"contents": "<span style=\"font-weight: 400;\">For years, people who are far overweight have had to put up with condescending attitudes – even from their doctors – as though their weight gain has been a failure of willpower. </span>\r\n\r\n<span style=\"font-weight: 400;\">But people’s </span><a href=\"https://www.nature.com/articles/nutd20157\"><span style=\"font-weight: 400;\">genes</span></a><span style=\"font-weight: 400;\"> and not being able to </span><a href=\"https://nutritionj.biomedcentral.com/articles/10.1186/1475-2891-10-104\"><span style=\"font-weight: 400;\">buy healthy food</span></a><span style=\"font-weight: 400;\"> easily – rather than a lack of self-discipline – have a much bigger role in why obese people struggle to lose weight and keep it off by exercising and restricting what they eat, research shows. </span>\r\n\r\n<a href=\"https://www.bmj.com/content/347/bmj.f5934\"><span style=\"font-weight: 400;\">Bariatric surgery</span></a><span style=\"font-weight: 400;\"> (which makes your stomach smaller so that you eat less) or </span><a href=\"https://www.nature.com/articles/s41591-022-02026-4\"><span style=\"font-weight: 400;\">appetite-suppressant medicines</span></a><span style=\"font-weight: 400;\"> (such as </span><a href=\"https://bhekisisa.org/health-news-south-africa/2023-02-09-ozempic-a-hashtag-a-helpful-effect-collide-drain-global-stocks-of-a-diabetes-drug/\"><span style=\"font-weight: 400;\">Ozempic)</span></a><span style=\"font-weight: 400;\"> are the most effective ways for people with a body mass index over 35 to lose weight. Someone is considered </span><a href=\"https://www.cdc.gov/obesity/basics/adult-defining.html\"><span style=\"font-weight: 400;\">overweight when their body mass index is 25 or higher, and obese when this number reaches 30</span></a><span style=\"font-weight: 400;\"> or more. </span>\r\n\r\n<span style=\"font-weight: 400;\">The attitudes that obese and overweight people face is similar to those seen in the early days of the HIV epidemic, when people used to say that it was someone’s own fault for getting infected, said Francois Venter, an HIV doctor and director of the </span><a href=\"https://www.ezintsha.org/\"><span style=\"font-weight: 400;\">Ezintsha</span></a><span style=\"font-weight: 400;\"> research centre, in an interview for an episode of </span><a href=\"https://bhekisisa.org/multimedia/podcasts/2023-02-02-podcast-weve-failed-as-clinicians-this-hiv-doctor-is-changing-how-he-treats-overweight-patients-heres-why/\"><i><span style=\"font-weight: 400;\">The Bhekisisa Podcast</span></i></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">Thanks to effective new HIV medicines such as dolutegravir, which </span><a href=\"https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0222229\"><span style=\"font-weight: 400;\">eliminates the disease as a medical issue</span></a><span style=\"font-weight: 400;\">, Venter’s patients are living long, healthy lives, and his role as a clinician is evolving too. </span>\r\n\r\n<span style=\"font-weight: 400;\">These days, weight gain troubles so many of his HIV patients that it prompted him to look for solutions to help them. What he’s learnt in the process has led him to believe that the treatment of obese people is “one of the last great stigmas”.</span>\r\n\r\n<span style=\"font-weight: 400;\">Until now, Venter said, he’d been giving his overweight and obese patients advice based on the outdated view that exercise and diet are the key to weight loss. “I’m shocked at my own behaviour,” he said on the podcast. </span>\r\n\r\n<span style=\"font-weight: 400;\">“Patients also used to think that they deserve to be HIV positive because they made bad life choices,” he added. Now, doctors and patients know that there are a whole lot of structural reasons for people becoming infected with HIV. </span>\r\n\r\n<span style=\"font-weight: 400;\">In some communities (</span><a href=\"https://journals.plos.org/globalpublichealth/article?id=10.1371/journal.pgph.0001502\"><span style=\"font-weight: 400;\">such as those where people live in poverty</span></a><span style=\"font-weight: 400;\"> and with little education), someone’s chance of getting infected with HIV is high. The same is true of how societal issues affect obesity, but there’s still so much shame around the condition. “It feels exactly like HIV,” Venter said.