an interactive map.
