"contents": "<span style=\"font-weight: 400;\">Something about the doctor’s room Mandla Ndlovu* was standing in didn’t feel right. It was small and hidden on the fifth floor of a run-down building in Johannesburg’s city centre. R100 got him a consultation with a doctor, whose desk was littered with medicine vials, some labelled and some not, some full and some empty.</span>\r\n\r\n<span style=\"font-weight: 400;\">“I’m looking for testosterone,” Ndlovu said. </span>\r\n\r\n<span style=\"font-weight: 400;\">Ndlovu is a</span><a href=\"https://transequality.org/issues/resources/frequently-asked-questions-about-transgender-people\"><span style=\"font-weight: 400;\"> transgender man</span></a><span style=\"font-weight: 400;\">, which means he was classified as female at birth but identifies as a man. </span>\r\n\r\n<span style=\"font-weight: 400;\">Many transgender men take testosterone to help them develop physical, male traits. The male hormone stimulates the development of </span><a href=\"https://www.mayoclinic.org/tests-procedures/masculinizing-hormone-therapy/about/pac-20385099#:~:text=You'll%20begin%20masculinizing%20hormone,gel%20applied%20to%20the%20skin.\"><span style=\"font-weight: 400;\">facial hair and a deeper voice and stops menstrual cycles</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Transgender people may experience </span><a href=\"https://www.mayoclinic.org/diseases-conditions/gender-dysphoria/symptoms-causes/syc-20475255\"><span style=\"font-weight: 400;\">gender dysphoria</span></a><span style=\"font-weight: 400;\"> – a feeling of intense body discomfort when the sex they were born with differs from the gender they identify as. </span>\r\n\r\n<span style=\"font-weight: 400;\">An </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/26835611/\"><span style=\"font-weight: 400;\">analysis</span></a><span style=\"font-weight: 400;\"> of many studies shows that people with gender dysphoria experience mental health problems such as anxiety and depression more often than the </span><a href=\"https://dictionary.apa.org/cisgender\"><span style=\"font-weight: 400;\">cisgender</span></a><span style=\"font-weight: 400;\"> population (people whose gender identity matches their sex at birth). However, these psychological conditions often decrease to similar levels as among cisgender people once trans people start gender-affirming medical treatment such as taking cross-sex hormones. </span>\r\n\r\n<span style=\"font-weight: 400;\">Taking cross-sex hormones such as testosterone can go a long way to help people with gender dysphoria. </span>\r\n\r\n<span style=\"font-weight: 400;\">Going without hormones can </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">reverse some of the effects of hormone therapy</span></a><span style=\"font-weight: 400;\"> depending on people’s individual anatomy, how long they’ve been on testosterone, and whether they’ve had any surgery. </span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-1464433\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/11/MC-Testosterone_2.jpg\" alt=\"\" width=\"720\" height=\"441\" /> A trans man takes a shot of Nebido testosterone. (Photo: Dylan Bush / Bhekisisa)</p>\r\n\r\n<span style=\"font-weight: 400;\">People could, for instance, feel their skin becoming softer and muscles becoming less defined, and fat would move to settle around their hips and on their chests, says Anastacia Tomson, a doctor and transgender activist in Cape Town. </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s also likely, she says, that without testosterone, people’s menstrual cycles will return. </span>\r\n\r\n<span style=\"font-weight: 400;\">The hormone treatment is often a crucial step for people considering gender-affirming surgery. Doctors may </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">recommend</span></a><span style=\"font-weight: 400;\"> that someone has hormone treatment for a year before gender-transitioning surgery, such as, in the case of trans men, </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6626317/\"><span style=\"font-weight: 400;\">bottom surgery</span></a><span style=\"font-weight: 400;\"> (a procedure to construct a penis). </span>\r\n\r\n<span style=\"font-weight: 400;\">But Depo-Testosterone, the cheapest version of the hormone, which is manufactured by Pfizer, has run out in South Africa three times for between one and 12 months at a time since 2018. The latest stockout started at the end of 2021 and </span><span style=\"font-weight: 400;\">a March statement from </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/10/Depo-Testosterone-Out-of-Stock-Letter-Updated-March-2022.png\"><span style=\"font-weight: 400;\">Pfizer</span></a><span style=\"font-weight: 400;\"> (which is dated incorrectly) said its product could only be back on the shelves – around the world – somewhere between April and June 2023.