</span>\r\n<h4><b>What’s the link between HIV and weight? </b></h4>\r\n<span style=\"font-weight: 400;\">Weight gain, weight loss and HIV have a complicated relationship, Venter said.</span>\r\n\r\n<span style=\"font-weight: 400;\">When an HIV infection goes untreated, a person could lose weight because the virus weakens their immune system and makes them more likely to pick up infections </span><a href=\"https://www.mayoclinicproceedings.org/article/S0025-6196(11)64035-4/fulltext#:~:text=Characteristics%20of%20HIV%2DAssociated%20Wasting,the%20onset%20of%20symptomatic%20AIDS.\"><span style=\"font-weight: 400;\">that cause diarrhoea or result in problems with the body’s ability to absorb the nutrients from food</span></a><span style=\"font-weight: 400;\">. This is called “wasting”.</span>\r\n\r\n<span style=\"font-weight: 400;\">Taking medicines that keep the virus in check has reduced cases of HIV-related wasting, but the drugs themselves can still have an impact on someone’s weight. </span>\r\n\r\n<b>Read more in Daily Maverick: </b><a href=\"https://www.dailymaverick.co.za/article/2022-11-07-sas-exploding-obesity-epidemic-department-of-health-launches-draft-prevention-and-management-strategy/\"><span style=\"font-weight: 400;\">SA’s exploding obesity epidemic — department of health launches draft prevention and management strategy</span></a>\r\n\r\n<span style=\"font-weight: 400;\">Older antiretroviral treatments such as stavudine made </span><a href=\"https://bmcinfectdis.biomedcentral.com/articles/10.1186/1471-2334-11-244\"><span style=\"font-weight: 400;\">people lose weight</span></a><span style=\"font-weight: 400;\"> (often in strange places, such as in their legs) because the medicine could affect the body’s ability to store and use fat. Stavudine is no longer used in South Africa’s HIV programme. </span>\r\n\r\n<span style=\"font-weight: 400;\">Taking </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7612404/\"><span style=\"font-weight: 400;\">efavirenz is also linked to weight loss</span></a><span style=\"font-weight: 400;\">, and researchers think this is because people’s bodies struggle to break down the drug quickly enough, which interferes with normal metabolism. </span>\r\n\r\n<span style=\"font-weight: 400;\">This drug’s use is declining too. The Health Department’s new HIV treatment guidelines (which haven’t been published yet) </span><a href=\"https://bhekisisa.org/health-news-south-africa/2022-12-13-hiv-treatment-in-sa-is-changing-here-are-7-things-you-need-to-know/\"><span style=\"font-weight: 400;\">will cycle most people who are on efavirenz onto a regimen that includes the more effective drug dolutegravir</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">People on dolutegravir pick up weight, but </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/32960272/\"><span style=\"font-weight: 400;\">research </span></a><span style=\"font-weight: 400;\">shows it’s not the medicine itself causing this. Instead, it’s a sign that someone is returning to health because the treatment is good at thwarting the virus. </span>\r\n<h4><b>Why would getting better on HIV treatment lead to weight gain? </b></h4>\r\n<a href=\"https://apps.who.int/iris/bitstream/handle/10665/325892/WHO-CDS-HIV-19.15-eng.pdf?ua=1\"><span style=\"font-weight: 400;\">Dolutegravir is better than efavirenz</span></a><span style=\"font-weight: 400;\"> at getting the levels of HIV in someone’s blood so low that it can no longer be transmitted during sex (this is called </span><a href=\"https://www.cdc.gov/hiv/risk/art/index.html#:~:text=This%20is%20called%20viral%20suppression,called%20an%20undetectable%20viral%20load.