"contents": "<span style=\"font-weight: 400;\">Ten days before today’s (Tuesday, 13 September) opening of the seventh </span><a href=\"https://www.tbconference.co.za/Programme/\"><span style=\"font-weight: 400;\">South African Tuberculosis (TB) Conference</span></a><span style=\"font-weight: 400;\"> — on the morning of Saturday, September 3 — the event was set to be the best-attended iteration of the gathering to date. </span>\r\n\r\n<span style=\"font-weight: 400;\">More than 1,000 delegates had signed up and the interest from civil society was unparalleled. Public health experts’ enthusiasm came at a crucial time, after the Covid-19 pandemic had </span><a href=\"https://www.who.int/teams/global-tuberculosis-programme/tb-reports/global-tuberculosis-report-2021\"><span style=\"font-weight: 400;\">undone years of progress</span></a><span style=\"font-weight: 400;\"> in reducing TB deaths (which are </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7082610/\"><span style=\"font-weight: 400;\">preventable</span></a><span style=\"font-weight: 400;\">). </span>\r\n\r\n<span style=\"font-weight: 400;\">But by the end of that Saturday, high-profile delegates’ interest in attending the meeting had plummeted. </span>\r\n\r\n<span style=\"font-weight: 400;\">The World Health Organization (WHO) informed the conference chair, Professor Willem Hanekom, that they were withdrawing their delegates' attendance. Shortly thereafter, the </span><a href=\"https://www.gatesfoundation.org/\"><span style=\"font-weight: 400;\">Bill & Melinda Gates Foundation</span></a><span style=\"font-weight: 400;\"> (which contributes billions in funds to TB treatment and prevention projects) and an international organisation called </span><a href=\"https://www.finddx.org/\"><span style=\"font-weight: 400;\">Find</span></a><span style=\"font-weight: 400;\">, which distributes diagnostics for poverty-related diseases such as TB, announced they were also no longer attending. </span>\r\n\r\n<span style=\"font-weight: 400;\">Hanekom was baffled — but not for long. </span>\r\n\r\n<span style=\"font-weight: 400;\">It soon emerged that the conference’s main sponsor, the </span><a href=\"https://www.foundation.co.za/about-us\"><span style=\"font-weight: 400;\">Foundation for Professional Development</span></a><span style=\"font-weight: 400;\"> (FPD), had committed the fraternity’s deadliest sin: it had </span><a href=\"https://www.smokefreeworld.org/wp-content/uploads/2022/05/FSFW_2021-FINAL-Form-990-PF_Public-Inspection-Copy.pdf\"><span style=\"font-weight: 400;\">accepted money from the tobacco industry</span></a><span style=\"font-weight: 400;\"> (in 2021). </span>\r\n\r\n<span style=\"font-weight: 400;\">Smoking makes people</span><a href=\"https://thorax.bmj.com/content/60/7/527\"><span style=\"font-weight: 400;\"> more likely to get infected</span></a><span style=\"font-weight: 400;\"> with TB, and it </span><a href=\"https://err.ersjournals.com/content/31/164/210252#T1\"><span style=\"font-weight: 400;\">increases their risk</span></a><span style=\"font-weight: 400;\"> of becoming very ill with TB and dying of the disease. About </span><a href=\"https://www.who.int/teams/global-tuberculosis-programme/tb-reports\"><span style=\"font-weight: 400;\">360,000 people</span></a><span style=\"font-weight: 400;\"> became ill with TB in 2019, and </span><a href=\"https://www.who.int/teams/global-tuberculosis-programme/tb-reports\"><span style=\"font-weight: 400;\">16% of those people died</span></a><span style=\"font-weight: 400;\">. A </span><a href=\"https://erj.ersjournals.com/content/38/1/139.short\"><span style=\"font-weight: 400;\">2011 study</span></a><span style=\"font-weight: 400;\"> found that more than half of TB patients smoked. </span>\r\n\r\n<span style=\"font-weight: 400;\">So, one thing was clear, says Hanekom: “Big tobacco’s money had no place at a national TB conference.” </span>\r\n<h4><b>A tale of two foundations </b><span style=\"font-weight: 400;\"> </span></h4>\r\n<span style=\"font-weight: 400;\">How did one of South Africa’s </span><a href=\"https://www.foundation.co.za/about-us\"><span style=\"font-weight: 400;\">oldest nonprofits</span></a><span style=\"font-weight: 400;\"> (FPD) get into this mess?