</span>\r\n\r\n<i><span style=\"font-weight: 400;\">Read in </span></i><span style=\"font-weight: 400;\">Daily Maverick</span><i><span style=\"font-weight: 400;\">: “</span></i><a href=\"https://www.dailymaverick.co.za/article/2021-09-16-gender-affirming-surgery-little-more-than-a-distant-dream-for-the-many-hamstrung-by-high-costs-and-long-waiting-lists/\"><i><span style=\"font-weight: 400;\">Gender-affirming surgery little more than a distant dream for the many hamstrung by high costs and long waiting lists</span></i></a><i><span style=\"font-weight: 400;\">”</span></i><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">The pharmaceutical company didn’t respond to requests for comment about the reason for the shortage, but the manufacturer’s spokespeople have told </span><a href=\"https://www.reuters.com/article/safrica-lgbt-medicine-trfn-idUSKBN2JU1AY\"><span style=\"font-weight: 400;\">numerous</span></a><span style=\"font-weight: 400;\"> other news outlets that it was prioritising Covid vaccines. </span>\r\n\r\n<span style=\"font-weight: 400;\">The current stockout means the just more than </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/11/TG-PSE-Draft-Report-v-0_district-level-final-editable-version-17April2022-1.pdf\"><span style=\"font-weight: 400;\">28,000 transgender men</span></a><span style=\"font-weight: 400;\"> in South Africa have few affordable options for getting hormone treatment. </span>\r\n\r\n<span style=\"font-weight: 400;\">A 10ml vial of Depo-Testosterone costs </span><a href=\"https://medicineprices.org.za/#related:718548019\"><span style=\"font-weight: 400;\">just under R650</span></a><span style=\"font-weight: 400;\">. The dose people inject depends on how long they’ve been on the treatment. </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">Local guidelines</span></a><span style=\"font-weight: 400;\"> say people should start with injecting </span><a href=\"https://www.pfizermedicalinformation.com/en-us/depo-testosterone/dosage-admin\"><span style=\"font-weight: 400;\">1ml</span></a><span style=\"font-weight: 400;\"> into the buttocks </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/11/Testosterone-Gender-Dysphoria-Masculinising_N_December-2019.pdf\"><span style=\"font-weight: 400;\">once every two weeks</span></a><span style=\"font-weight: 400;\">. This means one container will be enough for 20 weeks’ treatment. </span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-1464434\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/11/MC-Testosterone_3.jpg\" alt=\"\" width=\"720\" height=\"442\" /> Empty vials of Pfizer's Depo-Testosterone, which the company is no longer producing. (Photo: Dylan Bush / Bhekisisa)</p>\r\n\r\n<span style=\"font-weight: 400;\">This dose is usually increased gradually until the amount of testosterone in the blood is within </span><a href=\"https://d31kydh6n6r5j5.cloudfront.net/uploads/sites/161/2019/08/hormones_FTM.pdf\"><span style=\"font-weight: 400;\">the range usually found in males</span></a><span style=\"font-weight: 400;\">. The maximum dose is 2ml every two weeks. In that case, one vial will last for 10 weeks. </span>\r\n\r\n<span style=\"font-weight: 400;\">The other choice available in South Africa is the drug company Grünenthal’s longer-lasting </span><a href=\"https://www.nebido.com\"><span style=\"font-weight: 400;\">Nebido</span></a><span style=\"font-weight: 400;\"> injection, which costs</span><a href=\"https://medicineprices.org.za/#related:706461001\"><span style=\"font-weight: 400;\"> between R2,400 and R2,700</span></a><span style=\"font-weight: 400;\"> and needs to be administered every 10 to 14 weeks. </span>\r\n\r\n<span style=\"font-weight: 400;\">This is between three to four times more expensive than Depo-Testosterone, depending on the required dose. </span>\r\n\r\n<span style=\"font-weight: 400;\">In South Africa, testosterone is on the </span><a href=\"https://www.knowledgehub.org.za/system/files/elibdownloads/2022-07/Tertiary%20and%20Quaternary%20Level%20Essential%20Medicine%20Recommendations_June%202022%20%281%29.pdf\"><span style=\"font-weight: 400;\">essential medicines list for tertiary and district hospitals</span></a><span style=\"font-weight: 400;\">, which means the Health Department buys it for these facilities.