"contents": "<span style=\"font-weight: 400;\">Plans to kick off a study this month to test a new way for finding tuberculosis (TB) in kids – by looking at their stool samples (poop) – have been put on hold after a </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/07/twitter_f781c7d3ebddfd73425a3aa99cee7c91.jpeg.pdf\"><span style=\"font-weight: 400;\">cyberattack in late June shut down</span></a><span style=\"font-weight: 400;\"> the National Health Laboratory Service’s computer systems, but Farzana Ismail, a clinical microbiologist with the lab network, says it “remains a priority”.</span>\r\n\r\n<span style=\"font-weight: 400;\">Because of the IT problems at the country’s public pathology lab service, test results now have to be </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/07/NHLS-Statement.pdf\"><span style=\"font-weight: 400;\">processed and reported manually</span></a><span style=\"font-weight: 400;\">, slowing down feedback to doctors and patients – and upsetting plans like those for the TB testing study. </span>\r\n\r\n<span style=\"font-weight: 400;\">The pilot, in which stool samples were to be tested for signs of the TB germ, would have taken place over the next two months at six labs in Gauteng, the Eastern Cape and the Western Cape, she told Bhekisisa at the </span><a href=\"https://tbconference.co.za/\"><span style=\"font-weight: 400;\">8th TB Conference in Durban in early June</span></a><span style=\"font-weight: 400;\">. Together with the national TB programme, they are working towards rolling out the method at labs nationwide, says Ismail.</span>\r\n\r\n<span style=\"font-weight: 400;\">Finding TB in children is difficult, because kids younger than five often </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK214442/#:~:text=In%20young%20children%20TB%20is,and%20allows%20drug%20susceptibility%20testing.\"><span style=\"font-weight: 400;\">have only a small number of the bacteria in their lungs</span><span style=\"font-weight: 400;\">. This means </span></a><span style=\"font-weight: 400;\">the bug might not show up in a </span><a href=\"https://www.cancer.gov/publications/dictionaries/cancer-terms/def/sputum\"><span style=\"font-weight: 400;\">sputum</span></a><span style=\"font-weight: 400;\"> sample (a mix of saliva and mucus from the airways), and so the test result could come back negative even though the child is sick with TB. </span>\r\n\r\n<span style=\"font-weight: 400;\">About </span><a href=\"https://extranet.who.int/tme/generateCSV.asp?ds=estimates\"><span style=\"font-weight: 400;\">27,000 children younger than 15 in South Africa are thought to have TB</span></a><span style=\"font-weight: 400;\"> – almost 10% of the </span><a href=\"https://worldhealthorg.shinyapps.io/tb_profiles/?_inputs_&entity_type=%22country%22&iso2=%22ZA%22&lan=%22EN%22\"><span style=\"font-weight: 400;\">country’s total cases</span></a><span style=\"font-weight: 400;\"> – and about </span><a href=\"https://extranet.who.int/tme/generateCSV.asp?ds=notifications\"><span style=\"font-weight: 400;\">40% of them went untreated</span></a><span style=\"font-weight: 400;\"> in 2022. TB is the </span><a href=\"https://www.statssa.gov.za/publications/P03093/P030932020.pdf#page=57\"><span style=\"font-weight: 400;\">third-biggest killer</span></a><span style=\"font-weight: 400;\"> in this age group even though it is </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK538185/#:~:text=Bacillus%20Calmette%2DGuerin%20(BCG),the%20only%20vaccine%20against%20tuberculosis.\"><span style=\"font-weight: 400;\">preventable</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://www.cdc.gov/tb/topic/treatment/tbdisease.htm\"><span style=\"font-weight: 400;\">curable</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Roughly six out of every 10 </span><a href=\"https://extranet.who.int/tme/generateCSV.asp?ds=estimates\"><span style=\"font-weight: 400;\">childhood TB cases</span></a><span style=\"font-weight: 400;\"> are among kids under the age of five, the group in whom it’s particularly difficult to pick up the TB bug. </span>\r\n\r\n<span style=\"font-weight: 400;\">The difficulty in getting good sputum samples from kids means other, often more uncomfortable and sometimes painful, ways have to be used to get a specimen that will be good enough to give a reliable result. </span>\r\n\r\n<span style=\"font-weight: 400;\">For example, a health worker can take a sample from someone’s stomach (because people often </span><a href=\"https://tbksp.org/en/node/2031#:~:text=Children%20with%20TB%20may%20swallow%20mucus%20that%20contains%20M.