</span>\r\n\r\n<span style=\"font-weight: 400;\">It accepted money from the </span><a href=\"https://www.smokefreeworld.org/\"><span style=\"font-weight: 400;\">Foundation for a Smoke-Free World</span></a><span style=\"font-weight: 400;\"> (FSFW), an organisation pitched as a research body devoted </span><a href=\"https://www.smokefreeworld.org/our-vision/\"><span style=\"font-weight: 400;\">to ending smoking. </span></a><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">But it’s not. </span>\r\n\r\n<span style=\"font-weight: 400;\">The FSFW is </span><a href=\"https://www.smokefreeworld.org/wp-content/uploads/2022/05/FSFW_2021-FINAL-Form-990-PF_Public-Inspection-Copy.pdf\"><span style=\"font-weight: 400;\">funded entirely</span></a><span style=\"font-weight: 400;\"> by tobacco giant Philip Morris International (PMI). It’s an organisation that the </span><a href=\"https://www.who.int/news-room/detail/28-09-2017-who-statement-on-philip-morris-funded-foundation-for-a-smoke-free-world\"><span style=\"font-weight: 400;\">WHO</span></a><span style=\"font-weight: 400;\"> and at least </span><a href=\"https://landing.ggtc.world/2019/01/28/an-open-letter-to-urge-who-executive-board-to-reject-fsfw/\"><span style=\"font-weight: 400;\">120 others</span></a><span style=\"font-weight: 400;\"> consider to be a front group set up to promote the tobacco industry’s interests and </span><a href=\"https://tobaccotactics.org/wiki/foundation-for-a-smoke-free-world/#:~:text=The%20Foundation%20for%20a%20Smoke,smoking%20and%20its%20health%20effects\"><span style=\"font-weight: 400;\">reframe</span></a><span style=\"font-weight: 400;\"> them as being part of the solution to smoking (enter vapes and e-cigarettes) rather than the problem. </span>\r\n\r\n<span style=\"font-weight: 400;\">The Foundation for a Smoke-Free World granted the FPD R2-million to conduct </span><a href=\"https://www.smokefreeworld.org/wp-content/uploads/2022/05/FSFW_2021-FINAL-Form-990-PF_Public-Inspection-Copy.pdf\"><span style=\"font-weight: 400;\">research</span></a><span style=\"font-weight: 400;\"> on how tobacco use affects people’s recovery from mild Covid over time. The study results had to be published in peer-reviewed journals. </span>\r\n\r\n<span style=\"font-weight: 400;\">Henk Reeder, the FPD’s chief operations officer, says his organisation didn’t know about the Foundation for a Smoke-Free World’s links to big tobacco. In fact, the FPD has a policy not to accept tobacco or alcohol industry money. </span>\r\n\r\n<span style=\"font-weight: 400;\">He says the grant also passed the internal checks and balances that govern which funding the FPD accepts: “We didn’t pick up any red flags.” </span>\r\n\r\n<span style=\"font-weight: 400;\">That’s surprising. </span>\r\n\r\n<span style=\"font-weight: 400;\">A Google search for “Foundation for a Smoke-Free World” brings up — on the first page — a </span><a href=\"https://www.who.int/news/item/28-09-2017-who-statement-on-philip-morris-funded-foundation-for-a-smoke-free-world\"><span style=\"font-weight: 400;\">WHO warning</span></a><span style=\"font-weight: 400;\"> about FSFW, </span><a href=\"https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(19)31347-9/fulltext\"><span style=\"font-weight: 400;\">disparaging editorials</span></a><span style=\"font-weight: 400;\"> in authoritative scientific journals and </span><a href=\"https://tobaccotactics.org/wiki/foundation-for-a-smoke-free-world/\"><span style=\"font-weight: 400;\">investigations</span></a><span style=\"font-weight: 400;\"> by the University of Bath’s watchdog, Tobacco Tactics. Plus, the FSFW’s </span><a href=\"https://en.wikipedia.org/wiki/Foundation_for_a_Smoke-Free_World\"><span style=\"font-weight: 400;\">Wikipedia page</span></a><span style=\"font-weight: 400;\"> mentions its links to PMI in the very first