</span>\r\n\r\n<span style=\"font-weight: 400;\">But there aren’t many hospitals that have programmes to provide gender-affirming care – only </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/10/Spencer-The-care-is-the-best-you-can-give-at-the-time.pdf\"><span style=\"font-weight: 400;\">six government facilities nationwide</span></a><span style=\"font-weight: 400;\">, and they are located in just three of the nine provinces, according to a 2017 study. Only four of those offer endocrinology, the category in which testosterone falls. </span>\r\n\r\n<span style=\"font-weight: 400;\">In addition, state facilities have to stick to buying the product of the tender that the department accepted. Pfizer’s Depo-Testosterone is the only hormone treatment for trans men on the list, says Elma de Vries, </span><span style=\"font-weight: 400;\">the programme coordinator for medical students at the Nelson Mandela Metropolitan University in Gqeberha. </span>\r\n\r\n<span style=\"font-weight: 400;\">A few public health facilities have applied for special permission to buy Nebido for their existing patients, she explains. </span>\r\n\r\n<span style=\"font-weight: 400;\">The Health Department didn’t respond to queries about whether an alternative to Pfizer’s testosterone shot will be put on the essential medicines list permanently to ensure people don’t run out of treatment because of stockouts. </span>\r\n<h4>Third time unlucky</h4>\r\n<span style=\"font-weight: 400;\">During the first stockout of Pfizer’s Depo-Testosterone in 2018, Ndlovu had a vial that lasted him throughout most of the shortage. When it began to empty, he flew to Cape Town to a pharmacy that had stock </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> he could only get one vial at a time.</span>\r\n\r\n<span style=\"font-weight: 400;\">The next time the medicine ran out in 2019, it didn’t affect him because he had enough testosterone to use until stock was in again. </span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-1464435\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/11/MC-Testosterone_4.jpg\" alt=\"\" width=\"720\" height=\"433\" /> A needle and syringe with a shot of Nebido testosterone drawn. (Photo: Dylan Bush / Bhekisisa)</p>\r\n\r\n<span style=\"font-weight: 400;\">But at the end of 2021, Pfizer’s product ran out again </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> and so did Ndlovu’s luck. He couldn’t find the hormone anywhere. He emailed every pharmacy he could think of and even got in touch with doctors in the US to find out what it would cost to ship some here. </span>\r\n\r\n<span style=\"font-weight: 400;\">“My life almost crumbled,” he says. </span>\r\n\r\n<span style=\"font-weight: 400;\">Alongside physical changes, people who use testosterone and then suddenly stop also experience </span><a href=\"https://www.tandfonline.com/doi/pdf/10.1080/26895269.2022.2100644\"><span style=\"font-weight: 400;\">fatigue and brain fog</span></a><span style=\"font-weight: 400;\">, which affects their ability to think clearly or concentrate, and </span><a href=\"https://www.tandfonline.com/doi/pdf/10.1080/26895269.2022.2100644\"><span style=\"font-weight: 400;\">mood changes</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Taking testosterone had been a lifeline for Ndlovu for four years – that’s why he was at the dodgy doctor’s office: he was on his last vial, with only enough for three more injections (so he had six weeks’ supply). </span>\r\n<h4>Back-alley back-track: ‘I just felt unsafe’</h4>\r\n<span style=\"font-weight: 400;\">The man seated in the cramped back-alley doctor’s room offered Ndlovu one large dose of testosterone, which he said would last much longer than his usual shots would. </span>\r\n\r\n<span style=\"font-weight: 400;\">Before someone starts taking testosterone, their doctor does a </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/10/gardner2013.pdf\"><span style=\"font-weight: 400;\">blood test</span></a><span style=\"font-weight: 400;\"> to find out how much they should take per shot, usually </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">starting on 0.5ml a week and increasing to a maximum of 2ml every two weeks</span></a><span style=\"font-weight: 400;\">, De Vries says. </span>\r\n\r\n<i><span style=\"font-weight: 400;\">Read in </span></i><span style=\"font-weight: 400;\">Daily Maverick</span><i><span style=\"font-weight: 400;\">: “</span></i><a href=\"https://www.dailymaverick.co.za/article/2022-10-18-a-plea-to-parents-listen-to-trans-kids-not-moral-panics/\"><i><span style=\"font-weight: 400;\">A plea to parents: Listen to trans kids, not moral panics</span></i></a><i><span style=\"font-weight: 400;\">”</span></i>\r\n\r\n<span style=\"font-weight: 400;\">Taking the wrong dose can lead to </span><a href=\"https://link.springer.com/article/10.1007/s11154-018-9454-3\"><span style=\"font-weight: 400;\">heart diseases</span></a><span style=\"font-weight: 400;\">, she says. </span>\r\n\r\n<span style=\"font-weight: 400;\">Ndlovu decided to leave without the testosterone.