%20tuberculosis.%20Gastric%20aspiration%20is%20a%20technique%C2%A0used%20to%20collect%20gastric%20contents%20to%20try%20to%20confirm%20the%20diagnosis%20of%20TB%20by%20rapid%20molecular%20tests%20(or%C2%A0mycobacterial%20culture%20if%20rapid%20tests%20are%20not%20available)\"><span style=\"font-weight: 400;\">swallow phlegm, which can contain TB germs</span></a><span style=\"font-weight: 400;\"> if they’re infected). But for this, </span><a href=\"https://www.kznhealth.gov.za/family/National-Childhood-TB-Guidelines-2013-ZA.pdf\"><span style=\"font-weight: 400;\">a tube has to be fed through the child’s nose down into their stomach, and they can’t eat for three or more hours</span></a><span style=\"font-weight: 400;\"> before the test. </span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa’s guidelines on how to test and treat children for TB were </span><a href=\"http://www.kznhealth.gov.za/family/National-Childhood-TB-Guidelines-2013-ZA.pdf\"><span style=\"font-weight: 400;\">last revised in 2013</span></a><span style=\"font-weight: 400;\"> and, says Norbert Ndjeka, chief director for TB in the national Health Department, they are “impatiently waiting for the updated paediatric TB guidelines [to be published]” to help them better diagnose children, because “currently we’re not doing particularly well in this area”. </span>\r\n<h4><b>Stool is cool </b></h4>\r\n<span style=\"font-weight: 400;\">“To improve specimen collection [in children], we’re trying to get to child-friendly methods, like [taking] a [mouth] swab or stool [sample], so that it’s not so invasive,” says Karen du Preez, a senior clinical researcher at the </span><a href=\"https://blogs.sun.ac.za/dttc/\"><span style=\"font-weight: 400;\">Desmond Tutu TB Centre</span></a><span style=\"font-weight: 400;\">. “The reality is that because kids typically don’t have a lot of bugs, the diagnostic accuracy in the current tests is expected to be low.” </span>\r\n\r\n<span style=\"font-weight: 400;\">Using stool samples to test for TB will be included in the new guidelines, which are being written jointly by the Health Department and the </span><a href=\"https://tbthinktank.org/about-us/\"><span style=\"font-weight: 400;\">TB Think Tank</span></a><span style=\"font-weight: 400;\">, a group of experts who advise the government on policies for curbing TB infections, and come after the World Health Organization (WHO) </span><a href=\"https://iris.who.int/bitstream/handle/10665/352522/9789240046764-eng.pdf?sequence=1\"><span style=\"font-weight: 400;\">published an update of its standards document</span></a><span style=\"font-weight: 400;\"> in 2022. </span>\r\n\r\n<span style=\"font-weight: 400;\">The WHO has </span><a href=\"https://www.who.int/publications/i/item/9789240042650\"><span style=\"font-weight: 400;\">recommended testing faeces for TB</span></a><span style=\"font-weight: 400;\"> since 2021 – and in countries like </span><a href=\"https://www.usaid.gov/vietnam/news/jun-26-2020-usaid-supports-vietnams-national-tb-program-ntp-pilot-advance-tb-detection-children\"><span style=\"font-weight: 400;\">Vietnam</span><span style=\"font-weight: 400;\">,</span></a> <a href=\"https://www.usaid.gov/sites/default/files/2022-12/Zambia_Narrative_TBR22_Version_Final.pdf\"><span style=\"font-weight: 400;\">Zambia</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://www.usaid.gov/sites/default/files/2024-02/Ukraine_TB_roadmap_narrative-22_508.pdf\"><span style=\"font-weight: 400;\">Ukraine</span></a><span style=\"font-weight: 400;\"> the method has been widely used. Moreover, because the same rapid-result equipment can be used as for sputum tests, namely the </span><a href=\"https://www.cdc.gov/tb/php/laboratory-information/xpert-mtb-rif-assay.html\"><span style=\"font-weight: 400;\">GeneXpert machine</span></a><span style=\"font-weight: 400;\">, any lab that’s already set up for this </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8373220/\"><span style=\"font-weight: 400;\">will be able to run tests on stool samples too</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">With a GeneXpert test, a sample (such as sputum or stool) can be analysed for TB </span><a href=\"https://www.cdc.gov/tb/php/laboratory-information/xpert-mtb-rif-assay.html#:~:text=About%20the%20Xpert%20MTB%2FRIF%20Assay&text=The%20test%20simultaneously%20detects%20Mycobacterium,can%20add%203%20more%20weeks.