"contents": "<span style=\"font-weight: 400;\">Before the end of the year, 867 government health facilities will start to roll out the two-monthly HIV prevention injection CAB-LA, using 96,000 doses donated by the US government. </span>\r\n\r\n<span style=\"font-weight: 400;\">Studies show that CAB-LA, which, after being injected into someone’s buttocks, gradually releases the antiretroviral drug cabotegravir over two months into someone’s blood, can virtually </span><a href=\"https://www.hptn.org/news-and-events/press-releases/hptn-084-study-demonstrates-superiority-of-cab-la-to-oral-tdfftc-for\"><span style=\"font-weight: 400;\">eliminate</span></a><span style=\"font-weight: 400;\"> their chances of contracting HIV through sex, and can stop about a third of South Africa’s infections </span>\r\n\r\n<span style=\"font-weight: 400;\">In 2022, </span><a href=\"https://www.unaids.org/en/regionscountries/countries/southafrica\"><span style=\"font-weight: 400;\">about 150,000 adults</span></a> <span style=\"font-weight: 400;\">– 15 and older – </span><span style=\"font-weight: 400;\">in South Africa contracted HIV. </span><a href=\"https://hsrc.ac.za/press-releases/hsc/new-hiv-survey-highlights-progress-and-ongoing-disparities-in-south-africas-hiv-epidemic/\"><span style=\"font-weight: 400;\">About one in eight</span></a><span style=\"font-weight: 400;\"> adults have the virus. </span>\r\n\r\n<span style=\"font-weight: 400;\">Early results of four demonstration studies released at the </span><a href=\"https://www.iasociety.org/conferences/aids2024\"><span style=\"font-weight: 400;\">25th International Conference on Aids</span></a><span style=\"font-weight: 400;\"> in Munich over the weekend show that when people have a choice between taking a daily HIV prevention pill, using the monthly vaginal ring or getting a CAB-LA shot, which only has to be taken every other month, up to three-quarters go for the jab. For example, researchers from the Desmond Tutu Health Foundation in Cape Town found that among 1,084 people between 19 and 28 years old, </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/07/Session-4-Presentation-2_Elzette-Rousseau_updated.pptx\"><span style=\"font-weight: 400;\">74% chose the injection</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The jab, which is </span><a href=\"https://viivhealthcare.com/hiv-news-and-media/news/company-statements/progress-continues-to-accelerate-access/\"><span style=\"font-weight: 400;\">made by the British manufacturer ViiV Healthcare</span></a><span style=\"font-weight: 400;\">, was added to HIV prevention options at 16 research sites between February and June, Hasina Subedar, a senior technical adviser for the national Health Department, told Bhekisisa</span> <span style=\"font-weight: 400;\">at the conference.</span>\r\n\r\n<span style=\"font-weight: 400;\">Although it’s too early to comment on trends from these studies, or draw clear conclusions, Subedar says “the long-acting cabotegravir injection is gaining momentum as a popular choice among both young men and women”.</span>\r\n<h4><b>South Africa got a two-year donation of more than 200,000 doses</b></h4>\r\n<span style=\"font-weight: 400;\">The government accepted the 96,000 CAB-LA doses, which will arrive between October and December 2024, as part of a larger donation of 231,000 doses that Pepfar is releasing over two years, says the Health Department’s director-general, Sandile Buthelezi. </span>\r\n\r\n<span style=\"font-weight: 400;\">Buthelezi, who signed South Africa’s acceptance of the donation on 27 June, says the department accepted the donation on the basis that cheaper generic versions of CAB-LA are likely to become available in early 2027, which will help the country to sustain supplies, so that people “don’t have to stop and start”. </span>\r\n\r\n<span style=\"font-weight: 400;\">Buthelezi says other donors such as the Global Fund for Aids, TB and Malaria “also have shown appetite” to donate CAB-LA, and people started on this medicine will be able to switch to another injection, lenacapavir, which only has to be given once every six months, should they choose to and if it becomes available in South Africa. </span>\r\n\r\n<span style=\"font-weight: 400;\">A recent study </span><a href=\"https://www.gilead.com/news-and-press/press-room/press-releases/2024/6/gileads-twiceyearly-lenacapavir-demonstrated-100-efficacy-and-superiority-to-daily-truvada-for-hiv-prevention\"><span style=\"font-weight: 400;\">showed that none of 2,134 women</span></a><span style=\"font-weight: 400;\"> who received lenacapavir contracted HIV, but the drug is not yet registered with the country’s medicines regulator, the South African Health Products Regulatory Authority (Sahpra). CAB-LA was </span><a href=\"https://www.sahpra.org.za/press-releases/sahpra-registers-new-long-acting-hiv-pre-exposure-prophylaxis/\"><span style=\"font-weight: 400;\">registered with Sahpra in December 2022</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The 96,000 CAB-LA doses are enough for 13,728 people to protect themselves against HIV infection for one year, says Subedar. </span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa will receive the remainder of the 231,000 doses from October 2025, the beginning of the US financial year. </span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa will be the fourth African country </span><span style=\"font-weight: 400;\">– after </span><a href=\"https://www.prepwatch.org/wp-content/uploads/2024/07/June-2024-PLN-Presentation.pdf\"><span style=\"font-weight: 400;\">Zambia (which started roll-out in February), Zimbabwe</span></a><span style=\"font-weight: 400;\"> (in April) and Malawi (April) –</span><span style=\"font-weight: 400;\"> to offer CAB-LA for free in state clinics and hospitals as part of its health programmes.