</span>\r\n\r\n<span style=\"font-weight: 400;\">“I didn’t like it [the set-up]. It just felt unsafe,” he says as he rolls a cigarette and speaks to </span><i><span style=\"font-weight: 400;\">Bhekisisa </span></i><span style=\"font-weight: 400;\">at his home in Sandton. </span>\r\n\r\n<span style=\"font-weight: 400;\">He remembers thinking: “I guess I’m going to stop taking T [testosterone].” </span>\r\n<h4>Enter Fagron – an imperfect lifeline</h4>\r\n<span style=\"font-weight: 400;\">Just before Ndlovu’s final vial ran out, a friend messaged him with a lifeline. </span>\r\n\r\n<a href=\"https://fagron.co.za/\"><span style=\"font-weight: 400;\">Fagron</span></a><span style=\"font-weight: 400;\">, a pharmaceutical company in Cape Town, could make a customised treatment that would be similar to Depo-Testosterone by mixing the required ingredients on a case-by-case basis. This is called making up a </span><a href=\"http://www.acpsa.co.za/about/index.html\"><span style=\"font-weight: 400;\">compounded medicine</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">It was Tomson’s plan. She asked Fagron’s lead pharmacist to make up something similar to Depo-Testosterone when Pfizer announced the third stockout in 2021. </span>\r\n\r\n<span style=\"font-weight: 400;\">The Fagron formulation costs R475 for a 10ml vial. Adding a delivery fee to get it shipped from Cape Town to Johannesburg, for example, would bring the total to about R600. </span>\r\n\r\n<hr />\r\n\r\n<strong>Visit <a href=\"https://www.dailymaverick.co.za?utm_source=direct&utm_medium=in_article_link&utm_campaign=homepage\"><em>Daily Maverick's</em> home page</a> for more news, analysis and investigations</strong>\r\n\r\n<hr />\r\n\r\n<span style=\"font-weight: 400;\">But even with this workaround, treatment might still be unaccessible to many transgender men who get their testosterone for free from the few state facilities that have it. </span>\r\n\r\n<span style=\"font-weight: 400;\">Compounded medicines are </span><a href=\"https://www.sahpra.org.za/wp-content/uploads/2020/01/b335d1c0Act_101_of_1965_published_2003.pdf\"><span style=\"font-weight: 400;\">not allowed to be sold in retail stores and can only be bought from the compounding pharmacy with a doctor’s prescription</span></a><span style=\"font-weight: 400;\">. Even if a state patient gets a script from their clinic, Tomson says they would still have to pay the cost of the medicine themselves, since they would have to buy the product directly from the pharmacy that compounds it.</span>\r\n<h4>Getting by with a little help from friends</h4>\r\n<span style=\"font-weight: 400;\">For someone such as Malik Moyo (45), who makes a living selling handmade crafts, beanies, baskets and bracelets at flea markets in Johannesburg, the Fagron solution won’t work. </span>\r\n\r\n<span style=\"font-weight: 400;\">Money has been tight since the Covid lockdowns that started early in 2020 outlawed public gatherings, including markets, for a large part of two years. </span>\r\n\r\n<span style=\"font-weight: 400;\">Fortunately, Moyo could get their testosterone for free from the </span><a href=\"https://www.wrhi.ac.za/\"><span style=\"font-weight: 400;\">Wits Reproductive Health and HIV Institute</span></a><span style=\"font-weight: 400;\"> (Wits RHI), a nonprofit in Johannesburg. </span>\r\n\r\n<span style=\"font-weight: 400;\">Wits RHI provides gender-affirming hormone therapy as part of its HIV services. When gender-affirming hormones, counselling and hormone testing are available alongside HIV treatment and testing, </span><a href=\"https://pdf.usaid.gov/pdf_docs/PA00X63W.pdf\"><span style=\"font-weight: 400;\">research shows</span></a><span style=\"font-weight: 400;\"> it significantly increases how likely people are to start antiretrovirals for HIV prevention treatment. </span>\r\n\r\n<span style=\"font-weight: 400;\">But late in 2021, at the start of the third stockout, a Wits RHI doctor told Moyo the centre no longer had any of the hormone products to offer – and neither did any of the nearby government facilities. </span>\r\n\r\n<p><img loading=\"lazy\" class=\"size-full wp-image-1464436\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/11/MC-Testosterone_5.jpg\" alt=\"\" width=\"720\" height=\"450\" /> Mandla Ndlovu*, a transgender man living in Johannesburg. (Photo: Dylan Bush / Bhekisisa)</p>\r\n\r\n<span style=\"font-weight: 400;\">Moyo, who uses the non-binary pronoun “they”, stutters when recalling what the health worker told them: “Let me be honest with you, Malik. It’s either you find it [testosterone] somewhere else or you’re gonna lose what you want to achieve.” </span>\r\n\r\n<span style=\"font-weight: 400;\">What the doctor meant was that without regular testosterone shots, Moyo could lose some of the male traits the hormone brings about. </span>\r\n\r\n<span style=\"font-weight: 400;\">The stockout is </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/11/Pfizer-DSV-Stock-Update-11-November-2022-3.xlsx\"><span style=\"font-weight: 400;\">expected to end in July 2023</span></a><span style=\"font-weight: 400;\">, according to a stock update document provided to </span><i><span style=\"font-weight: 400;\">Bhekisisa </span></i><span style=\"font-weight: 400;\">by Fezeka Dlikilili, Pfizer’s conference coordinator in Johannesburg. </span>\r\n\r\n<span style=\"font-weight: 400;\">Until then, Moyo will continue to get money from their friends to cover the cost of consultation fees and a testostero</span><span style=\"font-weight: 400;\">ne shot from a clinic in Braamfontein. </span>\r\n\r\n<span style=\"font-weight: 400;\">“It’s just not an option to go back to a life before testosterone.” </span><b>DM/MC</b>\r\n\r\n<span style=\"font-weight: 400;\">* </span><i><span style=\"font-weight: 400;\">Not his real name.