This is according to</span><span style=\"font-weight: 400;\"> Dr </span><a href=\"https://chronicpain.co.za/meet-dr-raath/\"><span style=\"font-weight: 400;\">Russell Raath</span></a><span style=\"font-weight: 400;\">, a specialist anesthesiologist and owner of a “pain clinic” in Pretoria</span><span style=\"font-weight: 400;\">. </span>\r\n\r\n<span style=\"font-weight: 400;\">He says if pain is ongoing, don’t ignore it – seek help. </span>\r\n\r\n<span style=\"font-weight: 400;\">Even if it is intermittent pain after a sprain or surgery that doesn’t go away, its time to get help. </span>\r\n\r\n<span style=\"font-weight: 400;\">Chronic pain is defined as pain that lasts longer than expected. </span><a href=\"http://www.iasp-pain.org/\"><span style=\"font-weight: 400;\">The International Association for the Study of Pain</span></a><span style=\"font-weight: 400;\"> defines it as </span><span style=\"font-weight: 400;\">“</span><span style=\"font-weight: 400;\">an unpleasant, sensory and emotional experience which could be associated with tissue damage”.</span>\r\n\r\n<span style=\"font-weight: 400;\">Chronic pain is notoriously difficult to treat, and the success rate – for effective treatment – is not high, says Prof Sean Chetty,</span><span style=\"font-weight: 400;\"> president of the </span><a href=\"https://painsa.org.za/\"><span style=\"font-weight: 400;\">Pain Society of SA</span></a><span style=\"font-weight: 400;\"> and executive head of the </span><a href=\"http://www.sun.ac.za/english/faculty/healthsciences/anaesthesiology_critical_care\"><span style=\"font-weight: 400;\">Department of Anaesthesiology and Critical Care</span></a><span style=\"font-weight: 400;\"> at University Stellenbosch. </span>\r\n\r\n<span style=\"font-weight: 400;\">Raath says he believes “it is </span><span style=\"font-weight: 400;\">pathetic that patients who suffer from chronic pain are often told to make pain their friend”, adding that patients who suffer from pain shouldn’t simply be referred to psychologists or psychiatrists who put them on anti-depressants. </span>\r\n\r\n<span style=\"font-weight: 400;\">There is always hope for improvement. </span>\r\n\r\n<a href=\"https://open.spotify.com/artist/6a7yFEdaUPfs8WpUQI4zfK\"><span style=\"font-weight: 400;\">Anneli van Rooyen</span></a><span style=\"font-weight: 400;\">, a celebrated singer and songwriter, knows a lot about chronic pain. She won many awards for her angelic voice, until chronic pain dashed her hopes of a promising future. Van Rooyen consulted many therapists who referred her to psychologists and psychiatrists, until she was finally diagnosed with </span><a href=\"https://www.neurosurgeonsofnewjersey.com/blog/atypical-trigeminal-neuralgia-symptom/#:~:text=Atypical%20trigeminal%20neuralgia%20is%20a,baseline%20during%20normal%20functional%20activities.\"><span style=\"font-weight: 400;\">atypical trigeminal neuralgia</span></a><span style=\"font-weight: 400;\"> (TN2) in 2004. </span>\r\n\r\n<span style=\"font-weight: 400;\">“This came as a huge relief. I felt vindicated because now at least we knew what we were dealing with.” </span>\r\n\r\n<span style=\"font-weight: 400;\">The neurological surgery that followed was successful and Van Rooyen continued with her career. A few years later the pain was back and a second operation was performed – this time it was a disaster. </span>\r\n\r\n<span style=\"font-weight: 400;\">Van Rooyen woke up in agony. She could not move her jaw and had to be fed through a straw. Pain that she never imagined possible grabbed hold of her and extreme trauma, anger and a sense of hopelessness quickly followed. </span>\r\n\r\n<span style=\"font-weight: 400;\">Her independence, career and income were wiped out. </span>\r\n\r\n<span style=\"font-weight: 400;\">Since then, after many additional procedures, she exhausted all available therapies and has learned to cope as best she can.</span>\r\n\r\n<span style=\"font-weight: 400;\">Chetty says pain – its causes and remedies – is poorly taught at medical schools worldwide. </span>\r\n\r\n<span style=\"font-weight: 400;\">In South Africa the field is not recognised as a sub-specialty. </span>\r\n\r\n<span style=\"font-weight: 400;\">Pain patients are vulnerable and desperate. This puts them at risk of falling victim to unscrupulous practitioners who may perform unnecessary procedures that are not strictly scientifically based. </span>\r\n\r\n<span style=\"font-weight: 400;\">Chetty believes surgery should only be considered once all other options have been exhausted. Treating chronic pain successfully often involves shifting the focus from curing the pain to relieving it. </span>\r\n<h4><b>The difference between chronic pain and acute pain </b></h4>\r\n<b>Acute pain </b>\r\n\r\n<span style=\"font-weight: 400;\">Acute and chronic are very </span><a href=\"https://www.hopkinsmedicine.org/health/conditions-and-diseases/chronic-pain\"><span style=\"font-weight: 400;\">different</span></a><span style=\"font-weight: 400;\"> in nature. </span>\r\n\r\n<span style=\"font-weight: 400;\">Acute pain is often the result of damage to tissue. When an <a href=\"https://www.dailymaverick.co.za/article/2022-06-14-does-icing-an-injury-delay-recovery-the-jury-is-still-out/\">injury</a> occurs, the spinal nerves carry messages from the body to the brain to tell it what is going on.</span>\r\n\r\n<span style=\"font-weight: 400;\">Acute pain is often also called </span><a href=\"https://www.practicalpainmanagement.com/resource-centers/opioid-prescribing-monitoring/medical-management-acute-pain\"><span style=\"font-weight: 400;\">nociceptive</span></a><span style=\"font-weight: 400;\"> pain. </span><span style=\"font-weight: 400;\">Self -medication, analgesics, anti-inflammatory drugs, muscle relaxants and physio- therapy are usually sufficient to </span><span style=\"font-weight: 400;\">reduce or block these messages before they reach the brain.