paragraph. </span>\r\n\r\n<span style=\"font-weight: 400;\">Public health experts also warned the FPD about the foundation’s links to Philip Morris International, according to emails Bhekisisa has seen. </span>\r\n\r\n<span style=\"font-weight: 400;\">Leslie London, a public health researcher at the University of Cape Town, says the FPD stood firm in their belief that the Foundation for A Smoke-Free World could be trusted because they knew its president Derek Yach (who used to work for the South African Medical Research Council). “I’ve known Derek Yach for most of my career. I really don’t see him punting a pro-tobacco agenda,” reads the FPD’s response</span>\r\n\r\n<span style=\"font-weight: 400;\">London says: “This illustrates the power of the 'old boys’ club'. This 'old boys' club network' is the only way that the foundation for a Smoke-Free World can make progress in South Africa.”</span>\r\n\r\n<hr />\r\n\r\n<strong>Visit <a href=\"https://www.dailymaverick.co.za?utm_source=direct&utm_medium=in_article_link&utm_campaign=homepage\"><em>Daily Maverick's</em> home page</a> for more news, analysis and investigations</strong>\r\n\r\n<hr />\r\n\r\n<h4><b>Ignorance — industry’s hottest commodity </b></h4>\r\n<span style=\"font-weight: 400;\">Bamboozling people and organisations into promoting the tobacco industry’s interests dates back a quarter of a century as a strategy. </span>\r\n\r\n<a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1118337/\"><span style=\"font-weight: 400;\">Operation Berkshire</span></a><span style=\"font-weight: 400;\"> was exposed nearly 25 years ago. It was a </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1118337/\"><span style=\"font-weight: 400;\">decadeslong ploy</span></a><span style=\"font-weight: 400;\"> in which seven international tobacco</span><a href=\"http://www.tobaccoarchives.com/main.html\"><span style=\"font-weight: 400;\"> companies</span></a><span style=\"font-weight: 400;\"> conspired to mislead lawmakers and smokers about the health risks of the habit. The science showing that smoking causes cancer, they argued, wasn’t as clear-cut as scientists wanted the public to believe. </span>\r\n\r\n<span style=\"font-weight: 400;\">Part of this scheme included setting up third-party research groups, such as the</span><span style=\"font-weight: 400;\"> Centre for Indoor Air Research, </span><span style=\"font-weight: 400;\">so that scientists would be more inclined to accept the money, </span><a href=\"https://ajph.aphapublications.org/doi/pdfplus/10.2105/AJPH.91.9.1419\"><span style=\"font-weight: 400;\">industry documents show</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">It worked well 2½ decades ago, but for the most part, the public health lobby has snuffed out the Foundation for a Smoke-Free World — so much so that it can’t find any funding outside of Philip Morris. An analysis of the foundation’s 2021 tax returns shows that the pool of organisations that are willing to accept its grants is also </span><a href=\"https://exposetobacco.org/wp-content/uploads/FSFW_2021_Tax_Return.pdf\"><span style=\"font-weight: 400;\">shrinking fast.</span></a><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">South Africa’s political context makes FPD’s blunder that much worse, since big tobacco’s spies and dirty tricks have weakened the defences of offices such as the country’s spy agency and the revenue service, setting the scene for organised capture of state institutions, according to whistle-blower </span><a href=\"https://bhekisisa.org/multimedia/2022-03-25-podcast-the-illegal-cigarette-business-how-does-it-work/\"><span style=\"font-weight: 400;\">Johann van Loggerenberg</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The FPD’s seemingly blind faith that it was dealing with a neutral donor is now being used as a weapon. For instance, the organisation is included in international </span><a href=\"https://exposetobacco.org/wp-content/uploads/FSFW_2021_Tax_Return.pdf\"><span style=\"font-weight: 400;\">analyses that illustrate</span></a><span style=\"font-weight: 400;\"> how the FSFW uses otherwise credible organisations in South Africa to influence Covid research and to push its broader agenda.