"contents": "<span style=\"font-weight: 400;\">Being a doctor demands incredible emotional resilience and fortitude, as you serve people at their most vulnerable of times. Being a doctor in the South African public healthcare sector often means that the emotional reserve required is amplified tenfold. </span>\r\n\r\n<span style=\"font-weight: 400;\">Not only are you trying to serve sick people to the best of your capabilities, but you are constantly fighting multiple roadblocks completely out of your control. These present themselves in the ugliest of forms, hindering your every effort, and include lack of basic resources, environmental constraints and the cumulative burden you carry around of all the patients who suffered (or worse) when you were on duty, purely due to the lack of the former. </span>\r\n\r\n<span style=\"font-weight: 400;\">My colleagues and I will often joke that South African doctors are “traumatised” from the conditions we work under and that we all need a gap year or two to recover, and while we often use humour to make light of the situation we find ourselves in, the reality is surreal. </span>\r\n\r\n<span style=\"font-weight: 400;\">Entering medical school starry-eyed and idealistic is a dream quickly extinguished by any public hospital in South Africa and the challenges it poses, and many of us are faced with situations that scar us for a long time. </span>\r\n\r\n<span style=\"font-weight: 400;\">Seeing patients treated suboptimally, witnessing death that would have been avoidable were resources available, working hours and under conditions that leave us mentally and physically exhausted and, worst of all, behaving in ways completely uncharacteristic because you literally cannot cope any longer — it is certainly not as glamorous as what the medical dramas on TV portray.</span>\r\n\r\n<span style=\"font-weight: 400;\">It is a hallmark of deteriorating leadership and a corrupted society when honest men and women who speak out against injustice are victimised. </span>\r\n\r\n<p><a href=\"https://www.dailymaverick.co.za/article/2022-05-25-doctors-support-paediatricians-account-of-collapse-at-rahima-moosa-as-gauteng-health-officials-push-back/mc-rahimamoosa_12-2/\" rel=\"attachment wp-att-1269573\"><img loading=\"lazy\" class=\"size-full wp-image-1269573\" src=\"https://www.dailymaverick.co.za/wp-content/uploads/2022/05/MC-RahimaMoosa_12-1-e1653232347460.jpg\" alt=\"The sickening truth about South Africa's public hospitals\" width=\"720\" height=\"416\" /></a> Dr Tim De Maayer is a paediatric gastroenterologist at Rahima Moosa Mother and Child Hospital. (Photo: Supplied)</p>\r\n<h4><b>Boiling point</b></h4>\r\n<span style=\"font-weight: 400;\">I was privileged enough to work in the same department as Dr Tim de Maayer at Rahima Moosa Mother and Child Hospital during my training as an intern a couple of years ago. Paediatrics was one of the most challenging rotations as a junior doctor, not only because of the huge responsibility of dealing with the lives of little and very vulnerable humans, but also because of the increased numbers of patients and demanding working hours. </span>\r\n\r\n<span style=\"font-weight: 400;\">During that time, it was easy to draw inspiration from consultants such as Dr De Maayer, a doctor who not only is an exceptional clinician, but a great man of compassion and empathy. </span>\r\n\r\n<span style=\"font-weight: 400;\">When I read </span><a href=\"https://www.dailymaverick.co.za/article/2022-05-22-a-wake-up-call-for-health-department-heads-children-are-dying-because-of-horrendous-state-of-our-public-hospitals/\"><span style=\"font-weight: 400;\">his letter to the administrators of the healthcare system</span></a><span style=\"font-weight: 400;\">, it was heartbreaking to think of the magnitude of the problem that would drive an otherwise amicable, pleasant and unbelievably devoted doctor to take such a definitive and solid stand. </span>\r\n\r\n<span style=\"font-weight: 400;\">Something must be very wrong, and indeed it is. </span>\r\n\r\n<span style=\"font-weight: 400;\">I believe Dr De Maayer’s letter is the boiling point of the persistent and steady crumbling of the public healthcare system that has been grumbling along for years. </span>\r\n\r\n<span style=\"font-weight: 400;\">The question is, when will too many lives unnecessarily lost be accepted as enough? What is happening at Rahima Moosa hospital isn’t isolated to that hospital — it is a problem that has metastasised throughout the healthcare system in South Africa. </span>\r\n\r\n<span style=\"font-weight: 400;\">When people like Dr de Maayer and Professor Ebrahim