"contents": "<span style=\"font-weight: 400;\">An old medicine is back with a new promise: it can </span><a href=\"https://programme.aids2022.org/Abstract/Abstract/?abstractid=13231\"><span style=\"font-weight: 400;\">significantly cut your chances of getting a bacterial infection from condomless sex</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s an antibiotic from the 1960s called doxycycline used to </span><a href=\"https://www.medicines.org.uk/emc/product/4063/smpc#gref\"><span style=\"font-weight: 400;\">treat airway, skin or urinary tract infections</span></a><span style=\"font-weight: 400;\">. It’s also </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3062442/#:~:text=Doxycycline%20can%20be%20used%20for%20the%20prevention%20of%20malaria%20in,for%204%20weeks%20after%20leaving.\"><span style=\"font-weight: 400;\">given to travellers to prevent them from getting malaria</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">DoxyPEP is short for doxycycline post-exposure prophylaxis (PEP), which means the antibiotic is used to </span><i><span style=\"font-weight: 400;\">prevent</span></i><span style=\"font-weight: 400;\"> a bacterial infection after having been exposed to germs that cause certain sexually transmitted infections (STIs), instead of treating it once it’s already developed. In the case of malaria, doxycycline is used as pre-exposure prophylaxis, or PrEP, which means it’s given before someone has been exposed to the parasite that causes malaria. </span>\r\n\r\n<span style=\"font-weight: 400;\">New </span><a href=\"https://programme.aids2022.org/Abstract/Abstract/?abstractid=13231\"><span style=\"font-weight: 400;\">research conducted</span></a><span style=\"font-weight: 400;\"> in men who have sex with men and transgender women in the United States (US) shows taking a tablet of 200mg doxycycline up to 72 hours after unprotected oral or anal sex reduces someone’s chances of getting syphilis, chlamydia or gonorrhoea by two-thirds. </span>\r\n\r\n<span style=\"font-weight: 400;\">But although the drug itself is cheap (from 55c for a </span><a href=\"https://health-e.org.za/medicine-price-registry/\"><span style=\"font-weight: 400;\">100mg pill</span></a><span style=\"font-weight: 400;\">), it could come at a high cost to society because researchers </span><a href=\"https://programme.aids2022.org/Abstract/Abstract/?abstractid=13231\"><span style=\"font-weight: 400;\">don’t yet know</span></a><span style=\"font-weight: 400;\"> how the regular use of the medicine to prevent STIs might fuel </span><a href=\"https://www.yourgenome.org/facts/what-is-antibiotic-resistance/\"><span style=\"font-weight: 400;\">antibiotic resistance</span></a><span style=\"font-weight: 400;\"> (which is what happens when bugs learn to outsmart medicine). </span>\r\n\r\n<span style=\"font-weight: 400;\">Infectious-disease experts in South Africa say that, once local research has been done, DoxyPEP could work well here to prevent syphilis, chlamydia or gonorrhoea among </span><a href=\"https://www.cdc.gov/std/treatment-guidelines/trans.htm\"><span style=\"font-weight: 400;\">transgender women</span></a><span style=\"font-weight: 400;\"> and </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0208107\"><span style=\"font-weight: 400;\">men who have sex with men</span></a><span style=\"font-weight: 400;\">. These two groups are more vulnerable to developing STIs than the general population for a </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7527766/\"><span style=\"font-weight: 400;\">range</span></a><span style=\"font-weight: 400;\"> of reasons, </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/17855487/\"><span style=\"font-weight: 400;\">including that anal sex </span></a><span style=\"font-weight: 400;\">makes them more vulnerable to contracting the germs that cause the conditions. </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s a particularly hopeful plan for those who use state facilities. </span>\r\n<h4><b>Why? </b></h4>\r\n<span style=\"font-weight: 400;\">Because in South Africa’s public