"contents": "<span style=\"font-weight: 400;\">The blaming of foreign nationals for the state of South Africa’s public healthcare system does nothing to fix the very challenges that those espousing this unsubstantiated rhetoric claim to want to improve.</span>\r\n\r\n<span style=\"font-weight: 400;\">In fact, and as others have pointed out (for example see</span><a href=\"https://www.dailymaverick.co.za/article/2022-08-28-limpopo-health-mecs-berating-of-zimbabwean-patient-is-appalling-and-shameful/\"> <span style=\"font-weight: 400;\">here</span></a><span style=\"font-weight: 400;\">,</span><a href=\"https://www.politicsweb.co.za/politics/limpopo-mec-of-health-conducted-herself-unprofessi\"> <span style=\"font-weight: 400;\">here</span></a><span style=\"font-weight: 400;\"> and</span><a href=\"https://www.dailymaverick.co.za/opinionista/2022-09-05-blaming-immigrants-for-our-woes-is-the-oldest-and-worst-trick-in-the-book-why-do-people-still-fall-for-it/\"> <span style=\"font-weight: 400;\">here</span></a><span style=\"font-weight: 400;\">), this narrative of blame simply presents a distraction that successfully diverts the focus away from the very real financial and administrative</span><a href=\"https://idpjournal.biomedcentral.com/articles/10.1186/s40249-021-00804-9#:~:text=The%20South%20African%20health%20system%20faces%20a%20range%20of%20systemic,and%20patient%20management%20%5B8%5D.\"> <span style=\"font-weight: 400;\">challenges</span></a><span style=\"font-weight: 400;\"> crippling the system.</span>\r\n\r\n<span style=\"font-weight: 400;\">As a result, this scapegoating of foreign nationals undermines efforts currently under way to improve the public healthcare system that about</span><a href=\"https://www.statssa.gov.za/publications/Report-03-10-19/Report-03-10-192017.pdf\"> <span style=\"font-weight: 400;\">80%</span></a><span style=\"font-weight: 400;\"> of the population of South Africa is reliant on, while taking us further down a dangerous path of hostility and division.</span>\r\n\r\n<span style=\"font-weight: 400;\">We are </span><a href=\"https://www.dailymaverick.co.za/opinionista/2022-09-05-blaming-immigrants-for-our-woes-is-the-oldest-and-worst-trick-in-the-book-why-do-people-still-fall-for-it/\"><span style=\"font-weight: 400;\">again</span></a><span style=\"font-weight: 400;\"> witnessing the apportioning of blame for the poor state of South Africa’s public healthcare system on foreign nationals. This time, we have not only heard government officials — including the Limpopo</span><a href=\"https://www.dailymaverick.co.za/article/2022-08-28-limpopo-health-mecs-berating-of-zimbabwean-patient-is-appalling-and-shameful/\"> <span style=\"font-weight: 400;\">MEC for Health, Dr Phophi Ramathuba</span></a><span style=\"font-weight: 400;\"> and (</span><a href=\"https://www.dailymaverick.co.za/article/2019-09-26-in-sickness-and-in-health-why-the-minister-of-home-affairs-has-a-duty-of-care-to-tackle-endemic-anti-foreigner-sentiment/\"><span style=\"font-weight: 400;\">once again</span></a><span style=\"font-weight: 400;\">) </span><span style=\"font-weight: 400;\">the</span><a href=\"https://www.iol.co.za/pretoria-news/news/aaron-motsoaledi-limpopo-ancyl-back-phophi-ramathuba-as-joe-phaahla-set-to-get-zim-patient-report-29d9642a-a38d-4455-936a-3cf6e9a971f0\"> <span style=\"font-weight: 400;\">Minister of Home Affairs, Dr Aaron Motsoaledi</span></a><span style=\"font-weight: 400;\"> — dangerously inflate the numbers of foreign nationals in South Africa and scapegoat them for the state of the country’s healthcare system, but we have also seen foreign nationals</span><a href=\"https://www.msf.org.za/news-and-resources/press-release/intensifying-xenophobic-climate-and-politicisation-healthcare\"> <span style=\"font-weight: 400;\">targeted verbally and blocked from accessing</span></a><span style=\"font-weight: 400;\"> public clinics and hospitals.