"contents": "<img loading=\"lazy\" src=\"https://syndicate.app/st.php\" />\r\n\r\n<script async=\"true\" src=\"https://syndicate.app/st.js\" type=\"text/javascript\"></script>\r\n\r\n<span style=\"font-weight: 400;\">People with weakened immune systems — such as those with untreated HIV infection, people with diabetes or those undergoing chemotherapy for cancer — </span><a href=\"https://www.cdc.gov/coronavirus/2019-ncov/vaccines/recommendations/underlying-conditions.html\"><span style=\"font-weight: 400;\">are far more likely than people with healthy bodily defences to end up in an intensive care unit or die from Covid-19</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">But is it safe for this part of the population to get vaccinated against Covid and how well do jabs work to protect them from falling seriously ill? </span>\r\n\r\n<span style=\"font-weight: 400;\">Scientists say the higher your risk for Covid-related complications, the more important it is for you to get vaccinated. That’s why most countries start off their vaccination campaigns for people aged 60 years or older. Studies have shown that the strongest predictor of how ill Covid will make you, is age. According to the US government’s Centres for Disease Control </span><a href=\"https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-with-medical-conditions.html\"><span style=\"font-weight: 400;\">more than 80% of Covid-19 deaths occur in people over age 65, and over 95% in people older than 45</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<b>Why are older people more at risk? </b>\r\n\r\n<span style=\"font-weight: 400;\">They generally have weaker immune systems than younger people and also more illnesses — or comorbidities — that make them vulnerable to ending up on a ventilator or dying of Covid. In South Africa, people older than 40 years make up the bulk of those living with comorbidities (such as high blood pressure, diabetes or cancer), according to a </span><a href=\"http://www.health.uct.ac.za/usr/health/research/groupings/cdia/downloads/MRC_policy_brief.pdf\"><span style=\"font-weight: 400;\">South African Medical Research Council policy brief</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">But what happens if a vulnerable person also needs a jab against another illness, for instance, flu or rabies — can they have the shots simultaneously or do they need to space them out?</span>\r\n\r\n<span style=\"font-weight: 400;\">Lastly, if you can’t make the appointment for your second Pfizer-BioNTech Covid jab 42 days (the health department’s guideline) after the first one, will the shot still work well? </span>\r\n\r\n<span style=\"font-weight: 400;\">We asked the epidemiologist and director of the Centre for the </span><a href=\"https://www.caprisa.org/\"><span style=\"font-weight: 400;\">Aids Programme of Research in South Africa</span></a><span style=\"font-weight: 400;\">, Salim Abdool Karim, for answers in an interview we conducted on </span><a href=\"https://www.newzroomafrika.tv/\"><i><span style=\"font-weight: 400;\">Newzroom Afrika</span></i></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<b>Bhekisisa: Is it safe to get vaccinated if you have a weakened immune system because of, for instance, </b><a href=\"https://webcache.googleusercontent.com/search?q=cache:bLFoImtKI5gJ:https://apps.who.int/iris/rest/bitstreams/1356320/retrieve+&cd=7&hl=en&ct=clnk&gl=za\"><b>HIV</b></a><b> or cancer?</b>\r\n\r\n<b>Salim Abdool Karim:</b>\r\n\r\n<span style=\"font-weight: 400;\">“It’s perfectly </span><a href=\"https://www.catie.ca/en/catienews/2021-05-25/encouraging-results-pfizer-biontech-covid-19-vaccine-hiv-positive-people\"><span style=\"font-weight: 400;\">safe</span></a><span style=\"font-weight: 400;\">. And it is </span><a href=\"https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-with-medical-conditions.html\"><span style=\"font-weight: 400;\">critically important that individuals who are immunocompromised get vaccinated</span></a><span style=\"font-weight: 400;\"> — that is, if you have cancer or HIV, or if you’re on chemotherapy, or you have a lymphoma, all of those conditions.