\"><span style=\"font-weight: 400;\">within two hours</span></a><span style=\"font-weight: 400;\">, and </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10605857/\"><span style=\"font-weight: 400;\">in South Africa</span></a><span style=\"font-weight: 400;\">, most labs where TB gets tested are equipped with such machines.</span>\r\n\r\n<span style=\"font-weight: 400;\">Findings from a </span><a href=\"https://journals.asm.org/doi/epub/10.1128/jcm.02057-18\"><span style=\"font-weight: 400;\">systematic review</span></a><span style=\"font-weight: 400;\"> of studies in Africa and Asia, where stool samples were used to test for TB, found that the bug was correctly picked up in, on average, about 67% of close to 1,700 cases. This is in line with the </span><a href=\"https://iris.who.int/bitstream/handle/10665/135617/WHO_HTM_TB_2014.18_eng.pdf?sequence=1\"><span style=\"font-weight: 400;\">WHO’s ideal sensitivity rate</span></a><span style=\"font-weight: 400;\"> of 66% for rapid non-sputum-based TB tests in children. (Africa and Asia make up </span><a href=\"https://www.who.int/teams/global-tuberculosis-programme/tb-reports/global-tuberculosis-report-2023/tb-disease-burden/1-1-tb-incidence#:~:text=Among%20all%20incident%20TB%20cases,%25)%20and%20Europe%20(2.2%25).\"><span style=\"font-weight: 400;\">more than two-thirds of all TB cases globally</span></a><span style=\"font-weight: 400;\">.) </span>\r\n\r\n<span style=\"font-weight: 400;\">Correctly identifying kids who did </span><i><span style=\"font-weight: 400;\">not</span></i><span style=\"font-weight: 400;\"> have TB was almost perfect when using the stool sample test, </span><a href=\"https://journals.asm.org/doi/epub/10.1128/jcm.02057-18\"><span style=\"font-weight: 400;\">the review showed</span></a><span style=\"font-weight: 400;\">. (The stool test’s </span><a href=\"https://www.sciencedirect.com/science/article/pii/S1743181617304572?via%3Dihub\"><span style=\"font-weight: 400;\">specificity</span></a><span style=\"font-weight: 400;\"> was 99%, meaning that only one out of every 100 test results was a false positive.) Having a TB test with a high specificity means that fewer people will incorrectly be put on antibiotic treatment when they don’t need it, which can last between six and nine months and so add to the chances of </span><a href=\"https://bhekisisa.org/article/2024-06-05-faster-tests-shorter-treatments-how-new-tech-could-help-slow-down-drug-resistant-tb/\"><span style=\"font-weight: 400;\">drug-resistant germs developing</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">In the same way, using a TB test that yields accurate results fast and so identifies kids who </span><i><span style=\"font-weight: 400;\">do</span></i><span style=\"font-weight: 400;\"> have the germ, means that they can get onto treatment early, rather than having to wait for a </span><a href=\"https://www.cdc.gov/tb/webcourses/TB101/page17045.html\"><span style=\"font-weight: 400;\">bacterial culture test</span></a><span style=\"font-weight: 400;\">, which can take </span><a href=\"https://sahivsoc.org/FileUpload/SAHIVSOC%20TB%2025%20May%202017.pdf\"><span style=\"font-weight: 400;\">two to six weeks</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">“The longer we wait [to make a diagnosis], the sicker someone can get,” says Du Preez.