</span></i>\r\n\r\n<i><span style=\"font-weight: 400;\">This story was produced by the</span></i><a href=\"http://bhekisisa.org./\"> <i><span style=\"font-weight: 400;\">Bhekisisa Centre for Health Journalism</span></i></a><i><span style=\"font-weight: 400;\">. "description": "<span style=\"font-weight: 400;\">Something about the doctor’s room Mandla Ndlovu* was standing in didn’t feel right. It was small and hidden on the fifth floor of a run-down building in Johannesburg’s city centre. R100 got him a consultation with a doctor, whose desk was littered with medicine vials, some labelled and some not, some full and some empty.</span>\r\n\r\n<span style=\"font-weight: 400;\">“I’m looking for testosterone,” Ndlovu said. </span>\r\n\r\n<span style=\"font-weight: 400;\">Ndlovu is a</span><a href=\"https://transequality.org/issues/resources/frequently-asked-questions-about-transgender-people\"><span style=\"font-weight: 400;\"> transgender man</span></a><span style=\"font-weight: 400;\">, which means he was classified as female at birth but identifies as a man. </span>\r\n\r\n<span style=\"font-weight: 400;\">Many transgender men take testosterone to help them develop physical, male traits. The male hormone stimulates the development of </span><a href=\"https://www.mayoclinic.org/tests-procedures/masculinizing-hormone-therapy/about/pac-20385099#:~:text=You'll%20begin%20masculinizing%20hormone,gel%20applied%20to%20the%20skin.\"><span style=\"font-weight: 400;\">facial hair and a deeper voice and stops menstrual cycles</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Transgender people may experience </span><a href=\"https://www.mayoclinic.org/diseases-conditions/gender-dysphoria/symptoms-causes/syc-20475255\"><span style=\"font-weight: 400;\">gender dysphoria</span></a><span style=\"font-weight: 400;\"> – a feeling of intense body discomfort when the sex they were born with differs from the gender they identify as. </span>\r\n\r\n<span style=\"font-weight: 400;\">An </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/26835611/\"><span style=\"font-weight: 400;\">analysis</span></a><span style=\"font-weight: 400;\"> of many studies shows that people with gender dysphoria experience mental health problems such as anxiety and depression more often than the </span><a href=\"https://dictionary.apa.org/cisgender\"><span style=\"font-weight: 400;\">cisgender</span></a><span style=\"font-weight: 400;\"> population (people whose gender identity matches their sex at birth). However, these psychological conditions often decrease to similar levels as among cisgender people once trans people start gender-affirming medical treatment such as taking cross-sex hormones. </span>\r\n\r\n<span style=\"font-weight: 400;\">Taking cross-sex hormones such as testosterone can go a long way to help people with gender dysphoria. </span>\r\n\r\n<span style=\"font-weight: 400;\">Going without hormones can </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">reverse some of the effects of hormone therapy</span></a><span style=\"font-weight: 400;\"> depending on people’s individual anatomy, how long they’ve been on testosterone, and whether they’ve had any surgery. </span>\r\n\r\n[caption id=\"attachment_1464433\" align=\"alignnone\" width=\"720\"]<img class=\"size-full wp-image-1464433\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/11/MC-Testosterone_2.jpg\" alt=\"\" width=\"720\" height=\"441\" /> A trans man takes a shot of Nebido testosterone. (Photo: Dylan Bush / Bhekisisa)[/caption]\r\n\r\n<span style=\"font-weight: 400;\">People could, for instance, feel their skin becoming softer and muscles becoming less defined, and fat would move to settle around their hips and on their chests, says Anastacia Tomson, a doctor and transgender activist in Cape Town. </span>\r\n\r\n<span style=\"font-weight: 400;\">It’s also likely, she says, that without testosterone, people’s menstrual cycles will return. </span>\r\n\r\n<span style=\"font-weight: 400;\">The hormone treatment is often a crucial step for people considering gender-affirming surgery. Doctors may </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">recommend</span></a><span style=\"font-weight: 400;\"> that