</span><span style=\"font-weight: 400;\"> </span>\r\n\r\n<span style=\"font-weight: 400;\">Once the injury heals, the messages and the pain stop.</span>\r\n\r\n<b>Chronic pain</b>\r\n\r\n<span style=\"font-weight: 400;\">Raath explains that chronic pain occurs when nerves are damaged. </span>\r\n\r\n<span style=\"font-weight: 400;\">Actual physical changes take place in the nervous system which sends the messages to the brain. It happens primarily in the spinal cord. </span>\r\n\r\n<span style=\"font-weight: 400;\">These damaged nerves become the disease. </span>\r\n\r\n<span style=\"font-weight: 400;\">Chronic pain could start as acute pain; when this happens, the nervous system has been damaged and neuropathic changes have taken place. One example is shingles, which starts as a viral infection. Once it has cleared, damaged nerves could cause chronic pain. </span>\r\n<h4><b>Treatment of chronic pain</b></h4>\r\n<span style=\"font-weight: 400;\">Chetty says the sooner </span><a href=\"https://my.clevelandclinic.org/health/diseases/15833-neuropathic-pain\"><span style=\"font-weight: 400;\">chronic pain</span></a><span style=\"font-weight: 400;\"> is identified and treated, the better the chances of success. </span>\r\n\r\n<span style=\"font-weight: 400;\">Treatment could involve biological management, psychological therapy and physical rehabilitation. The approach taken to treat chronic pain will depend on the diagnosis and patient’s profile. </span>\r\n\r\n<span style=\"font-weight: 400;\">A doctor will usually discuss the benefits and risks of the various medical procedures with the patient. Early treatment will prevent neuropathic changes taking place. </span>\r\n\r\n<span style=\"font-weight: 400;\">Once these changes set in, it is difficult to remove pain completely. Chetty explains that when we have an acute injury, the sensory neurons that send messages to the brain are activated. </span>\r\n\r\n<span style=\"font-weight: 400;\">When the pain is ongoing, t</span><span style=\"font-weight: 400;\">he repeated messages can change the connections and structure of the brain. </span><span style=\"font-weight: 400;\">The brain memorises these signals. It sets off an increasing number of chemicals and connections in the brain and spinal cord, which could be compared to the click of a metronome. </span>\r\n\r\n<span style=\"font-weight: 400;\">After a while we anticipate these clicks or signals even before they happen until, eventually, the brain becomes over-sensitised and reacts to these anticipated signals. As a result, we may experience pain even though there are no new stimuli. </span>\r\n\r\n<span style=\"font-weight: 400;\">Raath adds that this partly explains phantom pain, where a patient may experience pain in a limb that has been amputated. Early medication could stop or block these anticipated messages. </span>\r\n\r\n<span style=\"font-weight: 400;\">Associate Professor </span><a href=\"https://www.wits.ac.za/staff/academic-a-z-listing/m/mot-moz/palesamotshabiwitsacza/\"><span style=\"font-weight: 400;\">Palesa Motshabi</span></a><span style=\"font-weight: 400;\">, </span><span style=\"font-weight: 400;\">Academic Head of the Department of Anaesthesiology at Wits</span><span style=\"font-weight: 400;\"> University, believes chronic pain could be prevented if </span><a href=\"https://painconcern.org.uk/chronic-pain-after-surgery/\"><span style=\"font-weight: 400;\">pre-habilitation</span></a><span style=\"font-weight: 400;\"> was introduced before major surgery. </span>\r\n\r\n<span style=\"font-weight: 400;\">A multi-disciplinary team could give patients who must undergo operations such as thoracic or cancer surgery, information and therapy beforehand. This could help speed up recovery in patients predisposed to chronic pain. </span>\r\n\r\n<a href=\"https://pubmed.ncbi.nlm.nih.gov/30940757/\"><span style=\"font-weight: 400;\">Patients</span></a><span style=\"font-weight: 400;\"> who may be prone to post operative chronic pain include, young people, females, smokers and those with a history of depression or anxiety. </span><a href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3654683/\"><span style=\"font-weight: 400;\">Ethnicity</span></a><span style=\"font-weight: 400;\"> has also shown to play a role in the experience of pain.</span>\r\n\r\n<span style=\"font-weight: 400;\">In conclusion, Van Rooyen says she has accepted that she will always have pain. </span>\r\n\r\n<span style=\"font-weight: 400;\">She has learnt that the only way to cope is to take one day at a time. </span>\r\n\r\n<span style=\"font-weight: 400;\">From moment to moment she chooses to be positive; when things become too much and she feels she is drowning in pain and grief, she simply surrenders and allows herself to feel angry and hopeless. </span>\r\n\r\n<span style=\"font-weight: 400;\">Her tip for those who might be feeling the same is “to celebrate every small achievement and do whatever you can, even if it means just pitching up”. </span>\r\n\r\n<span style=\"font-weight: 400;\">As a believer, she feels she can still make a difference and what she is enduring now, will not last forever. </span>\r\n\r\n<span style=\"font-weight: 400;\">Chetty advises anyone who has chronic pain to investigate and learn about the subject and the treatment options available. </span>\r\n\r\n<span style=\"font-weight: 400;\">He says “knowledge is our greatest weapon. There are lots of resources on the internet. Discuss these with your medical practitioner. There is always something we can do to help a patient – our goal is to improve their quality of life”. </span><b>DM/ML</b>",