sector, people are </span><a href=\"https://www.health.gov.za/wp-content/uploads/2020/11/sti-guidelines-27-08-19.pdf\"><span style=\"font-weight: 400;\">treated for STIs only when they show up to a facility with symptoms</span></a><span style=\"font-weight: 400;\">. Health workers then prescribe medicine based on what they see and what patients tell them about their sexual history. </span>\r\n\r\n<span style=\"font-weight: 400;\">This method is used in many poorer countries because it doesn’t require laboratory tests, which are expensive and are often not available. Prescribing medicine based on a patient’s symptoms is therefore </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/08/bosu1999.pdf\"><span style=\"font-weight: 400;\">cheap and allows them to get treatment on the day they’re at the clinic</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">But there are </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/08/bosu1999.pdf\"><span style=\"font-weight: 400;\">drawbacks</span></a><span style=\"font-weight: 400;\"> to this approach. </span>\r\n\r\n<span style=\"font-weight: 400;\">Many </span><a href=\"https://www.mayoclinic.org/diseases-conditions/sexually-transmitted-diseases-stds/in-depth/std-symptoms/art-20047081#:~:text=Asymptomatic%20STIs,as%20a%20condom%2C%20during%20sex.\"><span style=\"font-weight: 400;\">STIs don’t show symptoms</span></a><span style=\"font-weight: 400;\">, so those people won’t ever be treated, and they won’t know that they’re spreading the infection to others. </span>\r\n\r\n<span style=\"font-weight: 400;\">When there’s no laboratory result to confirm a diagnosis, health workers often opt for a medicine that could work for various infections, according to Johan Hugo, the senior clinical advisor at the </span><a href=\"https://www.anovahealth.co.za/\"><span style=\"font-weight: 400;\">Anova Health Institute</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Or, they may prescribe antibiotics </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/08/bosu1999.pdf\"><span style=\"font-weight: 400;\">when it’s not necessary</span></a><span style=\"font-weight: 400;\">. Both cases can drive antibiotic resistance.</span>\r\n\r\n<span style=\"font-weight: 400;\">While DoxyPEP could help prevent STIs from developing in the first place, the method could also drive drug resistance, says Remco Peters, an STI doctor and epidemiologist who’s in charge of putting together the STI treatment guidelines of the Southern African HIV Clinicians’ Society (SAHCS). </span>\r\n<h4><b>What does the research say? </b></h4>\r\n<span style=\"font-weight: 400;\">Doxycycline worked so well to prevent STIs in men who have sex with men and transgender women in the US DoxyPEP study that an ethical board ruled that it would be wrong to deprive the control group of the medicine. </span><span style=\"font-weight: 400;\">In a trial, the </span><a href=\"https://undsci.berkeley.edu/glossary/glossary.php?start=a&end=f\"><span style=\"font-weight: 400;\">control group are people who don’t get the drug being studied.</span></a><span style=\"font-weight: 400;\"> In this way, researchers can compare the results of the group that gets the actual drug with those that don’t to see how well the drug works.</span>\r\n\r\n<span style=\"font-weight: 400;\">Because of the good results, the DoxyPEP trial was stopped a year early, Annie Luetkemeyer, one of the principal researchers on the team, </span><a href=\"https://programme.aids2022.org/Abstract/Abstract/?abstractid=13231\"><span style=\"font-weight: 400;\">told delegates</span></a><span style=\"font-weight: 400;\"> at the </span><a href=\"https://aids2022.org/\"><span style=\"font-weight: 400;\">24th International Aids Conference</span></a><span style=\"font-weight: 400;\"> held in Montreal, Canada, in July. </span>\r\n\r\n<span style=\"font-weight: 400;\">The 501 participants were split into two groups. One group, of 327 people, took DoxyPEP after unprotected sex and the other group (174 people) did not. There would initially have been 554 participants in total, had the trial not ended early.