someone has hormone treatment for a year before gender-transitioning surgery, such as, in the case of trans men, </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6626317/\"><span style=\"font-weight: 400;\">bottom surgery</span></a><span style=\"font-weight: 400;\"> (a procedure to construct a penis). </span>\r\n\r\n<span style=\"font-weight: 400;\">But Depo-Testosterone, the cheapest version of the hormone, which is manufactured by Pfizer, has run out in South Africa three times for between one and 12 months at a time since 2018. The latest stockout started at the end of 2021 and </span><span style=\"font-weight: 400;\">a March statement from </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/10/Depo-Testosterone-Out-of-Stock-Letter-Updated-March-2022.png\"><span style=\"font-weight: 400;\">Pfizer</span></a><span style=\"font-weight: 400;\"> (which is dated incorrectly) said its product could only be back on the shelves – around the world – somewhere between April and June 2023.</span>\r\n\r\n<i><span style=\"font-weight: 400;\">Read in </span></i><span style=\"font-weight: 400;\">Daily Maverick</span><i><span style=\"font-weight: 400;\">: “</span></i><a href=\"https://www.dailymaverick.co.za/article/2021-09-16-gender-affirming-surgery-little-more-than-a-distant-dream-for-the-many-hamstrung-by-high-costs-and-long-waiting-lists/\"><i><span style=\"font-weight: 400;\">Gender-affirming surgery little more than a distant dream for the many hamstrung by high costs and long waiting lists</span></i></a><i><span style=\"font-weight: 400;\">”</span></i><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">The pharmaceutical company didn’t respond to requests for comment about the reason for the shortage, but the manufacturer’s spokespeople have told </span><a href=\"https://www.reuters.com/article/safrica-lgbt-medicine-trfn-idUSKBN2JU1AY\"><span style=\"font-weight: 400;\">numerous</span></a><span style=\"font-weight: 400;\"> other news outlets that it was prioritising Covid vaccines. </span>\r\n\r\n<span style=\"font-weight: 400;\">The current stockout means the just more than </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/11/TG-PSE-Draft-Report-v-0_district-level-final-editable-version-17April2022-1.pdf\"><span style=\"font-weight: 400;\">28,000 transgender men</span></a><span style=\"font-weight: 400;\"> in South Africa have few affordable options for getting hormone treatment. </span>\r\n\r\n<span style=\"font-weight: 400;\">A 10ml vial of Depo-Testosterone costs </span><a href=\"https://medicineprices.org.za/#related:718548019\"><span style=\"font-weight: 400;\">just under R650</span></a><span style=\"font-weight: 400;\">. The dose people inject depends on how long they’ve been on the treatment. </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">Local guidelines</span></a><span style=\"font-weight: 400;\"> say people should start with injecting </span><a href=\"https://www.pfizermedicalinformation.com/en-us/depo-testosterone/dosage-admin\"><span style=\"font-weight: 400;\">1ml</span></a><span style=\"font-weight: 400;\"> into the buttocks </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/11/Testosterone-Gender-Dysphoria-Masculinising_N_December-2019.pdf\"><span style=\"font-weight: 400;\">once every two weeks</span></a><span style=\"font-weight: 400;\">. (Photo: Dylan Bush / Bhekisisa)[/caption]\r\n\r\n<span style=\"font-weight: 400;\">This dose is usually increased gradually until the amount of testosterone in the blood is within </span><a href=\"https://d31kydh6n6r5j5.cloudfront.net/uploads/sites/161/2019/08/hormones_FTM.pdf\"><span style=\"font-weight: 400;\">the range usually found in males</span></a><span style=\"font-weight: 400;\">. The maximum dose is 2ml every two weeks. In that case, one vial will last for 10 weeks. </span>\r\n\r\n<span style=\"font-weight: 400;\">The other choice available in South Africa is the drug company Grünenthal’s longer-lasting </span><a href=\"https://www.nebido.com\"><span style=\"font-weight: 400;\">Nebido</span></a><span style=\"font-weight: 400;\"> injection, which costs</span><a href=\"https://medicineprices.org.za/#related:706461001\"><span style=\"font-weight: 400;\"> between R2,400 and R2,700</span></a><span style=\"font-weight: 400;\"> and needs to be administered every 10 to 14 weeks. </span>\r\n\r\n<span style=\"font-weight: 400;\">This is between three to four times more expensive than Depo-Testosterone, depending on the required dose. </span>\r\n\r\n<span style=\"font-weight: 400;\">In South Africa, testosterone is on the </span><a href=\"https://www.knowledgehub.org.za/system/files/elibdownloads/2022-07/Tertiary%20and%20Quaternary%20Level%20Essential%20Medicine%20Recommendations_June%202022%20%281%29.pdf\"><span style=\"font-weight: 400;\">essential medicines list for tertiary and district hospitals</span></a><span style=\"font-weight: 400;\">, which means the Health Department buys it for these facilities.