</span>\r\n\r\n<span style=\"font-weight: 400;\">This affects everyone reliant on accessing public healthcare: it restricts access to facilities and wastes the valuable resources of the Department of Health in having to respond to this, including, for example, the need to</span><a href=\"https://www.citizen.co.za/news/south-africa/3184634/gauteng-health-gets-interdict-august-2022/\"> <span style=\"font-weight: 400;\">obtain an interdict</span></a><span style=\"font-weight: 400;\"> against protesters.</span>\r\n<h4><b>Worrying misconception</b></h4>\r\n<span style=\"font-weight: 400;\">This inflation of the numbers of foreign nationals in the country is worrying: nearly one-quarter of the South Africans interviewed in a</span><a href=\"https://www.jstor.org/stable/27027853\"> <span style=\"font-weight: 400;\">2019 survey</span></a><span style=\"font-weight: 400;\"> conducted by the Human Sciences Research Council in Limpopo, KwaZulu-Natal, Gauteng and the Western Cape believed that the foreign-born population</span><a href=\"https://www.jstor.org/stable/27027853\"> <span style=\"font-weight: 400;\">exceeded 50%</span></a><span style=\"font-weight: 400;\"> of a country of</span><a href=\"https://www.statssa.gov.za/?p=15601\"> <span style=\"font-weight: 400;\">60.6 million</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">In reality — as</span><a href=\"http://www.statssa.gov.za/publications/P0302/P03022021.pdf\"> <span style=\"font-weight: 400;\">determined by the government’s own national statistics department, Stats SA</span></a><span style=\"font-weight: 400;\"> — just under four million, amounting to</span><a href=\"http://www.statssa.gov.za/publications/P0302/P03022021.pdf\"> <span style=\"font-weight: 400;\">6.7%</span></a><span style=\"font-weight: 400;\"> of the South African population, are estimated to be foreign nationals. This figure includes </span><a href=\"https://www.statssa.gov.za/?p=14569\"><span style=\"font-weight: 400;\">all categories of migrants</span></a><span style=\"font-weight: 400;\">, regardless of documentation status.</span>\r\n\r\n<span style=\"font-weight: 400;\">Critically, there is</span><a href=\"https://www.hst.org.za/publications/South%20African%20Health%20Reviews/9_Towards%20a%20migration%20aware%20health%20system%20in%20South%20Africa_a%20strategic%20opportunity%20to%20address%20health%20inequity.pdf\"> <span style=\"font-weight: 400;\">no evidence</span></a><span style=\"font-weight: 400;\"> to suggest that foreign nationals place an undue burden on the public healthcare system. While officials will often make unsubstantiated claims about the numbers of foreign nationals using services in specific facilities, the</span><a href=\"https://pmg.org.za/committee-question/19188/\"> <span style=\"font-weight: 400;\">minister of health has confirmed</span></a><span style=\"font-weight: 400;\">, in response to a question asked in Parliament in June 2022, that information on foreign nationals is</span><a href=\"https://pmg.org.za/committee-question/19188/\"> <span style=\"font-weight: 400;\">not collected</span></a><span style=\"font-weight: 400;\"> as part of South Africa’s Routine Health Information System.</span>\r\n\r\n<span style=\"font-weight: 400;\">There is no denying that</span><a href=\"https://theconversation.com/healthcare-in-south-africa-how-inequity-is-contributing-to-inefficiency-163753\"> <span style=\"font-weight: 400;\">South Africa’s public healthcare system faces multiple challenges</span></a><span style=\"font-weight: 400;\">, a reality recognised by the country’s National Department of Health. Just last week, Health Minister Dr Joe Phaahla publicly acknowledged that such challenges include “</span><a href=\"https://www.gov.za/speeches/minister-joe-phaahla-site-visit-%C2%A0kalafong-hospital-tshwane-1-sep-2022-0000\"><span style=\"font-weight: 400;\">reduced funding, inefficiency, maladministration, corruption and poor management</span></a><span style=\"font-weight: 400;\">.”