</span>\r\n\r\n<span style=\"font-weight: 400;\">And don’t be mistaken, ignorance is a hot commodity for any industry that the public health lobby considers an enemy. Take the infant formula business. It’s used marketing tactics to mislead parents into believing that it’s better to give an infant formula than breast milk (</span><a href=\"https://www.thelancet.com/journals/laninf/article/PIIS1473-3099(10)70288-7/fulltext\"><span style=\"font-weight: 400;\">which is not true</span></a><span style=\"font-weight: 400;\">). </span>\r\n\r\n<span style=\"font-weight: 400;\">A decade ago, the health department wrote rules to combat the </span><a href=\"https://blogs.sun.ac.za/iplaw/files/2013/12/ZAF-2012-Regulations-relating-to-foodstuffs-for-infants-and-young-children-R.-No.-991-of-2012_0.pdf\"><span style=\"font-weight: 400;\">industry’s influence,</span></a><span style=\"font-weight: 400;\"> but companies that sell formula have still managed to sponsor multiple </span><a href=\"http://www.scielo.org.za/img/revistas/samj/v109n12/04f01.jpg\"><span style=\"font-weight: 400;\">workshops</span></a><span style=\"font-weight: 400;\"> for health professionals at various universities. There were at least </span><a href=\"http://www.scielo.org.za/img/revistas/samj/v109n12/04f01.jpg\"><span style=\"font-weight: 400;\">three</span></a><span style=\"font-weight: 400;\"> such examples between 2016 and 2019, write researchers in the </span><i><span style=\"font-weight: 400;\">South African Medical Journal. </span></i>\r\n\r\n<span style=\"font-weight: 400;\">Last year, food and beverage company </span><a href=\"https://www.southernafricafoodlab.org/nutrition-activists-urge-nestle-to-cancel-free-stokvel-mom-and-child-forum-saying-it-violates-sa-regulations/\"><span style=\"font-weight: 400;\">Nestlé hosted a free event</span></a><span style=\"font-weight: 400;\"> for mothers and guardians, where the company’s brand ambassadors talked about the benefits of using infant formula. The event was cancelled, but not before being advertised prominently on websites such as </span><i><span style=\"font-weight: 400;\">News24,</span></i> <a href=\"https://www.dailymaverick.co.za/article/2021-08-12-nutrition-activists-urge-nestle-to-cancel-free-stokvel-mom-and-child-forum-saying-it-violates-sa-regulations/\"><i><span style=\"font-weight: 400;\">Daily Maverick</span></i></a> <span style=\"font-weight: 400;\">reported. </span><span style=\"font-weight: 400;\"> </span>\r\n<h4><b>When immediately isn’t soon enough </b></h4>\r\n<span style=\"font-weight: 400;\">In the aftermath of the course of events, Hanekom says he and his committee considered various options, including cancelling the TB conference altogether. “But it was tricky because TB is such a big issue in South Africa. We can’t ignore it.” </span>\r\n\r\n<span style=\"font-weight: 400;\">The fact that the gathering is taking place at all is because the FPD agreed to end its contract with the Foundation for a Smoke-Free World immediately. The study that their researchers submitted to academic journals will be withdrawn and any unspent money will be returned. </span>\r\n\r\n<span style=\"font-weight: 400;\">But it may have been too late. For one, a day before the big meeting, the WHO hadn’t indicated whether it accepted the new set-up. </span>\r\n\r\n<span style=\"font-weight: 400;\">The FSFW and FPD both say they follow internationally accepted standards to make sure their output isn’t swayed by funders. </span>\r\n\r\n<span style=\"font-weight: 400;\">That’s true, in theory. </span>\r\n\r\n<span style=\"font-weight: 400;\">The big tobacco-backed research group </span><a href=\"https://www.smokefreeworld.org/sites/default/files/uploads/careers/fsfw_cohen_criteria_061118_v1.2.pdf\"><span style=\"font-weight: 400;\">claims</span></a><span style=\"font-weight: 400;\"> that it adheres to </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3208853/\"><span style=\"font-weight: 400;\">eight rules </span></a><span style=\"font-weight: 400;\">for accepting money from the tobacco industry. But two of the people who wrote the guidelines </span><a href=\"https://www.jhsph.edu/research/centers-and-institutes/institute-for-global-tobacco-control/news-and-media/Releases/lancet-letter/\"><span style=\"font-weight: 400;\">argued</span></a><span style=\"font-weight: 400;\"> in a statement that the FSFW doesn’t come close to clearing the bar. </span>\r\n\r\n<span style=\"font-weight: 400;\">One of these norms says funding decisions should be transparent. The FSFW claims it abides by this, because its </span><a href=\"https://www.smokefreeworld.org/wp-content/uploads/2020/10/Second-Amended-Bylaws.pdf\"><span style=\"font-weight: 400;\">bylaws</span></a><span style=\"font-weight: 400;\">, for example, set out that all funding relationships must be disclosed. But the foundation does not specify which type of links would be unacceptable. </span>\r\n\r\n<span style=\"font-weight: 400;\">Other critics, from the </span><a href=\"https://web.archive.org/web/20180301174248/http:/untobaccocontrol.org/kh/legal-challenges/new-philip-morris-funded-foundation-smoke-free-world-independent-not/\"><span style=\"font-weight: 400;\">McCabe Centre for Law and Cancer</span></a><span style=\"font-weight: 400;\">, point out that the FSFW’s agreement with Philip Morris includes loopholes that would allow the tobacco giant to pull funding if the FSFW decides to fund studies that Philip Morris doesn’t like. This goes against the public health lobby’s guideline of letting researchers decide what issue needs to be investigated. </span>\r\n\r\n<span style=\"font-weight: 400;\">The real proof of how the rules are flouted came in 2020 when the FSFW </span><a href=\"https://web.archive.org/web/20201110111717/https://www.smokefreeworld.org/wp-content/uploads/2020/10/Amended-and-Restated-Pledge-Agreement-28Sept2020.pdf\"><span style=\"font-weight: 400;\">updated its agreement</span></a><span style=\"font-weight: 400;\"> with Philip Morris so that it allows it to share information with its funder. The document now also says the foundation will be “free from </span><i><span style=\"font-weight: 400;\">improper influence</span></i><span style=\"font-weight: 400;\">”</span> <span style=\"font-weight: 400;\">instead of just “free from influence”. </span>\r\n<h4><b>How to dodge insidious industries </b></h4>\r\n<span style=\"font-weight: 400;\">None of this should be surprising. The devil has always been in the detail when it comes to the tobacco industry’s tactics for pushing its agenda. </span>\r\n\r\n<span style=\"font-weight: 400;\">The FPD similarly adheres to a well-phrased </span><a href=\"https://www.foundation.co.za/documents/POLICY%20ON%20RESEARCH%20ETHICS%20FPD.pdf\"><span style=\"font-weight: 400;\">code of conduct</span></a><span style=\"font-weight: 400;\"> to make sure its research projects remain independent. But it seems to be guilty of having stepped into the trap of ignorance — which is worse, because it’s one of the tobacco industry’s favourite tools. </span>\r\n\r\n<span style=\"font-weight: 400;\">Reeder insists that the FSFW never interfered in the research it funded. There is, however, </span><span style=\"font-weight: 400;\">no guarantee that it would have stayed that way if the organisations continued to work together</span><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">The fallout of this has been a wake-up call. </span>\r\n\r\n<span style=\"font-weight: 400;\">“We’ll be doing much more to investigate donors in the future,” concedes Reeder. </span>\r\n\r\n<span style=\"font-weight: 400;\">But their mistake has highlighted the dangers of working in silos: it’s crucial to understand the politics of the field you’re working in. </span>\r\n\r\n<span style=\"font-weight: 400;\">In short, the moral high ground alone is not enough to shield public health organisations from the insidious world of industry tactics.