Variawa come bravely forward to point out the glaring deficiencies in one of the basic building blocks of our society, their pleas should be heard with earnestness. </span>\r\n\r\n<span style=\"font-weight: 400;\">What they say is a representation of what most — if not all — doctors in the public healthcare system experience and feel. Their frustrations originate from intentions of concern, genuine care and devotion to patients, as well as purity of service. </span>\r\n\r\n<a href=\"https://www.dailymaverick.co.za/article/2022-06-09-dr-tim-de-maayer-who-blew-the-whistle-on-shocking-state-of-childrens-healthcare-at-rahima-moosa-hospital-is-suspended/\"><span style=\"font-weight: 400;\">Actions of suspension</span></a><span style=\"font-weight: 400;\">, whether temporarily or not, reflect an ugly disposition of those in charge to shift the blame away from the true (albeit harsh) reality. Petitions, outpourings of support and objections should not be needed to </span><a href=\"https://www.dailymaverick.co.za/article/2022-06-10-whistleblower-paediatricians-suspension-lifted-after-massive-public-outcry/\"><span style=\"font-weight: 400;\">reverse suspensions</span></a><span style=\"font-weight: 400;\"> — they shouldn’t be happening in the first place.</span>\r\n\r\n<span style=\"font-weight: 400;\">Dr De Maayer’s letter is a desperate plea for reform and represents what most doctors in SA feel — he reiterates our frustrations, our anger at not being able to efficiently serve, our burnout and emotional distress and most importantly our dedication to the people who need it the most. </span>\r\n\r\n<span style=\"font-weight: 400;\">His letter and the response to it have been the topic of discussion among doctors nationwide — and the message is clear: we are tired of not being able to properly serve the people of this country because of a broken system that offers us inadequate support. His open letter was widely praised as he exposed the shocking state of children’s healthcare at the Rahima Moosa Mother and Child Hospital.</span>\r\n\r\n<span style=\"font-weight: 400\">Dr Tim De Maayer did what brave health workers have always done. Having done everything in his power to care for the desperately ill children at Rahima Moosa Mother and Child Hospital, he raised issues with management and, having seen no change, he went public. </span>\r\n\r\n<span style=\"font-weight: 400\">In June 2022, </span><a href=\"https://www.dailymaverick.co.za/article/2022-06-09-dr-tim-de-maayer-who-blew-the-whistle-on-shocking-state-of-childrens-healthcare-at-rahima-moosa-hospital-is-suspended/\"><span style=\"font-weight: 400\">he was placed on precautionary suspension</span></a><span style=\"font-weight: 400\"> with immediate effect after criticising the state of healthcare provided to patients at the hospital. The resulting public outcry saw him </span><a href=\"https://www.dailymaverick.co.za/article/2022-06-10-whistleblower-paediatricians-suspension-lifted-after-massive-public-outcry/\"><span style=\"font-weight: 400\">being reinstated.</span></a>\r\n\r\n<span style=\"font-weight: 400\">The consequences of the punitive action against those speaking out in healthcare led to many South African healthcare workers joining the</span><a href=\"https://www.dailymaverick.co.za/article/2022-06-21-i-am-movement-of-sa-health-workers-throws-down-the-gauntlet-to-health-minister-and-gauteng-premier/\"><span style=\"font-weight: 400\"> “I am” movement</span></a><span style=\"font-weight: 400\"> in 2022, speaking up against continued victimisation. </span>\r\n\r\n<span style=\"font-weight: 400\">The Office of the Health Ombud released </span><a href=\"https://healthombud.org.za/publications/reports/investigation-report-into-allegations-against-rahima-moosa-mother-and-child/\"><span style=\"font-weight: 400\">its report</span></a><span style=\"font-weight: 400\"> to minister of health Dr Joe Phaahla on Tuesday 14 March 2023, </span><a href=\"https://www.dailymaverick.co.za/article/2023-03-14-rahima-moosa-mother-child-hospital-is-a-filthy-and-neglected-mess-says-health-ombud/\"><span style=\"font-weight: 400\">noting</span></a><span style=\"font-weight: 400\"> that the health and dignity of patients and the wellbeing of healthcare workers were severely compromised.</span>\r\n\r\n<span style=\"font-weight: 400\">The investigative team spent close to a year investigating the allegations and concluded that there was incontrovertible proof that confirmed the complaints. The team obtained video footage that corroborated their findings, and whistle-blower Dr Tim De Maayer contributed significantly to the report.</span>",