</span>\r\n\r\n<span style=\"font-weight: 400;\">People who took the antibiotic had 65% fewer STI diagnoses than the control group, even though the DoxyPEP group was twice as big. </span>\r\n\r\n<span style=\"font-weight: 400;\">The drug was only slightly less effective at preventing STIs in HIV-positive people (62%) than in those not infected with HIV (66%). </span>\r\n\r\n<span style=\"font-weight: 400;\">A Kenyan study is also trialling the antibiotic to prevent STIs in </span><a href=\"https://trialsjournal.biomedcentral.com/articles/10.1186/s13063-022-06458-8\"><span style=\"font-weight: 400;\">cisgender women</span></a><span style=\"font-weight: 400;\"> (women whose </span><a href=\"https://www.cdc.gov/healthyyouth/terminology/sexual-and-gender-identity-terms.htm#:~:text=Cisgender%3A%20Individuals%20whose%20current%20gender,they%20were%20assigned%20at%20birth.\"><span style=\"font-weight: 400;\">gender identity</span></a><span style=\"font-weight: 400;\"> matches the sex they were born with). </span>\r\n\r\n<span style=\"font-weight: 400;\">It will end in June 2024.</span>\r\n<h4><b>What about resistance? </b></h4>\r\n<span style=\"font-weight: 400;\">In the US study, people were pretty good at taking DoxyPEP: almost nine out of 10 said they did so in time (between 24 and 72 hours after unprotected sex). About half of the participants used DoxyPEP up to 10 times per month and 30% took a pill between 10 and 20 times in that period. The remaining participants used the medicine on more than 20 occasions. </span>\r\n\r\n<span style=\"font-weight: 400;\">But researchers don’t know exactly how using the antibiotic so regularly will change the germs it’s meant to fight, or how it could affect the collection of bacteria </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK7617/\"><span style=\"font-weight: 400;\">normally living in our bodies</span></a><span style=\"font-weight: 400;\"> and which help to keep the bad ones in check. </span>\r\n\r\n<i><span style=\"font-weight: 400;\">Staphylococcus aureus</span></i><span style=\"font-weight: 400;\"> is one of the bugs that scientists worry about. It lives on our skin and is usually harmless. But in some cases (for example, </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK441868/\"><span style=\"font-weight: 400;\">when someone has a weak immune system</span></a><span style=\"font-weight: 400;\">), it can cause serious problems, such as heart inflammation or sepsis (when an infection you already have</span><a href=\"https://www.cdc.gov/sepsis/what-is-sepsis.html\"><span style=\"font-weight: 400;\"> triggers life-threatening tissue damage</span></a><span style=\"font-weight: 400;\"> throughout your body).</span>\r\n\r\n<span style=\"font-weight: 400;\">Infections caused by </span><i><span style=\"font-weight: 400;\">Staphylococcus aureus </span></i><span style=\"font-weight: 400;\">are already hard to treat because the germ is resistant to multiple antibiotics (also referred to as being multidrug-resistant). </span>\r\n\r\n<a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4697812/#:~:text=MRSA%20rate%20is%20high%20in,community%2D%20acquired%20infections%2C%20respectively.\"><span style=\"font-weight: 400;\">Nearly half of more than 2,000 patients admitted to hospitals with sepsis</span></a><span style=\"font-weight: 400;\"> in Gauteng, the Western Cape, the Free State and KwaZulu-Natal between 2010 and 2012 had been infected with a drug-resistant strain of the bug. </span><span style=\"font-weight: 400;\">When researchers tested blood samples from these patients to see how well the germ responded to 18 different antibiotics, they found that doxycycline only worked on drug-resistant </span><i><span style=\"font-weight: 400;\">Staphylococcus aureus </span></i><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4697812/table/pone.0145429.t002/?report=objectonly\"><span style=\"font-weight: 400;\">half the time</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<hr />\r\n\r\n<strong>Visit <a