\"><span style=\"font-weight: 400;\">viral suppression</span></a><span style=\"font-weight: 400;\">). </span>\r\n\r\n<span style=\"font-weight: 400;\">The drug also has fewer of the harsh side-effects of earlier medicines, including dizziness, bad dreams, liver issues and problems with metabolism. </span>\r\n\r\n<span style=\"font-weight: 400;\">Venter said: “It seems that the older regimens were actually masking the fact that people gain weight when they get better on HIV [treatment].” </span>\r\n\r\n<span style=\"font-weight: 400;\">With the newer drugs not preventing people from gaining weight any longer, there are also two other factors to consider. </span>\r\n\r\n<span style=\"font-weight: 400;\">First, South Africans are </span><a href=\"https://www.nature.com/articles/nutd20157\"><span style=\"font-weight: 400;\">genetically prone to obesity</span></a><span style=\"font-weight: 400;\">. A study that tracked a group of 990 black South African teens from the well-known </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2702039/\"><span style=\"font-weight: 400;\">Birth to 20 study</span></a><span style=\"font-weight: 400;\">, found three genes that were linked to having a higher body mass index. “The best way to avoid becoming obese is to choose your parents well,” Venter said an obesity expert told him once. </span>\r\n\r\n<span style=\"font-weight: 400;\">Second, </span><a href=\"https://nutritionj.biomedcentral.com/articles/10.1186/1475-2891-10-104\"><span style=\"font-weight: 400;\">research</span></a><span style=\"font-weight: 400;\"> shows that the way South Africa’s cities and transport systems are set up isn’t helping. People who live far from work may struggle to have meals at home and are more likely to buy food on the go, the researchers argue. </span>\r\n<blockquote>The tools that reliably help overweight and obese people lose weight (surgery and medicines) are out of reach for poorer people once more.</blockquote>\r\n<span style=\"font-weight: 400;\">These are often high in fat and refined starch, but have few other nutrients. Savoury snacks such as vetkoek, fried chips and kotas (a hollowed-out quarter loaf of bread usually filled with fried chips), soft drinks and ready-to-eat fast food are generally also </span><a href=\"https://nutritionj.biomedcentral.com/articles/10.1186/1475-2891-10-104\"><span style=\"font-weight: 400;\">cheaper than healthier options</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The food industry is also constantly</span><a href=\"https://www.cambridge.org/core/journals/public-health-nutrition/article/global-trends-in-added-sugars-and-nonnutritive-sweetener-use-in-the-packaged-food-supply-drivers-and-implications-for-public-health/A6375EB569DCDA4899730EC40C69D1CC\"><span style=\"font-weight: 400;\"> tailoring their products to make them more delicious,</span></a><span style=\"font-weight: 400;\"> Venter said, so that you just can’t stop eating when you’ve had the first mouthful.</span>\r\n<h4><b>What happens once someone has picked up weight? </b></h4>\r\n<span style=\"font-weight: 400;\">It’s </span><a href=\"https://ajph.aphapublications.org/doi/10.2105/AJPH.2015.302773\"><span style=\"font-weight: 400;\">next to impossible</span></a><span style=\"font-weight: 400;\"> for heavily overweight and obese people to lose weight without surgery or medicines, Venter said. </span>\r\n\r\n<span style=\"font-weight: 400;\">He cautioned that doctors should make sure they explain to patients starting dolutegravir that they can expect to gain weight on the medicine. </span>\r\n\r\n<span style=\"font-weight: 400;\">“As a medical community – and as an HIV community – we need to start thinking very hard about what we do for people who are gaining weight. It would be terrible for people to stop taking their medicines because they’re worried about weight gain.”