</span>\r\n\r\n<span style=\"font-weight: 400;\">Getting results from a culture test takes a long time because a small dish with a </span><a href=\"https://www.cdc.gov/tb/webcourses/TB101/page17045.html\"><span style=\"font-weight: 400;\">gel-like food source in which bacteria can grow</span></a><span style=\"font-weight: 400;\"> has to stand at a suitable temperature for a few weeks to allow enough time for a clump of bacteria to grow. </span>\r\n\r\n<span style=\"font-weight: 400;\">Moreover, given that taking only one type of sample from children isn’t guaranteed to identify the bug in someone who is indeed sick, the WHO </span><a href=\"https://iris.who.int/bitstream/handle/10665/353599/9789240042650-eng.pdf?sequence=1#page=9\"><span style=\"font-weight: 400;\">says</span></a><span style=\"font-weight: 400;\"> using stool together with another specimen type may improve the chance of an accurate diagnosis.</span>\r\n<h4><b>What about other options? </b></h4>\r\n<span style=\"font-weight: 400;\">Researchers are also looking at adding other ways of sample collection to the TB test choices. For example, taking a swab from the </span><a href=\"https://www.nature.com/articles/srep08668\"><span style=\"font-weight: 400;\">mouth and/or tongue</span></a><span style=\"font-weight: 400;\"> may be promising. </span>\r\n\r\n<span style=\"font-weight: 400;\">An </span><a href=\"https://deohs.washington.edu/cangelosilab/oral-swab-assay\"><span style=\"font-weight: 400;\">oral swab</span></a><span style=\"font-weight: 400;\"> is taken by scraping something that looks like a long earbud against the lining inside someone’s cheek or at the back of their tongue. It’s more comfortable for the patient than having to cough up sputum, </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6425180/\"><span style=\"font-weight: 400;\">easier to collect and safer for health workers too</span></a><span style=\"font-weight: 400;\">, because seeing that the patient doesn’t have to cough hard, there’s little chance of breathing in the TB germ from </span><a href=\"https://www.cdc.gov/tb/webcourses/tb101/page1699.html\"><span style=\"font-weight: 400;\">tiny droplets of spit expelled into the air when someone coughs</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">While the WHO doesn’t have guidelines on oral swab tests yet, it was the topic of </span><a href=\"https://documents.theunion.org/web-uploads/UNION2023_Abstracts_High.pdf?_gl=1*cgmytz*_ga*MTkzODY0NDgzOS4xNzE4MjY3MjIz*_ga_8PZRTR8JPW*MTcxODI2NzIyMi4xLjEuMTcxODI2NzMyNC41Ny4wLjA.\"><span style=\"font-weight: 400;\">many presentations</span></a><span style=\"font-weight: 400;\"> at the </span><a href=\"https://theunion.org/\"><span style=\"font-weight: 400;\">International Union Against TB and Lung Disease</span></a><span style=\"font-weight: 400;\"> conference on lung health </span><a href=\"https://conf2023.theunion.org/\"><span style=\"font-weight: 400;\">in November</span></a><span style=\"font-weight: 400;\">. Moreover, research has found a </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6425180/\"><span style=\"font-weight: 400;\">similar accuracy level</span></a><span style=\"font-weight: 400;\"> (of about 83%) when comparing sputum and oral swab samples. </span>\r\n\r\n<span style=\"font-weight: 400;\">However, since children often have fewer germs in their lungs than adults, it’s not sure how well oral swabs will work in finding the bug in children, says Du Preez.</span>\r\n\r\n<span style=\"font-weight: 400;\">Indeed, findings of a </span><a href=\"https://www.thelancet.com/journals/langlo/article/PIIS2214-109X(23)00469-2/fulltext\"><span style=\"font-weight: 400;\">systematic review</span></a><span style=\"font-weight: 400;\"> of studies in which mouth samples were used for TB tests, found that correct results showed up in only 5% to 42% of children. </span>\r\n\r\n<span style=\"font-weight: 400;\">Says Ismail about the research on oral swabs for finding TB in kids: “It’s shown sensitivity [how accurate a test is in identifying a disease] of around 30%, which means that three in 10 tests will be confirmed and the other seven will need further testing.” </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">This story was produced by the</span></i><a href=\"http://bhekisisa.org./\"><i><span style=\"font-weight: 400;\"> Bhekisisa Centre for Health Journalism</span></i></a><i><span style=\"font-weight: 400;\">.