href=\"https://www.dailymaverick.co.za?utm_source=direct&utm_medium=in_article_link&utm_campaign=homepage\"><em>Daily Maverick's</em> home page</a> for more news, analysis and investigations</strong>\r\n\r\n<hr />\r\n\r\n<h4><b>What do we know about STI resistance in SA? </b></h4>\r\n<span style=\"font-weight: 400;\">South Africa will need local studies to help policymakers gauge how well DoxyPEP will work in the country, Peters says. </span>\r\n\r\n<span style=\"font-weight: 400;\">The country’s public sector facilities are </span><a href=\"https://bhekisisa.org/article/2022-07-22-superbugs-could-cost-you-an-arm-or-a-leg-why-hospitals-need-more-money-to-fight-drug-resistance/\"><span style=\"font-weight: 400;\">already battling</span></a><span style=\"font-weight: 400;\"> to stop common bacterial infections seen in hospitals from becoming untreatable because there aren't enough infectious disease experts or money to run programmes to curb antimicrobial resistance. A </span><a href=\"https://www.mdpi.com/2079-6382/11/7/881/htm\"><span style=\"font-weight: 400;\">study</span></a><span style=\"font-weight: 400;\"> conducted in KwaZulu-Natal, published in the journal </span><i><span style=\"font-weight: 400;\">Antibiotics</span></i><span style=\"font-weight: 400;\"> in June, for instance, found that only three out of 57 state hospitals had set aside funds to roll out South Africa’s </span><a href=\"https://health-e.org.za/wp-content/uploads/2015/09/Antimicrobial-Resistance-National-Strategy-Framework-2014-2024.pdf\"><span style=\"font-weight: 400;\">action plan</span></a><span style=\"font-weight: 400;\"> to fight antimicrobial resistance.</span>\r\n\r\n<span style=\"font-weight: 400;\">From a </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/32868325/\"><span style=\"font-weight: 400;\">Johannesburg </span></a><span style=\"font-weight: 400;\">study that monitored changes in resistance over 10 years, it already seems as if an antibiotic such as doxycycline may not work as well to prevent gonorrhoea in South Africa as it did in the US study, Peters says. </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/32868325/\"><span style=\"font-weight: 400;\">Up to 83%</span></a><span style=\"font-weight: 400;\"> of gonorrhoea cases were resistant to the antibiotic in the Johannesburg trial. </span>\r\n\r\n<span style=\"font-weight: 400;\">Fortunately, Peters says, doxycycline resistance in chlamydia and syphilis is rare, so DoxyPEP will likely be very effective in preventing those infections in South Africa. </span>\r\n\r\n<span style=\"font-weight: 400;\">Untreated STIs are a serious threat, research shows. </span>\r\n\r\n<span style=\"font-weight: 400;\">When STIs such as gonorrhoea, syphilis or chlamydia aren’t treated, they can </span><a href=\"https://www.tandfonline.com/doi/pdf/10.1016/S0968-8080(12)40653-X\"><span style=\"font-weight: 400;\">cause infertility</span></a><span style=\"font-weight: 400;\">, put you at a higher risk of </span><a href=\"https://www.who.int/news-room/fact-sheets/detail/sexually-transmitted-infections-(stis)\"><span style=\"font-weight: 400;\">contracting HIV</span></a><span style=\"font-weight: 400;\"> or lead to </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6525108/\"><span style=\"font-weight: 400;\">stillbirth</span></a><span style=\"font-weight: 400;\"> if a woman is pregnant. People with HIV who aren’t on antiretroviral (ARV) treatment also </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6715951/\"><span style=\"font-weight: 400;\">become more infectious</span></a><span style=\"font-weight: 400;\"> when they have gonorrhoea. </span>\r\n\r\n<span style=\"font-weight: 400;\">In short, it’s a double bind: the way South Africa treats STIs at the moment drives drug resistance and a possible new prevention method could do the same. </span>\r\n\r\n<span style=\"font-weight: 400;\">So how should policymakers decide?</span>\r\n\r\n<span style=\"font-weight: 400;\">Says Peters: “It’s hard to compare the two [approaches] without clear answers about how well DoxyPEP will work in different populations, and how that could impact drug resistance.”