</span>\r\n\r\n<b>Read more in Daily Maverick: </b><a href=\"https://www.dailymaverick.co.za/article/2023-03-02-more-than-half-of-the-world-will-be-overweight-or-obese-by-2035-report/\"><span style=\"font-weight: 400;\">More than half of the world will be overweight or obese by 2035 – report</span></a>\r\n\r\n<span style=\"font-weight: 400;\">Telling heavily overweight people to diet and exercise is not the way to do it, Venter argues, because research shows that this course of action only leads to weight loss in a </span><a href=\"https://ajph.aphapublications.org/doi/10.2105/AJPH.2015.302773%E2%80%9D\"><span style=\"font-weight: 400;\">small number of people</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">He explained: “Imagine telling an overweight person to diet and exercise. Then, when they don’t lose weight, you say, ‘you’re a bad person and you’re not taking our advice’.</span>\r\n\r\n<span style=\"font-weight: 400;\">“As clinicians, we’ve failed our patients.” </span>\r\n<h4><b>What advice should doctors give their patients instead? </b></h4>\r\n<span style=\"font-weight: 400;\">Bariatric surgery and weight loss medicines such as </span><a href=\"https://www.wegovy.com/taking-wegovy/dosing-schedule.html\"><span style=\"font-weight: 400;\">Wegovy</span></a><span style=\"font-weight: 400;\"> and Ozempic “offer a great deal of hope”, but, says Venter, they’re not available in </span><a href=\"https://knowledgehub.health.gov.za/system/files/elibdownloads/2021-02/Primary%20Healthcare%20STGs%20and%20EML%207th%20edition%20-%202020-v2.0.pdf\"><span style=\"font-weight: 400;\">South Africa’s public sector</span></a><span style=\"font-weight: 400;\"> because they’re so </span><a href=\"https://bhekisisa.org/health-news-south-africa/2023-02-09-ozempic-a-hashtag-a-helpful-effect-collide-drain-global-stocks-of-a-diabetes-drug/\"><span style=\"font-weight: 400;\">expensive</span></a><span style=\"font-weight: 400;\">. The medicine comes in a 1.5ml injection pen, which costs between </span><a href=\"https://medicineprices.org.za/#search:ozempic\"><span style=\"font-weight: 400;\">R1,200 and R1,500</span></a><span style=\"font-weight: 400;\">. It’s injected weekly, and one pen can last somebody between one and eight weeks, depending on the weekly dose that their doctor prescribes. </span>\r\n\r\n<span style=\"font-weight: 400;\">This situation too reminds Venter of the darker days of South Africa’s HIV epidemic. Back then, rich people had access to antiretrovirals, while those who relied on the state for healthcare did not. </span>\r\n\r\n<a href=\"https://link.springer.com/article/10.1007/s11904-015-0259-7\"><span style=\"font-weight: 400;\">For nearly 10 years</span></a><span style=\"font-weight: 400;\">, from 1999 to 2008, the South African government largely denied that Aids was a disease caused by becoming infected with HIV. </span>\r\n\r\n<span style=\"font-weight: 400;\">The country’s decision-makers at the time (former president Thabo Mbeki and his health minister, Manto Tshabalala-Msimang) claimed </span><a href=\"https://www.aidstruth.org/myths/\"><span style=\"font-weight: 400;\">poverty, malnutrition, haemophillia (a blood disorder) and drug use</span></a><span style=\"font-weight: 400;\"> – and not HIV – was responsible for the devastating number of deaths Africa was seeing at the time. In 2005 alone, Aids deaths were estimated at </span><a href=\"https://data.unaids.org/publications/irc-pub06/epi_update2005_en.pdf\"><span style=\"font-weight: 400;\">2.4 million</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The government portrayed a drug known as AZT, </span><a href=\"https://www.aidstruth.org/myths/#m7\"><span style=\"font-weight: 400;\">by then a proven antiretroviral treatment</span></a><span style=\"font-weight: 400;\">, as </span><a href=\"https://vdoc.pub/documents/aids-and-south-africa-the-social-expression-of-a-pandemic-6ksrl5kh6sc0\"><span style=\"font-weight: 400;\">poisonous</span></a><a href=\"https://www.aidstruth.org/myths/\"><span style=\"font-weight: 400;\"> and even claimed that it caused Aids</span></a><span style=\"font-weight: 400;\">.</span>\r\n\r\n<span style=\"font-weight: 400;\">Now, Venter says, the tools that reliably help overweight and obese people lose weight (surgery and medicines) are out of reach for poorer people once more. </span>\r\n\r\n<span style=\"font-weight: 400;\">He says: “As doctors we’ll have to start thinking hard about better ways to help obese and overweight South Africans who want to lose weight.