</span>\r\n\r\n<span style=\"font-weight: 400;\">He also stated that the government is, “</span><a href=\"https://www.gov.za/speeches/minister-joe-phaahla-site-visit-%C2%A0kalafong-hospital-tshwane-1-sep-2022-0000\"><span style=\"font-weight: 400;\">ready to be held accountable in the improvement of quality of health services for the entire South African population</span></a><span style=\"font-weight: 400;\">”.</span>\r\n\r\n<span style=\"font-weight: 400;\">The task then should be to focus on holding the government to account for addressing these challenges, and not allowing dangerous discourse that fuels</span><a href=\"https://theconversation.com/rising-vigilantism-south-africa-is-reaping-the-fruits-of-misrule-179891\"> <span style=\"font-weight: 400;\">anti-immigrant activism and vigilantism</span></a><span style=\"font-weight: 400;\"> to distract from the issue by scapegoating foreign nationals. </span>\r\n\r\n<span style=\"font-weight: 400;\">As a systems problem, it is only by addressing service delivery that we can address the challenges facing South Africa’s public healthcare system. Interventions designed to progress towards</span><a href=\"https://www.who.int/health-topics/universal-health-coverage\"> <span style=\"font-weight: 400;\">Universal Health Coverage</span></a><span style=\"font-weight: 400;\"> (UHC) — with an emphasis on “universal” — </span><a href=\"https://theconversation.com/migrants-must-be-part-of-south-africas-universal-health-plan-heres-why-120925\"><span style=\"font-weight: 400;\">are needed</span></a><span style=\"font-weight: 400;\"> to ensure that everyone in South Africa will benefit.</span>\r\n\r\n<span style=\"font-weight: 400;\">At a national level, South Africa has a progressive policy framework that supports UHC, and upholds the</span><a href=\"https://www.spotlightnsp.co.za/2022/05/13/opinion-what-the-law-says-about-medical-xenophobia-in-south-africa/\"> <span style=\"font-weight: 400;\">right to healthcare for all</span></a><span style=\"font-weight: 400;\">, clearly outlined in the</span><a href=\"https://www.mahpsa.org/wp-content/uploads/2017/09/S27migranthealthA5-2017-print.pdf\"> <span style=\"font-weight: 400;\">Constitution, the Refugees Act and the National Health Act</span></a><span style=\"font-weight: 400;\"> (see footnote).</span>\r\n\r\n<hr />\r\n\r\n<strong>Visit <a href=\"https://www.dailymaverick.co.za?utm_source=direct&utm_medium=in_article_link&utm_campaign=homepage\"><em>Daily Maverick's</em> home page</a> for more news, analysis and investigations</strong>\r\n\r\n<hr />\r\n\r\n<span style=\"font-weight: 400;\">But current debates about how the right to health should be afforded to foreign migrants is — as always — polarised, with very little space for understanding the complex reality of South Africa’s relationship to migration and health.</span>\r\n\r\n<span style=\"font-weight: 400;\">The blaming of foreign migrants for resource, staffing and infrastructure challenges of the public health system is</span><a href=\"https://www.dailymaverick.co.za/article/2019-09-26-in-sickness-and-in-health-why-the-minister-of-home-affairs-has-a-duty-of-care-to-tackle-endemic-anti-foreigner-sentiment/\"> <span style=\"font-weight: 400;\">nothing new</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">However, what is new — and increasingly gaining currency across political and public debates — is the suggestion that the rights afforded to everyone in South Africa to access healthcare are wrong and that immigration should be treated as a matter of national security.</span>\r\n\r\n<span style=\"font-weight: 400;\">This is reflected in amendments to the Refugee and Immigration Acts, the White Paper on International Migration and the restrictions outlined in the National Health Insurance Bill.