</span>\r\n\r\n<span style=\"font-weight: 400;\">At an Aids conference session, Ashneil Jain of the US department of state, said Pepfar plans to sponsor CAB-LA in at least 12 countries before the end of the year. </span>\r\n<h4><b>CAB-LA costs four times more than the daily prevention pill</b></h4>\r\n<span style=\"font-weight: 400;\">Research shows CAB-LA </span><a href=\"https://www.hptn.org/news-and-events/press-releases/hptn-084-study-demonstrates-superiority-of-cab-la-to-oral-tdfftc-for\"><span style=\"font-weight: 400;\">works better</span></a><span style=\"font-weight: 400;\"> than the daily HIV prevention pill, which, by the end of April, was available for free in </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/07/240628_June-2024-Data-Mia-1.pptx\"><span style=\"font-weight: 400;\">93% (3,241 out of 3,484) of the government’s primary health clinics</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The </span><a href=\"https://www.hiv.gov/hiv-basics/hiv-prevention/using-hiv-medication-to-reduce-risk/pre-exposure-prophylaxis\"><span style=\"font-weight: 400;\">pill contains two antiretroviral drugs</span></a><span style=\"font-weight: 400;\"> (ARVs), </span><span style=\"font-weight: 400;\">emtricitabine and tenofovir disoproxil fumarate. Once</span><span style=\"font-weight: 400;\"> there is enough of these two medicines in an HIV-negative person’s vaginal or anal tissues, their white blood cells are shielded against HIV. </span>\r\n\r\n<span style=\"font-weight: 400;\">If the pill, also referred to as oral pre-exposure prophylaxis (PrEP), is used consistently, in other words each day, it can lower someone’s chances of contracting HIV </span><a href=\"https://medlineplus.gov/hivprepandpep.html\"><span style=\"font-weight: 400;\">by at least 90%</span></a><span style=\"font-weight: 400;\">, but many people struggle with adherence. </span>\r\n\r\n<span style=\"font-weight: 400;\">CAB-LA, on the other hand, only has to be </span><a href=\"https://www.who.int/news/item/28-07-2022-who-recommends-long-acting-cabotegravir-for-hiv-prevention\"><span style=\"font-weight: 400;\">taken every other month</span></a><span style=\"font-weight: 400;\">, making it easier to stick to it. </span>\r\n\r\n<span style=\"font-weight: 400;\">But it </span><a href=\"https://bhekisisa.org/health-news-south-africa/2023-11-16-slash-the-price-by-three-quarters-government-on-anti-hiv-jab/\"><span style=\"font-weight: 400;\">is four times more expensive</span></a><span style=\"font-weight: 400;\"> than the daily pill. The department buys a month’s supply of the pill </span><a href=\"https://bhekisisa.org/wp-content/uploads/2024/07/HP13-2025ARV-Reference-Price-list_15-July-2024.pdf\"><span style=\"font-weight: 400;\">for R64.50</span></a><span style=\"font-weight: 400;\">. The injection, however, costs the government £23.50</span><span style=\"font-weight: 400;\"> (about R555 at the curr</span><span style=\"font-weight: 400;\">ent exchange rate) a shot, </span><a href=\"https://bhekisisa.org/multimedia/2023-11-10-how-and-when-cabla-two-monthly-hiv-prevention-injection-will-get-to-sa/\"><span style=\"font-weight: 400;\">which is the price </span><span style=\"font-weight: 400;\">ViiV Healthcare said in November 2023</span></a><span style=\"font-weight: 400;\"> it will sell the medicine at in </span><a href=\"https://data.worldbank.org/country/XO\"><span style=\"font-weight: 400;\">90 countries that the World Bank classifies as having low- and middle-income status</span></a><span style=\"font-weight: 400;\">, including South Africa, and also to donors. </span>\r\n\r\n<span style=\"font-weight: 400;\">The Health Department’s essential medicines list committee decides whether to recommend a PrEP product, based on whether it is cost effective. For this, it compares the cost of the medicine with that of the HIV prevention pill. </span>\r\n\r\n<span style=\"font-weight: 400;\">The department can only buy medicines that are on the essential medicines list. Because CAB-LA has not been approved yet by the committee, the department cannot procure it. </span>\r\n\r\n<span style=\"font-weight: 400;\">The committee will be able to consider recommending CAB-LA. ViiV Healthcare offers CAB-LA to the department at a competitive price, comparable to that of the daily pill. </span>\r\n\r\n<span style=\"font-weight: 400;\">The 867 clinics chosen to stock Pepfar-sponsored CAB-LA have been selected because they have had a high uptake of oral PrEP </span><span style=\"font-weight: 400;\">– at least 20 new clients had to be given the pill in the past financial year, which started in March, says Subedar. </span>\r\n\r\n<span style=\"font-weight: 400;\">She says the department also had to ensure the donations are given to at least one facility in each local municipality. </span><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">“If we run out of donations, and if there are no cheaper generic formulations or the department has not been successful in securing CAB-LA at a lower price,” she says,” we do have oral PrEP that is effective.” </span><b>DM</b>\r\n\r\n<i><span style=\"font-weight: 400;\">This story was produced by the</span></i><a href=\"http://bhekisisa.org./\"> <i><span style=\"font-weight: 400;\">Bhekisisa Centre for Health Journalism</span></i></a><i><span style=\"font-weight: 400;\">.