</span>\r\n\r\n<span style=\"font-weight: 400;\">But there aren’t many hospitals that have programmes to provide gender-affirming care – only </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/10/Spencer-The-care-is-the-best-you-can-give-at-the-time.pdf\"><span style=\"font-weight: 400;\">six government facilities nationwide</span></a><span style=\"font-weight: 400;\">, and they are located in just three of the nine provinces, according to a 2017 study. Only four of those offer endocrinology, the category in which testosterone falls. </span>\r\n\r\n<span style=\"font-weight: 400;\">In addition, state facilities have to stick to buying the product of the tender that the department accepted. Pfizer’s Depo-Testosterone is the only hormone treatment for trans men on the list, says Elma de Vries, </span><span style=\"font-weight: 400;\">the programme coordinator for medical students at the Nelson Mandela Metropolitan University in Gqeberha. </span>\r\n\r\n<span style=\"font-weight: 400;\">A few public health facilities have applied for special permission to buy Nebido for their existing patients, she explains. </span>\r\n\r\n<span style=\"font-weight: 400;\">The Health Department didn’t respond to queries about whether an alternative to Pfizer’s testosterone shot will be put on the essential medicines list permanently to ensure people don’t run out of treatment because of stockouts. </span>\r\n<h4>Third time unlucky</h4>\r\n<span style=\"font-weight: 400;\">During the first stockout of Pfizer’s Depo-Testosterone in 2018, Ndlovu had a vial that lasted him throughout most of the shortage. When it began to empty, he flew to Cape Town to a pharmacy that had stock </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> he could only get one vial at a time.</span>\r\n\r\n<span style=\"font-weight: 400;\">The next time the medicine ran out in 2019, it didn’t affect him because he had enough testosterone to use until stock was in again. </span>\r\n\r\n[caption id=\"attachment_1464435\" align=\"alignnone\" width=\"720\"]<img class=\"size-full wp-image-1464435\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/11/MC-Testosterone_4.jpg\" alt=\"\" width=\"720\" height=\"433\" /> A needle and syringe with a shot of Nebido testosterone drawn. (Photo: Dylan Bush / Bhekisisa)[/caption]\r\n\r\n<span style=\"font-weight: 400;\">But at the end of 2021, Pfizer’s product ran out again </span><span style=\"font-weight: 400;\">–</span><span style=\"font-weight: 400;\"> and so did Ndlovu’s luck. He couldn’t find the hormone anywhere. He emailed every pharmacy he could think of and even got in touch with doctors in the US to find out what it would cost to ship some here. </span>\r\n\r\n<span style=\"font-weight: 400;\">“My life almost crumbled,” he says. </span>\r\n\r\n<span style=\"font-weight: 400;\">Alongside physical changes, people who use testosterone and then suddenly stop also experience </span><a href=\"https://www.tandfonline.com/doi/pdf/10.1080/26895269.2022.2100644\"><span style=\"font-weight: 400;\">fatigue and brain fog</span></a><span style=\"font-weight: 400;\">, which affects their ability to think clearly or concentrate, and </span><a href=\"https://www.tandfonline.com/doi/pdf/10.1080/26895269.2022.2100644\"><span style=\"font-weight: 400;\">mood changes</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Taking testosterone had been a lifeline for Ndlovu for four years – that’s why he was at the dodgy doctor’s office: he was on his last vial, with only enough for three more injections (so he had six weeks’ supply). </span>\r\n<h4>Back-alley back-track: ‘I just felt unsafe’</h4>\r\n<span style=\"font-weight: 400;\">The man seated in the cramped back-alley doctor’s room offered Ndlovu one large dose of testosterone, which he said would last much longer than his usual shots would. </span>\r\n\r\n<span style=\"font-weight: 400;\">Before someone starts taking testosterone, their doctor does a </span><a href=\"https://bhekisisa.org/wp-content/uploads/2022/10/gardner2013.pdf\"><span style=\"font-weight: 400;\">blood test</span></a><span style=\"font-weight: 400;\"> to find out how much they should take per shot, usually </span><a href=\"https://sahivsoc.org/Files/SAHCS%20GAHC%20guidelines.pdf\"><span style=\"font-weight: 400;\">starting on 0.5ml a week and increasing to a maximum of 2ml every two weeks</span></a><span style=\"font-weight: 400;\">, De Vries says. </span>\r\n\r\n<i><span style=\"font-weight: 400;\">Read in </span></i><span style=\"font-weight: 400;\">Daily Maverick</span><i><span style=\"font-weight: 400;\">: “</span></i><a href=\"https://www.dailymaverick.co.za/article/2022-10-18-a-plea-to-parents-listen-to-trans-kids-not-moral-panics/\"><i><span style=\"font-weight: 400;\">A plea to parents: Listen to trans kids, not moral panics</span></i></a><i><span style=\"font-weight: 400;\">”</span></i>\r\n\r\n<span style=\"font-weight: 400;\">Taking the wrong dose can lead to </span><a href=\"https://link.springer.com/article/10.1007/s11154-018-9454-3\"><span style=\"font-weight: 400;\">heart diseases</span></a><span style=\"font-weight: 400;\">, she says. </span>\r\n\r\n<span style=\"font-weight: 400;\">Ndlovu decided to leave without the testosterone.