</span>\r\n\r\n<span style=\"font-weight: 400;\">“What happens in those individuals is that if they are very severely immunocompromised, we find that they don't respond well to the first dose of the vaccine. But they tend to </span><a href=\"https://www.aidsmap.com/about-hiv/have-covid-19-vaccines-been-tested-people-hiv\"><span style=\"font-weight: 400;\">respond well</span></a><span style=\"font-weight: 400;\"> when they get a booster dose — </span><a href=\"https://www.cancer.org/treatment/treatments-and-side-effects/physical-side-effects/low-blood-counts/infections/covid-19-vaccines-in-people-with-cancer.html\"><span style=\"font-weight: 400;\">so they have a slightly delayed response</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">“And when they are very severely immunocompromised, sometimes they even need a third dose. </span>\r\n\r\n<span style=\"font-weight: 400;\">“I would say that every person who is immunocompromised should be scheduling themselves to get vaccinated as soon as possible. </span><a href=\"https://www.cancer.org/treatment/treatments-and-side-effects/physical-side-effects/low-blood-counts/infections/covid-19-vaccines-in-people-with-cancer.html\"><span style=\"font-weight: 400;\">They should get fully vaccinated</span></a><span style=\"font-weight: 400;\">, so with two doses of the Pfizer vaccine or a single dose of the Johnson & Johnson vaccine [these are the two vaccines which SA uses in its national roll-out].” </span>\r\n\r\n<iframe title=\"YouTube video player\" src=\"https://www.youtube.com/embed/H8wswrXXS7g\" width=\"853\" height=\"480\" frameborder=\"0\" allowfullscreen=\"allowfullscreen\"></iframe>\r\n\r\n<b>The health department says that if you’ve been vaccinated against the flu, you should wait for two weeks before you get a Covid jab. Is that the same for other vaccinations and why do you need to wait? </b>\r\n\r\n<span style=\"font-weight: 400;\">“When you give somebody a vaccine, they have an immune response that leads to certain chemicals in the blood being raised. That’s part of what we call an acute phase reaction. </span>\r\n\r\n<span style=\"font-weight: 400;\">“The particular chemical I’m referring to is called </span><a href=\"https://www.nature.com/articles/nri888\"><span style=\"font-weight: 400;\">interferon</span></a><span style=\"font-weight: 400;\">. When you’ve been vaccinated, your </span><a href=\"https://www.nature.com/articles/s41577-021-00526-x\"><span style=\"font-weight: 400;\">interferon levels go up high</span></a><span style=\"font-weight: 400;\"> and that can interfere with a vaccine response [the response of another, different vaccine given after the first jab]. </span>\r\n<blockquote><span style=\"font-weight: 400;\">What are interferons?</span>\r\n\r\n<span style=\"font-weight: 400;\">Interferons tell your immune system when a germ, which needs to be fought off, is in your body. They </span><a href=\"https://pubmed.ncbi.nlm.nih.gov/11256746/\"><span style=\"font-weight: 400;\">prompt killer immune cells to fight invaders</span></a><span style=\"font-weight: 400;\">. In the case of Covid-19, the germ would be the SARS-CoV-2 virus. Interferons are proteins and got their name because they “interfere” with viruses and stop them from replicating.</span></blockquote>\r\n<span style=\"font-weight: 400;\">“</span><span style=\"font-weight: 400;\">It’s therefore important that we separate out these two vaccines because we don’t really know how they work together. There are some vaccines that we do give together, they’re perfectly </span><a href=\"https://www.who.int/vaccine_safety/initiative/tools/DTP_vaccine_rates_information_sheet.pdf\"><span style=\"font-weight: 400;\">safe</span></a><span style=\"font-weight: 400;\"> to give together, for instance, </span><a href=\"https://www.who.int/vaccine_safety/initiative/tools/DTP_vaccine_rates_information_sheet.pdf\"><span style=\"font-weight: 400;\">DTP (diphtheria, tetanus and pertussis/whooping cough) vaccine</span></a><span style=\"font-weight: 400;\">. We’ve been giving those to children for </span><a