</span>\r\n<h4><b>Lessons from HIV PEP</b></h4>\r\n<span style=\"font-weight: 400;\">The country’s public health system already provides PEP for HIV. This four-week ARV course cuts people’s risk of HIV infection by about </span><a href=\"https://stacks.cdc.gov/view/cdc/38856\"><span style=\"font-weight: 400;\">80%</span></a><span style=\"font-weight: 400;\">, as long as they start taking the pills a day after a possible exposure. The medicine has been available for free at state facilities since </span><a href=\"https://www.gov.za/sites/default/files/gcis_document/201409/aidsoperationalplan10.pdf\"><span style=\"font-weight: 400;\">2003</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Hugo, who has worked at Anova’s </span><a href=\"https://ivantomscentre.africa/\"><span style=\"font-weight: 400;\">Ivan Toms Centre for Health</span></a><span style=\"font-weight: 400;\"> in Cape Town since 2009, says he’s seen first-hand that health workers’ attitudes can determine if a strategy like this works in real life. </span>\r\n\r\n<span style=\"font-weight: 400;\">He explains: “Health workers often judge when people say they need PEP. But people are sexual beings and stuff happens. When a condom breaks or maybe you just didn’t use one, people need to be comfortable talking to health workers about it.” </span>\r\n<h4><b>Where should people get DoxyPEP?</b></h4>\r\n<span style=\"font-weight: 400;\">South Africa still battles to get HIV treatment and prevention to the people who need it most, says Juliet Houghton, who heads the SAHCS. </span>\r\n\r\n<span style=\"font-weight: 400;\">To fix this, they, together with other organisations, are working on a project that will train pharmacists to start people on ARVs, HIV PEP or the HIV-prevention pill (PrEP), Truvada. </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s an extension of a 2010 drive to upskill nurses to administer ARVs so that patients didn’t have to queue at hospitals to see a doctor for treatment. Instead, they could just go to the nurse at their nearest clinic. </span><a href=\"https://sajhivmed.org.za/index.php/hivmed/article/view/1196/2310#CIT0026_1196\"><span style=\"font-weight: 400;\">This took pressure off bigger facilities and ultimately more people started treatment</span></a><span style=\"font-weight: 400;\">.</span><a href=\"https://sajhivmed.org.za/index.php/hivmed/article/view/1196/2310#CIT0026_1196\"><span style=\"font-weight: 400;\"> </span></a>\r\n\r\n<span style=\"font-weight: 400;\">Now, pharmacists too will be able to bring ARVs, HIV PEP and Truvada to people. </span>\r\n\r\n<span style=\"font-weight: 400;\">And, says Houghton, pharmacies could be an ideal place to offer DoxyPEP, since many people prefer going to chemists rather than clinics. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Pharmacies are often near shopping centres, and usually close to a taxi rank, even in the rural areas.” Unlike state clinics, they’re also open on weekends and after work. </span>\r\n\r\n<span style=\"font-weight: 400;\">But part of preparing health workers to roll out DoxyPEP should include explaining to patients what they can expect from the medicine, Hugo cautions. </span>\r\n\r\n<span style=\"font-weight: 400;\">Doxycycline can cause heartburn, so Hugo usually tells his patients to eat something before taking the drug, to take it with enough water to swallow easily and to avoid dairy. </span><a href=\"https://www.ncbi.nlm.nih.gov/books/NBK555888/\"><span style=\"font-weight: 400;\">Dairy products such as milk decrease the amount of doxycycline the body can absorb</span></a><span style=\"font-weight: 400;\">, so people might be getting a lower dose than they should. Sitting down while swallowing the pill can make the heartburn worse, so it’s better to stand upright while taking it. </span>\r\n\r\n<span style=\"font-weight: 400;\">This kind of information may seem arbitrary, says Hugo, but it must be included when health workers are trained to administer DoxyPEP. </span>\r\n\r\n<span style=\"font-weight: 400;\">Houghton agrees.