</span>\r\n\r\n<span style=\"font-weight: 400;\">“I didn’t like it [the set-up]. It just felt unsafe,” he says as he rolls a cigarette and speaks to </span><i><span style=\"font-weight: 400;\">Bhekisisa </span></i><span style=\"font-weight: 400;\">at his home in Sandton. </span>\r\n\r\n<span style=\"font-weight: 400;\">He remembers thinking: “I guess I’m going to stop taking T [testosterone].” </span>\r\n<h4>Enter Fagron – an imperfect lifeline</h4>\r\n<span style=\"font-weight: 400;\">Just before Ndlovu’s final vial ran out, a friend messaged him with a lifeline. </span>\r\n\r\n<a href=\"https://fagron.co.za/\"><span style=\"font-weight: 400;\">Fagron</span></a><span style=\"font-weight: 400;\">, a pharmaceutical company in Cape Town, could make a customised treatment that would be similar to Depo-Testosterone by mixing the required ingredients on a case-by-case basis. This is called making up a </span><a href=\"http://www.acpsa.co.za/about/index.html\"><span style=\"font-weight: 400;\">compounded medicine</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">It was Tomson’s plan. She asked Fagron’s lead pharmacist to make up something similar to Depo-Testosterone when Pfizer announced the third stockout in 2021. </span>\r\n\r\n<span style=\"font-weight: 400;\">The Fagron formulation costs R475 for a 10ml vial. Adding a delivery fee to get it shipped from Cape Town to Johannesburg, for example, would bring the total to about R600. </span>\r\n\r\n<hr />\r\n\r\n<strong>Visit <a href=\"https://www.dailymaverick.co.za?utm_source=direct&utm_medium=in_article_link&utm_campaign=homepage\"><em>Daily Maverick's</em> home page</a> for more news, analysis and investigations</strong>\r\n\r\n<hr />\r\n\r\n<span style=\"font-weight: 400;\">But even with this workaround, treatment might still be unaccessible to many transgender men who get their testosterone for free from the few state facilities that have it. </span>\r\n\r\n<span style=\"font-weight: 400;\">Compounded medicines are </span><a href=\"https://www.sahpra.org.za/wp-content/uploads/2020/01/b335d1c0Act_101_of_1965_published_2003.pdf\"><span style=\"font-weight: 400;\">not allowed to be sold in retail stores and can only be bought from the compounding pharmacy with a doctor’s prescription</span></a><span style=\"font-weight: 400;\">. Even if a state patient gets a script from their clinic, Tomson says they would still have to pay the cost of the medicine themselves, since they would have to buy the product directly from the pharmacy that compounds it.</span>\r\n<h4>Getting by with a little help from friends</h4>\r\n<span style=\"font-weight: 400;\">For someone such as Malik Moyo (45), who makes a living selling handmade crafts, beanies, baskets and bracelets at flea markets in Johannesburg, the Fagron solution won’t work. </span>\r\n\r\n<span style=\"font-weight: 400;\">Money has been tight since the Covid lockdowns that started early in 2020 outlawed public gatherings, including markets, for a large part of two years. </span>\r\n\r\n<span style=\"font-weight: 400;\">Fortunately, Moyo could get their testosterone for free from the </span><a href=\"https://www.wrhi.ac.za/\"><span style=\"font-weight: 400;\">Wits Reproductive Health and HIV Institute</span></a><span style=\"font-weight: 400;\"> (Wits RHI), a nonprofit in Johannesburg. </span>\r\n\r\n<span style=\"font-weight: 400;\">Wits RHI provides gender-affirming hormone therapy as part of its HIV services. When gender-affirming hormones, counselling and hormone testing are available alongside HIV treatment and testing, </span><a href=\"https://pdf.usaid.gov/pdf_docs/PA00X63W.pdf\"><span style=\"font-weight: 400;\">research shows</span></a><span style=\"font-weight: 400;\"> it significantly increases how likely people are to start antiretrovirals for HIV prevention treatment. </span>\r\n\r\n<span style=\"font-weight: 400;\">But late in 2021, at the start of the third stockout, a Wits RHI doctor told Moyo the centre no longer had any of the hormone products to offer – and neither did any of the nearby government facilities. </span>\r\n\r\n[caption id=\"attachment_1464436\" align=\"alignnone\" width=\"720\"]<img class=\"size-full wp-image-1464436\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/11/MC-Testosterone_5.jpg\" alt=\"\" width=\"720\" height=\"450\" /> Mandla Ndlovu*, a transgender man living in Johannesburg.