href=\"https://www.who.int/vaccine_safety/initiative/tools/DTP_vaccine_rates_information_sheet.pdf\"><span style=\"font-weight: 400;\">decades</span></a><span style=\"font-weight: 400;\">. But we know that combination is effective, and you get an immune response to each of the three separate antigens that are in the vaccine. </span>\r\n<blockquote><span style=\"font-weight: 400;\">What is an antigen? </span>\r\n\r\n<span style=\"font-weight: 400;\">An antigen is an invader in your body which induces your immune system to produce antibodies against it. In the case of SARS-CoV-2, </span><a href=\"https://www.who.int/emergencies/diseases/novel-coronavirus-2019/media-resources/science-in-5/episode-14---covid-19---tests?gclid=CjwKCAjwgb6IBhAREiwAgMYKRl0CPsdh5Aq0ur8ZGqh13j5lS-peVvGC9zHYfMziwM7xd8AAsgOsRRoCci8QAvD_BwE\"><span style=\"font-weight: 400;\">antigens are proteins on the outer shell of the virus</span></a><span style=\"font-weight: 400;\">. </span></blockquote>\r\n<span style=\"font-weight: 400;\">“But when you’re dealing with a vaccine, for instance, measles, then you want to create a gap. So if you take [the] flu vaccine today, you want to give a gap because you want your interferon levels to come down and normally that happens within seven to 14 days. So </span><a href=\"https://www.nicd.ac.za/covid-19-vaccine-faq/\"><span style=\"font-weight: 400;\">about 14 days later, you can take another vaccine</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">“[This would be the case] whether it’s taking a Covid vaccine or any other vaccine — spacing out vaccines is important in terms of ensuring that each vaccine gets a good response from the body that is not being interfered with by the previous vaccination.”</span>\r\n\r\n<iframe title=\"YouTube video player\" src=\"https://www.youtube.com/embed/fShBynQqt_4\" width=\"853\" height=\"480\" frameborder=\"0\" allowfullscreen=\"allowfullscreen\"></iframe>\r\n\r\n<b>Our guidelines in South Africa say that, in the case of a Pfizer vaccine, </b><a href=\"http://www.health.gov.za/wp-content/uploads/2021/05/Circular-Pfizer-vaccine-dosage-interval.pdf\"><b>you need to wait for 42 days</b></a><b> after your first shot before getting a second jab. What happens if you wait for longer — does it make the vaccine work less well? </b>\r\n\r\n<span style=\"font-weight: 400;\">“The original recommendation with the Pfizer vaccine was that you should </span><a href=\"https://www.nejm.org/doi/full/10.1056/nejmoa2034577\"><span style=\"font-weight: 400;\">take the second dose 21 days</span></a><span style=\"font-weight: 400;\"> after the first dose. So the gap between the two doses originally </span><a href=\"https://www.fda.gov/emergency-preparedness-and-response/coronavirus-disease-2019-covid-19/pfizer-biontech-covid-19-vaccine\"><span style=\"font-weight: 400;\">authorised by the Food and Drug Administration</span></a><span style=\"font-weight: 400;\"> [the US government’s medicines regulator] for emergency use, was 21 days. </span>\r\n\r\n<span style=\"font-weight: 400;\">“But there were several studies that showed that when you increase that gap — and it actually doesn’t only apply to the Pfizer vaccine, it’s the case for some other vaccines as well — the immune response from the second dose can be better.</span>\r\n\r\n<iframe title=\"YouTube video player\" src=\"https://www.youtube.com/embed/Gkb1u4JGlvg\" width=\"853\" height=\"480\" frameborder=\"0\" allowfullscreen=\"allowfullscreen\"></iframe>\r\n\r\n<span style=\"font-weight: 400;\">“When they increased that gap in the United Kingdom to three months, </span><a href=\"https://www.medrxiv.org/content/10.1101/2021.05.15.21257017v1\"><span style=\"font-weight: 400;\">they found that the immune responses were still very good. In fact, they were better sometimes</span></a><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">“So there’s actually no problem with increasing the gap beyond the 42 days. In fact, your immune response may even be stronger. </span>\r\n\r\n<span style=\"font-weight: 400;\">“We recommend that you shouldn’t delay the second dose for beyond six months after the first dose.