</span>\r\n\r\n<span style=\"font-weight: 400;\">Most recently, it has been clearly shown in the suggestion by the ANC government, during its</span><a href=\"https://cisp.cachefly.net/assets/articles/attachments/88865_6th_npc_report.pdf\"> <span style=\"font-weight: 400;\">sixth policy conference</span></a><span style=\"font-weight: 400;\">, that South Africa should overhaul its entire governance of immigration, including reconsidering the country’s accession of the</span><a href=\"https://www.unhcr.org/1951-refugee-convention.html\"> <span style=\"font-weight: 400;\">1951 Refugee Convention and its 1967 Protocol</span></a><span style=\"font-weight: 400;\">.</span>\r\n<h4><b>Dangerous developments</b></h4>\r\n<span style=\"font-weight: 400;\">These developments are dangerous for everyone in South Africa. They push the</span><a href=\"https://www.dailymaverick.co.za/article/2022-06-06-proposed-legal-and-policy-reform-will-entrench-sa-discrimination-and-xenophobia/\"> <span style=\"font-weight: 400;\">unsubstantiated idea</span></a><span style=\"font-weight: 400;\"> that immigration is</span><a href=\"https://theconversation.com/xenophobia-is-on-the-rise-in-south-africa-scholars-weigh-in-on-the-migrant-question-181288\"> <span style=\"font-weight: 400;\">to blame</span></a><span style=\"font-weight: 400;\"> for failures of the state to deliver services, for high crime rates and rising unemployment. They also provide fodder for growing</span><a href=\"https://theconversation.com/rising-vigilantism-south-africa-is-reaping-the-fruits-of-misrule-179891\"> <span style=\"font-weight: 400;\">anti-immigrant activism</span></a><span style=\"font-weight: 400;\">, typified by the increasingly violent actions of</span><a href=\"https://www.npr.org/2022/06/07/1103445432/south-africa-anti-immigrant-operation-dudula\"> <span style=\"font-weight: 400;\">Operation Dudula</span></a><span style=\"font-weight: 400;\"> and the</span><a href=\"https://www.iol.co.za/sundayindependent/news/were-not-xenophobic-says-south-african-first-president-mario-khumalo-e9f129df-61ee-47a9-8794-fd100e994f1d\"> <span style=\"font-weight: 400;\">South Africa First Party.</span></a>\r\n\r\n<span style=\"font-weight: 400;\">Blaming foreign nationals for a struggling public healthcare system serves as a</span><a href=\"https://www.politicsweb.co.za/politics/limpopo-mec-of-health-conducted-herself-unprofessi\"> <span style=\"font-weight: 400;\">convenient distraction</span></a><span style=\"font-weight: 400;\"> from the real problems associated with service delivery. The suggestion that foreign nationals are the sole problem (and their removal is the sole solution) prevents us from understanding why the South African public healthcare system is struggling and what is needed to improve the quality of life</span><a href=\"https://theconversation.com/migrants-must-be-part-of-south-africas-universal-health-plan-heres-why-120925\"> <span style=\"font-weight: 400;\">for all in South Africa</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Moreover, this distraction stops us from holding officials to account for the actions required to improve the delivery of services to the majority of the population — citizens and non-citizens alike — who are reliant on the public healthcare system.</span>\r\n\r\n<span style=\"font-weight: 400;\">Ultimately, everyone loses: the scapegoating of migrants not only undermines progress towards quality healthcare for all, but also poses real risks to the health and wellbeing of everyone living in South Africa.</span>\r\n<h4><b>Footnote</b></h4>\r\n<span style=\"font-weight: 400;\">Everyone in South Africa has the right to free emergency healthcare at the point of use — no fees can be charged before care is provided. All pregnant and breastfeeding women, and all children under six years of age, have the right to free services at all levels of healthcare regardless of nationality and documentation status. This includes all antenatal care, childbirth and vaccination services.</span>\r\n\r\n<span style=\"font-weight: 400;\">In South Africa, a means-tested co-payment system exists for all when accessing higher levels of care, ie hospitals; all users — including South African citizens — are means-tested on the basis of income and are charged accordingly.
